Surveillance for equity in maternal care in Zimbabwe.

The great hope and promise of post-independence efforts to promote equitable health care in Zimbabwe started with three years of dramatic improvement. Commitment to correcting inequities which were as discriminatory as any country in the world produced rapid extension of health centre infrastructure and the improvement of district hospitals. The major constraint was the entrenched pattern of sophisticated, high-technology health care left by colonial administrators which continued to monopolize resources. In spite of the excellent beginning, development of services for the poor was thwarted by

Identification of a recurrent nonsense mutation in HR gene responsible for atrichia with papular lesions in two Kashmiri families.

BACKGROUND: Congenital atrichia (CA) is a rare form of irreversible alopecia with an autosomal recessive mode of inheritance. This form of hair loss is mainly associated with mutations in the human hairless (HR) gene located at chromosome 8p21.3. An additional unique feature atrichia with papular lesions (APL) comprises keratin-filled cysts known as papules. The present study aimed to uncover the underlying genetic causes of APL in two consanguineous Kashmiri families. METHODS: In the present study, two consanguineous families of Kashmiri origin with APL displaying an autosomal recessive mode

A million-cow genome-wide association study of productive life in U.S. Holstein cows.

BACKGROUND: Productive life (PL) of a cow is the time the cow remains in the milking herd from first calving to exit from the herd due to culling or death and is an important economic trait in U.S. Holstein cattle. The large samples of Holstein genomic evaluation data that have become available recently provided unprecedented statistical power to identify genetic factors affecting PL in Holstein cows using the approach of genome-wide association study (GWAS). METHODS: The GWAS analysis used 1,103,641 Holstein cows with phenotypic observations on PL and genotypes of 75,282 single nucleotide po

The impact of the economic crisis and reported food insecurity in Lebanon on vitamin B12 and folate levels: a retrospective study.

BACKGROUND: Lebanon experienced a severe economic crisis, exacerbated by the COVID-19 pandemic and the Beirut port explosion in August 2020. This crisis pushed over half of the population into poverty resulting in widespread food insecurity. The latter often leads to inadequate intake of essential micronutrients such as vitamin B12 and folate. The aim of this study is to investigate and compare the influence of the economic crisis on the prevalence of these micronutrient deficiencies. METHODS: This retrospective study, conducted at Al Zahraa Hospital University Medical Center in Beirut, inves

Burden of Duchenne muscular dystrophy in Australia: a scoping review.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a rare X-linked recessive disorder characterised by progressive muscle degeneration leading to severe disability and early mortality, with no cure. This disease process affects nearly every aspect of daily functioning, from basic movements to respiratory and cardiac functions, and consequently imposes a significant burden on patients, caregivers and families. We aimed to review the available literature examining the epidemiological, health-related quality of life (HRQoL) and economic burden of DMD in Australia from a societal perspective. METHO

COVID-19, economic downturn, and long-term trajectories of population mental health: Evidence from two nationally representative British birth cohorts at the intersection of gender and socioeconomic position.

BACKGROUND: We examined long-term trajectories of mental (ill-)health in two British generations ('Baby Boomers' and 'Generation X') across the life-course, including the COVID-19 lockdowns and the following cost-of-living increases. We analysed inequalities by generation, gender, socioeconomic position (SEP), and their intersections, and explored the relationship between inflation and mental (ill-)health post-lockdown. METHODS AND FINDINGS: We used data from the National Child Development Study (NCDS/1958, n = 8215) and the 1970 British Cohort Study (BCS/1970, n = 7789), with repeated measur

Common Mental Disorders and Economic Uncertainty: Evidence from the COVID-19 Pandemic in the U.S.

Mental health disorders represent an enormous cost to society, are related to economic outcomes, and have increased markedly since the COVID-19 outbreak. Economic activity contracted dramatically on a global scale in 2020, representing the worst crisis since the Great Depression. This study used the COVID Impact Survey to provide insights on the interactions of mental illness and economic uncertainty during COVID-19. We used a probability-based panel survey, COVID Impact Survey, conducted in the U.S. over three waves in the period April-June 2020. The survey covered individual information on e

Economic conditions, illicit drug use, and substance use disorders in the United States.

We provide the first analysis of the relationship between economic conditions and the use of illicit drugs other than marijuana. Drawing on US data from 2002 to 2015, we find mixed evidence on the cyclicality of illicit drug use. However, we find robust evidence that economic downturns lead to increases in the intensity of prescription pain reliever use as well as increases in clinically relevant substance use disorders involving opioids. These effects are concentrated among working-age white males with low educational attainment. We conclude that policymakers should consider devoting more, no

Socioeconomic status and cardiovascular health in the COVID-19 pandemic.

The goals of this review are to evaluate the impact of socioeconomic (SE) status on the general health and cardiovascular health of individuals during the COVID-19 pandemic and also discuss the measures to address disparity. SE status is a strong predictor of premature morbidity and mortality within general health. A lower SE status also has implications of increased cardiovascular disease (CVD) mortality and poorer CVD risk factor profiles. CVD comorbidity is associated with a higher case severity and mortality rate from COVID-19, with both CVD and COVID-19 sharing important risk factors. The

Individuals experiencing chronic homelessness: A 10-year follow-up of a cohort in Spain.

With the start of the great economic recession in 2007, homelessness increased fivefold in some regions of southern Europe. Larger numbers of people experiencing homelessness, compounded by a lowered capacity for social and health services to respond to their needs, precipitated an increase in so-called 'chronic homelessness'. The aim of this study was to establish the presence of chronic homelessness in a defined geographical area of Spain, and to determine the prevalence of diagnosed mental disorders within both the chronic and non-chronic homeless population. A prospective and descriptive s

Coronavirus disease 2019 (COVID-19) and the renin-angiotensin system: A closer look at angiotensin-converting enzyme 2 (ACE2).

Since the first cases of atypical pneumonia linked to the Huanan Seafood Wholesale Market in Wuhan, China, were described in late December 2019, the global landscape has changed radically. In March 2020, the World Health Organization declared COVID-19 a global pandemic, and at the time of writing this review, just over three million individuals have been infected with more than 200,000 deaths globally. Numerous countries are in 'lockdown', social distancing is the new norm, even the most advanced healthcare systems are under pressure, and a global economic recession seems inevitable. A novel c

Gene therapy advances using canine and feline animal models of inherited retinal degeneration.

Inherited retinal degenerations (IRDs) are a genetically heterogenous group of visually impairing conditions that affect many people worldwide. They are caused by mutations in a variety of genes with a range of vision loss onset from childhood to middle-age. Many IRDs are inherited in an autosomal recessive fashion and are due to loss of function of the gene product, allowing for a standard gene augmentation approach in which a normal copy of the mutated gene is introduced. Retinal gene delivery using adeno-associated viral (AAV) vectors has proven to be the safest and most effective approach

Homozygous variants in the HEXB and MBOAT7 genes underlie neurological diseases in consanguineous families.

BACKGROUND: Neurological disorders are a common cause of morbidity and mortality within Pakistani populations. It is one of the most important challenges in healthcare, with significant life-long socio-economic burden. METHODS: We investigated the cause of disease in three Pakistani families in individuals with unexplained autosomal recessive neurological conditions, using both genome-wide SNP mapping and whole exome sequencing (WES) of affected individuals. RESULTS: We identified a homozygous splice site variant (NM_000521:c.445 + 1G > T) in the hexosaminidase B (HEXB) gene confirming a dia
