The impact of smoking on surgical complications after head and neck reconstructive surgery with a free vascularised tissue flap: a systematic review and meta-analysis.

Reconstructive surgery with a free vascularised tissue flap is indicated in large defects in the head and neck region, which arise mostly because of head and neck cancer. Tobacco smoking is a major risk factor for head and neck cancer, and many patients undergoing reconstructive surgery in the head and neck have a history of smoking. The objective of this meta-analysis was to determine the impact of smoking on surgical complications after head and neck reconstructive surgery with a free vascularised tissue flap. A systematic review was undertaken for articles reporting and comparing the incide

Reconstruction of Extensive Soft-Tissue Defects with Concomitant Bone Defects in the Lower Extremity with the Latissimus Dorsi-Serratus Anterior-Rib Free Flap.

BACKGROUND: The combined latissimus dorsi-serratus anterior-rib (LD-SA-rib) free flap provides a large soft-tissue flap with a vascularized bone flap through a solitary vascular pedicle in a one-stage reconstruction. METHODS: Seven LD-SA-rib free flaps were performed in seven patients to reconstruct concomitant bone and extensive soft-tissue defects in the lower extremity (tibia, five; femur, one; foot, one). The patients were all male, with an average age of 34 years (range, 20-48 years). These defects were secondary to trauma in five patients and posttraumatic osteomyelitis in two patients.

Repair of a large soft tissue defect in the leg with cross-leg bridge free transfer of a latissimus dorsi myocutaneous flap: a case report.

A 48-year-old man sustained a traffic accident injury to his left leg. It was an open fracture of the left tibia and fibula accompanied by a large soft tissue defect (27 cm multiply 7 cm). Doppler examination revealed the posterior tibial artery was occluded due to thrombosis. Three weeks after injury, the latissimus dors myocutaneous flap was elevated with a T-shaped vascular pedicle and was interposed between the two vascular ends of the posterior tibial vessel of the contralateral leg. Two end to end anastomoses were performed between the two vascular ends of the posterior tibial vessel of

Widespread cutaneous angiosarcoma of the scalp: diagnosis and soft tissue reconstruction with a combined double-muscle-free flap and split-thickness-skin graft.

BACKGROUND: Cutaneous angiosarcoma is a rare vascular tumor, which is usually located in the head and neck region. Because of the unspecific appearance, diagnoses are only secured after a histological examination of a tissue sample. CASE REPORT: After three punch biopsies, a 67-year-old patient, who had been healthy so far, was diagnosed with a cutaneous multilocated angiosarcoma of the scalp. Computed tomography of the head, neck, thorax, and abdomen was done for staging. It was without pathological findings. The tumor removal caused a scalp defect with a size of 30 x 30 cm. The cover of the

One-Staged Simultaneous Soft Tissue and Metatarsal Reconstruction With Free Vascularized Bone Flaps for Traumatic Open Type III Foot Injury: A Clinical Series and Literature-Based Reconstructive Strategy.

BACKGROUND: Severe Gustilo-III foot injuries that combine segmental bone loss with extensive soft tissue damage remain challenging. One-stage reconstruction with a free fibular vascularized bone flap (FVBF) or free iliac vascularized bone flap (IVBF) offers the advantages of immediate skeletal stability, soft tissue resurfacing, and early mobilization, but contemporary outcome data are limited. METHODS: From December of 2000 to November 2021, 6 patients who sustained from severe traumatic metatarsal, tarsal bone, and soft tissue defects were treated with a FVBF or IVBF incorporating soft tiss

Microsurgical Reconstruction of Extensive Lower Limb Defects: Latissimus Dorsi Free Flap for Circumferential Soft Tissue Loss Following High-Energy Trauma.

Background/Objectives: High-energy trauma to the lower limb often results in extensive soft tissue loss with exposure of critical structures, posing a serious threat to limb viability. Early and effective coverage is crucial to prevent infection, promote bone healing, and preserve function. This report presents the use of a latissimus dorsi free flap for circumferential soft tissue reconstruction following a severe crush injury. Methods: We describe the case of a young female patient who sustained a high-energy crush trauma with a comminuted, displaced fracture of the middle and distal third o

Free Flap Surgery for Elbow Soft Tissue Reconstruction Using the Brachial Artery as Recipient Vessel: Evaluation of MPETS Cases and Comparative Literature Review.

