Adaptive geometric-attention network for two-stage lung nodule segmentation and malignancy classification in federated healthcare IoT edge environments.

Accurate segmentation and classification of lung nodules in computed tomography (CT) scans remains a critical challenge in early lung cancer detection within distributed healthcare Internet of Things (IoT) environments. This paper presents a novel two-stage framework called Adaptive Geometric-Attention Network (AGA-Net) that integrates geometric constraints with multi-scale attention mechanisms for precise nodule segmentation followed by uncertainty-aware malignancy classification in federated learning scenarios. Unlike existing approaches that rely on traditional convolutional architectures,

YOMO TF based edge cloud collaborative surveillance framework for tobacco warehouse safety management.

Tobacco warehousing requires continuous surveillance to mitigate risks like unauthorized access, fire hazards, and moisture-induced decay. To address these challenges, this paper proposes an edge-cloud collaborative surveillance framework with adaptive deep learning, termed YOMO-TF (YOLO + MobileOne + Transformer + Federated self-distillation). The architecture consists of three layers: edge layer- employing lightweight models (YOLOv8-nano for real-time object detection and MobileOne-S for effective image classification) for performing fast, on-device video analytics without storing data in cl

Regime-Aware Federated Aggregation (RAFA) for privacy-preserving energy load forecasting across heterogeneous European grid clients.

BACKGROUND: Federated Learning (FL) enables collaborative model training across distributed clients without sharing raw data, making it attractive for privacy-sensitive energy forecasting. However, standard aggregation algorithms, most notably FedAvg and FedProx, assume stationary client data distributions, an assumption routinely violated in multi-national power grids where seasonal patterns, demand crises, and policy changes induce persistent regime shifts. METHODS: We present RAFA (Regime-Aware Federated Aggregation), a novel FL framework that explicitly detects and adapts to distribution

Intelligent Real-Time Healthcare and Biomedical Monitoring Systems: A Narrative Review of AI, IoT, and Emerging Technologies.

BACKGROUND: Chronic disease management, an ageing global population, and the aftermath of the COVID-19 pandemic have pushed real-time, continuous health monitoring from a research curiosity toward routine clinical practice. Artificial intelligence (AI), the Internet of Medical Things (IoMT), edge computing, and next-generation wireless networks are converging to enable systems that sense, interpret, and act on physiological data outside the traditional hospital setting. METHODS: This is a narrative review, not a PRISMA-guided systematic or scoping review; it synthesises 75 sources (a mixture

Machine Learning and Deep Learning for Neurological Disease Analysis: A Systematic Review Across Five Major Disorders.

Artificial Intelligence (AI) has become integral to the research of neurological diseases due to the rapid expansion of neuroimaging, clinical, physiological, and wearable data. However, the concise synthesis of recent machine learning (ML) and deep learning (DL) remains limited. This systematic review analyzes studies published between January 2021 and March 2026 on five major conditions- Alzheimer's disease, stroke, Parkinson's disease, brain tumors, and traumatic brain injury (TBI)-following the PRISMA 2020 guidelines and a structured search of PubMed, Scopus, and Web of Science, yielding 2

A causal multimodal framework for privacy-preserving early-stage cancer detection and adaptive testing.

Early detection of cancer at stage I is critical for improving survival rates, yet existing diagnostic tools often face trade-offs between sensitivity, specificity, and clinical scalability. While liquid biopsies, radiomics, and breathomics independently offer promise, their isolated use struggles with robustness, leading to false positives or missed early lesions. To overcome these challenges, this research proposes CausaLMED, a causal multimodal framework that integrates cfDNA fragmentomics, exhaled breathomics, imaging radiomics, and digital pathology embeddings through a causal graph-based

Alzheimer's detection using quanvolutional neural networks with federated training.

Alzheimer's disease detection from MRI data is challenging because of subtle anatomical patterns, high-dimensional image data, and privacy constraints associated with centralized medical-data training. We propose a hybrid quantum-classical federated learning framework using a frozen four-qubit quanvolutional front-end. The 128 × 128 grayscale MRI images are divided into 2 × 2 patches, and quantum features are pre-extracted using a fixed circuit implemented in PennyLane. The resulting features are classified using trainable classical dense layers implemented in TensorFlow/Keras. Federated train

FedEdgeR: federated and privacy-preserving edgeR for differential gene expression analysis.

MOTIVATION: Multi-center RNA-seq studies improve statistical power, but privacy regulations restrict patient-level data sharing. Meta-analysis methods avoid this restriction but lose power, especially under per-site imbalance. Federated learning allows sites to share only summary statistics. Among the three dominant differential expression (DE) frameworks, Flimma federates limma-voom and FedPyDESeq2 federates DESeq2. edgeR, still preferred for small-sample and high-variability designs, lacks a federated counterpart. Its iteratively reweighted least squares (IRLS) and Cox-Reid dispersion estima

Data Interoperability in Context: The Importance of Open-Source Implementations When Choosing Open Standards.

Following the proposal by Tsafnat et al (2024) to converge on three open health data standards, this viewpoint offers a critical reflection on their proposed alignment of openEHR, Fast Health Interoperability Resources (FHIR), and Observational Medical Outcomes Partnership (OMOP) as default data standards for clinical care and administration, data exchange, and longitudinal analysis, respectively. We argue that open standards are a necessary but not sufficient condition to achieve health data interoperability. The ecosystem of open-source software needs to be considered when choosing an approp

Trustworthy AI-Powered Intrusion Detection for the Internet of Medical Things (IoMT): A Review.

