During laparoscopic myomectomy, it is difficult to determine the actual blood loss volume because physiological saline is used for lavaging to detect the bleeding point or to ensure no active bleeding site. We attempted to estimate blood loss by measuring the waste irrigation fluid's hemoglobin (Hb) concentration using HemoCue (HemoCue, Angelholm, Sweden). Twenty-three women enrolled in our study. After the end of laparoscopic myomectomy, the waste irrigation fluid's Hb content was measured using HemoCue. We estimated blood loss using the following formula. Estimated blood loss (ml)= [waste ir

Genital tract and perineal electrical burns during resectoscopic surgery are occasionally encountered. However, perineal or buttock burns from other causes have not been reported.A 44-year-old overweight woman with a history of menometrorrhagia underwent endometrial ablation with a thermal balloon device uneventfully under general anesthesia. Upon removal of the weighted speculum it was noted that the buttocks had sustained two oval burns under the speculum ball. Experiments demonstrated that the weighted speculum cools off differentially, with the ball temperature remaining at higher than 45C

The surgical repair of vesicovaginal fistula (VVF) over the last two decades has evolved from the transabdominal/transvaginal route to minimally invasive techniques of laparoscopy and robotic surgery. The indications for laparoscopic repair include supratrigonal fistulae, and stenotic/narrow vaginas that make vaginal access to the fistula site difficult. In the current published literature, comparable results have been reported with open surgery. The initial techniques were performed to simulate the open classic technique described by O'Conor, but with better imaging, exposure and magnificatio

Female pelvic medicine and reconstructive surgery (FPMRS) is a rapidly expanding surgical specialty that focuses on the evaluation and treatment of pelvic floor disorders in women. Evaluation and treatment of several pelvic floor disorders can be accomplished with office-based procedures, yielding significant advantages to the patient, physician, and health system. This article describes the common and more useful office-based procedures for the FPMRS physician, covering 4 major categories: cystourethroscopy, procedures for stress urinary incontinence, procedures for urge urinary incontinence,

Laparoscopy is the gold standard in gynecological surgery. Single-port laparoscopic surgery, has been developed in order to improve minimally invasive surgery. We analyzed single-port approach in benign gynecologic pathology and made a bibliographic research on Pubmed and Medline from January 2000 to January 2015. From what emerges from the papers taken into consideration, single-port laparoscopy can be used for salpingostomy or salpingectomy to treat tubal ectopic pregnancy, benign adnexal disease (ovarian cystectomy, salpingo-oophorectomy), and for hysterectomy. We do not have enough data fo

PURPOSE OF REVIEW: This article provides an update on the best practices for the prevention, recognition, and management of urinary tract injuries that may occur during gynecologic laparoscopic surgery.
RECENT FINDINGS: Higher surgical volume is directly associated with improved surgical outcomes, denoted by consistently lower rates of complications for commonplace procedures such as hysterectomy. As a result, expert opinion on prevention of iatrogenic urologic injury suggests a real need for improved education and training of gynecologic surgeons. Discontinued manufacturing of indigo carmine

OBJECTIVE: The association of intraoperative neurologic injuries with gynecologic surgical procedures is well established. The sequelae of such injuries are usually transient and resolve with minimal intervention, although long-term disability can and does occasionally occur. The purpose of this study was to examine the mechanisms by which these injuries occur in order to reduce the risk of their occurrence.
DATA SOURCES: A MEDLINE search was performed cross-referencing the index terms "neurological injury" and "gynecological surgery," from January 1, 1960 to December 31, 2002.

BACKGROUND: The Obstetrics and Gynecology (OB/GYN) flight at Keesler Medical Center sheltered 36 pregnant women and their families during Hurricane Katrina to ensure that appropriate medical care would be available. Ironically, the hospital's emergency generators were destroyed during the 30-foot storm surge, while one woman with a previous cesarean section went into active labor. She ultimately underwent a cesarean delivery using battery-operated flashlights for illumination. After the storm, the damage to the hospital was so extensive as to require aeromedical evacuation of the pregnant pati

Background: Laparoscopy is rapidly replacing laparotomy in the field of gynecologic surgery. Generally, there are limited data concerning the utility of single-incision laparoscopic surgery (SILS) in gynecologic surgery. Specifically, in Saudi Arabia, a third-world country, data are further limited; only one related study has been conducted so far. The purpose of this study is to retrospectively report our single-institutional experience of SILS in terms of feasibility, safety and perioperative outcomes in the management of various gynecologic conditions. Methods: The study took place at the W

OBJECTIVE: Surgical intervention remains the cornerstone of treatment for gynecologic malignancies. However, the complex nature of these procedures leads to a significant risk of perioperative and post-operative complications. The European Society of Gynecological Oncology recognized the need for standardized, evidence-based consensus statements with the aim to minimize surgical morbidity and mortality and decrease surgical complications.
METHODS: A literature review was conducted by the methodologist of the group, using the MEDLINE database, based on 5 patient, intervention or exposure, comp

As vaginal hysterectomy rates continued to decline, vNOTES (vaginal natural orifice transluminal endoscopic surgery) emerged. vNOTES offers the advantage of combining the benefits of laparoscopy with improved visualization, instrumentation, and hemostasis with the vaginal route. In this review, we describe the vNOTES technique in gynecologic surgical procedures, review available evidence, and discuss future directions.

A 71-year-old woman with POP-Q stage IV uterine prolapse with cystocele underwent robot-assisted sacrocolpopexy (RSC) using the hinotori Surgical System. During the initial surgery, bleeding from the median sacral vein led to suboptimal mesh fixation at the sacral promontory. Within 1 month post-surgery, the patient experienced recurrence of prolapse symptoms. Examination revealed descent of the uterine cervix to nearly the pre-operative state. Robot-assisted repair surgery was performed, which confirmed detachment of the mesh from the sacral fixation site. The repair involved excising the upp

INTRODUCTION AND HYPOTHESIS: Sacrocolpopexy (SCP) is a recognized treatment for apical pelvic organ prolapse (POP). However, mesh erosion remains a concern, particularly when performed with concomitant hysterectomy. This video presents data on one case of a modified technique aimed at potentially minimizing mesh erosion in robotic SCP.
METHODS: This technique focuses on reinforcing the vaginal cuff and using a pedicled peritoneal graft to create a tissue barrier between the mesh and the vaginal vault. Procedural steps include intrafascial colpotomy, edge-to-edge cuff closure using barbed sutu

Surgical site infections are a common complication of obstetric and gynaecological surgeries; up to 10% of gynaecological patients undergoing an operative procedure will develop a surgical site infection. In surgeries with high rates of post-operative infection, antibiotic prophylaxis (using an antibiotic with an appropriate microbiological spectrum and administered in a timely manner) can play a major role in improving outcomes. This review examines the medical literature to assess the indications and appropriate antibiotic choices for prophylaxis to prevent surgical site infection in obstetr
