The European journal of esthetic dentistry : official journal of the European Academy of Esthetic Dentistry2009eng
The aim of this two-part treatment series is on the one hand to emphasize the difficulties a clinician is confronted with when planning complex cases, and on the other hand to reveal the rationale supporting the final treatment plan selection. Among the challenging cases to be considered are periodontal compromised rest dentitions requiring prosthodontic rehabilitation. For these patients the decision-making process deals with the indications and limitations of both the fields of fixed and removable prosthodontics. The first part of this article deals with the various prosthodontic treatment o

AIM: The objective of this literature review and subsequent case report is to discuss and illustrate the tunnel preparation procedure as a treatment alternative for furcation-involved molars.
REVIEW OF THE LITERATURE: Furcation tunnel preparation, i.e., the creation of access for plaque control between periodontally diseased roots, has not been discussed in the literature as comprehensively as root resective therapy, though success rates appear to fall within the same range. A short root trunk and a wide furcation entrance diameter are prerequisites for the indication of the tunnel preparatio

OBJECTIVE: To assess the correspondence between periodontitis categories defined by 1999 AAP and 2018 AAP/EFP classification systems, and to characterize clinical features within Grade C periodontitis.
METHODS: A cross-sectional study was conducted on patients with periodontitis. Participants were examined and classified according to both classifications. Comparative analyses between aggressive (AgP) and chronic (CP) periodontitis within Grade C included age, bone loss patterns, clinical attachment loss, furcation involvement, and tooth mobility. The association between classifications was il

Gingival recession (GR) is a condition resulting in root exposure which leads to root sensitivity, pain, root caries, plaque retention, poor esthetics, and tooth loss. Sites exhibiting Miller Class III and IV GR are not suitable for treatment with surgical root coverage techniques, and their prognosis are very poor with current techniques. In this case report, pedicled buccal fat pad (PBFP) was employed as subepithelial graft technique for root coverage of maxillary tooth with Class III GR defect along with furcation involvement and the absence of keratinized gingiva. PBFP as the subepithelial

Efficacious guided tissue regeneration for intrabony, furcation class II, and recession defects can be accomplished with both nonresorbable and bioresorbable barriers. The potential for regeneration of the periodontium is highly dependent on defect morphology and the availability of "progenitor cells." Many factors associated with surgical technique and barrier properties influence the regenerative outcome of guided tissue regeneration. Maintained flap coverage of the barrier minimizes epithelial down-growth as well as the risk of bacterial contamination of the barrier and the healing wound. C

BACKGROUND: Granulation tissue is routinely discarded in periodontal surgical procedures, but few studies have characterized it. The present study aimed to compare global gene expression in granulation tissue derived from different types of periodontal osseous defects.
METHODS: Total RNA was isolated from granulation tissue harvested during routine periodontal surgical procedures. Nineteen sites were analyzed-seven infrabony, six suprabony, and six furcation defects. Following quality control checks, samples underwent bulk mRNA sequencing (20-30 million read pairs per sample) before bioinform

The present clinical trial was designed to evaluate the effects of a bioactive glass, Perioglas, in the treatment of periodontal intrabony defects. 20 patients, 23-55 years of age (44 sites), with intrabony defects completed the 1-year study. Teeth with furcation involvement were excluded. After completion of initial therapy, defects were randomly assigned to either a test or control procedure. Following flap reflection, root planing and removal of chronic inflammatory tissue in both groups, the test defects were restored with the bioactive glass particulate material. Mucoperiosteal flaps were

Here, we describe the treatment course and 2-year follow-up in a case of multiple deep intrabony defects treated with periodontal regenerative therapy. The patient was a 50-year-old woman presenting with the chief complaint of mobile teeth in the maxillary molar region. Examination at her initial visit revealed sites with a probing depth of ≥7 mm in the molar region. Radiographic examination revealed generalized bone resorption. Angular bony defects were evident in the molar region. Initial periodontal therapy was commenced based on a clinical diagnosis of generalized chronic periodontitis. At

Periodontitis is a biofilm-induced chronic inflammatory disease that, if left untreated, can result in alveolar bone and tooth loss. Intrabony defects and furcation involvement (FI) are particularly difficult to manage, as they often persist after step 1 and step 2 periodontal therapy. In this case, we report a relatively novel therapeutic approach to managing deep furcation involvement in the first mandibular right molar (#46). The concerned area of furcation involvement presented a localized periodontitis with a Silness-Loe plaque index of 3, positive bleeding on probing, gingival recession

BACKGROUND: Radiographs play a key role in diagnosis of periodontal diseases. Deep learning models have been explored for image analysis in periodontal diseases. However, there is lacuna of research in the deep learning model-based detection of furcation involvements [FI]. The objective of this study was to determine the accuracy of deep learning model in the detection of FI in axial CBCT images.
METHODOLOGY: We obtained initial dataset 285 axial CBCT images among which 143 were normal (without FI) and 142 were abnormal (with FI). Data augmentation technique was used to create 600(300 normal

BACKGROUND: Hajdu-Cheney syndrome (HCS) is an inheritable, rare disorder of bone metabolism, associated with acro-osteolysis of the distal phalanges, short stature, distinctive craniofacial and skull changes, premature tooth loss, and periodontitis. This report focuses on the periodontal manifestations of HCS.
METHODS: A 22-year-old female presented with the characteristic clinical features of HCS, including short stature, small face, prominent epicanthal folds, thin lips, small mouth, and short hands. There were no abnormal biochemical, hematological, or hormonal data. Tests for bone mineral

A review of the literature was performed related to the frequency of closure of Grade II furcations with various regenerative therapies such as bone replacement grafts (BRG), coronally positioned flaps (CPF), guided tissue regeneration barriers (GTR), or open flap debridement (OFD). Fifty papers involving 1,016 furcations were evaluated. Complete furcation closure was reported only 20% of the time. Clinical change from Grade II to Grade I (partial furcation fill) was found in an additional 33% of the cases. Therefore, general improvement in clinical furcation status has been reported only abou

BACKGROUND: Management of inflammation at previously surgically treated periodontal sites represents a clinical challenge. Although surgical retreatment may be indicated in selected cases, a conservative non-surgical approach may restore periodontal stability while avoiding the additional morbidity associated with repeated surgery. This case report describes the long-term management of a previously surgically treated mandibular molar furcation using site-specific non-surgical retreatment combined with adjunctive application of a collagen-based hydrogel.
METHODS: A 45-year-old non-smoking male

Tonsilloliths are frequently detected as incidental findings, and their microbial similarity to periodontal biofilms suggests potential clinical significance, although robust CBCT-based evidence supporting this association has been limited. This retrospective study evaluated the association between the presence, size, and number of tonsilloliths detected on cone-beam computed tomography (CBCT) images and periodontal bone loss, furcation involvement, and other dental pathologies. CBCT scans of 342 systemically healthy patients obtained between November 2020 and October 2024 were retrospectively

BACKGROUND: Severe buccal vertical bone defects accompanied by combined periodontal-endodontic lesions are challenging to manage, and evidence regarding the long-term outcomes of digitally assisted autotransplantation combined with sticky bone grafting remains limited. We aimed to report a case of digitally assisted tooth autotransplantation combined with autologous alveolar bone and sticky bone grafts for the reconstruction of a severe buccal vertical defect.
CASE DESCRIPTION: A 23-year-old female patient underwent autotransplantation of the right maxillary third molar (#1) to replace the le
