OBJECTIVE: In temporal lobe epilepsy (TLE), detection of the epileptogenic zone predicts a good surgical outcome. When submitted to 18F-fluorodeoxyglucose positron emission tomography (PET), some patients display lateralized, focal hypometabolism in the temporal lobe (PET+), whereas others appear normometabolic (PET-). However, the mechanism behind this metabolic difference remains unclear. This study aimed to identify differential molecular mechanisms in these patient subtypes.
METHODS: Neocortical and hippocampal biopsies of TLE patients (n = 3 PET+, n = 3 PET-) and nonepileptic postmortem

OBJECTIVES: The present study investigates the explicit and implicit timing abilities of patients with mesial temporal lobe epilepsy (MTLE). Based on previous studies, it was hypothesized that timing abilities were decreased in MTLE patients.
METHODS: The performance of 21 MTLE patients and 21 neurologically healthy probands was tested on two separate tasks. The time bisection task was used to investigate explicit timing and the foreperiod task to test implicit timing.
RESULTS: For the time bisection task, less precise temporal judgements were found in the patient group compared to the contr

Non-invasive neuroimaging is important in epilepsy to help identify cerebral abnormalities. Abnormally reduced fractional anisotropy (FA) in deep white matter (WM) from diffusion-weighted imaging (DWI) is widely reported in large multi-cohort studies across all types of epilepsies. However, abnormalities in FA for superficial WM are rarely investigated in epilepsy. To gain a greater understanding of the nature of WM abnormality at different WM depths, we investigated DWI abnormalities at a range of superficial and deep WM in two separate temporal lobe epilepsy (TLE) cohorts. The first cohort (

Temporal lobe epilepsy (TLE) is the most common pharmacoresistant epilepsy in adults, yet few patients receive curative surgery due to diagnostic and prognostic uncertainty. In a multicenter cohort, we analyzed multimodal MRI and clinical data from 282 TLE patients, 298 healthy controls, and 45 disease controls. Patient-specific deviations from typical lifespan trajectories of intrinsic brain function were mapped using normative modeling. Regional functional alterations were heterogeneous but overlapped most in the mesiotemporal cortex. Connectome-based simulations revealed abnormality spread

Ictal bruxism is a rare motor manifestation in temporal lobe epilepsy with unclear network underpinnings. We report the first case demonstrating SEEG-guided network characterization of ictal bruxism in a 55-year-old man with drug-resistant temporal lobe epilepsy who continued to experience stereotyped seizures with rhythmic teeth grinding after a failed anterior temporal lobectomy. SEEG recorded one spontaneous and two stimulation-induced seizures sampling from the superior temporal gyrus, anterior ventral insula (aVInsula), and nine additional regions of interest. Recordings identified seizur

Encephaloceles are abnormal protrusions of brain tissue, meninges, and cerebrospinal fluid that result from defects in the skull base or cranial vault. These abnormalities can lead to seizure disorders and focal pharmacoresistant epilepsies. However, clinical suspicion and diagnosis are frequently delayed due to the significant challenges in interpreting initial imaging. Interpretation omissions can contribute to false diagnosis as non-lesional epilepsy in patients with encephalocele. This warrants appropriate imaging paradigms and careful interpretation. When clinical suspicion remains, furth

BACKGROUND: Classical type of superficial hemosiderosis (SH) is subpial hemosiderin deposition mainly affecting the cerebellum, brainstem, and spinal cord, which generally presents with cerebellar ataxia and sensorineural hearing disturbance. We here report a rare case of the classical type of SH presenting with temporal lobe epilepsy and perform a literature review on similar cases.
CASE DESCRIPTION: A 63-year-old man with four episodes of impaired awareness and confusion lasting for around 5 minutes after feeling vague uneasiness, suggesting focal impaired awareness seizure, visited a neuro

PURPOSE: Reduced glucose metabolism in the hippocampus is commonly observed in cases of medial temporal lobe epilepsy (MTLE) with hippocampal sclerosis (HS). Glucose metabolism among the various hippocampal subfields has not been thoroughly investigated.
PATIENTS AND METHODS: This study examined 29 patients (18 females; 15-58 years) diagnosed with HS who underwent surgery for drug-resistant epilepsy. FreeSurfer 7.1.1 was used in the processing of MRI data and 18 F-FDG PET scans to derive volumetric data and the FDG SUVr in the whole hippocampus and hippocampal subfields, including the CA1, CA

