Improving Consent in Trauma: Recall (ICIT: Recall) : a multicentre study protocol of consent for hip fractures.

AIMS: This study investigates the effectiveness and adequacy of the informed consent process for patients undergoing hip fracture surgery. While informed consent is a legal and ethical responsibility, factors in the trauma setting can impair patients' understanding and retention of information. This study seeks to evaluate patients' recall of perioperative complications and explore their perceptions of the consent process. METHODS: A mixed-methods, multicentre cohort study will be conducted in the Southeast of Scotland. Adult patients with hip fractures will be recruited via purposive samplin

Hidden in plain sight: the covering of patients' eyes and a microethics of medical photography.

This article uses the author's experience of researching historical photographs of facial injury and surgical reconstruction to think through the ethics of writing about and publishing images of patients anonymised by excising or covering their eyes. This article specifically highlights tensions between the British Medical Journal's guidelines for patient anonymity in imagery and those of the archives of the British Association of Plastic, Reconstructive and Aesthetic Surgeons. The rules for reproducing these sensitive images are not standardised across disciplines nor across journals and medi

Negotiating international bioethics: a response to Tom Beauchamp and Ruth Macklin.

Can the bioethical theories that have served American bioethics so well, serve international bioethics as well? In two papers in the previous issue of the Kennedy Institute of Ethics Journal, I contend that the form of principlist fundamentalism endorsed by American bioethicists like Tom Beauchamp and Ruth Macklin will not play on an international stage. Deploying techniques of postmodern scholarship, I argue that principlist fundamentalism justifies neither the condemnation of the Nazi doctors at Nuremberg, nor, as the Report of the Advisory Committee on the Human Radiation Experiments (ACHRE

Beyond mentorship: the promise and perils of sponsorship in health professions education research.

Mentorship has long been regarded as foundational for academic advancement. However, the potentially transformative role of sponsorship remains underexplored in health professions education (HPE) research. Sponsorship, the proactive leveraging of influence to advocate for others, can offer critical support to early-career researchers. Yet, while sponsorship has clear benefits, it is neither universally necessary, nor free of potential challenges. This article offers a critical examination of sponsorship in HPE research, discussing its advantages, complexities, and potential risks. Drawing from

Modeling and measuring graduate students' generative AI literacy: a study based on Marzano's taxonomy.

BACKGROUND: Generative AI is rapidly reshaping graduate students' academic research practices. Existing research lacks both literacy models specific to graduate students and a unifying theoretical framework to explain each dimension's functional role and interrelationships, constraining its cultivation and evaluation. METHODS: Semi-structured interviews with 14 professors were analyzed using grounded theory, with Marzano's taxonomy of educational objectives serving as the theoretical framework for selective coding. A corresponding scale was developed and empirically validated with 308 graduat

The Moral Brain: a Neuropsychiatric Perspective on Moral Behavior and Motivation.

This review explores the interplay between neurobiological, psychological, and social factors that underpin moral behavior and motivation. Integrating insights from neuropsychiatry, it examines the roles of key brain regions such as the prefrontal cortex, amygdala, and anterior cingulate cortex, as well as neurotransmitters like serotonin and dopamine, in shaping moral reasoning and ethical decision-making. The historical evolution of moral motivation theories, from ancient philosophy to modern psychological and neurobiological perspectives, provides a foundation for understanding intrinsic an

Maternal grief in cross-cultural context: Selective neglect, replaceable infants and lifesaving names.

Scheper-Hughes divides mothers onto "better off" vis-à-vis "poor" mothers stuck in "old" reproductive strategy with high fertility. Cultural construction of mother love allows the latter group to neglect their "worst bets" to death without grief. Based on the bio-evolutionary theory, Hrdy hints that "modern" Western mothers, guided by ethical behavior, care for unviable infants while mothers in "non-Western societies" might dispose them of due to innate responses. This article warns against such binary division of mothers. Ethnographic research indicates that notions of replaceable infants, fa

An epistemology of inclusion for nursing: Lessons from male infertility.

Male infertility-defined by the World Health Organization as the inability to achieve pregnancy after 12 months of unprotected intercourse due to male-factor reproductive impairment-contributes to nearly half of infertility cases worldwide. Yet within nursing scholarship, men's reproductive experiences remain only partially visible, reflecting longstanding gendered assumptions that equate reproduction with femininity. This epistemic underdevelopment has ethical consequences: it sustains stigma, delays help-seeking, and limits nursing's capacity to provide equitable, relationally attuned care.

Ethics of sedation for existential suffering: palliative medicine physician perceptions - qualitative study.

OBJECTIVES: Palliative sedation for existential suffering (PS-ES) is considered a controversial practice to control refractory suffering at the end of life. This study aims to explore Belgian palliative care physicians' perceptions regarding the ethics of PS-ES. METHODS: This nationwide qualitative study follows a Grounded Theory approach. We conducted semistructured interviews with 25 palliative care physicians working in 23 Belgian hospitals and hospices (Flanders, Brussels, Wallonia). We analysed the data using the Qualitative Analysis Guide Of Leuven and we followed the Consolidated Crite

The dead donor rule: effect on the virtuous practice of medicine.

