Sexual Identity Fluidity Among Sexual Minorities: Gender Differences in Determinants of Fluidity and Health-Related Outcomes.

Drawing upon the Generations Study, a longitudinal U.S. national probability survey, we examine sexual fluidity among 894 sexual minority adults over three waves of survey data. We find that plurisexual individuals (i.e. attracted to more than one gender) are more sexually fluid than are monosexual individuals, and those who hold more emergent sexual identities (e.g. asexual, same-gender loving, pansexual, and queer) are more sexually fluid than those with more traditional sexual identities (gay, bisexual, and lesbian). We also find that, although sexual minority men are less fluid than sexual

Effect of Intravenously Administered Crystalloid Solutions on Acid-Base Balance in Domestic Animals.

Intravenous fluid therapy can alter plasma acid-base balance. The Stewart approach to acid-base balance is uniquely suited to identify and quantify the effects of the cationic and anionic constituents of crystalloid solutions on plasma pH. The plasma strong ion difference (SID) and weak acid concentrations are similar to those of the administered fluid, more so at higher administration rates and with larger volumes. A crystalloid's in vivo effects on plasma pH are described by 3 general rules: SID > [HCO3-] increases plasma pH (alkalosis); SID < [HCO3-] decreases plasma pH (alkalosis); and SID

Acute postburn edema: role of strongly negative interstitial fluid pressure.

Interstitial fluid hydrostatic pressure (Pif) was measured with micropipettes during the acute edema generation that followed thermal skin injury in rats. Intradermal Pif was reduced from normal level of -1 mmHg to very negative values after thermal injury. The strongly negative Pif reflects a tissue imbibition pressure created by the thermal injury. The magnitude and duration of this pressure was dependent on the extent of the injury and the availability of fluid. After in vivo injury to 10 and 40% of the total body surface area (TBSA), mean intradermal Pif was temporarily reduced to -20 and

A Fluid Challenge Test for the Diagnosis of Occult Heart Failure.

A right heart catheterization with measurements of pulmonary artery wedge pressure (PAWP) may be necessary for the diagnosis of left heart failure as a cause of pulmonary hypertension or unexplained dyspnea. Diagnostic cutoff values are a PAWP of ≥ 15 mm Hg at rest or a PAWP of ≥ 25 mm Hg during exercise. However, accurate measurement of PAWP can be challenging and heart failure may be occult. Left heart catheterization, with measurement of left ventricular end-diastolic pressure, may also be indecisive. Measurements are then best repeated in stress conditions. Exercise is an option, but the e

Ultrafiltration for management of fluid overload in patients with heart failure.

Heart failure is the number one cause of death in the United States and a significant burden to the healthcare system. One of the primary complications of heart failure is fluid overload, for which current treatments are limited. Medical therapy is first-line; however, rates of diuretic insensitivity are high, medications are not easily titrated, and they do not address the underlying physiologic derangement that leads to hypervolemia. Removal of isotonic fluid via hemofiltration and peritoneal dialysis is an understudied but promising therapy that enables decongestion without maladaptive stim

Prevalence of obstructive sleep apnea syndrome in patients with lymphedema referred for complete decongestive therapy.

OBJECTIVE: Obstructive sleep apnea (OSA) syndrome is one of the most frequent chronic diseases in the general population. The nocturnal rostral fluid shift is accepted as a key mechanism in OSA pathogenesis in medical conditions associated with fluid overload. The main objective of this study was to assess the prevalence of OSA in patients with lymphedema. METHODS: A prospective study was conducted in patients with lymphedema hospitalized for complete decongestive therapy between December 2016 and November 2017. A nocturnal respiratory polygraphy, an Epworth sleepiness scale, and lymphedema v

Extracorporeal ultrafiltration for acute heart failure: patient selection and perspectives.

Most patients presenting with acute heart failure (AHF) show signs and symptoms of fluid overload, which are closely associated with short-term and long-term outcomes. Ultrafiltration is an extremely appealing strategy for patients with AHF and concomitant overt fluid overload not fully responsive to diuretic therapy. However, although there are several theoretical beneficial effects associated with ultrafiltration, published reports have shown controversial findings. Differences in selection of the study population and in ultrafiltration indications and protocols, and high variability in the

Reorganized neural activation in motor cortex following subdural fluid collection: an fMRI and DTI study.

