Spinal compression fractures in osteoporotic women: patterns and relationship to hyperkyphosis.

A prospective study of thoracic and lumbar spinal fracture distribution and its relationship to thoracic kyphosis was performed in 87 women with osteoporosis. Anterior wedge fractures were most commonly seen in the midthoracic spine and about the thoracolumbar junction, whereas central compression fractures were most common from the first to the fourth lumbar levels. Solitary wedge fractures did not occur above the seventh thoracic vertebra, suggesting that a cause other than osteoporosis must be suspected in any patient with an isolated high thoracic fracture. Analysis of the spinal radiograp

Clinical evaluation of the sliding compression screw in 121 hip fractures.

Between August 1975 and June 1978, the orthopedic residents at the Jacksonville Health Education Program have used the sliding compression screw to treat 135 hip fractures. The 121 of these being reported include 27 subcapital and femoral neck fractures, 88 intertrochanteric fractures, and six subtrochanteric fractures. Patients' ages ranged from 23 to 98 years, with a mean age of 64. Accurate anatomic reduction of the fracture fragments, be it open or closed, and attention to detail in the placement of the compression screws were thought to be major factors favoring a good result. As confiden

Nonunion of Isolated Medial Cuneiform Fracture Fixed With a Compression Screw and Compression Staple: A Case Report.

Isolated cuneiform fractures are rare and account for only 1.7% of all midfoot fractures. Medial cuneiform fractures can be treated conservatively or surgically, with good clinical outcomes. However, nonunion is a rare complication of medial cuneiform fractures, and only a few cases have been reported in the literature. We report a case of a medial cuneiform fracture requiring surgical treatment that had a good clinical outcome. A 15-year-old boy presented to an orthopedic clinic with a complaint of pain in his right foot. The patient had landed on the foot during a handball game and was treat

Lumbar Spine Orientation Affects Compressive Fracture Outcome.

PURPOSE: Understanding how spinal orientation affects injury outcome is essential to understand lumbar injury biomechanics associated with high-rate vertical loading. METHODS: Whole-column human lumbar spines (T12-L5) were dynamically loaded using a drop tower to simulate peak axial forces associated with high-speed aircraft ejections and helicopter crashes. Spines were allowed to maintain natural lordotic curvature for loading, resulting in a range of orientations. Pre-test X-rays were used to quantify specimen orientation at the time of loading. Primary fracture types were identified (wedge

Effect of compression on mandibular fracture haematoma-derived cells.

Mechanical stress induces a variety of biochemical and morphological reactions in bone cell biology. This study aimed to investigate appropriate pressures of osteogenesis on the biological responses of 3-dimensional cultured human mandibular fracture haematoma-derived cells by compressive loading. Six patients with mandibular fractures who underwent open reduction and internal fixation were included in the study. During the operation, fracture haematomas that formed fibrin clots were manually removed before irrigation. First, pressures were applied to human mandibular fracture haematoma-derive

Percutaneous vertebroplasty-induced adjacent vertebral compression fracture.

BACKGROUND: The risks associated with percutaneous vertebroplasty (PV) are low. Patients show marked improvement and are able to rapidly resume normal activities after PV. The sudden development of postoperative vertebral compression fracture (VCF) is a common complication, and additional PV is frequently performed in these cases. However, there have been no studies reporting acute compression fractures of an adjacent vertebra immediately after PV. OBJECTIVE: This case report presents a rare case in which the patient had to undergo a second PV because of PV-induced adjacent VCF. Further, we r

Lordoplasty: an alternative technique for the treatment of osteoporotic compression fracture.

We report here on a new technique using polymethylmethacrylate to manage vertebral osteoporotic compression fractures in three patients. These patients presented with acute back pain that manifested itself after minor trauma. Osteoporotic compression fractures were diagnosed via plain X-ray and magnetic resonance imaging studies. The patients were treated with absolute bed rest and non-steroidal anti-inflammatory drugs. Despite of the conservative treatment, the patients experienced severe, recalcitrant and progressive pain. The vertebrae were collapsed over 50% or kyphotic deformity was seen

Sacral fracture with compression of cauda equina: surgical treatment.

