Central nervous system hypoxia in children due to near drowning.

Fourteen children who experienced acute, profound central nervous system hypoxia secondary to near drowning, aspiration, or respiratory arrest underwent CT examination. During the first week after the episode, the most frequent finding was a loss of gray-white matter differentiation. Other findings included effacement of sulci and cisterns, focal areas of edema in the cerebral cortex or basal ganglia, and hemorrhagic infarctions of the basal ganglia. Subsequent CT scans obtained from two weeks to five months after the hypoxic episode showed progression of cerebral loss from cortical infarction

[Drowning, swimming pool death and other emergencies related to swimming].

In the Federal Republic of Germany, some 700 people drown every year. Of particular importance is the so-called swimming bath black-out that occurs while swimming under water, which must always be avoidable when the causes are known and appropriate precautionary measures taken. Of life-saving importance in a case of drowning is the rapid and proper use of first aid. The severe pulmonary injury induced by freshwater or salt water aspiration urgently requires the treatment of the victim in an intensive care unit of a hospital, even after successful resuscitation.

Immersion Deaths in Seoul: Implications of Decomposition for Postmortem Diagnosis of Drowning.

BACKGROUND: Drowning is defined as a process of respiratory impairment caused by submersion in water and is a significant cause of death worldwide. However, postmortem diagnosis of drowning remains a major challenge in forensic pathology, primarily because it relies on the exclusion of all other possible causes based on only a few pathognomonic findings during autopsy, particularly in cases involving decomposition. This study reviewed a series of immersion deaths in Seoul by analyzing demographic and epidemiological factors, their associations with the cause and manner of death, and the impact

Muay Thai Almost Drowned My Heart - A Case Report.

Rib fractures are frequently encountered in contact sports, with pyopneumothorax as a potentially severe complication. However, it is infrequent for pleural empyema to progress to purulent pericarditis and cardiac tamponade. In this report, we present a successful case of a 41-year-old previously healthy Muay Thai fighter who developed purulent pericarditis with cardiac tamponade due to the contiguous spread of infection from an empyemic hydropneumothorax caused by traumatic rib fracture. Emergency pericardial and pleural drainage were performed, and despite negative cultures from both pericar

Assessment of maxillary sinus fluid volume for postmortem diagnosis of drowning.

INTRODUCTION: Drowning is a comprehensive and exclusive diagnosis at autopsy. Autopsy findings such as pleural effusion and waterlogged lungs contribute to the diagnosis. Herein, we aim to reveal the practical usefulness and postmortem changes of the maxillary sinus fluid volume to diagnose drowning. METHODS: We evaluated 52 drowning and 59 nondrowning cases. The maxillary sinus fluid volume was measured using a computed tomography (CT) scan, and pleural effusion volume and lung weight were manually measured at autopsy. The utility of these three indices for diagnosing drowning and its postmo

An autopsy case of drowning under the influence of multiple psychotropic drugs.

A fatal case of drowning under the influence of multiple psychotropic drugs, such as quetiapine, escitalopram, aripiprazole and flunitrazepam, is presented. Quantitative toxicological analysis of a femoral blood sample revealed concentrations of quetiapine, escitalopram, aripiprazole and 7-aminoflunitrazepam (a metabolite of flunitrazepam) of 1.266 µg/ml, 0.609 µg/ml, 0.124 µg/ml and 0.055 µg/ml, respectively. From the autopsy findings, results of toxicological examination and investigation by the authorities, we concluded that the cause of death was drowning under the influence of mainly quet

Lung transplantation following death by drowning: a review of the current literature.

While multiple donor characteristics have been cited as ideal for lung transplantation, there are minimal widely accepted exclusion criteria. One criterion that many centers view with hesitation is death by drowning. However, recent literature suggests such donors may result in acceptable outcomes following transplantation. This review highlights a case of a patient who underwent a successful bilateral lung transplant from a donor following a drowning event. A review of the current literature is presented, concluding with a new proposed set of favorable donor criteria following death by drowni

Bathtub drowning: An 11-year retrospective study in the state of Maryland.

