New onset left frontal lobe seizure presenting with ictal asystole.

Ictal asystole is a presumably rare but potentially fatal complication of seizures, most often of temporal lobe origin. It is believed that at least some cases of sudden unexplained death in epilepsy (SUDEP) might be triggered by ictal bradycardia or asystole. Current standard practice is to implant a permanent pacemaker in these patients to prevent syncope and/or death. However, emerging data suggests that effective medical or surgical treatment of epilepsy might be enough to prevent cardiac asystole, eliminating the need for permanent pacemaker placement. We describe a case of new onset left

Frontal lobe dementia and subcortical vascular dementia: a neuropsychological comparison.

We compared the performance of 40 patients with frontal lobe dementia to that of 40 patients with subcortical vascular dementia (80 patients including, 46 men and 34 women) in a set of tasks assessing attentional, executive, and behavioural tasks. The frontal lobe dementia represents an important cause for degenerative disruption and is increasingly recognised as an important form (up to 25%) of degenerative dementia among individuals of late-middle-age. The main involvement is the frontal-subcortical pathway, which is the final target of impairment even in subcortical vascular dementia. A wid

Hereditary spastic paraplegia with frontal lobe dysfunction: a clinicopathologic study.

The authors report an unusual family with hereditary spastic paraplegia (HSP) with frontal lobe dysfunction having the onset in the sixth decade. All the patients showed hypoperfusion in the frontal lobes and thalami on SPECT. Neuropathologic findings revealed thin corpus callosum and degeneration in the thalamic dorsomedial nuclei as well as degeneration of the corticospinal tracts. This family was likely affected by a novel form of HSP characterized by frontal lobe dysfunction caused by thalamic degeneration.

Frontal lobe disinhibition, Rett syndrome and attention deficit hyperactivity disorder.

Motor cortex hyperexcitability in Rett Syndrome (RS) has been ascribed to the failure of an inhibiting system arising from the frontal lobes, a system needed in childhood and early adolescence. It is also presumed that, on a virtually dysfunctional (nonstructural) basis, motor cortex hyperexcitability in ADHD is also due to insufficient frontal lobe inhibition ("lazy frontal lobe"). A series of schematic illustrations is added for the sake of easier comprehension of the presented concept.

Frontal lobe degeneration: novel ubiquitin-immunoreactive neurites within frontotemporal cortex.

Ubiquitin-protein conjugates have been identified in filamentous inclusions in various neurodegenerative disorders. In frontal lobe degeneration (FLD) no distinctive histological features have been reported with the exception of some ubiquitin-positive intraneuronal inclusions in cases associated with motor neuron disease. In the present study, we investigated five FLD cases without motor neuron disease using immunohistochemistry. A constant feature in all cases consisted of ubiquitin-positive neurites in layers I-III of the frontotemporal neocortex. These neurites were not argyrophilic, and c

Autosomal dominant nocturnal frontal lobe epilepsy. A distinctive clinical disorder.

The disorder of autosomal dominant nocturnal frontal lobe epilepsy has recently been identified, and is now delineated in detail. A phenotypically homogeneous group of five families from Australia, Britain and Canada, containing 47 affected individuals, was studied. The largest family contained 25 affected individuals spanning six generations. This disorder is characterized by clusters of brief nocturnal motor seizures, with hyperkinetic or tonic manifestations. Subjects often experienced an aura, and remained aware throughout the attacks. Seizures occurred in clusters (mean eight attacks/nigh

Clinical picture of frontal lobe degeneration of non-Alzheimer type.

Frontal lobe degeneration of non-Alzheimer type (FLD) is the second most common primary degenerative dementia in southern Sweden. Clinical findings in 30 FLD cases with postmortem-verified diagnoses are described. FLD starts in the presenium with a mean disease duration of 7.5 years (range 3-17 years). Clinical onset is insidious and slow and the early stage is dominated by personality changes with lack of insight and judgement and signs of disinhibition. A typical feature is progressive loss of expressive speech with stereotyped phrases, late mutism and amimia. Restlessness, changes of oral/d

Deficits in strategy application following frontal lobe damage in man.

