An overview of genetic counseling in Cuba.

This brief report provides an overview of the history and current status of genetic services in Cuba. In 1971, the University of Medical Sciences of Havana began to train doctors in medical genetics according to the medicine development plan in Cuba. With the aim of introducing genetic services to the population, two main issues were identified: the impact of neural tube defects as a cause of infantile mortality, and a founder effect resulting in a high frequency of sickle cell anemia, which increased the mortality rate and impacted the quality of peoples' lives. The impact of consanguinity is

Estimates of genetic parameters for Canadian Holstein female reproduction traits.

Age at first insemination, days from calving to first insemination, number of services, first-service nonreturn rate to 56 d, days from first service to conception, calving ease, stillbirth, gestation length, and calf size of Canadian Holstein cows were jointly analyzed in a linear multiple-trait model. Traits covered a wide spectrum of aspects related to reproductive performance of dairy cows. Other frequently used fertility characteristics, like days open or calving intervals, could easily be derived from the analyzed traits. Data included 94,250 records in parities 1 to 6 on 53,158 cows fro

Global retinoblastoma service availability in 2024 and equity trends since 2017.

BACKGROUND/AIMS: To assess the global availability of retinoblastoma (Rb) diagnostic and treatment services in 2024 and examine changes in their income-related distribution since 2017. METHODS: We conducted an international cross-sectional study with a longitudinal comparison. In 2024, 438 Rb treatment centres from 145 countries participated; 49 countries had no identified Rb centres, and four were unconfirmed. Longitudinal analyses included 246 centres from 124 countries that participated in both the 2017 and 2024 assessments. Centres reported availability of key diagnostic services (CT, MRI

Architectures of genetic medicine: comparing genetic testing for breast cancer in the USA and the UK.

This paper compares the development of genetic testing for breast cancer (BRCA testing) in the USA and the UK. It argues that national political cultures played an important role in how these genetic testing technologies were shaped, and that the shapes of these technologies had important implications for the users of these systems. In order to demonstrate the roles of national social and political elements in the development of new genetic testing technologies, I introduce the concept of a technology's architecture, which is made up of components and the specific ways in which these component

Understanding the impact of genetic testing for inherited retinal dystrophy.

The capability of genetic technologies is expanding rapidly in the field of inherited eye disease. New genetic testing approaches will deliver a step change in the ability to diagnose and extend the possibility of targeted treatments. However, evidence is lacking about the benefits of genetic testing to support service planning. Here, we report qualitative data about retinal dystrophy families' experiences of genetic testing in United Kingdom. The data were part of a wider study examining genetic eye service provision. Twenty interviewees from families in which a causative mutation had been id

Genetic analysis of fertility in the Italian Brown Swiss population using different models and trait definitions.

The aim of this study was to estimate genetic parameters for fertility and production traits in the Brown Swiss population reared in the Alps (Bolzano-Bozen province, Italy). Fertility indicators were interval from parturition to first service, interval from first service to conception (iFC), and interval from parturition to conception, either expressed as days and as number of potential 21-d estrus cycles (cPF, cFC, and cPC, respectively); number of inseminations to conception; conception rate at first service; and non-return rate at 56 d post-first service. Production traits were peak milk y

FH Genetic Testing in Preventive Cardiology: Unpacking Challenges and Enablers Beyond Cost.

IMPORTANCE: Familial hypercholesterolemia (FH) is a common genetic condition associated with premature cardiovascular disease. Genetic testing enables definitive diagnosis and cascade screening, yet its uptake remains limited even in cost-free settings. As Singapore implements a national subsidized genetic testing service for FH, understanding persisting barriers to testing is critical to improving service design and implementation. OBJECTIVE: This qualitative study examined individual-level barriers and facilitators of genetic testing (GT) uptake and assessed experiences with genetic testing

Genetic counseling for prion disease: Updates and best practices.

Prion disease is a rare, fatal, and often rapidly progressive neurodegenerative disease. Ten to fifteen percent of cases are caused by autosomal dominant gain-of-function variants in the prion protein gene, PRNP. Rarity and phenotypic variability complicate diagnosis, often obscuring family history and leaving families unprepared for the genetic implications of an index case. Several recent developments inspire this update in best practices for prion disease genetic counseling. A new prion-detection assay has transformed symptomatic diagnosis. Meanwhile, penetrance, age of onset, and duration

Uptake of genetic testing for germline BRCA1/2 pathogenic variants in a predominantly Hispanic population.

