Histological examination of the auricular cartilage and pseudocyst of the auricle.

In our histological examination of 42 ears (auricles) which were taken from 18 fetus and three adults, the intracartilaginous fibrous tissue with blood vessels and lymphatics were found in 12 ears and interruption of the auricular cartilage in 22 ears. The intracartilaginous connective tissue with blood vessels and lymphatics were linked together with the connective tissue outside the auricular cartilage in other sections. These findings support the hypothesis of congenital embryonic dysplasia as the origin of the formation of pseudocyst of the auricle.

A new method for training of ear framework creation by silicon dental impression material.

This article presents the novel method of training of creating cartilage framework for total ear reconstruction in microtia. Replica of costal cartilage harvested for real surgery was simulated by silicon dental impression material. Carving of framework was done with wood carving instruments. Silicon dental impression material gives the consistency and texture almost comparable to real costal cartilage. Sequential steps similar to actual surgery were simulated to create the three-dimensional framework.By using this novel technique, novice surgeons can practice creating ear framework and improv

Totally avulsed ear: new technique of immediate ear reconstruction.

INTRODUCTION: Reconstruction of the avulsed auricle remains a challenge to the plastic surgeon. Many surgeons suggest reattachment of the total or subtotal amputated ear as a composite graft, but this solution is unreliable. Some surgeons advocate the use of the retro auricular pocket to enhance survival of the reattached ear. However, this burying method entails multistage procedures with somewhat poor results due to the associated fibrosis. Microsurgical replantation can be performed but its applicability is limited due to difficulty with venous drainage. In this paper we advocate the use of

Effects of hardness and dimension of pinna and auricular hematoma on detection threshold of cartilage conduction.

In cartilage conduction (CC), a vibrator is presented onto the cartilage of the ear instead of the bony parts of the head used in ordinary bone conduction (BC). Because the auricle cartilage is softer and lighter than the bone, it doesn't require as much pressure as BC, which may cause discomfort (or pain) in the area where a BC transducer is being pressed. However, CC is a relatively new technology, and whether the less dense characteristics of cartilage, which varies from person to person, result in a better sound perception is still being studied. In this paper, we focused on investigating

Long-term Outcomes on Patients with Microtia after Autogenous Costal Cartilage Reconstruction.

Autologous costal cartilage grafts remain the gold standard method for microtia reconstruction. However, reports on its long-term outcomes are limited. We present two cases with >40-year outcomes after auricular reconstruction with autologous costal cartilage. A 56 year-old woman and a 53 year-old man presented to our institution with complaints of wire exposure. In both cases, the reconstructed ear was deformed. A computerized tomography scan revealed calcification of the reconstructed costal cartilage graft framework. To our knowledge, these cases present the longest outcomes (50 and 42 year

A True Single-Stage Reconstruction of a Projected Auricle for Concha-Type Microtia Incorporating Endoscopically Harvested Temporoparietal Fascia.

Reconstruction of microtia with autogenous costal cartilage that produces a well-projected ear in a single stage is a challenging endeavor. In this case report, we describe a single-stage, projected costal cartilage-based reconstruction of concha type mitoria. Due to the patient's low hairline, his hair-bearing scalp would encroach on the ear framework if placed subcutaneously in the standard fashion. Thus, a large TPF flap harvested with endoscopic assistance was planned to achieve both a color-matched hairless skin envelope and a well-projected ear in single stage.

Recurrence of an Oval Window Rupture 25 Years After Traumatic Stapediovestibular Luxation Repair: A Case Report of a Novel Cartilage Sealing Technique.

Stapediovestibular luxations are often caused by trauma to the external ear canal. Sealing the oval window while leaving the stapes in the vestibule is a safer procedure than stapedectomy, considering the risk of additional inner ear damage, but it risks recurrence. We report a rare case of oval window rupture recurrence 25 years after initial surgery and introduce a new method using cartilage to seal the oval window. A 37-year-old woman experienced a recurrence of oval window rupture after nasal blowing. She had a history of left ear ossicular chain dislocations and oval window rupture caused

Two-stage reconstruction of the auricle in congenital microtia using autogenous costal cartilage.

The current authors reconstructed the auricles of congenital microtia patients with a two-stage method using autogenous costal cartilage. In the first stage, lobule rotation, fabrication of the cartilage framework, and implantation of the framework were performed. In the second stage, elevation of the auricle using transplantation of the costal cartilage and two local transposition flaps, formation of the tragus, and deepening of the concha were performed. A total of 125 consecutive patients were treated, ranging from seven to 53 years old, from 1990 to 2003. Sixty-nine patients were males and

Rating the aesthetic results after auricular reconstructive surgery for congenital aural atresia with microtia.

OBJECTIVE: This retrospective study aimed to establish a robust rating system for assessing post-operative outcomes in congenital aural atresia patients undergoing auricular reconstruction. The newly introduced EAR scale, a weighted grading system, not only considers anatomical landmarks but also factors such as ear alignment. In addition, the outer-ear cartilage scale and the visual analogue scale (VAS) were introduced. These scales were compared among themselves and against two established scales. METHODS: Nine raters assessed 17 eligible patients who underwent auricular reconstruction betw

Perichondrium/cartilage composite graft for repairing large tympanic membrane perforations and hearing improvement.

BACKGROUND: The main risk factors for postoperative failure in tympanoplasties are large perforations that are difficult to repair, annular perforations, and a tympanic membrane (TM) with extensive granular myringitis that require middle ear exploration and mastoidectomy. The aim of this study was to investigate a novel technique of perichondrium/cartilage composite graft for repairing the large TM perforation in the patient of otitis media. METHODS: Retrospective chart reviews were conducted for 102 patients with large tympanic membrane perforations, who had undergone tympanoplasty from Augu

Improving In Vitro Cartilage Generation by Co-Culturing Adipose-Derived Stem Cells and Chondrocytes on an Allograft Adipose Matrix Framework.

BACKGROUND: Microtia is an inherited condition that results in varying degrees of external ear deformities; the most extreme form is anotia. Effective surgical reconstruction techniques have been developed. However, these usually require multistage procedures and have other inherent disadvantages. Tissue engineering technologies offer new approaches in the field of external ear reconstruction. In this setting, chondrocytes are cultured in the laboratory with the aim of creating bioengineered cartilage matrices. However, cartilage engineering has many challenges, including difficulty in culturi

A comparison of ear reattachment methods: a review of 25 years since Pennington.

BACKGROUND: A failed ear reattachment tends to complicate subsequent reconstruction with autogenous rib cartilage. The various reattachment methods differ in technical demand, in the secondary damage of periauricular tissue, and in aesthetic outcome. In the past 25 years, the microsurgical method has proven to be especially successful. The aim of this work was to assess case reports of auricular trauma since the first microsurgical ear replantation by Pennington. METHODS: An extensive review of the literature of acute ear trauma published between 1980 and 2004 was performed. For a better comp

Modified Methods of Fabricating Helix and Antihelix in Total Auricular Reconstruction Based on Different Length of Eighth Costal Cartilage.

BACKGROUND: Attaining an ideal appearance of the reconstructed ear depends primarily on the fabricated cartilaginous ear framework. Despite the role of eighth costal cartilage, the length of the eighth rib cartilage is difficult to anticipate, and growth of the eighth cartilage is variable. The authors discussed modified methods of fabricating helix and antihelix based on different length of eighth costal cartilage. METHODS: Based on the actual length of the eighth costal cartilage, patients were divided into group A and group B. In group A, the eighth costal cartilage was divided into 2 part
