Münchhausen syndrome: psychopathology and management.

A case report on a patient with Münchhausen syndrome is presented and the psychopathological features are compared to other reports in the literature. Diagnosis and differential diagnosis are discussed with regard to other somatoform disorders such as conversion disorder and somatization disorder as well as to malingering and cenesthesic schizophrenia. The management of Münchhausen syndrome is aggravated by the low compliance in these patients, who expose themselves to serious complications of unnecessary medical and surgical procedures. Early diagnosis could to a considerable extent exclude t

Case report: Hereditary sensory autonomic neuropathy presenting as bifid deformity to the tongue.

Hereditary sensory autonomic neuropathy (HSAN) is a group of rare genetic disorders in which affected patients have a diminished capacity to feel pain. Patients with HSAN may present with a wide range of factitial injuries, where injury to the oral cavity may be an early presenting sign. While existing literature on HSAN is scant, many reports highlight the long-term outcomes that may include enucleation of eyes, amputation of fingers and limbs, and disfigurement of the tongue. This case describes a five-month-old female with repetitive injury to the tongue causing it to heal with a bifid defo

Rhinotillexomania: a manifestation of psychiatric illness.

OBJECTIVE: To increase the knowledge of rhinotillexomania, or compulsive nose picking, as a manifestation of psychiatric disease through the presentation of a case series and a review of the literature. MATERIAL: We present three clinical cases with self-destructive nasal injuries as a symptom of different psychiatric diseases. RESULTS: One patient presented amputation of the middle turbinate as a manifestation of an obsessive-compulsive disorder of bipolar disease. Two patients had a septal perforation. In the first patient it was the first symptom of factitious dermatitis and in the second

[Pseudodementia, what are we talking about? Part II: From Stertz to Alzheimer: A psychogenic disease after trauma].

OBJECTIVE: The aim of this work is to give a new anchorage for the initial concept of pseudodementia, its basic definitions and actual clinical implications. The original definition and framework of the concept are misidentified and therefore need to be reintroduced and analysed here. This approach, beyond its heuristic and epistemological goal, has a practical aim: to adjust the psychopathological point of view on pseudodementia. METHODOLOGY: In addition to a conventional survey based on search engines (Medline, PsychINFO and google book), we conducted a systematic review of the concomitant

Recurrent subcutaneous emphysema of the face: a challenging clinical problem.

In the neck or face, there are different causes for subcutaneous emphysema such as injury to the sinuses, the hypopharynx, the laryngotracheal complex, the pulmonary parenchyma, the esophagus or the presence of gas-forming organisms. However, factitious subcutaneous emphysema, a rare cause, must be considered in the differential diagnosis. In this clinical report, we discuss a 20-year-old girl who was under follow-up because of recurrent subcutaneous emphysema of the face and periorbital area. After 2 years of work-ups, including a period of close observation in the intensive care unit, self a

The clenched fist syndrome. A psychiatric syndrome mimicking reflex sympathetic dystrophy.

We describe a patient with swelling and pain in his right hand, which persisted for 1 year after minimal trauma. We think this represents a case of "clenched fist syndrome," a conversion disorder with unique somatic features that is easily mistaken for reflex sympathetic dystrophy syndrome. Included is a comparison of other psychological illnesses in which unusual physical findings involve the hand.

Tourette syndrome, atypical pervasive developmental disorder and Ganser syndrome in a 15-year-old, visually impaired, mentally retarded boy.

This is a case report of a 15-year-old visually impaired, mentally retarded male who presents with symptoms consistent with Tourette Syndrome, a Syndrome of approximate answers (Ganser's Syndrome) and Atypical Pervasive Developmental Disorder. The authors feel that this follow-up on the case presented earlier by Parraga and Butterfield raises the possibility of a link between a number of the symptoms of adult schizophrenia, appearing in attenuated form in these two cases, and Tourette Syndrome.

A patient with distinct dissociative and hallucinatory fugues.

A 62-year-old man presented with a history suggesting both dissociative fugue and a distinct fugue-like hallucination. The dissociative fugues included unplanned travel, loss of personal identity, inability to recall his past and amnesia for the fugue interval. The subjective fugues consisted of a stereotyped hallucination wherein he would travel to a social gathering place, meet his 'imaginary friends' and engage with them in conversation. He experienced the subjective fugues as if they were real, recognised them as hallucinations when he was normally conscious, and remembered them in great d

Raynaud's phenomenon--or just skin with dye?

Munchausen's syndrome is a factitious disorder resulting in unnecessary investigations and operative treatments in a small and well-defined population. Autobiographical falsification is the characteristic of the entity. The case history is presented of a 28-year-old woman admitted to the emergency department with severe pain of acute onset in her fingers and discoloration while washing dishes. She had been diagnosed with Raynaud's phenomenon and had been on antiepileptic drugs. The fingertips of both hands looked cyanotic. Radial and ulnar pulses were intact. She had argued with the personnel

Scintigraphic manifestations of thyrotoxicosis.

The term thyrotoxicosis refers to the clinical syndrome of increased systemic metabolism that results when the serum concentrations of free thyroxine, free triiodothyronine, or both are elevated. The term hyperthyroidism refers to overactivity of the thyroid gland with a resultant increase in thyroid hormone synthesis and release into the systemic circulation. These terms are not interchangeable, since thyrotoxicosis can develop in thyroid conditions that are not associated with increased thyroid function, such as thyroiditis, or in so-called factitious hyperthyroidism. The clinical signs and

Dermatitis Artefacta: A Multidisciplinary Approach.