The Internet of Medical Things (IoMT) is transforming healthcare through continuous patient monitoring, telemedicine, cloud-edge services, and Healthcare 5.0. However, the rapid growth of interconnected medical devices has expanded the healthcare cyberattack surface, making intelligent intrusion detection essential for protecting sensitive medical data and ensuring resilient clinical operations. Existing reviews examine specific aspects of AI-powered intrusion detection but rarely provide a deployment-oriented synthesis linking technical performance with operational and clinical requirements.

Design considerations for in silico drug discovery platform that can accelerate targeted drug repurposing: a focus on rare, neglected, and emerging diseases.

INTRODUCTION: Over the past decades, considerable debate has arisen regarding the decline in productivity within the pharmaceutical industry. In this context, drug repurposing has become an increasingly important strategy. Because of its cost- and time-efficient nature, it offers a particularly attractive avenue for identifying therapeutic solutions for rare, neglected, and emerging diseases. However, the implementation of rational drug repurposing strategies in these fields is hindered by data scarcity. AREAS COVERED: The author focuses on the challenge of data availability in the context of

An adaptive client selection method for long tailed scene classification in autonomous driving.

Collaborative autonomous driving utilizing Federated Learning (FL) is frequently constrained by long-tailed data distributions and perceptual imbalances. Implementing federated learning for environmental scene classification in autonomous driving faces severe challenges due to the long-tailed and non-IID distribution of real-world climatic data. To address this, this paper proposes FedRare, a client selection framework based on a multi-dimensional utility function, integrating scenario criticality ([Formula: see text]), distribution rarity ([Formula: see text]), and local update quality ([Form

GEN-Guard: correcting generalization failures for deployable federated surgical AI.

PURPOSE: Federated Learning (FL) in surgical video AI enables collaborative model training without sharing sensitive data. However, standard evaluation practices-selecting the "best" global model based only on validation data from participating hospitals-can lead to suboptimal deployment choices. We identify this critical failure mode as performance leakage, where the selected model overfits internal federation data and fails to generalize to unseen institutions, thereby undermining the core goal of FL: robust real-world generalization. METHOD: We propose GEN-Guard, a practical post-hoc frame

Head and neck cancers survival in Europe, Taiwan, and Japan: results from RARECAREnet Asia based on a privacy-preserving federated infrastructure.

BACKGROUND: The head and neck cancers (HNCs) incidence differs between Europe and East Asia. Our objective was to determine whether survival of HNC also differs between European and Asian countries. METHODS: We used population-based cancer registry data to calculate 5-year relative survival (RS) for the oral cavity, hypopharynx, larynx, nasal cavity, and major salivary gland in Europe, Taiwan, and Japan. We modeled RS with a generalized linear model adjusting for time since diagnosis, sex, age, subsite, and histological grouping. Analyses were performed using federated learning, which enables

FedMamba-IoMT: Federated state space models with differential privacy and byzantine resilience for privacy-preserving intrusion detection in Internet of Medical Things.

The proliferation of Internet of Medical Things (IoMT) devices has created critical cybersecurity challenges demanding intrusion detection systems that achieve high accuracy across diverse attack taxonomies while preserving patient privacy across institutional boundaries. Existing federated learning (FL) approaches face an inherent tension: Transformer-based architectures achieve strong detection performance but incur quadratic computational complexity and substantial communication overhead, while lightweight classifiers sacrifice representational capacity. Moreover, most FL-based intrusion de

Next generation preventive neurology: how artificial intelligence and machine learning are reshaping Alzheimer's disease research.

A neurological condition that worsens over time, Alzheimer’s disease (AD) is typified by memory loss, cognitive decline, and functional degradation. Traditional diagnostic techniques such as neuroimaging, cerebrospinal fluid biomarkers, and neuropsychological testing are often intrusive, costly, or insensitive in the early stages. Recent years have seen the emergence of AI and ML as game-changing technologies for AD risk assessment, early detection, and customized prevention. Using sophisticated models such as deep learning, convolutional neural networks (CNNs), and graph-based algorithms, AI-

FedFound: a federated foundation model for lifespan brain morphological connectome analysis.

The brain morphological connectome derived from structural MRI reflects inter-regional morphological relationships, providing a powerful representation for characterizing individual variability and detecting abnormalities across the lifespan. However, these abnormal alterations are subtle and complex, posing significant challenges for accurate and generalizable diagnosis using machine learning. Here, we present FedFound, the first federated foundation model inspired by the structured educational and residency training pathway of radiologists, designed for robust and scalable analysis of lifesp

Beyond the Usual Players: Evidence-Building Priorities for Behavioral Health Among all U.S. Federal Agencies.

There are great and growing behavioral needs in the United States. The Department of Health and Human Services (HHS) already invests in many lines of research and services to address those needs. But there are opportunities for other federal agencies to provide investments as well. All federal agencies are now required to develop evidence-building plans, called Learning Agendas, per the Evidence-Based Policy Making Act. This commentary reviews the content of 2022-2026 Learning Agendas for priorities related to behavioral health across all U.S. federal agencies besides HHS. Across 4 other feder

Decoupling forgetting and preservation in federated unlearning via knowledge distillation.

Federated Learning (FL) enables collaborative model training without sharing raw data, but compliance with data privacy regulations such as the "Right to be Forgotten" requires mechanisms to remove specific clients' contributions from trained models. A key challenge in federated unlearning is selectivity: removing a target client's influence while preserving performance for other designated clients. This is inherently difficult because deep neural networks learn shared feature representations, and aggressive forgetting operations degrade overall model utility. We propose a Three-Phase Selectiv
