Application of novel burst wave lithotripsy and ultrasonic propulsion technology for the treatment of ureteral calculi in a bottlenose dolphin (Tursiops truncatus) and renal calculi in a harbor seal (Phoca vitulina).

Marine mammals may develop kidney stones, which can be challenging to treat. We describe burst wave lithotripsy (BWL) and ultrasonic propulsion to treat ureteral calculi in a 48-year-old female bottlenose dolphin (Tursiops truncatus) and to reduce renal stone burden in a 23-year-old male harbor seal (Phoca vitulina). BWL and ultrasonic propulsion were delivered transcutaneously in sinusoidal ultrasound bursts to fragment and reposition stones. Targeting and monitoring were performed with real-time imaging integrated within the BWL system. Four dolphin stones were obtained and fragmented ex viv

S-ketamine plus dexmedetomidine vs. S-ketamine plus propofol for sedation-analgesia during positioning for spinal anesthesia in older adults undergoing lower extremity fracture surgery.

BACKGROUND: Positioning for neuraxial anesthesia in geriatric lower extremity fracture surgery is painful and can destabilize hemodynamics. Sedation-analgesia must balance effective analgesia with respiratory safety and physiologic stability. METHODS: This single-center retrospective cohort study included patients aged ≥65 years undergoing lower extremity fracture surgery under spinal anesthesia who received S-ketamine plus dexmedetomidine (Group A, n = 48) or S-ketamine plus propofol (Group B, n = 46). Primary outcomes included a numerical rating scale (NRS) positioning pain score of 0-10 an

Spinal Anesthesia Reduces Perioperative Polypharmacy and Opioid Burden in Patients Over 65 Who Undergo Transforaminal Lumbar Interbody Fusion.

BACKGROUND: Polypharmacy and opioid administration are thought to increase the risk of postoperative cognitive dysfunction and delirium in elderly patients. Spinal anesthesia (SA) holds potential to reduce perioperative polypharmacy in spine surgery. As more geriatric patients undergo spine surgery, understanding how SA can reduce polypharmacy and opioid administration is warranted. We aim to compare the perioperative polypharmacy and dose of administered opioids in patients ≥65 years who undergo transforaminal lumbar interbody fusion (TLIF) under SA versus general anesthesia (GA). METHODS: A

Post-operative delirium in older hip fracture patients: a new onset or was it already there?

PURPOSE: Hip fractures are a worldwide health issue primarily for older patients, conditioning major morbidity and mortality. An experienced multidisciplinary team is essential to manage surgery and peri-operative implications, to enable rapid functional and cognitive recovery. Delirium is a recognizable problem associated with negative outcomes. Our study aims to determine the influence of pre-operative delirium in the incidence of post-operative delirium, and to evaluate the association between other known peri-operative risk factors with both conditions. METHODS: A single-center, retrospec

Percutaneous Endoscopic Lumbar Foraminotomy for Foraminal Stenosis with Postlaminectomy Syndrome in Geriatric Patients.

INTRODUCTION: Postlaminectomy syndrome (PLS) or failed back surgery syndrome is a condition characterized by persistent pain following a back surgery. Degenerative processes may result in foraminal stenosis development over time, even after a successful surgery. Percutaneous endoscopic lumbar foraminotomy (PELF) offers a minimally invasive means of treating foraminal stenosis after a back surgery. The objective of this study was to evaluate the outcomes of PELF for foraminal stenosis with PLS in geriatric patients. METHODS: Two-year follow-up data were collected from 21 consecutive patients a

Exposure to surgery under general anaesthesia and brain magnetic resonance imaging changes in older adults.

BACKGROUND: Preclinical studies suggest that exposure to general anaesthesia (GA) could cause neurodegeneration consistent with Alzheimer's disease (AD) pathology. Brain magnetic resonance imaging (MRI) is useful to study structural brain changes. We tested the hypothesis that exposure to surgery with GA (surgery/GA) is associated with greater cortical thinning and increased frequency of white matter lesions. METHODS: This is a cross-sectional analysis of 70-91-yr-old participants enrolled in the Mayo Clinic Study of Aging who had baseline MRI. The thickness of selected cortical regions, the

Comparison of emergence agitation between sevoflurane and desflurane in elderly orthopedic patients: a retrospective cohort study.

BCKGROUND: Emergence agitation frequently occurs during emergence from general anesthesia and may result in potentially serious complications, especially in elderly patients. Although desflurane and sevoflurane are widely used, their differential effects on emergence agitation in elderly populations remain unclear. METHODS: In this retrospective cohort study, we analyzed 376 elderly patients (≥65 years) who underwent orthopedic surgery under general anesthesia with either sevoflurane or desflurane between August 2022 and February 2024. Propensity score matching (1:1 ratio) was performed to ad

Remimazolam for induction of anesthesia in elderly patients with severe aortic stenosis: a prospective, observational pilot study.

