The changing pattern of fever of unknown origin in the Republic of North Macedonia.

INTRODUCTION: The study aimed to compare the etiologic spectrum of diseases causing fever of unknown origin (FUO) and methods for definitive diagnosis in a tertiary care hospital in the Republic of North Macedonia during two different time periods. PATIENTS AND METHODS: There were analysed retrospectively the causes for FUO and final diagnostic approaches in 185 patients with classic FUO that were treated at the University Hospital for Infectious Diseases in Skopje during two time periods. Seventy nine patients were treated during 1991 to 1995 and 106 patients during 2011 to 2015. RESULTS: W

Mycoplasma Pneumonia: An Unrecognized Cause of Fever of Unknown Origin in an Adult.

A 26-year-old female was admitted for fever of unknown origin (FUO), headaches, left ankle edema, and a lower extremity rash consistent with erythema nodosum. She had no respiratory symptoms or family history of autoimmune diseases. A chest X-ray was negative for pneumonia or hilar adenopathy. Extensive autoimmune workup was negative. A chest, abdomen, and pelvis computed tomography scan was unremarkable and laboratory studies revealed no source of infection. On hospital day 5, the patient developed a mild productive cough. Her Mycoplasma pneumonia (MP) IgM was high, confirming the diagnosis o

[A patient with fever of unknown origin - a common cause with an atypical presentation].

Fever of unknown origin (FUO) is a common medical diagnosis by exclusion. In these cases, fever is the predominant symptom of an underlying disease. We describe the case of a 60-year old patient with FUO. Intensive search for the causative disease was carried out. Unfortunately all the investigations remained fruitless. Eventually, the patient was discharged with the diagnosis of common variable immunodeficiency, based on hypogammaglobulinemia and Cytomegalovirus replication. Two weeks after discharge, the patient presented in the outpatient clinic with the typical symptoms of giant cell arter

An unusual cause for fever of unknown origin in a patient with a prosthetic valve.

A case of Wegener granulomatosis as a cause for fever of unknown origin (FUO) and secondary involvement of endocardium in association with mitral valve prosthesis is presented. Although the patient was referred as a case of unresolving pneumonia, her unresponsiveness to broad spectrum antibiotics including infective endocarditis treatment prompted investigation for a non-infectious aetiology for her FUO, leading to a diagnosis of Wegener granulomatosis.

[Tuberculosis as a cause of fever of unknown origin in the elderly: a case report].

In the last decade, the incidence of tubercular disease in Italy has increased. The age-groups 20-39 and 65-80 years are the ones mainly involved. However, compared to younger age-groups, the presentation of the disease in the elderly is often poorly characteristic, and for this reason a correct diagnosis is often delayed. We describe the case of an elderly man who came to our observation for the persistence of fever (since 20 days, mainly in the evening) associated with decreased leukocyte and platelet counts and with progressively worsening of his general conditions. The laboratory data and

Children with fever of unknown origin in Argentina: an analysis of 113 cases.

The aim of this study was to determine the causes of fever of unknown origin, to evaluate new diagnostic tests and to elucidate risk factors for chronic or life-threatening disorders. The medical records of 113 children who had undiagnosed fever for at least 3 weeks were reviewed. Infection (N = 41) was the most frequent cause of fever of unknown origin. Respiratory tract infections were the most common causes in infants and endocarditis and tuberculosis were more frequent in older children. Neoplastic disorders (N = 11) occurred in children older than one year. Juvenile rheumatoid arthritis (

Pathogenesis of fever of unknown origin in patients with acute leukemia and granulocytopenia.

Interleukin-1 (IL-1) is an endogenous pyrogen produced by blood monocytes or tissue macrophages in response to infection. Since hepatic macrophages (Kupffer cells) make up more than 90% of tissue macrophages and have a possibility of remaining intact after cytotoxic chemotherapy, they may be the major producers of IL-1 in infection during prolonged periods of severe monocytopenia in leukemic patients having modern chemotherapy treatment. Hence, the majority of febrile episodes probably due to infection in patients with acute leukemia and granulocytopenia may be caused by an inflammatory respon

Computed tomography of the abdomen in evaluation of patients with fever of unknown origin.

Applying Petersdorf's classic criteria for fever of unknown origin (FUO), 75 of 6,654 CT scans were reviewed to evaluate the diagnostic effectiveness of abdominal CT. Twenty-three (29%) of these 78 scans had positive CT findings and 8 (10%) had positive but nonspecific findings. Abdominal exploration or biopsy was carried out in 20 of these 31 patients. In addition, 10 cases with negative CT results were explored. In no case was any gross anatomic abnormality found in these negative cases.

