AIM: Femoral fractures are one of the most debilitating injuries presenting to the emergency departments (EDs). The pain caused by these fractures is typically managed with opioids and adjunctive regional analgesia. These approaches are often associated with adverse side effects. Thus, appropriate alternative methods should be thoroughly investigated. To evaluate ultrasound-guided femoral nerve block (FNB) with ultrasound-guided fascia iliaca compartment block (FICB) in femoral fractures, to determine which provides better analgesia and less opioid requirement.
METHODS: This study was a rando

BACKGROUND: Transient femoral nerve palsy is a potential complication of the use of a Pavlik harness to treat developmental dysplasia of the hip. Our hypothesis was that patients who develop a femoral nerve palsy while undergoing Pavlik harness treatment for developmental dysplasia of the hip are more likely to have unsuccessful orthotic treatment and to require closed or open hip reduction.
METHODS: We performed a retrospective review of all patients who underwent Pavlik harness treatment for developmental dysplasia of the hip within a seventeen-year period (1992 to 2008). All cases of femor

The origins, courses and relations of lateral femoral cutaneous nerves (LFCNs) were examined bilaterally in 28 cadavers, and the variations were observed in two. On the right side of one cadaver, the ventral rami of the first and second lumbar spinal nerves were united and then this nerve was divided into four branches. From medial to lateral, these branches were the obturator nerve, the femoral nerve, the medially located LFCN and the laterally located LFCN. On the left side of another cadaver, there were three LFCNs. All of these nerves pierced the psoas major muscle anterolaterally. Two of

INTRODUCTION: Missed compartment syndrome can have devastating long-term impact on a patient's function. Femoral fracture has been reported in 52-58% of acute thigh compartment syndromes in the existing literature. Time to diagnosis of compartment syndrome is cited as a key determinant of outcome. Use of femoral nerve blocks in splinting of femoral fractures may mask signs of early compartment syndrome. We present the attitudes of emergency department and orthopaedic staff in NHS trusts in England with regard to this issue.
METHODS AND MATERIALS: Survey of all 171 acute hospitals in the Unite

BACKGROUND: Femoral nerve injury causes knee dysfunction, and high femoral nerve injury is difficult to repair.
OBJECTIVE: To evaluate the anatomic feasibility of transferring the sciatic nerve motor branches in high femoral nerve injury.
METHODS: The femoral nerve was exposed in both lower extremities of 3 adult fresh-frozen cadavers; each branch was noninvasively dissected to its proximal nerve fiber intersection point and distal muscle entry point. The branches of the sciatic nerve were also exposed. The length, diameter, and number of myelinated fibers were measured in each femoral and s

CASE: A 7-year-old boy presented with left femoral and obturator nerves (ONs) palsy after an asthmatic attack with a viral prodrome, and his right lower limb was unaffected. He was diagnosed with acute flaccid myelitis (AFM) after positive spinal magnetic resonance imaging findings. After contralateral ON to femoral nerve transfer (CONFNT), his left quadriceps was reinnervated at 5.5 months, full knee extension was recovered at 14 months, and good functional outcomes were achieved at 31 months.
CONCLUSIONS: This first clinical report on CONFNT demonstrated a feasible good alternative in treat

BACKGROUND: Anterior branch of the obturator nerve transfer has been proven as an effective method for femoral nerve injuries, but the patient still has difficulty in rising and squatting, up and downstairs. Here, we presented a novel neurotization procedure of selectively repairing 3 motor branches of the femoral nerve by transferring motor branches of the obturator nerve in the thigh level and assessing its anatomical feasibility.
METHODS: Eight adult cadavers (16 thighs) were dissected. The nerve overlap distance between the gracilis branch and the rectus femoris (RF) branch, the adductor

Animal studies are essential to biomedical research and the cornerstone is a reproducible animal model. While there are many reports on rodent peripheral nerve injury models, a large animal model is essential to confirm the effects of nerve regeneration over the longer distances of regeneration required in humans. Swine have long been used as a large animal model for other surgical and biomedical studies. This paper represents a novel neurovascular injury model in the Sus scrofa domesticus swine (American Yorkshire pig). This paper will describe our experience and recommendations with pre-oper

Peripheral nerve cuff electrodes (NCEs) in motor system neuroprostheses can generate strong muscle contractions and enhance surgical efficiency by accessing multiple muscles from a single proximal location. Predicting chronic performance of high contact density NCEs based on intraoperative observations would facilitate implantation at locations that maximize selective recruitment, immediate connection of optimal contacts to implanted pulse generators (IPGs) with limited output channels, and initiation of postoperative rehabilitation as soon as possible after surgery. However, the stability of