A quantitative investigation of the ability to carry out a variety of cognitive tasks was performed in 3 patients who had sustained traumatic injuries which involved prefrontal structures. All 3 had severe difficulties in 2 tests which required them to carry out a number of fairly simple but open-ended tasks over a 15-30 min period. They typically spent too long on individual tasks. All patients scored well on tests of perception, language and intelligence and 2 performed well on a variety of other tests of frontal lobe function. Explanations for their difficulty on the multiple subgoal tasks

Functional frontal lobectomy in the surgical treatment of pharmacoresistant frontal lobe epilepsy: how I do it.

BACKGROUND: Frontal lobe epilepsy is pharmacoresistant in 30% of cases, constituting 10-20% of epilepsy surgeries. For cases of no lesional epilepsy (negative MRI), frontal lobectomy is a crucial treatment, historically involving Frontal Anatomical Lobectomy (AFL) with a 33.3% complication risk and 55.7% seizure control. METHODS: We describe Frontal Functional Lobectomy (FFL), in which the boundaries are defined on the patient's functional cortico-subcortical areas, recognized with advanced intraoperative technologies such as tractography and navigated transcranial magnetic stimulation (nTMS)

The Connections of the Short Arcuate Fibers of the Frontal Lobe: An Anatomical Study.

BACKGROUND: We present the anatomy of the short arcuate fibers (SAF) of the brain. This anatomy is not very well depicted in literature. AIM: The aim was to study the connections of the short association fibers of the frontal lobe of the brain. MATERIAL AND METHODS: Five formalin-fixed frozen cadaveric brains were studied by using Klinger's method of white fiber dissection. The superficial gray matter was cored out and the connections of the SAF were studied. The SAF of each dissected frontal lobe were compared with the other specimens and the similarities and differences between them were r

Aging modulates frontal lobes involvement in emotion regulation processing.

Emotion regulation (ER) is the process by which individuals can modulate the intensity of their emotional experience and it plays a crucial role in daily life. So far, behavioral analyses seem to suggest that ER ability remains stable throughout the lifespan. However, imaging studies evaluating the neural correlates of ER performance during the aging process have shown mixed results. In this study, we used the "Cambridge Centre for Ageing and Neuroscience cohort sample" to investigate: (1) ER behavioral performance and (2) the differential association between brain measures (based on both stru

Learning the hierarchical organization of the frontal lobe with differential causal effects.

In this video article, accompanying the paper "An approach to learning the hierarchical organization of the frontal lobe", we discuss a data driven approach to learning brain connectivity. Hierarchical models of brain connectivity are useful to understand how the brain can process sensory information, make decisions, and perform other high-level tasks. Despite extensive research, understanding the structure of the prefrontal cortex (PFC) remains a crucial challenge. In this work, we propose an approach to studying brain signals and uncovering characteristics of the underlying neural circuity,

The alterations of degree centrality in the frontal lobe of patients with panic disorder.

Objective: The brain network in panic disorder (PD) is still an intriguing issue for research. In this study, we hoped to investigate the role of DC (degree centrality) for the pathophysiology of PD, especially for the fear network. Methods: We enrolled 60 patients with PD and 60 controls in the current study. The gender and age were matched for two groups. All participants received the resting-state functional magnetic resonance imaging to survey the baseline brain activity. Then the DC values of all participants were using REST toolbox. We also compared the DC values between PD and controls.

Differences in Frontal Lobe Dysfunction in Patients with Episodic and Chronic Migraine.

Neuroimaging and neuropsychological investigations have indicated that migraineurs exhibit frontal lobe-related cognitive impairment. We investigated whether orbitofrontal and dorsolateral functioning differed between individuals with episodic migraine (EM) and chronic migraine (CM), focusing on orbitofrontal dysfunction because it is implicated in migraine chronification and medication overuse headache (MOH) in migraineurs. This cross-sectional study recruited women with CM with/without MOH (CM + MOH, CM - MOH), EM, and control participants who were matched in terms of age and education. We c
