Study of antipsychotic drug use in long-term care facilities.

We chose to conduct this study because neuroleptic use has not been well defined in nursing homes. To our knowledge, it represents the only attempt at implementing the AIMS procedure in a group of nursing homes. We have provided evidence for what appears to be reasonably typical use of antipsychotic medication in skilled and intermediate care facilities. Senility and dementia were the most common diagnoses noted for neuroleptic recipients. Inservice education had little, if any, impact on antipsychotic drug use. Efforts to reduce the incidence of inappropriate use of neuroleptics should be aim

Health professionals need to be able to identify depression in older people.

Healthcare literature shows that depression in older people, as a mental health problem, is often under-recognized and under-managed. This may partly be the result of multiple pathology, a common feature in later life, masking the features of depressive illness. It may also be the consequence of inadequate use of mental health specialists' skills. As the population is ageing rapidly, healthcare professionals will encounter more cases of depression among the older population. Some therapeutic approaches have been suggested for nurses to adopt. More importantly, nurses and other care workers hav

[Multicomponent nurse-led intervention for the prevention of delirium in hospitalized frail older patients: PREDELENF study protocol].

INTRODUCTION: Due to population aging, hospital care increasingly involves a higher number of individuals with chronic conditions, frailty, and disability, who are at risk of developing geriatric syndromes and require complex management during hospitalization. Delirium is one of the most prevalent, underdiagnosed geriatric syndromes and it is associated with serious consequences in terms of morbidity, mortality, and clinical management. Despite being a potentially preventable event and an indicator of geriatric patient safety and care quality, few programs address it systematically. MATERIALS

Nurse-led speech therapist referral initiative for older adult patients at risk of dysphagia: a best practice implementation project.

INTRODUCTION AND OBJECTIVES: Dysphagia is common among older patients, affecting their nutritional status, hospital readmission, length of hospital stay, and hospitalization costs. Nurses can play a key role in early identification of dysphagia through systematic screening. This project sought to (i) achieve 80% compliance among nurses in using the 4-point questionnaire test (4QT) swallow screening test on patients and (ii) ensure all patients screening positive for dysphagia were referred to a speech therapist within 1 day. METHODS: A short swallow screening tool was adapted from an evidence

Work motivation profiles and work engagement among nursing assistants in Chinese aged care institutions: a cross-sectional latent profile analysis.

BACKGROUND: Geriatric nursing assistants are central to long-term care, but average-score analyses may conceal differences in how autonomous and controlled forms of work motivation combine within individuals. OBJECTIVE: To identify latent work motivation profiles among geriatric nursing assistants, compare the primary outcome of work engagement across profiles, and examine factors associated with profile membership. METHODS: A cross-sectional survey was conducted between May and August 2025 in nine aged care institutions in Xi'an and Weinan, Shaanxi Province, China. Eligible participants wer

Teaching medicine interns minimum geriatrics competencies within a "4 + 2" schedule.

The increased ambulatory training time in an "X + Y" (inpatient + ambulatory) residency schedule affords more opportunities to teach geriatrics principles of care. We describe our internal medicine (IM) residency program's experience in teaching the IM-family medicine (FM) minimum geriatrics competencies (MGC) during a longitudinal geriatrics rotation embedded within interns' yearlong "4 + 2" schedule. Interns spend 1 day of the ambulatory block in a geriatrics outpatient setting (Program of All-Inclusive Care for the Elderly, house calls, nursing home), during which geriatrics division facult

Impact of Acute Functional Decline on Frailty and Mortality in Acutely Hospitalized Older Persons.

OBJECTIVES: Older persons are often referred to emergency departments due to acute functional decline, that is, reduced activities of daily living occurring in the days before hospitalization, frequently driven by acute medical conditions. We aimed to describe frailty in the oldest hospitalized patients presenting with acute functional decline and evaluate predictors of death, severe frailty and nursing home admission after 3-6 months. METHODS: We included 119 persons with acute functional decline, aged ≥ 80 years, ≥ 5 systemic diagnoses, ≥ 5 drugs daily, from an acute geriatric ward and reas

Back to Basics: Giant Challenges to Addressing Isaac's "Geriatric Giants" Post COVID-19 Crisis.

The COVID-19 pandemic, being the greatest challenge to our healthcare system for over a century, has its greatest impact on older patients. This subgroup has higher morbidity and mortality than younger age groups. Superimposed on this, the major preventative intervention resulting in social isolation has negative consequences. Prof. Bernard Isaacs described the "Geriatric Giant Symptoms" in 1965 and encouraged the development of interventions for immobility, instability, incontinence and impaired intellect/memory with careful management of these symptoms resulting in better outcomes for older

The administrative tasks burden in inpatient units: an observational study to quantify the impact on nursing staff's working time.

Background Administrative tasks (ATs) are perceived by healthcare workers as something that takes up an inappropriate number of nurses' working time and limits their ability to provide an adequate amount of 'direct care' to their patients. Understanding the kind of ATs mistakenly attributed in this period of staff shortage can be significant in the analysis of the congruence between the nurse's skills and the work/organizational process. In the inpatient units there are some ATs performed by nurses, such as organizing documentation and forms, answering phone calls, planning visits and appointm