BACKGROUND Dermatitis artefacta is a challenging and often under-recognized psychodermatosis characterized by self-induced skin injury. Its clinical presentation frequently mimics other chronic ulcers, leading to frequent misdiagnosis and diagnostic delays. This delay prolongs symptoms and perpetuates the detrimental itch-injury cycle, whereby chronic pruritus drives compulsive scratching, creating a barrier to effective healing. CASE REPORT We report the case of a 59-year-old man with a 3-year history of a refractory, intensely pruritic ulcer on the right temporal scalp, which subsequently ex

Ganser-like syndrome after loss of psychic self-activation syndrome: psychogenic or organic?

Very few data are available on the long-term changes in the cognitive abilities of patients with loss of psychic self-activation syndrome (LPSAS). Here, we present a 25-year follow-up study on a case of LPSAS resulting from bilateral pallidal lesions caused by carbon monoxide intoxication. Typical signs of LPSAS were observed, showing no changes in severity, but Ganser syndrome (GS) gradually developed and worsened during the follow-up period. GS is generally assumed to be a psychogenic syndrome, but an organic etiology has been suspected by the authors of several case reports. Here, atypical

A case of factitious subfascial emphysema.

A 44-year-old woman with a history of borderline personality disorder, Hepatitis C, and multiple hospital admissions for cellulitis and cutaneous abscesses presented with pain of several days duration in her left upper extremity following getting her left arm "stuck" in a subway turnstile. The pain and swelling had progressively worsened following the injury. At the time of presentation the patient's temperature was 98.6 degrees F, heart rate was 82 beats/minute, blood pressure of 116/60, and an oxygen saturation of 98% on room air. The left upper extremity was diffusely swollen but minimally

Dermatitis artefacta.

BACKGROUND: Dermatitis artefacta (DA) is difficult to diagnose by clinical findings alone. Histopathologic analysis may rule out organic disorders and in some cases establish, in conjunction with microanalytic techniques, a definite diagnosis. OBJECTIVE: We report a young woman with DA in the form of recurrent abscesses whose diagnosis was aided by the identification of subcutaneous foreign material through histologic and microanalytic methods. METHODS: Histologic examination showed foreign body giant cell reaction with polarizable material, which was identified as cellulose by energy disper

[Reversible dementia].

A 65 year old woman, referred for differential diagnosis of dementia, presented with cognitive and mnestic deficits. Typical signs and symptoms of hypothyroidism led to the diagnosis of severe autoimmune thyroiditis. After six months of treatment with thyroid hormone, dementia, myopathy and ataxia had disappeared. The neurologic and neuropsychologic manifestations of hypothyroidism are discussed. In dementia the diagnosis of secondary and reversible forms is of great importance with regard to prognosis and treatment. Hypothyroidism is known to be a common and treatable cause of pseudodementia.

Childhood chronic daily headache: a biopsychosocial perspective.

The aim of our observational study was to highlight some clinical observations on chronic daily headache (CDH) in children and adolescents. Data on patients < or =18 years aged presenting with CDH to the Pediatric Headache Clinic at the Royal University Hospital, Saskatoon, Canada, were collected prospectively and sequentially from February 2004 to July 2006. Standardized data sheets and definitions were used. Follow-up information on the 70 participants (22 males, 48 females) was current to February 2007. Fifty-four participants (77%) had had recurrent headaches before transformation to CDH.

Munchausen Syndrome Presented as Guillain-Barré Syndrome: A Case Report and Literature Review.

Munchausen syndrome (MS), a complex form of factitious disorder (FD), presents significant diagnostic and management challenges in emergency and hospital settings. Patients deliberately fabricate or induce symptoms to gain medical attention, often leading to unnecessary interventions, resource misallocation, and iatrogenic harm. This study highlights the diagnostic complexity and the need for multidisciplinary management of Munchausen syndrome through a detailed case report and literature review. A 30-year-old woman presented with neurological symptoms mimicking Guillain-Barré syndrome (GBS),

A second visually impaired, mentally retarded male with pervasive developmental disorder, Tourette disorder and Ganser's syndrome: diagnostic classification and treatment.

The authors report a second visually impaired, mentally retarded male who presents with symptoms consistent with atypical pervasive developmental disorder, Tourette disorder, and a syndrome of approximate answers (Ganser's syndrome). Both this case and an earlier reported case are significant in that they identify two children with strikingly similar long-standing symptoms and developmental histories. Since these findings may have broader relevance to children with multiple coexisting disorders, diagnostic and intervention strategies in this population are reviewed.

An adolescent girl with signs and symptoms of anaphylaxis and negative immunologic workup: a case report.

BACKGROUND: The increasing incidence of allergies and allergic reactions among children and adults has become a major public health concern. The etiology of allergic reactions can often be confirmed based on a detailed history and supportive testing. However, there are cases where the underlying factors are more complex and difficult to identify. CASE PRESENTATION: Here we present the case report of a 14-year-old Caucasian  girl with weight loss and a 3-year history of reported angioedema culminating in five intensive care unit admissions over the course of 2.5 months.
