CASE: A 17-year-old adolescent boy with primary osteosarcoma of the proximal femur underwent cement-augmented internal fixation with a femoral neck system for a pathologic hip fracture, allowing for pain control, neoadjuvant chemotherapy, and diagnostic work-up to be completed before resection and endoprosthetic reconstruction. No hardware complications or local recurrences were noted at 3 years postoperatively.
CONCLUSION: Cement-augmented internal fixation with a femoral neck system is an option for certain patients with pathologic hip fractures. This approach allows for diagnostic work-up,

BACKGROUND: A hibernoma, also known as a brown fat tumor, is a rare benign soft tissue tumor, which originates from brown adipose tissue remaining in the fetus after the gestational period. It is often detected in adult men, presenting as a painless slow-growing mass. Hibernomas of the thigh have been reported; however, motor and sensory disorders caused by the tumors compressing the femoral nerve have not been reported. Between 1997 and 2003, a total of 2223 patients presented to our hospital with the neck of femur fracture. This retrospective study examines the histological results of 90 patients (4%) suspected of having a fracture associated with serious underlying pathology. On the basis of a considerable experience in the use of Ender's nails in basal and trochanteric fractures of the neck of the femur, the authors are convinced of the excellent mechanical properties of this fixation device. They have therefore used it in five atypical and complex cases of subtrochanteric fracture. They describe and discuss the clinical and radiological results obtained. Periprosthetic fractures after total hip arthroplasty (THA) were found in 0.77% (33 fractures in 4280 THA). At the time of fracture the prosthesis was loose in about one-third of the cases. A technique of modified Van Nes rotationplasty has been used since 1981 for limb salvage surgery in children and adolescents with malignant sarcoma of the lower extremity. The value of xeroradiography in the diagnosis of peripheral skeletal lesions is illustrated by some typical examples and compared with standard radiographic procedures. The advantages of xeroradiography depend on its convenient format, easy appreciation of structural changes and easy viewing. On the other hand, density gradations are flattened. Compared with a technically satisfactory film examination, there is no increase in diagnostic information. Xeroradiography can therefore only be regarded as complementary to conventional radiography for the diagnosis of skeletal conditions.

AIMS: Following the resection of an extensive amount of bone in the treatment of a tumour, the residual segment may be insufficient to accept a standard length intramedullary cemented stem.