A bathtub drowning is one of the leading causes of death in a bathtub. The purpose of this study was to evaluate how reliable the drowning-related signs could be for identifying a bathtub drowning in the cases of death in the bathtub. Performing a retrospective review of 92 deaths in the bathtub in Maryland, 71.7 percent were the presence of bathtub drowning and 28.3 percent were the absence of bathtub drowning. Three leading contributory causes of death were cardiovascular disease, drug/alcohol-related death, and seizure disorder in both groups. There was a statistically significant differenc

Where children and adolescents drown in Queensland: a population-based study.

OBJECTIVE: This retrospective population-based study examined drowning location by the site of immersion for both fatal and non-fatal drowning events in Queensland. Drowning location is not routinely collected, and this study used data linkage to identify drowning sites. The resulting enhanced quality data quantify drowning incidence for specific locations by geographic region, age group and by severity for the first time. DESIGN: Linked data were accessed from the continuum of care (prehospital, emergency, hospital admission and death data) on fatal and non-fatal drowning episodes in childre

Do not drown the patient: appropriate fluid management in critical illness.

Administering intravenous fluids to support the circulation in critically ill patients has been a mainstay of emergency medicine and critical care for decades, especially (but not exclusively) in patients with distributive or hypovolemic shock. However, in recent years, this automatic use of large fluid volumes is beginning to be questioned. Analysis from several large trials in severe sepsis and/or acute respiratory distress syndrome have shown independent links between volumes of fluid administered and outcome; conservative fluid strategies have also been associated with lower mortality in t

Weight ratio of the lungs and pleural effusion to the spleen in the diagnosis of drowning.

An accurate and reliable method of diagnosing death by drowning is an important requirement in forensic autopsies. In this study, we compared the weight ratio of the lungs and pleural effusion to the spleen for 55 cases of drowning (37 males, 18 females), 36 cases of mechanical asphyxiation (16 males, 20 females), and 26 cases of acute cardiac death (19 males, 7 females). In the case of the males, there were significant differences in the weight of the spleen and the total weight of the lungs and pleural effusion between drowning and the other causes of death; however, there was no such signif

The use of the Heimlich maneuver in near drowning: Institute of Medicine report.

The application of the Heimlich maneuver as the initial and perhaps only step for opening the airway in all near-drowning victims has been proposed by Henry Heimlich and Edward Patrick, contrary to current resuscitation guidelines for the treatment of near-drowning victims established by the Emergency Cardiac Care (ECC) Committee of the American Heart Association. Although the Heimlich maneuver is useful for the removal of aspirated solid foreign bodies, there is no evidence that death from drowning is frequently caused by aspiration of a solid foreign body that is not effectively treated by t

[Cerebral anoxia in near-drowning of children. The prognostic value of EEG].

The retrospective electroclinical evaluation of anoxia by near-drowning in 23 children observed between 1985 and 1989 revealed 2 groups, each with a distinct evolution: the first group, with good prognosis of 17 children, which recovered consciousness without neurological complications between 2 d and 1 wk after the accident. The second group of 6 children with a poor outcome--either i), death; or ii), state of permanent injury; or iii), a high level of clinical deficits. Ice-related winter drowning: an area for evidence-informed injury prevention.

BACKGROUND: As global temperatures increase, ice formation on water bodies might slow and/or be of shorter duration, increasing risk for drowning injury and mortality during wintertime activities. This research explores the state of the scholarly literature on ice-related drownings and applies that knowledge to vital records data from Michigan in the Great Lakes region of the USA. METHODS: We conducted a systematic review of the scholarly literature on wintertime drownings to characterise the current state of the scholarly literature. Using death records from the Great Lakes region of the USA

Lung Abscess Caused by Pseudomonas aeruginosa after a Near-drowning Episode.

Aspiration pneumonia is one of the most severe complications associated with near-drowning and is referred to as drowning-associated pneumonia (DAP). Inhaled water may be contaminated by various pathogens, thereby facilitating DAP development. The causative organisms of DAP differ from those of community-acquired pneumonia, with Gram-negative bacilli being predominant. We herein report an 83-year-old Japanese man with chronic kidney disease who presented to the emergency department after a near-drowning episode. His oxygen saturation level was 86% despite receiving oxygen via a 10 L/min reserv