Kikuchi Disease With Marfanoid Features in a Background of Fever of Unknown Origin.

Kikuchi-Fujimoto disease (KFD) is a self‑limiting necrotizing lymphadenitis that usually affects young women. It can mimic infection, malignancy, or autoimmune disease, which often leads to diagnostic delays. Marfanoid habitus encompasses a constellation of physical traits associated with Marfan syndrome: tall stature, disproportionate limbs, arachnodactyly, joint hypermobility, and chest wall deformities. Co-existence of KFD and prominent marfanoid features has not previously been reported. Based on a structured PubMed/MEDLINE search from database inception through January 2026 using MeSH ter

Pulmonary and Urologic Sarcoidosis as a Cause of Intermittent Fever of Unknown Origin.

Diagnosing fever of unknown origin (FUO) presents a substantial challenge due to its potential association with various diseases affecting different organs. In 1961, Petersdorf and Beeson initially defined FUO as a condition characterized by a temperature exceeding 38.3 °C on at least three occasions over a minimum three-week period. Despite a week of inpatient investigation, a definitive diagnosis remains unclear.  Sarcoidosis, a granulomatous disease impacting multiple systems, is among the causes of FUO. While the lungs are commonly affected, any organ can be involved, leading to diverse ma

Serological investigation of orthohantaviruses in patients with fever of unknown origin in Kazakhstan.

OBJECTIVE: Orthohantaviruses are geographically widely distributed and present various clinical manifestations from mild symptoms to the severe form of haemorrhagic fever with renal syndrome (HFRS) in Eurasia. Official registration of HFRS in Kazakhstan started in the year 2000. However, the true prevalence of human infections by orthohantaviruses within Kazakhstan is unknown. The aim of this study was to investigate of the seroprevalence of orthohantavirus infections in patients with fever of unknown origin (FUO) in two regions, Almaty and Kyzylorda region. METHODS: Paired serum samples from

Recent trends in the distribution of causative diseases of fever of unknown origin.

Fever of unknown origin is a challenging diagnostic problem and the aim of this research was to analyze trends in the distribution of its causative diseases. This retrospective study makes a comparison between two different clinical series of patients from two different periods: 227 from period 1 (1998-2002) and 602 from period 2 (2008-2012). There were fewer infections (31.72% vs.16.45%) and more miscellaneous causes (5.29% vs. 13.12%) in the period 2 series, whereas no significant differences in autoimmune diseases, malignancies and undiagnosed cases were found. Adult onset Still's disease a

Fever of unknown origin and the role of Plasmodium vivax in Sao Paulo.

Malaria is a disease that hits the northern part of Brazil in an endemic form. Recorded cases outside this region are unusual and, in most cases, limited to travellers. The southeast of Brazil has an extensive wilderness area with a type of vegetation similar to the one found in the equatorial rainforest. We present the case of a patient living in the southeast, with no reported travel history, who evolved to periodic fever, associated with myalgia and chills. With the positive test result for Plasmodium, the treatment for malaria was started. This case enabled the study in the Anopheles mosqu

Outcomes in patients with an emergency department diagnosis of fever of unknown origin.

OBJECTIVE: To describe the outcomes in patients given an ED diagnosis of fever of unknown origin (FUO). METHODS: A retrospective analysis of ED records linked to hospital morbidity, mortality and microbiology records of patients presenting to Western Australia's teaching hospitals from July 2000 to July 2003. RESULTS: There were 3218 presentations diagnosed with FUO, 2049 (63.7%) children (median age 1.8 years) and 1169 (36.3%) adults (median age 56.0 years). FUO accounted for 0.3% of adult and 1.5% of paediatric ED presentations. Overall, 1997 (62.1%, 95% confidence interval 60.4-63.8%) wer

Fever management in children and insights into fever of unknown origin: a survey among Italian pediatricians.

BACKGROUND: Fever is a common symptom in children, but despite existing guidelines, pediatricians may not fully apply recommendations. Fever of Unknown Origin (FUO) is generally referred to as an unexplained prolonged fever. However, a standardized FUO definition and management is missing. OBJECTIVE: To collect updated data on the approach to fever and FUO among Italian pediatricians. METHODS: A cross-sectional anonymous survey was conducted among a large sample of primary care and hospital pediatricians. The panel group formulated and proposed a practical FUO definition, using a modified De