Genetic counseling is under-utilized in women who meet family history criteria for BRCA1 and BRCA2 (BRCA1/2) testing, particularly among racial/ethnic minorities. We evaluated the uptake of BRCA1/2 genetic testing among women presenting for screening mammography in a predominantly Hispanic, low-income population of Washington Heights in New York City. We administered the Six-Point Scale (SPS) to women presenting for screening mammography at Columbia University Irving Medical Center (CUIMC) in the Washington Heights neighborhood of New York, NY. The SPS is a family history screener to determine

Challenges of genomic testing for patients and clinicians in Latin America: Foundations of a qualitative multi-country study.

Genomic medicine is expanding across Latin America (LATAM), yet access to essential ancillary services such as genetic counselling remains limited. 'Latin-SEQ' is a study that provides whole exome sequencing (WES) for neuromuscular diseases across 18 countries, aiming to improve diagnostic rates and generate region-specific genetic insights. However, funding constraints exclude genetic counselling and cascade testing, raising concerns about equitable and harm-free care. This paper reports early findings from 'Latin-SEQ Plus,' a mixed-methods study exploring patient and healthcare practitioner

Valuing the benefits of genetic testing for retinitis pigmentosa: a pilot application of the contingent valuation method.

BACKGROUND: Technological advances present an opportunity for more people with, or at risk of, developing retinitis pigmentosa (RP) to be offered genetic testing. Valuation of these tests using current evaluative frameworks is problematic since benefits may be derived from diagnostic information rather than improvements in health. This pilot study aimed to explore if contingent valuation method (CVM) can be used to value the benefits of genetic testing for RP. METHODS: CVM was used to elicit willingness-to-pay (WTP) values for (1) genetic counselling and (2) genetic counselling with genetic t

Understanding facilitators and barriers to genetic testing for black ovarian cancer patients: A qualitative study utilizing interview data with patients and providers.

Studies consistently demonstrate that Black ovarian cancer patients are less likely to receive guideline-recommended genetic testing. The current study explored barriers to genetic testing among Black women from the perspective of key stakeholders. We conducted semi-structured interviews with patients (n = 5), advanced practice providers (n = 4), genetic counselors (n = 5), and physicians (n = 6). A framework analysis was used to identify themes. All patients identified as Black and completed genetic testing. Among providers, 13% identified as Black and the majority (66%) focused on gynecologi

Genetic screening: marvel or menace?

Genetic screening is a systematic search in the population for persons of certain genotypes. The usual purpose is to detect persons who themselves or whose offspring are at risk for genetic diseases or genetically determined susceptibilities to environmental agents. Is genetic screening a marvel about to free us from the scourge of genetic disease or a menace about to invade our privacy and determine who may reproduce? There are three different types of genetic screening. Newborn screening identifies serious genetic disease at birth, permitting prompt treatment to prevent mental and physical r

Are we ready for genetic testing for primary open-angle glaucoma?

Following a dramatic reduction in the cost of genotyping technology in recent years, there have been significant advances in the understanding of the genetic basis of glaucoma. Glaucoma patients represent around a quarter of all outpatient activity in the UK hospital eye service and are a huge burden for the National Health Service. A potential benefit of genetic testing is personalised glaucoma management, allowing direction of our limited healthcare resources to the glaucoma patients who most need it. Our review aims to summarise recent discoveries in the field of glaucoma genetics and to di

Referral patterns for genetic counselling of women diagnosed with tubo-ovarian or peritoneal high-grade serous carcinoma (HGSC) within the Auckland Gynaecological Oncology Centre.

BACKGROUND: Germline BRCA1/2 mutations account for approximately 15% of invasive ovarian carcinomas. Referral of all women with non-mucinous epithelial tubo-ovarian and peritoneal cancer, especially high-grade serous carcinoma (HGSC) to genetic services for genetic counselling and subsequent BRCA testing, has become standard of care in many countries. Publicly funded BRCA testing was restricted to women ≤70 years old with HGSC in New Zealand for most of our study period. Referral rates of women with HGSC for BRCA mutation testing in New Zealand have not previously been reported. AIMS: To dete

Recent Advances in Genetic Testing and Clinical Management of Hereditary Breast and Ovarian Cancer (HBOC) in India.

BACKGROUND: Hereditary breast and ovarian cancer (HBOC) syndromes, responsible for 5%-10% of all breast and ovarian cancers in the general population, are largely associated with pathogenic variants of the BRCA1 and BRCA2 genes. Yet, the role of other cancer susceptibility genes highlights the genetic etiology of HBOC as complex, thus requiring thorough investigation beyond these main mutations, highlighting a need for a comprehensive genetic assessment in disease management strategies. SUMMARY: In India, advances in genetic research and clinical management have significantly impacted the kno
