Optimization of older adults by a geriatric assessment-guided multidisciplinary clinic before CAR T-cell therapy.

AbstractThe optimal means of assessing candidacy of older adults (≥65 years) for chimeric antigen receptor T-cell (CAR-T) therapy are unknown. We explored the role of a geriatric assessment (GA)-guided multidisciplinary clinic (GA-MDC) in selecting and optimizing older adults for CAR-T. Sixty-one patients were evaluated in a GA-MDC (median age, 73 years; range, 58-83). A nonbinding recommendation ("proceed" or "decline") regarding suitability for CAR-T was provided for each patient based on GA results. Fifty-three patients ultimately received CAR-T (proceed, n = 47; decline, n = 6). Among pati

Multidimentional Preoperative Frailty Assessment and Postoperative Complication Risk in Egyptian Geriatric Patients Undergoing Elective Cardiac Surgery.

BACKGROUND: Frailty affects up to 51%of the geriatric population in developing countries which leads to increased morbidity and mortality. OBJECTIVE: To determine the association between pre-operative frailty through multidimentional assessment score, and the incidence of post-operative complications and to validate Robinson score in geriatric Egyptian patients undergoing elective cardiac surgery. METHODS: We recruited 180 elderly participants aged 60 years old and above, who underwent elective cardiac surgery. They were divided into frail, pre-frail, and non-frail groups after application o

Geriatric assessment predicts survival and toxicities in elderly myeloma patients: an International Myeloma Working Group report.

We conducted a pooled analysis of 869 individual newly diagnosed elderly patient data from 3 prospective trials. At diagnosis, a geriatric assessment had been performed. An additive scoring system (range 0-5), based on age, comorbidities, and cognitive and physical conditions, was developed to identify 3 groups: fit (score = 0, 39%), intermediate fitness (score = 1, 31%), and frail (score ≥2, 30%). The 3-year overall survival was 84% in fit, 76% in intermediate-fitness (hazard ratio [HR], 1.61; P = .042), and 57% in frail (HR, 3.57; P < .001) patients. The cumulative incidence of grade ≥3 nonh

Feasibility of computer-based self-administered cancer-specific geriatric assessment in older patients with gastrointestinal malignancy.

BackgroundThe Cancer-Specific Geriatric Assessment (CSGA) is a primarily self-administered paper survey of validated measures.MethodsWe developed and tested the feasibility of a computer-based CSGA in patients ≥70 years of age who were receiving treatment for gastrointestinal malignancies at the Dana-Farber Cancer Institute. From December 2009 to June 2011, patients were invited to complete the CSGA at baseline (start of new treatment) and follow-up (at the first of 4 months later or within 4 weeks of completing treatment). Feasibility endpoints were proportion of eligible patients consented,

Frailty as defined by the comprehensive geriatric assessment frailty index (CGA-FI) is associated with in-hospital complications and mortality in geriatric hip fracture patients, A retrospective cohort study.

BACKGROUND: Increased frailty is associated with higher rates of adverse events after hip fracture surgery. Unfortunately, considerable uncertainty remains about which tool or index best quantifies frailty and therefore the associated risk of complications following hip fracture surgery. This study tried to evaluate whether a Comprehensive Geriatric Assessment-Frailty Index could predict in-hospital complications following hip fracture surgery. METHODS: A retrospective cohort study was conducted among 1469 patients aged 70 years and older with an operatively managed hip fracture. Patients gro

Clinical utility of geriatric assessment for older surgical patients with gastrointestinal cancer: Evidence, gaps and future directions.

As the global population ages, the number of older adults undergoing surgery for gastrointestinal cancer continues to increase. Although advances in surgical techniques have expanded treatment options for older patients with gastrointestinal cancer, age-related vulnerability and heterogeneity in functional reserve pose substantial challenges to surgical decision-making. Geriatric assessment (GA) is a multidimensional evaluation designed to identify vulnerabilities that are not captured by conventional surgical risk assessments. While GA has been widely recommended in medical oncology, its role

Development and pilot implementation of a digitally integrated frailty-triggered comprehensive geriatric assessment system for older inpatients.

BACKGROUND: Frailty is prevalent among hospitalized older adults and associated with adverse outcomes, yet it is rarely assessed systematically in acute care. Comprehensive geriatric assessment (CGA), a key component of frailty care, remains underutilized. System-level integration of frailty identification with structured CGA may facilitate its implementation in routine care. METHODS: We developed a digitally integrated frailty-triggered CGA system incorporating three steps: frailty identification using the Clinical Frailty Scale (CFS), stratified care pathways by CFS levels, and FINDNEEDS-ba

Impact of Geriatric Assessment on Radiotherapy Intensity in Older Patients With Nonmetastatic Cancer: A Multi-institutional Study.

AIMS: This study prospectively investigated an appropriate geriatric assessment tool for predicting the decisions by radiation oncologists to reduce radiotherapy intensity. MATERIALS AND METHODS: We enrolled patients with nonmetastatic cancer (aged ≥70 years) in this multicenter study. Reduced-intensity radiotherapy implied changes to the irradiation field and/or radiotherapy schedule from the standard based on the primary tumour guidelines. Geriatric assessment was performed by calculating the Geriatric 8 (G8) and Vulnerable Elders Survey (VES-13) scores before radiotherapy and at 3 to 5 and

Geriatric assessment for older adults receiving less-intensive therapy for acute myeloid leukemia: report of CALGB 361101.

Geriatric assessment (GA) predicts survival among older adults with acute myeloid leukemia (AML) treated intensively. We evaluated the predictive utility of GA among older adults treated with low-intensity therapy on a multisite trial. We conducted a companion study (CALGB 361101) to a randomized phase 2 trial (CALGB 11002) of adults ≥60 years and considered "unfit" for intensive therapy, testing the efficacy of adding bortezomib to decitabine therapy. On 361101, GA and quality of life (QOL) assessment was administered prior to treatment and every other subsequent cycle. Relationships between

The Role of Smoking and Body Mass Index in Mortality Risk Assessment for Geriatric Hip Fracture Patients.

Background Smoking, obesity, and being below a healthy body weight are known to increase all-cause mortality rates and are considered modifiable risk factors. The purpose of this study is to assess whether adding these risk factors to a validated geriatric inpatient mortality risk tool will improve the predictive capacity for hip fracture patients. We hypothesize that the predictive capacity of the Score for Trauma Triage in the Geriatric and Middle-Aged (STTGMA) tool will improve. Methodology Between October 2014 and August 2021, 2,421 patients >55-years-old treated for hip fractures caused b

Risk Stratification of Patients Undergoing Percutaneous Repair of Mitral and Tricuspid Valves Using a Multidimensional Geriatric Assessment.

BACKGROUND: Given their advanced age and high comorbidity, individual risk assessment is crucial in patients undergoing transcatheter mitral and tricuspid valve repair. Therefore, we evaluated the use of a comprehensive geriatric assessment score, the multidimensional prognostic index (MPI), for risk stratification in these patients. METHODS: We conducted a prospective, observational single-center study, including 226 patients undergoing percutaneous repair for mitral or tricuspid regurgitation. The MPI was calculated preprocedural and covers 8 domains (activities of daily living, instrumenta
