Sialochemistry of patients with rheumatoid arthritis. Electrolytes, protein, and salivary IgA.

Sialochemistry was undertaken to detect subclinical involvement of salivary glands in patients with rheumatoid arthritis (RA). The concentrations of salivary sodium and IgA were significantly elevated in 24 percent of the RA patients. Eighty per cent of the patients with elevated sodium and IgA concentrations had keratoconjunctivitis sicca as well. No correlation between serum and salivary IgA levels was found. The results indicate that salivary sodium and IgA levels can be used for detection of subclinical salivary gland involvement and for follow-up of RA patients.

[Persistent electrolyte deficiency in an oncology patient].

We report the case of a 47-year-old patient with a dedifferentiated mediastinal-thoracic liposarcoma who developed electrolyte disturbances during chemotherapy with Ifosfamide and Doxorubicin. Initially, a refeeding syndrome was suspected, supported by hypokalemia, hypomagnesemia, and hypophosphatemia. However, the prolonged course of electrolyte abnormalities, along with the presence of normoglycemic glucosuria and proteinuria, were not consistent with this diagnosis. Over time, a partial Fanconi syndrome was confirmed, reflecting a proximal tubular dysfunction - a well-known complication of

[Lithium-induced electrolyte disturbances and catatonia].

In this case report, a 57-year-old man with bipolar disorder on long-term lithium treatment was admitted with catatonia. Laboratory tests revealed hypercalcemia with parathyroid hormone levels in the upper third of the normal reference interval, consistent with lithium-associated hyperparathyroidism, as well as nephrogenic diabetes insipidus. Pausing lithium and treating hypercalcemia led to a gradual improvement in catatonia. The case emphasises the importance of considering endocrine and electrolyte disturbances in cases of acute deterioration of psychiatric symptoms during lithium treatment

An electrolyte disturbance masquerading as a panic attack.

Because proton pump inhibitors (PPIs) are so commonly used, their safety and potential adverse reactions often are not considered. This article describes a patient whose paresthesias and severe muscle cramps were attributed to panic attacks but actually were caused by hypocalcemia secondary to PPI-induced hypomagnesemia. This case serves as a reminder that PPIs should be used cautiously and appropriately with consideration for regular monitoring of electrolytes and vitamin B12.

Oral Electrolyte and Water Supplementation in Horses.

Horses that sweat for prolonged periods lose considerable amounts of water and electrolytes. Maintenance of hydration and prevention of dehydration requires that water and electrolytes are replaced. Dehydration is common in equine disciplines and can be avoided, thus promoting equine wellness, improved performance and enhanced horse and rider safety. Significant dehydration occurs through exercise or transport lasting one hour or more. Oral electrolyte supplementation is an effective strategy to replace water and electrolytes lost through sweating. The stomach and small intestine serve as a re

FAM111A is dispensable for electrolyte homeostasis in mice.

Autosomal dominant mutations in FAM111A are causative for Kenny-Caffey syndrome type 2. Patients with Kenny-Caffey syndrome suffer from severe growth retardation, skeletal dysplasia, hypoparathyroidism, hypocalcaemia, hyperphosphataemia and hypomagnesaemia. While recent studies have reported FAM111A to function in antiviral response and DNA replication, its role in regulating electrolyte homeostasis remains unknown. In this study, we assessed the role of FAM111A in the regulation of serum electrolyte balance using a Fam111a knockout (Fam111a-/-) C57BL/6 N mouse model. Fam111a-/- mice displayed

A girl with lethargy and severe electrolyte imbalance.

BACKGROUND: Autoimmune polyendocrine syndrome type 1 (APS-1) is a rare autosomal recessive disorder characterised by a triad of mucocutaneous candidiasis, hypoparathyroidism and primary adrenal failure. It is caused by a mutation in the autoimmune regulator gene AIRE, involved in negative selection of self-reacting T-lymphocytes. CASE PRESENTATION: A young girl had been lethargic for 10 weeks following an upper airway infection. Attempts to perform blood sampling were interrupted due to severe anxiety. The patient was admitted to hospital after transient dizziness, aphasia, and paresthesia of

Magnesium: The overlooked electrolyte in blood cancers?

Magnesium is an important element that has essential roles in the regulation of cell growth, division, and differentiation. Mounting evidence in the literature suggests an association between hypomagnesemia and all-cause mortality. In addition, epidemiologic studies have demonstrated that a diet poor in magnesium increases the risk of developing cancer, highlighting its importance in the field of hematology and oncology. In solid malignancies, hypomagnesemia at diagnosis portends a worse prognosis. However, little is known about prognosis in patients with hypomagnesemia and blood cancers in ge

Neurologic manifestations of major electrolyte abnormalities.

The brain operates in an extraordinarily intricate environment which demands precise regulation of electrolytes. Tight control over their concentrations and gradients across cellular compartments is essential and when these relationships are disturbed neurologic manifestations may develop. Perturbations of sodium are the electrolyte disturbances that most often lead to neurologic manifestations. Alterations in extracellular fluid sodium concentrations produce water shifts that lead to brain swelling or shrinkage. If marked or rapid they can result in profound changes in brain function which ar

Perioperative Acid-Base and Electrolyte Disturbances.

Obtaining and interpreting blood gas and electrolyte levels is essential in the management of perioperative veterinary patients. Metabolic and electrolyte alterations are common in critically ill surgical patients, and can lead to alterations in cardiovascular function, neurologic status, respiratory function, and even response to various drug therapies. Several common perioperative conditions are discussed in this article, including metabolic disturbances, electrolyte abnormalities (hyponatremia and hypernatremia, hyperkalemia), and respiratory abnormalities.

[Electrolyte disorders as a hallmark of monogenetic diseases].

In daily clinical practice, the term electrolyte generally refers to sodium, potassium, chloride, calcium, and magnesium ions. In addition to their many functions, such as neuronal and muscular transmission, some electrolytes also contribute to osmolality and maintenance of electrochemical gradients, which, in turn enable many transport processes. The absorption and reabsorption of electrolytes occurs via polarized cell assemblies, i.e., epithelia. Besides the intestine (absorption), the most important organ is the kidney. Here, following glomerular filtration, electrolytes are reabsorbed via

[Electrolyte disorders in preeclampsia. A case report].

The occurrence of electrolyte disorders as hypocalcemia and/or hyponatremia is an uncommon event in preeclampsia, which can be the sign of serious situation, with potentially unfavourable consequences for the mother and her fœtus. Hyponatremia in the setting of preeclampsia is an indicator of severity, and requires the understanding of the etiologic mechanisms to initiate an appropriate treatment. Indeed the often-considered fluid restriction is rarely a treatment option for pregnant women. Hypocalcemia is a complication that must be monitored when a treatment with high doses of intravenous ma

A case of myotonic dystrophy with electrolyte imbalance.

Type 1 myotonic dystrophy (DM1) is an autosomal-dominant inherited disorder with a multisystem involvement, caused by an abnormal expansion of the CTG sequence of the dystrophic myotonia protein kinase (DMPK) gene. DM1 is a variable multisystem disorder with muscular and nonmuscular abnormalities. Increasingly, endocrine abnormalities, such as gonadal, pancreatic, and adrenal dysfunction are being reported. But, Electrolytes imbalance is a very rare condition in patients with DM1 yet. Herein we present a 42-yr-old Korean male of DM1 with abnormally elevated serum sodium and potassium. The pati