BACKGROUND: Remimazolam, a novel benzodiazepine, has been reported to cause less hypotension than propofol during induction of anesthesia. Therefore, remimazolam might be a valuable option in elderly patients with severe aortic stenosis who are considered to be the most vulnerable to hemodynamic instability. We aimed to evaluate the feasibility and hemodynamic effects of remimazolam as an induction agent in elderly patients with severe aortic stenosis. METHODS: This prospective, open-label, single-arm, observational pilot study was conducted in a university hospital between November 2020 and

Timing of anesthetic emergence and early postoperative recovery in older adults undergoing thoracic surgery: a retrospective cohort study.

BACKGROUND: Older adults are at heightened risk of adverse early postoperative outcomes, yet whether the time of day at which patients emerge from anesthesia influences these outcomes remains unclear. We therefore examined the association between late-day anesthetic emergence and early postoperative outcomes after thoracic surgery in older adults. METHODS: We conducted a retrospective cohort study of 8,118 patients aged 65-90 year undergoing elective non-cardiac thoracic surgery. Patients were classified into the daytime group (08:00-16:00) or late-day group (16:00-24:00) according to the sta

Comparison of opioid-free versus opioid-based total intravenous anaesthesia in elderly patients undergoing short-duration surgery: a randomized controlled trial.

INTRODUCTION: Older adults who undergo short-duration surgery are vulnerable to opioid-related complications. It is uncertain whether an opioid-free total intravenous anaesthesia (OFA) can reduce these events. We aimed to determine whether OFA reduces the incidence of major postoperative adverse events compared with standard opioid-based total intravenous anaesthesia (OBA). PATIENTS AND METHODS: This single-center randomized clinical trial was conducted in China. From May to August 2025, 400 patients aged ≥60 years undergoing elective, short-duration surgery (anticipated duration of less than

NIRS-assisted cerebral autoregulation in elderly patients during thoracic surgery: a prospective observational study.

PURPOSE: Postoperative cerebral complications may be related to abnormal intraoperative cerebral blood flow, and blood pressure fluctuations are common during thoracoscopic surgery and affect the cerebral blood supply, especially in elderly patients. Here, we investigated the clinical value of continuous cerebral autoregulation (CA) monitoring in protecting against cerebral dysfunction in elderly patients undergoing thoracoscopic surgery. METHODS: This study included 84 patients who underwent video-assisted thoracoscopic lobectomy under general anaesthesia and were divided by age into an elder

Pre-induction nociception indices are associated with post-induction hypotension during general anesthesia in older patients: a prospective observational study.

We investigated whether pre-induction High Frequency Variability Index (HFVI)/Analgesia Nociception Index (ANI) and Nociception Level (NOL) values could predict hypotension during general anesthesia induction in older patients. This prospective observational study included patients aged ≥ 65 years undergoing elective surgery under general anesthesia. After recording HFVI/ANI and NOL values, remifentanil was administered at 0.2 µg/kg/min for 2 min, followed by a rapid intravenous bolus administration of propofol at 2 mg/kg. Noninvasive blood pressure was measured at 1-min intervals over 5 min.

Urinary Retention in Orthopaedic Surgery: An Evidence-based Algorithm.

Postoperative urinary retention (POUR), the inability to urinate spontaneously after surgery, affects up to 43% of orthopaedic patients and can lead to pain, bladder injury, urinary tract infections, increased healthcare costs, and delayed rehabilitation. POUR arises from decreased neurologic signaling, reduced bladder sensation and contractility, or mechanical obstruction. Risk factors include advanced age, prior urinary retention, comorbidities (eg, benign prostatic hyperplasia, diabetes, renal dysfunction), anesthesia type, perioperative opioid use, and catheterization. Despite its prevalen

Geriatric intertrochanteric hip fractures: an economic analysis.

Hip fractures have a high economic and social cost to society. Rising healthcare costs, coupled with managed healthcare systems, have forced a close inspection of healthcare expenditures. To evaluate the impact of geriatric intertrochanteric hip fractures upon a hospital system, an economic cost-analysis was undertaken. An analysis was made of financial and hospital data of elderly patients who sustained an intertrochanteric hip fracture and subsequently underwent open reduction and internal fixation. Increasing patient age did not correlate with length of stay, overall hospital cost, or net h
