Acute pulmonary edema in pregnancy.

OBJECTIVE: To describe the incidence, predisposing conditions, and inciting factors culminating in pulmonary edema in the pregnant patient. METHODS: A review of 62,917 consecutive pregnancies delivered at our institution from January 1, 1989 to June 1, 1999 was undertaken for the diagnosis of pulmonary edema. Each chart was reviewed for maternal demographics, admission diagnoses, medication use, gestational age at diagnosis, fluid balance, coexisting maternal illness, tocolytic use, evidence of preeclampsia, and diagnostic criteria. After careful review of the records, the most likely cause o

Update: High altitude pulmonary edema.

Recent high altitude studies with pulmonary artery (PA) catheterization and broncho-alveolar lavage (BAL) in early high altitude pulmonary edema(HAPE) have increased our understanding of the pathogenetic sequence in HAPE. High preceding PA and pulmonary capillary pressures lead to a non-inflammatory leak of the alveolar-capillary barrier with egress of red cells, plasma proteins and fluid into the alveolar space. The mechanisms accounting for an increased capillary pressure remain speculative. The concept that hypoxic pulmonary vasoconstriction (HPV) is uneven so that regions with less vasocon

[Treatment of acute brain edema].

OBJECTIVE: To review the basic concepts and mechanisms of cerebral edema and to analyze current management of this complex clinical problem. DEVELOPMENT: Cerebral edema is a stereotyped reaction of the brain to any aggression and is a very common complication in patients with alterations of the blood-brain barrier. The cerebral edema should be recognized and treated early and aggressively to avoid disastrous results such as cerebral herniation. The most important types of edema are the vasogenic and cytotoxic. The first implies primary alterations of the blood-brain barrier, whilst in the sec

Vitrectomy for traction macular edema.

PURPOSE: Traction macular edema may develop through contraction of macular epiretinal membranes (ERM), or due to persistent vitreomacular traction during the evolution of vitreomacular traction syndrome (VMS). The purpose of this retrospective study was to determine the effect of vitreous surgery and the release of the vitreomacular traction or the removal of epiretinal membranes, on the evolution of traction induced macular edema. MATERIAL AND METHODS: Fourteen eyes from 14 patients presenting with idiopathic or secondary epiretinal membranes, and 11 eyes from 10 patients presenting with vit

Latanoprost-associated cystoid macular edema.

PURPOSE: To report two cases in which cystoid macular edema developed after initiation of topical latanoprost for glaucoma. METHODS: Case reports. One pseudophakic eye in each of two patients treated with latanoprost for glaucoma developed decreased vision and cystoid macular edema. Latanoprost was discontinued, and the cystoid macular edema was treated with topical corticosteroids and ketorolac. RESULTS: After discontinuing latanoprost and starting corticosteroids and ketorolac, visual acuity improved from counting fingers to 20/60 in one patient's left eye and from 20/100 to 20/25 in the o

[Chronic brain edema in neurocysticercosis].

In a retrospective study, we report the clinical characteristics of chronic brain oedema (CBO) in 34 patients with neurocysticercosis (NCC) who presented diffuse brain oedema upon computed tomography (CT) as a common point. All patients received dextrochlorpheniramine, and, 94.1% of them also albendazole. A predominance of females (73.5%) aged 11-40 years (92.3%) was seen. Headache occurred in 94.1% of patients, nausea/vomiting in 47.1%, epileptic manifestations in 41.1% and psychiatric disorders in 38.2%. Hyperreflexia was obtained in 82.3% of patients, papilloedema in 58.8% and normal neurol

Pulmonary edema of scuba divers.

A syndrome of acute pulmonary edema has been previously reported among scuba divers in cold, European waters. Because of the temperatures involved, the name "cold-induced pulmonary edema" was coined in the original 1989 description. We report six individuals who developed the identical syndrome, five while diving in Puget Sound and one in the Gulf of Mexico. The four women and two men ranged in age from 24 to 60 yr. They experienced one to six episodes apiece, each with the development severe dyspnea at depth without excessive exertion. Associated symptoms included cough, weakness, expectorati

Disk edema and peripheral neuropathy.

A 55-year-old woman presented with an eight-year history of progressive gait dysfunction and a 2-3 week history of horizontal diplopia. Examination of the patient revealed acute papilledema, intermittent esotropia, and upper and lower motor neuron findings in her lower extremities. CT scan of the head showed communicating hydrocephalus. A spinal cord tumor extending from T11 to L3 was visualized by MR imaging of the spine. All of her symptoms, except for a left foot drop, resolved with aggressive removal of the tumor, which was a myxopapillary ependymoma. Mechanisms of the neuro-ophthalmic pic

[Prevention of postoperative scrotal edema].