Sciatic nerve injuries cause debilitating functional impairment, particularly when the injury mechanism and level preclude reconstruction with primary grafting. The purpose of this study was to demonstrate the anatomical feasibility of nerve transfers from the distal femoral nerve terminal branches to the tibial nerve and to detail the successful restoration of tibial function using the described nerve transfers. Six cadaveric legs were dissected for anatomical analysis and the development of tension-free nerve transfers from femoral nerve branches to the tibial nerve. In 2 patients with compl

Femoral nerve lesions are uncommon, but very distressing at the functional level because of the absence of knee locking mechanism by the quadriceps muscle. We propose here a new neurotization procedure of obturator nerve motor branches to the motor portion of the femoral nerve in the thigh. This study was conducted on five cadavers. The motor portion of the femoral nerve and the motor branches of the obturator nerve, supplying the gracilis and adductor longus muscles, were isolated. The distance between nerve endings and diameter were measured to determine if a direct neurorrhaphy was possible

The treatment of peripheral nerve injuries with nerve gaps largely consists of autologous nerve grafting utilizing sensory nerve donors. Underlying this clinical practice is the assumption that sensory autografts provide a suitable substrate for motoneuron regeneration, thereby facilitating motor endplate reinnervation and functional recovery. This study examined the role of nerve graft modality on axonal regeneration, comparing motor nerve regeneration through motor, sensory, and mixed nerve isografts in the Lewis rat. A total of 100 rats underwent grafting of the motor or sensory branch of t

The influence of chronic hypotension on density and fluorescence intensity (after glyoxylic acid treatment) of periarterial plexus of nerve fibers in saphenous artery wall and on reinnervation of this vessel was studied in Wistar rats. Regional hypotension in the rat hindquarters vascular system was created by partial occlusion of abdominal aorta distally to the renal arteries. Four weeks later, the segment of femoral nerve was resected in one of the limbs to denervate the saphenous artery. In the limb with intact nerve, chronic (6-7 weeks-long) hypotension resulted in 20% decrease of fluoresc

STUDY DESIGN: A retrospective analysis of a case series was performed.
OBJECTIVE: To describe a novel technique to monitor femoral nerve function by analyzing the saphenous nerve somatosensory evoked potential (SSEP) during transpsoas surgical exposures of the lumbar spine.
SUMMARY OF BACKGROUND DATA: During transpsoas direct lateral approaches to the lumbar spine, electromyography monitoring is frequently advocated; however, sensory and motor neurological complications are still being reported. Femoral nerve injury remains a feared complication at the L3-L4 and L4-L5 levels. The current neu

Meralgia paresthetica (MP) is a mononeuropathy caused by compression of the lateral femoral cutaneous nerve (LFCN), producing pain, numbness, and paresthesia over the anterolateral thigh. While MP is commonly due to benign compressive or metabolic causes, it may rarely result from structural lesions such as LFCN schwannomas. We report a 50-year-old woman with a one-month history of right lower quadrant abdominal pain and anterolateral thigh numbness, followed by cramping pain extending into the right leg. She also described a burning sensation in both soles. Examination revealed right iliac fo

BACKGROUND Psoas quartus is a rare anatomical variant of the psoas major muscle that can alter the course and branching of the femoral nerve as it descends from the posterior abdominal wall. Understanding its anatomical implications is crucial for clinicians and surgeons. CASE REPORT During a routine cadaveric dissection, we identified a unilateral psoas quartus muscle on the left side. This muscle was situated between the psoas major, psoas minor, and iliacus, originating from the transverse process of L5 and the quadratus lumborum. The femoral nerve exhibited an unusual branching pattern as

Proximal femoral fractures are frequent injuries that typically require surgical treatment. A rare complication is iatrogenic injury to the sciatic nerve, often caused by cerclages or suboptimally reduced fracture fragments. Our retrospective study of 224 cases in 39 months revealed an incidence of 1.8% for postoperative sensory motor deficits. The treatment includes revision surgery, such as neurolysis or improved reduction of fracture fragments. Despite surgical revision, long-term deficits often persist due to slow nerve regeneration. A precise operative technique and early diagnostic work-

BACKGROUND: Femoral nerve palsy can cause loss in quadriceps function and knee extension disability, which may lead to severe lower extremity impairment. The obturator nerve trunk transfer in the pelvic, the obturator nerve mortal branches transfer out of the pelvic, along with nerve graft, was introduced years ago to restore femoral nerve function. However, the outcomes of these procedures have never been compared. The aims of this study were to give our experiences in surgical reconstruction for femoral nerve injury and to compare the outcomes of different approaches.
METHODS: Nine patients

Autologous skin grafting from the thigh is frequently required for treatment of burns and is associated with intense pain at the donor site. Local anesthetic-based (LA) nerve blocks of the lateral femoral cutaneous nerve (LFCN) have been demonstrated to provide analgesia when the graft is taken from the lateral thigh. However, the duration of these single injection blocks has been reported to average only 9 hours, whereas the pain from the procedure lasts days or weeks. Continuous LA nerve blocks can also be used to provide analgesia during serial debridement of burns, although this requires p
