The Fingerprint of Scalp-EEG in Drug-Resistant Frontal Lobe Epilepsies.

PURPOSE: Scalp-EEG incompletely covers the frontal lobe cortex. Underrepresentation of frontobasal or frontomesial structures, fast ictal spreading, and false lateralization impede scalp-EEG interpretation. Hence, we investigated the significance of scalp-EEG in the presurgical workup of frontal lobe epilepsy. METHODS: Using descriptive statistical methods and Pearson chi-squared test for group comparisons, we retrospectively investigated postsurgical outcome, interictal epileptiform discharges (iiEDs), and electrographic seizure patterns on scalp-EEG in 81 consecutive patients undergoing res

Short-term effectiveness and side effects of ketogenic diet for drug-resistant epilepsy in children with genetic epilepsy syndromes.

BACKGROUND: Drug-resistant epilepsy (DRE) impacts a significant portion, one-third, of individuals diagnosed with epilepsy. In such cases, exploring non-pharmacological interventions are crucial, with the ketogenic diet (KD) standing out as a valuable option. KD, a high-fat and low-carb dietary approach with roots dating back to the 1920s for managing DRE, triggers the formation of ketone bodies and modifies biochemistry to aid in seizure control. Recent studies have increasingly supported the efficacy of KD in addressing DRE, showcasing positive outcomes. Furthermore, while more research is n

Upregulation of breast cancer resistance protein and major vault protein in drug resistant epilepsy.

PURPOSE: Identifying factors involved in the development of drug resistant epilepsy (DRE) remains a challenge. Candidate gene studies have shown modulation of resistance to drugs by various multidrug resistance proteins in DRE. However the resistance to drugs in DRE could be more complex and multifactorial involving molecules in different pharmacokinetic processes. In this study for the first time we have analyzed the relative expression of four molecules with different drug resistance mechanisms in two most common DRE pathologies, mesial temporal lobe epilepsy (MTLE) and focal cortical dyspla

Characterization of a lamotrigine-resistant kindled model of epilepsy in mice: evaluation of drug resistance mechanisms.

The aims of this study were to characterize a lamotrigine-resistant kindled model of epilepsy in mice, to study the anticonvulsant effect of carbamazepine (CBZ) and valproic acid (VPA), and to probe into the mechanism for resistance. Swiss albino mice were kindled by a subconvulsive dose of pentylenetetrazole (PTZ, 30 mg/kg, i.p., every other day for 6 weeks). The mice were pre-treated (30 min.) either with a low dose of LTG (5 mg/kg, i.p.) or with vehicle, and the seizures were scored. The acute treatment with LTG (15 mg/kg, i.p.) on the last day blocked seizure in the vehicle-treated group,

Precision management of cenobamate in drug-resistant epilepsy: integrating pharmacogenetics, therapeutic drug monitoring, and real-world clinical strategies.

Cenobamate (CNB) demonstrates high efficacy in drug-resistant focal epilepsy, yet optimal management requires personalized strategies. This exploratory case series describes five patients selected from a cohort of 125 individuals treated with CNB, examining the relationship between pharmacogenetic (PGx) profiles and clinical outcomes. Clinical data and therapeutic drug monitoring (TDM) were integrated with targeted Next-Generation Sequencing of key metabolic genes (UGT2B7, UGT2B4, CYP2E1, CYP2B6, CYP2A6, CYP3A4, CYP2C19) to predict metabolizer phenotypes. Three patients (Cases 1, 3, 4) achieve

Circulating polymer-resolved microplastics in pediatric epilepsy: higher internal burden in drug-resistant cases.

Drug-resistant epilepsy (DRE) in children remains a major clinical challenge, and environmental contributors to pharmacoresistance are not fully understood. In this study, we quantified polymer-resolved microplastics (MPs) in peripheral blood from pediatric patients with controlled epilepsy (CE), drug-resistant epilepsy (RE), and healthy controls (NC) using pyrolysis-gas chromatography-mass spectrometry (Py-GC/MS) with laser direct infrared (LDIR) validation. Eight polymer types were detected. Total circulating polymer-resolved MP burden was observed to be higher in epilepsy patients, with the

Plasma volume fraction in drug-resistant epilepsy: A DCE-MRI marker of microvascular pathology.

OBJECTIVES: Microvascular remodeling and blood-brain barrier dysfunction (BBBD) are increasingly recognized as contributors to epilepsy. However, commonly used vascular imaging markers are often state-dependent and lack spatial specificity. We aimed to (1) validate plasma volume fraction (vₚ) derived from dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) as a sensitive marker of microvascular changes; (2) characterize the vₚ alterations in patients with drug-resistant epilepsy (DRE); and (3) assess the spatial relationship between vₚ abnormalities and BBBD. METHODS: We analyzed D

In vivo 9.4 Tesla MRI of a patient with drug-resistant epilepsy: Technical report.

PURPOSE: In resective epilepsy surgery for drug-resistant focal epilepsy (DRE), good seizure outcome is strongly associated with visualization of an epileptogenic lesion on MRI. Standard clinical MRI (≤ 3 Tesla (T)) may fail to detect subtle lesions. 7T MRI enhances detection and delineation, the potential benefits of increasing field strength to 9.4T are explored. METHODS: A 36 years old male patient with DRE evaluated for resective surgery, in which 3T and 7T MRI failed to detect any epileptogenic lesions, was submitted to a dedicated epilepsy scan protocol using T1 and T2* weighted imaging

Breaking the Seizure Cycle: Surgical Outcomes of Medial Temporal Lobectomy in Drug-Resistant Epilepsy.

BACKGROUND: Medial temporal lobe (MTL) lesions are among the most common causes of drug-resistant epilepsy. Surgical management typically involves lesion excision, with or without amygdalohippocampectomy. An alternative approach includes selective hippocampal transection to disrupt the epileptogenic circuit without affecting the memory circuit. Objective: To assess epilepsy control after medial temporal lobectomy for management of drug-resistant temporal lobe epileptic patients. METHODS: This is a prospective cohort interventional study of consecutive series of patients coming to Al-Azhar Uni

Nuclei-Specific Amygdala Enlargement Is Linked to Psychiatric Comorbidities in Drug-Resistant Focal Epilepsy.

OBJECTIVE: Amygdala enlargement has been the subject of controversial studies regarding its significance in terms of pathogenicity both in epilepsy and in psychiatric comorbidities such as anxiety, depression, and post-traumatic stress disorder. However, no causal link has been established in either direction, and the role of distinct amygdala nuclei remains unknown. We investigated volumetric changes of the amygdala and its nine main nuclei and their associations with psychiatric comorbidities in patients with drug-resistant focal epilepsy. METHODS: Eighty-seven adult patients with drug-resi
