Regenerative endodontics aims not only to control infection but also to restore a functional pulp-dentin complex. Achieving this predictably remains challenging because disinfection, inflammation, blood vessel formation, matrix remodeling, and stem cell behavior are closely connected. Polymeric nanoparticles are increasingly being explored as a way to influence this complex environment because they can deliver therapeutic agents in a controlled and, in some cases, responsive manner. This review examines how the pulp microenvironment affects regenerative outcomes and discusses nanoscale strateg

Persistent periapical lesions challenge clinicians between periapical surgery and extraction-implant, yet long-term comparative outcomes remain limited. Hence, this retrospective cohort followed 166 patients (78 per group) for ≥24 months, comparing success rates, clinical/ radiographic outcomes, satisfaction, cost and duration after failed endodontics. Periapical surgery achieved 89.7% success, compared with 92.3% for implants (p=0.571), with comparable survival, satisfaction and periapical healing; surgery excelled in shorter duration with lower cost. Both modalities yield reliable long-term

BACKGROUND: Endodontic prognosis is based on clinical and radiographic factors that estimate the likelihood of treatment success. Because periapical healing reflects complex interactions among microbial, host, anatomical, and treatment-related factors, individual outcomes may occasionally differ from conventional prognostic expectations. This paper aimed to describe and critically analyze favorable outcomes in teeth presenting conditions traditionally associated with reduced probabilities of treatment success.
CASE PRESENTATION: Three clinical cases were described in accordance with the Prefe

DESIGN: The study is a prospective, double-blinded randomised control trial that compares the mineral trioxide aggregate (MTA) and Biodentine as the pulp space barrier material after induction of a periapical blood clot by over-instrumentation for endodontic regeneration in single-rooted mature permanent anterior tooth (closed apex) with apical periodontitis (periapical pathology of more than 3 mm) and having necrosed pulp. A total of 36 patients were included in the study and after randomisation were allocated equally to both the groups out of which 31 patients returned for follow-up over an

OBJECTIVES: To (1) validate the use of a post-operative intraoral scan (IOS) versus Cone Beam Computed Tomography (CBCT), gold standard, on its ability to measure the accuracy of guided endodontics, and (2) present clinical data on the accuracy of guided endodontics.
METHODS: Four models, including 10 extracted teeth each, were created. Forty guided access cavities were planned on dentin to simulate pulp canal obliteration (PCO). Two operators performed guided access cavities. A post-operative CBCT and IOS were acquired. The coronal, apical, and angular deviations were measured with CBCT and

AIM: This study investigated how Colombian dentists with different academic levels indicate antibiotics with therapeutic purposes in endodontics.
MATERIALS AND METHOD: A cross-sectional survey was conducted among 559 dentists in the form of an online questionnaire.
RESULTS: Three hundred and twenty questionnaires were answered (57.2%). There were significant differences among respondents. For irreversible pulpitis, 140 dentists (43.7%) said they prescribe antibiotics (57.5% of general practitioners, 20.1% of specialists and 38.9% of those with Master's and/or PhD degrees), while for symptoma

Matrix metalloproteinases (MMPs) are a group of over 25 secreted and membrane-bound enzymes responsible for pericellular substrate degeneration. In response to injury, they play key roles in morphogenesis, wound healing, tissue repair and remodeling. They have been isolated from dentin, odontoblasts, pulp and periapical tissue. They play a major role in the formation of dentin matrix and secondary and tertiary dentin. These are also responsible for releasing dentinal growth factors. MMP family proteins elicit a dual role in the pathogenesis of inflammation, stimulating protective innate and/or

As the root canal system shows different and complicated anatomies, mechanical instrumentation alone has not the ability to provide a bacteria-free environment in root canals. On the other aspect, necrotic tissue remaining can decrease the effects of root canal irrigants and medicaments and also interfere with the adaptation of root canal fillings to dentin. As a result, certain disinfection and irrigation procedures are required to remove the remaining tissues from the root canal area thoroughly and also be able to eliminate the microorganisms. Triple antibiotic paste (TAP) containing metroni

Endodontic inflammatory diseases (EIDs), such as pulpitis and apical periodontitis (AP), are common chronic oral inflammatory conditions, which are most commonly caused by untreated dental caries. Over the past two decades, several studies have reported positive bidirectional associations between AP and various systemic conditions, including cardiovascular disease (CVD), diabetes, hepatic disease, inflammatory bowel disease (IBD), hematological disease, and adverse pregnancy outcomes (APOs). These associations indicate that chronic oral inflammation may contribute to systemic low-grade inflamm

Primary dental treatments for odontogenic sinusitis (ODS) due to endodontic infections (pulpal necrosis and apical periodontitis ± periapical abscess) include extraction and root canal treatment (RCT). Published evidence is lacking on the success of primary endodontic treatment for purulent ODS, with the majority of RCT-related series reporting on its success at resolving reactive maxillary sinus mucositis. Dental extraction is the most definitive treatment of endodontic disease causing ODS, but compromises the functional dentition and still often fails to resolve the purulent sinusitis. This

Extrusion of root filling material had been shown to reduce the success of endodontic treatment. This case report describes the management of a patient who reported prolonged, persistent, and increasing pain on an upper root filled central incisor with extruded root filling material. A 28-year-old female patient came with the chief complaint of pain and tenderness on the upper left central incisor. The pain was mostly triggered by mastication. Upon examination and investigation, the tooth of concern was tooth 21 which was a root treated many years ago. It appeared to have tenderness on percuss

Endodontic therapy is intended to preserve the function of mature teeth with irreversible pulpitis or pulp necrosis or to maintain the vitality of endodontically compromised immature teeth. Standard root canal therapy and vital pulp therapy are 2 mainstays of endodontic treatment. Recent knowledge has improved the outcomes of endodontic treatment with newer materials, such as mineral trioxide aggregate. Composite or prosthodontic crown restoration is also a critical key to success.

In 2009 a new type of endodontic sealers was introduced to the market. The so called "bioceramic" sealers are a promising alternative to the present golden standard of root canal fillings. Now a decade later, still very little is known about the ability to remove these sealers in cases of non-surgical endodontic retreatment (NSER). There are only a limited number of articles that provide such information. The commonly used hand files are not efficient in removing thoroughly the sealer from the main canal walls. Machine driven files are much easier and faster alternative to the hand ones, but a

INTRODUCTION: The objective of this study was to evaluate the antimicrobial effects of photodynamic therapy (PDT) on infected human teeth ex vivo.
METHODS: Fifty-two freshly extracted teeth with pulpal necrosis and associated periradicular radiolucencies were obtained from 34 subjects. Twenty-six teeth with 49 canals received chemomechanical debridement (CMD) with 6% NaOCl, and 26 teeth with 52 canals received CMD plus PDT. For PDT, root canal systems were incubated with methylene blue (MB) at concentration of 50 μg/mL for 5 minutes, followed by exposure to red light at 665 nm with an energy

Endodontic infections are polymicrobial infections resulting in bone destruction and tooth loss. The host response to these infections is complex, including both innate and adaptive mechanisms. Osteopontin (OPN), a secreted, integrin-binding protein, functions in the regulation of immune responses and enhancement of leucocyte migration. We have assessed the role of OPN in the host response to endodontic infection using a well-characterized mouse model. Periapical bone loss associated with endodontic infection was significantly more severe in OPN-deficient mice compared with wild-type 3 weeks a