Background and Objectives: The elbow joint, essential for daily activities, often requires soft tissue reconstruction following trauma, infection, or tumor excision. Free flap surgery using the brachial artery (BA) as the recipient vessel offers stable vascular support, but preserving distal blood flow is crucial. Due to vessel diameter differences, end-to-side (ETS) anastomosis is usually necessary, as flow-through anastomosis can be challenging. Although reports exist on soft tissue reconstruction using the BA as the recipient vessel, complications and outcomes related to using the sole main

Comparing the Clinical Effects of Free and Pedicled Flap in Repairing Small Area of Distal Lower Limb Soft Tissue Defects: A Retrospective Comparative Study.

BACKGROUND AND AIMS: The exposure of tendons, blood vessels, nerves, and bone due to soft tissue defects in the foot poses a significant challenge for microsurgeons. Free perforator flaps and pedicled perforator flaps are currently the most common methods used for repairing small areas of soft tissue defects in the distal lower limb. METHODS: A retrospective analysis was carried out on 124 patients with small soft tissue defects in the distal lower limb from January 2009 to December 2021. The evaluation criteria encompassed the patient's intraoperative condition, the occurrence of short-term

A case report of reconstruction of ocular and complete upper eyelid avulsion with severe facial soft tissue injuries using anterolateral thigh free flap.

INTRODUCTION AND IMPORTANCE: Ocular injury is second mostly caused by motor vehicle accident (MVA) and often leads to severe ocular injury even to visual loss and various aesthetic problems. The outcome is determined by the magnitude of the initial damage and treatment availability. Treating ocular and facial injury due to MVA is challenging given the scope of the damage and may result in various outcomes. CASE PRESENTATION: This case presented a 22-years old woman with a history of car accident assessed with total upper eyelid avulsion with corneal perforation and involvement of multiple fac

Supermicrosurgical free sensate superficial circumflex iliac artery perforator flap for reconstruction of a soft tissue defect of the ankle in a 1-year-old child.

Although a superficial circumflex iliac artery perforator (SCIP) flap has recently been widely used owing to its various advantages, reports on its use in the pediatric population are limited. A case of a supermicrosurgical reconstruction of a soft tissue defect of the ankle associated with the congenital deficiency of the tibia using a free sensate SCIP flap in a 1-year-old child has been presented. The correction of the valgus deformity of the ankle resulted in a soft tissue defect, which required flap coverage. The lateral cutaneous branch of the intercostal nerve of the flap was coapted wi

Reconstruction using a three-dimensional orbitozygomatic skeletal model of titanium mesh plate and soft-tissue free flap transfer following total maxillectomy.

The surgical strategy for maxillary reconstruction after maxillectomy has yet to be standardized. The authors developed a technique using a three-dimensional orbitozygomatic skeletal model of a titanium mesh for skeletal reconstruction after maxillectomy. From May of 1996 to September of 2000, 18 patients underwent reconstruction using the titanium mesh model in conjunction with a soft-tissue free flap following total maxillectomy for a maxillary malignancy. The soft-tissue free flap was conventional and consisted of two skin paddles to the maxillary defect. One skin paddle became the lateral

Incision-free, coronally advanced flap with subepithelial connective tissue graft placed by the molar or canine access (MOCA) technique: 13 case series.

INTRODUCTION: Root coverage procedures are not always predictable, and outcomes depend on several factors. This technique provides a predictable alternative to managing facial gingival recessions. CASE SERIES: A new grafting technique is introduced that requires no incisions at the recipient site, thereby preserving the integrity of the local blood supply to optimize the healing process. The graft is placed through the gingival sulcus via a molar or canine access (MOCA) approach, and there is minimal tension on the coronally advanced flap through use of suspension sutures. Remote peripheral tissue oxygenation does not predict postoperative free flap complications in complex head and neck cancer surgery: A prospective cohort study.

BACKGROUND: Patients undergoing free flap reconstruction after head and neck cancer may develop free flap complications. In the perioperative period, haemoglobin content and oxygen tissue saturation (StO2) measured directly on the free flap reflect peripheral tissue oxygenation. However, in this type of surgery, StO2 cannot always be applied directly or proximate to the free flap. The aim of this study was to assess the possible value of StO2 measured at the thenar eminence and other 24hour perioperative factors on free flap complications. METHODS: Inclusion criteria corresponded to patients
