Targeting endocarditis-associated Streptococcus biofilms: The antibacterial potential of Lippia sidoides essential oil.

OBJECTIVES: Streptococcus species such as Streptococcus sanguinis and Streptococcus gordonii are key contributors to infective endocarditis due to their ability to form resilient biofilms. This study evaluated the antibacterial and antibiofilm potential of Lippia sidoides essential oil (LSEO) against clinical isolates obtained from blood and the oral cavity. DESIGN: Sixteen clinical isolates of S. sanguinis and S. gordonii, comprising eight oral isolates and eight blood isolates obtained from patients with infective endocarditis, were evaluated. Antimicrobial activity was determined using min

Contemporary diagnostic strategies for blood culture-negative infective endocarditis: from conventional microbiology to emerging molecular and multimodality approaches.

BACKGROUND: Blood culture-negative infective endocarditis (BCNE) remains one of the most challenging presentations of infective endocarditis (IE) because the absence of microbiological confirmation delays diagnosis and complicates therapeutic management. BCNE may result from prior antibiotic exposure, infection with fastidious or intracellular microorganisms, fungal pathogens, or non-infectious conditions mimicking IE. METHODS: This narrative review provides a clinically oriented overview of contemporary diagnostic strategies for BCNE based on current evidence and recent international guideli

Ten years of infective endocarditis: a clinicopathologic study.

The records of 46 patients with infective endocarditis diagnosed either clinically or post-mortem were analyzed. Twenty-six patients were over 60 years of age. S. aureus was the predominant organism, almost exclusively found in patients with acute endocarditis. Thirty-six patients had pre-existing heart disease, the most common being non-rheumatic valvular calcification and congenital defects. Two thirds of the patients, especially those with aortic valve regurgitation, developed new or progressive heart failure. A correct clinical diagnosis was established in only 30 patients. Twenty-three pa

The Dilemma of Surgical Timing in Acute Aortic Valve Endocarditis: Does Early Surgery Improve Risks or Prognosis?

BACKGROUND: Acute aortic valve infective endocarditis (IE) presents a critical surgical timing dilemma. This study investigates whether early surgical intervention (within seven days of targeted antibiotic therapy initiation) affects mortality and clinical outcomes compared to delayed/conventional surgery. METHODS: A retrospective, multicenter analysis of 204 patients with aortic IE was conducted, excluding emergency cases requiring immediate intervention. Patients were stratified into EARLY (≤7 days) and LATE (>7 days) surgical groups. Primary endpoints included in-hospital mortality and maj

Endocarditis, drug use and biological sex: A statewide analysis comparing sex differences in drug use-associated infective endocarditis with other drug-related harms.

OBJECTIVES: Hospitalizations for drug use-associated infective endocarditis (DUA-IE) have risen sharply across the United States over the past decade. The sex composition of DUA-IE remains less clear, and studies have indicated a possible shift to more females. We aimed to compare more recent statewide hospitalization rates for DUA-IE in females versus males and contextualize them among other drug-related harms in North Carolina (NC). METHODS: This study was a retrospective analysis using public health datasets of all NC hospital discharges for infective endocarditis from 2016 to 2020. Drug u

[Aneurysm of the aortic cusp; report of a case].

We performed surgical therapy to the rare case of the perforation of an aneurysm of the aortic cusp which had caused the congestive heart failure. 71-year-old man having developed coughing was diagnosed as a severe aortic regurgitation and came to our hospital for surgery. The non coronary cusp was dilated and perforated. The aortic valves were removed and replaced with a bioprosthetic valve. The pathological examination of the valves showed healed infective endocarditis. The patient discharged home uneventfully.

[Call for attention to the role of nuclear molecular imaging in the diagnosis of inflammation in cardiovascular disease].

Inflammation is a key pathological feature and significant pathogenic factor in the development and progression of cardiovascular diseases (CVD). Nuclear molecular imaging enables non-invasive visualization and quantification of CVD related inflammation, reflecting its pathological evolution. Nuclear molecular imaging plays a crucial role and holds immense potential in the precise diagnosis of inflammation in CVD. This review highlights the clinical applications and future prospects of nuclear inflammation imaging in vulnerable atherosclerotic plaques, acute myocardial infarction, infective en

An unusual presentation of infective endocarditis.