We report a patient with a cavernous malformation involving the right lentiform nucleus. Pre-surgical planning included fMRI localization of language, motor, and sensory processing, and DTI of white matter tracts. fMRI results revealed no activation near the planned resection zone. However, post-surgery the patient developed a subdural fluid collection, which applied pressure to the primary motor cortex (M1). Follow-up scans revealed that motor activation had shifted due to pressure, and then shifted to a new location after the fluid collection subsided. This case report suggests that long-ter

Central pontine myelinolysis: sequele of hyponatremia during transcervical resection of endometrium.

Transcervical resection of endometrium is an alternative to hysterectomy for women with menorrhagia. The procedure involves the use of cutting loop diathermy to resect the endometrium while the uterine cavity is irrigated with 1.5% glycine which can absorb consequent fluid and electrolyte shifts. Severe hyponatremia leading to central pontine myelinolysis is an extremely rare complication of this procedure. We report a case of a young female undergoing transcervical resection of endometrium for menorrhagia, who developed central pontine myelinolysis but made a complete recovery after three mon

Association of Sleep Problems With Dialysis Shifts in Patients Undergoing Hemodialysis in Tbilisi, Georgia.

Introduction According to a 2023 poll by the International Society of Nephrology, 850 million individuals worldwide suffer from chronic kidney disease (CKD) and hemodialysis (HD) is the primary treatment for 69% of the patients with CKD. While HD effectively regulates fluid balance and electrolyte levels, patients often face challenges such as weakness, exhaustion, and cognitive changes, which impact their quality of life. Sleep-related issues, including poor quality, excessive morning sleepiness, insomnia, and restless leg syndrome (RLS), are particularly common among HD patients. These distu

A Systematic Review: Is Early Fluid Restriction in Preterm Neonates Going to Prevent Bronchopulmonary Dysplasia?

Preterm birth causes constant challenges, with bronchopulmonary dysplasia (BPD) being a major concern. Immediately after birth, it takes time to establish feeding between the mother and the premature baby. During this time, the telological shifting of fluid from extracellular space to intracellular space will help the baby; this transition should be smooth. Both normal physiologic changes and pathophysiologic events are capable of disrupting this delicate fluid shifting that occurs in very low-birth-weight infants during the first week of life. The immaturity of the renal system and evaporativ

Carpal tunnel syndrome in pregnancy.

During pregnancy, hormonal fluctuations, fluid shifts, and musculoskeletal changes predispose women to carpal tunnel syndrome. While the clinical presentation is similar to other patients, the history obtained must include information regarding the pregnancy itself. Currently, the indication for electrodiagnostic testing is not clearly defined. Given that symptoms often improve with conservative treatment and abate after delivery, EMG/NCV testing can often be avoided. However, if symptoms are severe or persist, carpal tunnel release is indicated and is considered a safe procedure for both moth

[Electrolyte composition of the cerebrospinal fluid and neuromuscular excitability in epilepsy].

Clinical (Chvostek symptom and Trousseau-Bonsdorf test) and electromyographical investigations of the neuromuscular excitability were performed in patients with different forms of epilepsy. Ionized Ca, Na, K, Cl, and total Mg were measured in the cerebrospinal fluid (CSF). Seventy-five percent of the patients showed clinical and electromyographic signs of the tetanic syndrome. In patients with general seizures the CSF ionized Ca content was decreased as related to that of normal subjects. Brain and neuromuscular excitability increase was related with the shifts in Ca metabolism.

Hysteresis Phenomenon Coupled with Capillary Phase Transition of N2/CH4 Binary Mixtures in Cylindrical Nanopores.