The authors describe the technique of scrotal elevation and compression for the prevention of postoperative scrotal hematoma. This technique was used in 30 patients undergoing hydrocelectomy, spermatocelectomy or epididymectomy. There have been no untoward effects associated with this technique and this technique is applicable to a wide variety of scrotal procedures. The method described is simple, rapid to perform, comfortable for the patients and most effective to prevent postoperative complications in patients undergoing a scrotal operation.

Phosphatic metabolism and corneal edema.

From three groups of patients with corneal edema, the degree of edema was clinically quantified prior to penetrating keratoplasty. Corneas from patients with failed grafts (n = 16) had the greatest edema (mean score 3.8 +/- 0.6 SE), corneas with Fuchs' endothelial dystrophy (n = 12) had the least edema (1.4 +/- 0.4), and corneas with aphakic and pseudophakic bullous keratopathy (n = 40) had intermediate edema (2.7 +/- 0.5). Phosphorus-31 nuclear magnetic resonance spectroscopy was used to quantitate five high-energy phosphates and nine low-energy phosphates and calculate 12 metabolic indices f

[Hereditary angioneurotic edema and pregnancy].

Hereditary angioneurotic oedema (HANE) is an autosomal dominant disorder, relatively uncommon, in which patients experience recurrent bouts of oedema of the extremities, face and larynx. Attacks of HANE, sometimes fatal when involving the larynx may, among other causes, be triggered by the anesthetist's manipulation of the upper airway (tracheal intubation). HANE may affect women during their reproductive years and present serious problems in case of pregnancy. Labour can precipitate laryngeal oedema in case of general anesthesia with tracheal intubation indicated for a caesarean section or a

[Prevention of decubitus, edema, contractures].

This paper presents a historical reminder of the development of medical care, a contemporary consideration of the division of care, as well as the definition of the term-quality medical care, and a review of the preventive measures for bed-sore, edema and contracture. In regard to the fact that care and therapy are a tightly bound process, they must also be applied as such. The definition of the term-quality medical care is an open question. The prevention of bed-sore, edema and contracture is the element of quality care.

Syphilitic disc edema and periphlebitis.

This photo essay shows the retinal findings in a patient with presumed syphilitic disc edema and periphlebitis. Resolution of these findings after treatment with penicillin is documented by improvement in visual acuity from 20/50 to 20/25 in the right eye and from 20/200 to 2/20 in the left eye. Photographic documentation of the resolution of retinal findings, including disc edema and periphlebitis, is presented.

Cerebral edema associated with meningioma.

Cerebral edema associated with meningiomas is a rather common phenomenon. A patient with a small meningioma presented with severe cerebral edema, out of proportion to the size of the tumor. During surgery the meningioma was removed and the adjacent edematous brain tissue biopsied. The histopathological study showed conspicuous plasma cell and lymphocyte infiltration in both the tumor and the cerebral edematous tissue. The pathological findings are presented, and the possible mechanism for cerebral edema associated with meningioma is discussed.

Pulmonary edema in cocaine smokers.

Cocaine smoking can cause a number of medical complications. Pneumomediastinum and pneumothorax due to barotrauma have been the most common radiographic abnormalities reported in the medical literature. The hospital records of five patients with pulmonary edema who smoked cocaine just before admission were reviewed. Except for cocaine abuse no other possible cause for the pulmonary edema was found. Although chest radiographic abnormalities in these patients are uncommon, these cases are reported to inform the radiologist of this possible complication of cocaine smoking. The presence of pulmona

[Pheochromocytoma revealed by pulmonary edema].

We report the case of a young patient of 37 presenting with two attacks of pulmonary oedema which revealed an underlying pheochromocytoma. The diagnosis was suggested by the raised level of vanyl-mandelic acid in the urine and the abdominal CT scan. After an adrenalectomy the outcome was satisfactory. Pheochromocytoma should be considered in the differential diagnosis of the acute respiratory distress syndrome in the adult linked to the action of the circulating catecholamines on the pulmonary capillaries.

[Pulmonary edema following airway obstruction].

This case report describes an adult patient who developed post extubation laryngeal spasm and later pulmonary edema following relief of the laryngeal spasm. The mechanism that causes pulmonary edema following upper airway obstruction is very probably due to a modified Müller maneuver (inspiration against a closed glottis), which will increase negative intrapleural pressure, and thereby cause increased pulmonary capillary permeability.