This paper reports a case of a 22 years old young male who presented in emergency department with some non-specific symptoms such as abdominal pain, vomiting, chest heaviness and shortness of breath. Chest x-ray revealed a combined picture of pneumonia and congestive heart failure. Echocardiogram showed ventricular septal defect, aortic regurgitation and mitral stenosis. Later on diagnosed with infective endocarditis and the culprit was his native valves. He went through aortic valve replacement and discharged after full recovery.

[Surgical treatment for non-ischemic mitral regurgitation].

Non-ischemic mitral regurgitation( MR) is classified in degenerative MR, infective endocarditis( IE) and rheumatic MR. For degenerative MR, although 100% achievement of successful mitral valve plasty (MVP), it still important to evaluate mitral morphology preoperatively and to select suitable procedure.MR due to IE, the most important thing is to exclude vegetation completely. Then MVP is considered depends on its area of defect. Rheumatic MR is still difficult to accomplish MVP. Mitral valve replacement(MVR) is a main strategy for rheumatic MR.

Non-bacterial thrombotic endocarditis and metastatic lung adenocarcinoma.

A 69-year-old Chinese man presented with exertional dyspnoea and subjective left upper limb weakness. Initial clinical impressions were community-acquired pneumonia and cerebrovascular accident. Further imaging studies revealed an incidental mitral valve vegetation and left lung upper lobe nodule likely a lung malignancy with possible lymphangitis carcinomatosis. Appropriate empirical antibiotic treatment for infective endocarditis was instituted early in admission, but multiple blood cultures were negative. The patient progressively developed worsening neurological dysfunction and subconjunct

Aspergillus Endocarditis in Native Valves in Non-Traditional Hosts: A Systematic Review of a Case in a Patient with CREST Syndrome and Advanced Liver Cirrhosis.

Background: Aspergillus endocarditis is a rare but life-threatening form of infective endocarditis that typically occurs in patients with a history of cardiac surgery, prosthetic valve implantation, or profound immunosuppression. Native valve involvement in non-traditional hosts remains exceptionally rare and is diagnostically challenging. Case presentation: We describe a 56-year-old woman with CREST syndrome and advanced liver disease awaiting transplantation who developed native aortic valve endocarditis. Blood cultures and serum biomarkers (galactomannan and β-d-glucan) were also negative.

Non-bacterial thrombotic endocarditis in a patient with COVID.

Non-bacterial thrombotic endocarditis is mainly associated with malignancies and rheumatological diseases. We report the case of mildly symptomatic COVID-19 infection with non-bacterial aortic valve vegetation complicated by transient ischemic attack (TIA) and pulmonary embolism during his hospitalisation. This case emphasised rare life-threatening complications from a hypercoagulable state related to COVID-19 infection. To the best of our knowledge, this is the third case report of non-bacterial endocarditis in a patient with COVID-19 patients as a potential rare complication of COVID-19.

Listeriosis: A Rare Cause of Cardiac Implantable Electronic Device Infection.

Listeriosis is a series of diseases caused by the bacteria Listeria monocytogenes. Typically presenting with mild symptoms of diarrhea, fever, and myalgias, it can, in some cases, assume an invasive form with systemic involvement. Cardiac involvement in listeriosis is rare, with very few reports describing the involvement of cardiac implantable electronic devices (CIED), which hampers the development of conclusive recommendations regarding CIED management or antibiotic courses. We present the case of an 82-year-old woman who had a pacemaker and developed gastroenteritis complicated by bacterem

Acquisition of FKS2 mutation after echinocandin treatment of infective endocarditis by Candida glabrata.