Temporal lobe epilepsy (TLE) is the most common pharmaco-resistant epilepsy in adults. While primarily associated with mesiotemporal pathology, recent evidence suggests that brain alterations in TLE extend beyond the paralimbic epicenter and impact macroscale function and cognitive functions, particularly memory. Using connectome-wide manifold learning and generative models of effective connectivity, we examined functional topography and directional signal flow patterns between large-scale neural circuits in TLE at rest. Studying a multisite cohort of 95 patients with TLE and 95 healthy contro

OBJECTIVE: Pathological amyloid-β (Aβ) accumulation and hyperphosphorylated tau proteins have been described in resected temporal lobe specimens of epilepsy patients. We aimed to determine cerebrospinal fluid (CSF) Aβ1-42 and p181-tau levels and cerebral Aβ deposits on positron emission tomography (Aβ PET) and correlate these findings with cognitive performance in adults with drug-resistant temporal lobe epilepsy (TLE).
METHODS: In this cross-sectional study, we enrolled individuals with drug-resistant TLE who were 25-55 years old. Each participant underwent 18F-flutemetamol PET, determinatio

Temporal lobe epilepsy (TLE) can cause different types of memory impairments. Here, we report a case of immediate improvement of memory impairment following antiepileptic drug (AED) treatment in a patient with TLE with amygdala enlargement (TLE-AE), who rapidly developed recurrence. The patient was a man in his 60s whose family members complained of his amnesia. Neuropsychological investigations detected obvious recent and remote memory loss and executive function impairments. Our examinations revealed evidence of TLE and bilateral amygdala enlargement without any results suggesting organic di

Temporal-lobe epilepsy in humans is often associated with widespread, synchronized neuron firing that co-occurs with traveling waves in local field potential. These traveling waves generate stochastic oscillations in a time series of microelectrode voltage, and previous work has deemed it informative for traveling-wave analysis to study the mean periodicity. This manuscript reveals that: a) mean voltage (i.e., traveling-wave periodicity) adequately explains the observed voltage periodicity only for a select few time intervals during seizure; and b) mean voltage has a 7 Hz cosine-series represe

INTRODUCTION: Patients with epilepsy are particularly vulnerable to the negative effects of anxiety disorders. In particular, temporal lobe epilepsy with anxiety disorders (TLEA) has attracted more attention in epilepsy research. The link between intestinal dysbiosis and TLEA has not been established yet. To gain deeper insight into the link between gut microbiota dysbiosis and factors affecting TLEA, the composition of the gut microbiome, including bacteria and fungi, has been examined.
METHODS: The gut microbiota from 51 temporal lobe epilepsy patients has been subjected to sequencing targe

Temporal lobe epilepsy (TLE) is one of the most common pharmaco-resistant epilepsies in adults. While hippocampal pathology is the hallmark of this condition, emerging evidence indicates that brain alterations extend beyond the mesiotemporal epicenter and affect macroscale brain function and cognition. We studied macroscale functional reorganization in TLE, explored structural substrates, and examined cognitive associations. We investigated a multisite cohort of 95 patients with pharmaco-resistant TLE and 95 healthy controls using state-of-the-art multimodal 3T magnetic resonance imaging (MRI)

Drug-resistant mesial-temporal lobe epilepsy is a devastating disease with seizure onset in the hippocampal formation. A fraction of hippocampi samples from epilepsy-surgical procedures reveals a peculiar histological pattern referred to as 'gliosis only' with unresolved pathogenesis and enigmatic sequelae. Here, we hypothesize that 'gliosis only' represents a particular syndrome defined by distinct clinical and molecular characteristics. We curated an in-depth multiparameter integration of systematic clinical, neuropsychological as well as neuropathological analysis from a consecutive cohort

The ability to mentally travel forward through time allows humans to envisage a diverse array of possible events taking place in the future, helping us to choose which pathway to take in life. In epilepsy, we assume that patients use this cognitive ability when deciding between various treatment options, but this assumption has not been robustly tested. The temporal lobes are key contributors to this 'future thinking' and its building blocks include cognitive functions commonly impaired in temporal lobe epilepsy such as memory and language, giving rise to a hypothesis that 'future thinking' is