OBJECTIVE: The President's Council on Bioethics in 2008 reaffirmed the necessity of the dead donor rule and the legitimacy of the current criteria for diagnosing both neurological and cardiac death. In spite of this report, many have continued to express concerns about the ethics of donation after circulatory death, the validity of determining death using neurological criteria and the necessity for maintaining the dead donor rule for organ donation. I analysed the dead donor rule for its effect on the virtuous practice of medicine by physicians caring for potential organ donors. RESULTS: The d

The words we work with that work on us: clinical paradigm and cumulative relational trauma.

This paper addresses a gap between analytic clinical theory and practice which emerges when examining the words we work with via textual and narrative research of case histories. Both subject matter and methodology fit with the remit of conceptual research in psychoanalysis, currently ranging from inductive to nomothetical approaches. Research of clinical language reveals an implicit account of human nature and the world which undergirds clinical practice. Based in the critical philosophy of the previous century, this is termed clinical paradigm. Such implicit views are induced rather than exp

Threat of COVID-19 Vaccine Hesitancy in Pakistan: The Need for Measures to Neutralize Misleading Narratives.

Immediately after declaring COVID-19 as a pandemic, numerous wild conspiracy theories sprouted through social media. Pakistan is quite vulnerable to such conspiracy narratives and has experienced failures of polio vaccination programs because of such claims. Recently, two well-known political figures raised conspiracy theories against COVID-19 vaccines in Pakistan, stating that COVID-19 is a grand illusion and a conspiracy against Muslim countries. This theory is much discussed in the local community, supporting COVID-19 vaccine hesitancy. We urge healthcare authorities in Pakistan to take nec

Navigating fairness in artificial intelligence-based prediction models: theoretical constructs and practical applications.

Artificial intelligence (AI)-based prediction models, including risk scoring systems and decision support systems, are being increasingly adopted in health care. Addressing AI fairness is essential to fighting health disparities and ensuring equitable model performance and patient outcomes. However, numerous and conflicting definitions of fairness complicate this effort. In this Viewpoint, we aim to support the transition of AI fairness from theory to practice using appropriate fairness metrics. We assess the relation of 27 fairness definitions identified in the literature to the model's inten

Explicating the process of moral courage in clinical nurses: a grounded theory study.

Moral courage in nurses is the product of a complex process and can be enhanced by identifying the steps through which nurses make morally courageous decisions. This qualitative study aimed to explicate the process of moral courage in clinical nurses and present a descriptive model. The study was conducted in Shahrekord University of Medical Sciences in 2022 using the grounded theory approach. Twenty-one clinical nurses were selected through purposive and theoretical sampling. Data were analyzed using Strauss and Corbin's 2015 approach and data management was supported by MAXQDA software (vers

On transplanting human fetal tissue: presumptive duties and the task of casuistry.

The procurement of fetal tissue for transplantation may promise great benefit to those suffering from various pathologies, e.g., neural disorders, diabetes, renal problems, and radiation sickness. However, debates about the use of fetal tissue have proceeded without much attention to ethical theory and application. Two broad moral questions are addressed here, the first formal, the second substantive: Is there a framework from other moral paradigms to assist in ethical debates about the transplantation of fetal tissue? Does the use of fetal tissue entail cooperation in abortion? To answer thes

Realist synthesis of co-production of services in a local authority context: what works, for whom, how and in what circumstances?

BACKGROUND: Co-production is increasingly promoted as a means of improving the design and delivery of local authority services, yet evidence on how and why it works remains limited. This realist synthesis explored how co-production operates within complex local authority systems, identifying the circumstances and mechanisms that enable or constrain meaningful involvement of stakeholders in co-production processes and sustained change to ensure that services are co-produced with professionals and those with lived experience. METHODS: Scoping searches identified key literature on co-production

Public value management in rural China through digital engagement, identity recognition and moral legitimacy.

This study investigates how public value is constructed in rural governance through the interplay of digital civic embeddedness, moral legitimacy, procedural inclusion, and eco-cultural identity salience. Drawing on Public Value Theory (PVT), Social Identity Theory (SIT), and Digital Citizenship Theory (DCT), the study develops and empirically tests a structural model using survey data from 412 rural residents in Henan Province, China. Results from PLS-SEM analysis reveal that while digital civic embeddedness does not directly enhance perceived public value, it significantly influences it thro

Challenges in the conduct of vaginal microbicide effectiveness trials in the developing world.

Conducting a phase III trial of a vaginal microbicide in a developing country poses several important and complex ethical challenges. As part of a process to bridge the gap between ethical theory and practice, we share our experiences in performing a phase III trial of Col 1492 (Advantage S) among female sex workers at four sites world-wide; Durban, Abidjan, Cotonou and Hat Yai. The ethical challenges included: (i) difficulties in obtaining informed consent. Participants were unable to grasp the concepts of a clinical trial for several weeks to months. In Cotonou, 30% of the women did not know

Implementing socially assistive robots in geriatric care: a qualitative study of caregivers' practical and moral experiences.

BACKGROUND: Socially assistive robots (SARs) are increasingly introduced into geriatric care as a potential solution to workforce shortages and growing demands. However, their implementation raises ethical concerns about how these technologies can support good care, concerns that often remain technology-centered, with limited attention to how they emerge in everyday practices. Understanding professional caregivers' perspective is crucial, as they are responsible for implementing this technology in practice. AIM: This study examines how professional caregivers experience SAR implementation in