Capillary condensation/evaporation-induced hysteresis is a fundamental characteristic of confined fluid-phase transitions. Although extensively studied for pure fluids, the hysteresis behavior of confined fluid mixtures remains poorly understood. Here, grand canonical Monte Carlo-molecular dynamics (GCMD) simulations are employed to investigate adsorption-desorption hysteresis of N2/CH4 mixtures in cylindrical carbon nanopores. While pure CH4 and N2 exhibit the expected pore-size- and temperature-dependent hysteresis behavior, binary mixtures show strongly composition-dependent phase transitio

Shared mechanisms in neuromyelitis optica spectrum disorder and spaceflight-associated neuro-ocular syndrome: Insights into central nervous system fluid dynamics, glymphatic function, and astrocyte dysregulation.

Spaceflight-Associated Neuro-ocular Syndrome (SANS) and Neuromyelitis Optica Spectrum Disorder (NMOSD) represent distinct neurological challenges with intriguing parallels in their disruption of central nervous system (CNS) fluid dynamics and the clinical neuro-ophthalmic manifestations. SANS, affecting astronauts during prolonged spaceflight, is characterized by optic disc edema, globe flattening, and vision changes resulting from microgravity-induced cephalad fluid shifts. NMOSD, an autoimmune astrocytopathy, is driven by aquaporin-4 (AQP4) autoantibodies that compromise astrocytic water reg

Peritoneal dialysis as therapy for electrolyte and acid base disorders.

Peritoneal dialysis (PD) does not demand special equipment and its fluid composition can be easily changed according to the individual condition. Nine patients with chronic or acute renal failure presented severe metabolic alkalosis (MA). Hemodialysis (HD) proved virtually ineffective and the MA persisted. Physiological saline solution was adopted as the main component of the PD fluid for the treatment of MA. By this method, Cl- can be shifted from PD fluid to extracellular fluid (ECF) and HCO3- from ECF to PD fluid by ionic gradient. Therefore, pH and base excess (BE) of these patients both i

Current aspects of perioperative fluid handling in vascular surgery.

PURPOSE OF REVIEW: Perioperative fluid management influences patient outcome. Vascular surgery unites various surgical procedures, mainly with a high impact on patients who often have relevant preexisting illnesses. There are only scarce data on this specialty, forcing the clinician to extrapolate existing data when planning perioperative fluid management. This review aims to summarize the underlying facts. RECENT FINDINGS: Perioperative insensible perspiration does not exceed 1 ml/kg per hour. A third space shift does not exist; therefore, its primary substitution is erroneous. Rather, a cry

Integrated haemodynamic monitoring of fluid responsiveness and fluid tolerance in critical care: from physiology to bedside applications.

The framework of fluid responsiveness addresses only whether volume expansion will augment cardiac output, not whether the circulation can accept that volume without producing congestive organ injury. The complementary concept of fluid tolerance has accumulated empirical support over recent years and is integral to understanding patients' fluid status. This narrative review synthesises the physiological basis of fluid responsiveness and fluid tolerance, describes the bedside monitoring tools available to assess each, and considers the clinical implications of their joint interpretation in crit

[Retinitis pigmentosa with uveal effusion syndrome, a case report].

We reported a 65 year old female patient with retinitis pigmentosa accompanied by unilateral uveal effusion syndrome. Reduction in visual acuity and loss of depth of the anterior chamber were followed by peripheral choroidal detachment and then by retinal detachment. Subretinal fluid shifted to the lower part of the fundus for a short period and easily shifted to the posterior pole in a supine position. The axial length of both eyes was 21.3 mm. Fluorescein angiography revealed no definite points of leakage or abnormally dilated vessels. Chorioretinal detachment gradually disappeared with syst

Dynamic changes in perivascular space morphology predict signs of spaceflight-associated neuro-ocular syndrome in bed rest.

During long-duration spaceflight, astronauts experience headward fluid shifts and expansion of the cerebral perivascular spaces (PVS). A major limitation to our understanding of the changes in brain structure and physiology induced by spaceflight stems from the logistical difficulties of studying astronauts. The current study aimed to determine whether PVS changes also occur on Earth with the spaceflight analog head-down tilt bed rest (HDBR). We examined how the number and morphology of magnetic resonance imaging-visible PVS (MV-PVS) are affected by HDBR with and without elevated carbon dioxid