Bloodstream infections caused by non-albicans Candida species are increasing and echinocandins have been extensively used especially in patients with hemodynamic instability, previous antifungal treatment and hospital risk factors for intrinsic or acquired resistance to azoles. Candida glabrata resistance to echinocandins is reported and is generally associated with previous use of echinocandins; FKS gene mutations have been associated with a worse outcome. We report the case of a 65-year-old woman who developed candidemia and endocarditis by C. glabrata with a newly acquired FKS mutation 24 m

Are we missing anaerobic infective endocarditis in some acute coronary syndromes?

A 76-year-old man presented with a 3-week history of intermittent fevers and dyspnoea on exertion after a dental bridge placement 2 months ago. The patient's medical history was significant for mild to moderate mitral valve prolapse. Initial evaluation was notable for a 3/6 systolic apical murmur. Laboratory investigations revealed leucocytosis and elevated erythrocyte sedimentation rate, C reactive protein and cardiac biomarkers. Patient was treated initially for non-ST elevation myocardial infarction. A 2-dimensional echocardiography was concerning for a new mitral regurgitation and a questi

Atypical mitral valve infective endocarditis presenting with fever and left upper abdominal pain due to splenic infarction: a case report.

Infective endocarditis (IE) may present with non-specific systemic symptoms and embolic complications, making early diagnosis challenging, particularly when initial transthoracic echocardiography (TTE) is non-diagnostic. We report a case of atypical mitral valve IE in a 40-year-old man who presented with persistent fever and left upper abdominal pain rather than overt cardiac symptoms. On admission, inflammatory markers were elevated, whereas emergency chest and abdominal computed tomography showed no definite acute infectious focus. Initial TTE demonstrated only mild mitral and tricuspid regu

Streptococcus bovis endocarditis secondary to colorectal cancer: A case report.

BACKGROUND: Streptococcus bovis (S. bovis) bacteremia and infective endocarditis have a well-established association with colorectal cancer (CRC), though the mechanisms underlying this potentially bidirectional relationship remain poorly understood. CASE SUMMARY: This case report describes a 55-year-old male with a history of hypertension and hemicolectomy due to advanced colorectal adenomas who presented with syncope, septic shock, and a 12-pound weight loss over 3 weeks. Subsequent blood cultures grew S. bovis, with a transthoracic echocardiogram revealed mobile vegetations on both the aort

Convexal Subarachnoid Hemorrhage Caused by Infective Endocarditis in a Patient with Advanced Human Immunodeficiency Virus (HIV): The Culprits and Bystanders.

BACKGROUND Convexal subarachnoid hemorrhage (cSAH), a rare form of non-aneurysmal subarachnoid hemorrhage, is confined to cerebral convexities without extension into basal cisterns or ventricles. Typical presentation includes thunderclap/progressive headache or transient focal neurological symptoms; rare manifestations include seizures, intractable vomiting, or altered mental status. Here, we report the first case of convexal subarachnoid hemorrhage and multifocal ischemic lesions caused by infective endocarditis (IE) in a treatment-naïve advanced HIV patient. Patient characteristics, treatment patterns, and prognosis in drug-use-associated infective endocarditis in Denmark from 1999 to 2018.

BACKGROUND: While the proportion of drug-use-associated infective endocarditis (DU-IE) has been increasing during the opioid crisis in the United States, it is unknown whether this is seen in Denmark, where several preventive means have been implemented. We aimed to assess the temporal proportion of DU-IE and examine the rate of IE recurrence and mortality. METHODS: This nationwide cohort study identified all patients with first-time infective endocarditis in 1999-2018. Drug use was defined using ICD-8/10 codes or prescription filling of medication for opioid use disorder. Long-term mortality

Ruptured sinus of Valsalva aneurysm due to Infective Endocarditis managed by device closure - a case report.

Ruptured sinus of Valsalva aneurysm (RSOV) is an uncommon cause of high output heart failure. RSOV most commonly opens into the right ventricle followed by the right atrium and non-coronary cusp involvement is relatively uncommon. Infective endocarditis (IE) is a rare cause of RSOV. We report an interesting clinical scenario of IE causing RSOV managed by device closure. A 16-year-old male patient presented to the emergency department with acute chest pain, fever, and engorged neck veins. On cardiorespiratory system examination he had features of left ventricular failure. Blood culture revealed