Fractures of the sacrum are rare and generally accompany fractures of the pelvis. Isolated transverse sacral fractures are even less frequent, and extensive neurologic deficits may accompany these injuries. This report describes an unusual case of extradural hemorrhage accompanying a complex fracture of the sacrum. The reversal of a serious neurologic deficit was notably aided by sacral laminectomy.

Compression of the suprascapular nerve after fracture of the scapular notch.

A 32-year-old man with a fracture of the scapular notch associated with a lesion of the suprascapular nerve is reported. A nerve decompression operation was performed 20 months after the injury with relief of pain. The possibility that residual disability following a fracture of the scapula can be due to a lesion of the suprascapular nerve is underlined. The value of X-ray examination with projections visualizing the notch and diagnostic local anaesthetic block of the nerve passing through the notch are emphasized. Primary wide resection of the scapular notch is a preferable procedure preventi

Application of a lateral compression clamp in the management of mandibular fractures.

A lateral compression clamp system has been used in the treatment of fractures of the mandible. It provides rigid fixation and promotes earlier osteogenesis at the fracture site. Intermaxillary fixation can often be eliminated in edentulous persons and greatly reduced in duration when necessary as an adjunct to fixation in patients with teeth. These significant qualities make the compression clamp quite beneficial in the treatment plan and postoperative management of many otherwise difficult cases. Having evaluated our experiences with compression clamps, we believe that this approach deserves

Mechanics of dynamic compression plate application in fracture fixation.

BACKGROUND: Dynamic compression plating is a fundamental type of bone fracture fixation used to generate interfragmentary compression. The goal of this study was to investigate the mechanics of the surgical application of these plates, specifically how plate prebend, screw location, fracture gap, and applied torque influence the resulting compressive pressures. METHODS: Synthetic bones with transverse fractures were fixed with locking compression plates. One side of the fracture was fixed with locking screws. On the other side of the fracture, a nonlocking screw was inserted eccentrically to

Balloon kyphoplasty for the treatment of pathological vertebral compressive fractures.

BACKGROUND: Previous clinical studies have shown the safety and effectiveness of balloon kyphoplasty in the treatment of pathological vertebral compression fractures (VCFs). However, they have not dealt with the impact of relatively common comorbid conditions in this age group, such as spinal stenosis, and they have not explicitly addressed the use of imaging as a prognostic indicator for the restoration of vertebral body height. Neither have these studies dealt with management and technical problems related to surgery, nor the effectiveness of bone biopsy during the same surgical procedure. T

[Osteosynthesis of trochanteric fracture with percutaneus compression plate (PC.C.P.)].

In terms of frequency, trochanteric fractures represent one of the most common problems in adult traumatology. Considering their physical and psychological consequences, and the patients mean age, they constitute one of the most acute public health problems faced by the elderly. Surgical reduction is currently considered as preferred treatment. Stable fixation of the fracture focus by osteosynthesis allows early mobilization of the treated member, central axis of the body. A series of 35 patients with a mean age of 78, surgically treated with Gotfried percutaneous plating between June 1998 and

Osteoporotic vertebral compression fracture causing neurologic deficit.

Osteoporotic vertebral fractures rarely cause neurologic deficits. We discuss a 69-year-old man who presented with the signs of impaired function of the left L5 nerve root. Imaging revealed a fracture of the posterior upper edge of the fifth lumbar vertebra with a bony fragment protruding into the spinal canal. The fracture was attributed to sneezing and coughing in this man with osteoporosis secondary to hypogonadism. Literature search revealed 64 cases of neurologic deficit associated with osteoporotic vertebral fracture. Most osteoporotic fractures with neurologic deficit (72%) occur at the
