Recovery After Bariatric Surgery: The effects of Mobilization-A Study Protocol.

BACKGROUND: Patients recovering from bariatric surgery report the highest average pain during hospital admission, compared to patients recovering from other abdominal surgeries. Studies have shown many benefits from early mobilization after surgery, however, the effect of early mobilization after bariatric surgery on pain and physical function is unknown. This study aims to investigate the effects of an early initiated mobilization at the post anesthesia care unit and continued for the first 4 postoperative weeks. METHODS/DESIGN: This randomized controlled trial with a nested interview study

Factors Impacting Early Mobilization Following Hip Fracture: An Observational Study.

BACKGROUND AND PURPOSE: Hip fracture guidelines emphasize mobilization within 48 hours of surgery. The aims of this audit were to determine the proportion of patients with hip fracture who mobilize within 48 hours, identify factors associated with delayed mobilization, and identify barriers to mobilization. METHODS: Single-site prospective audit of 100 consecutive patients (age 82 ± 9 years) admitted for surgical management of hip fracture. Data collected included time to mobilization, factors that may impact mobilization (age, weight-bearing status, additional injuries, premorbid mobility st

Supervised Immediate Postoperative Mobilization After Elective Colorectal Surgery: A Feasibility Study.

BACKGROUND: Early mobilization is a significant part of the ERAS® Society guidelines, in which patients are recommended to spend 2 h out of bed on the day of surgery. However, it is not yet known how early patients can safely be mobilized after completion of colorectal surgery. The aim of this study was to evaluate the feasibility, and safety of providing almost immediate structured supervised mobilization starting 30 min post-surgery at the postoperative anesthesia care unit (PACU), and to describe reactions to this approach. METHODS: This feasibility study includes 42 patients aged ≥18 year

Early mobilization for children in intensive therapy: A protocol for systematic review and meta-analysis.

INTRODUCTION: Intensive care units focus primarily on life support and treatment of critically ill patients, but there are many survivors with complications, such as generalized muscle disorders, functional disability and reduced quality of life after hospital discharge, resulting from prolonged stays in these units. The current evidence suggests that early mobilization-based rehabilitation (exercise initiated immediately after the patient's significant physiological changes have stabilized) in critically ill adults can alleviate these complications from immobility and critical illness. Howeve

ABCDEs of ICU: Early mobility.

The intensive care unit (ICU) provides a critical level of care to medically unstable patients. Patients need intensive monitoring and treatment that may require emergency interventions. The vulnerability and complexity of the ICU unintentionally creates an environment that limits and poorly defines the intervention of early mobility in the unstable critically ill patients. The short- and long-term effects of immobility and bed rest increase acute complications, the length of stay in the ICU and hospital, and mortality and morbidity rates. According to current research, instituting early mobil

Postoperative pain management of the obese patient.

In the obese patient, the goal of postoperative pain management is provision of comfort, early mobilisation and improved respiratory function without causing inadequate sedation and respiratory compromise. The pathophysiology of obesity, typical co-morbidities and the high prevalence of obstructive sleep apnoea (OSA) amongst obese patients make safe analgesic management difficult. In particular, pain control after bariatric surgery is a major challenge. Although several reviews covering anaesthesia and analgesia for obese patients are published, there is mainly expert opinion and a paucity of

Primary patellar tendon repair and early mobilization: results in an active-duty population.

We retrospectively reviewed 13 patellar tendon repairs done over 32 months at a tertiary care, military medical center. Early mobilization was initiated within 2 weeks postoperatively. Clinical and functional results were statistically examined with relation to age, timing of surgery, length of follow-up, quadriceps atrophy, extensor lag, patella position, and time to full duty. At an average of 24 months' follow-up, six patients (46%) had thigh girth atrophy, and one patient (8%) had an extensor lag >5 degrees. Mean Lysholm score was 84 (range, 57 to 100). Maximum postoperative Tegner activit

Early mobilization of isolated ulnar-shaft fractures.

A consecutive series of 21 isolated ulnar-shaft fractures were treated with early mobilization. All the fractures healed with good function and without serious complications. As long as the radioulnar joints are intact, this type of fracture can safely be treated with immediate mobilization or a short period in a below-elbow cast.

Cemented Bipolar Hemiarthroplasty with a Wiring Technique for Unstable Intertrochanteric Hip Fractures in Elderly Patients.

PURPOSE: Intertrochanteric hip fractures are commonly seen in the elderly population. Osteosynthesis is technically demanding and has a high rate of failure, especially in osteoporotic bones. Furthermore, delayed ambulation after surgery can be a risk factor for systemic complications. Active prevention of prolonged bed-ridden can improve the quality of life postoperatively. For early ambulation, the authors chose hemiarthroplasty as the surgical method. The purpose of this study was to assess the efficacy of cemented bipolar hemiarthroplasty with a wiring technique for unstable intertrochante

"Mind the Gap": Brazilian Physiotherapists Are Not Implementing Early Mobilization Interventions for Postoperative Patients With Hip Fractures.

BACKGROUND AND PURPOSE: Early mobilization, such as standing and walking within the first day after surgery, is crucial to enhance recovery after a hip fracture, yet its implementation remains a challenge. This paper aims to understand the Brazilian physiotherapists' (PTs) knowledge and practices regarding early mobilization after hip fracture repair. METHODS: An online survey was conducted from September 1, 2022 to May 31, 2023 with PTs involved in postoperative care for hip fracture patients. Participants were recruited via social media and email, and answered questions assessing their know

Phase-1 Cardiac Rehabilitation in Acute Heart Failure: Development of an Early Mobilization Algorithm Through Delphi Consensus.

PURPOSE: Literature on early mobilization and exercise training in patients recovering from acute heart failure (AHF) is promising. However, there lacks uniformity in the time of initiation, exercise prescription, safety criteria, and termination criteria. Thus, the aim was to develop a mobilization algorithm for patients recovering from AHF. METHODS: A modified web-based Delphi process was undertaken involving 15 panelists from across the globe. In Round 1, new variables, modifications to suggested variables, and agreement-disagreement within the panelists were obtained. In Round 2, agreemen

Postoperative Complications and Mobilization Following Major Abdominal Surgery With Versus Without Fitness Tracker-based Feedback (EXPELLIARMUS): A Student-led Multicenter Randomized Controlled Clinical Trial of the CHIR-Net SIGMA Study Group.

OBJECTIVE: To determine whether daily postoperative step goals and feedback through a fitness tracker (FT) reduce the rate of postoperative complications after surgery. BACKGROUND: Early and enhanced postoperative mobilization has been advocated to reduce postoperative complications, but it is unknown whether FT alone can reduce morbidity. METHODS: EXPELLIARMUS was performed at 11 University Hospitals across Germany by the student-led clinical trial network SIGMA. Patients undergoing major abdominal surgery were enrolled, equipped with an FT, and randomly assigned to the experimental (visibl

[Treatment of deep vein thrombosis in the leg and pelvis].

The most important step for the treatment of deep vein thrombosis in the leg and pelvis is the immediate and sufficient anticoagulation by subcutaneous application of low-molecular-weight heparins or fondaparinux. Prevention of a relapse can be achieved by subsequent oral anticoagulation with vitamin K antagonists, whereas treatment duration is based on localisation of the thrombus and underlying risk factors. Immediate mobilisation of the patient is a fundamental step. Treatment with a compression bandage and subsequent use of compression stockings is a further effective step in reducing pain
