The Hoffmann Reflex.

The human spinal cord is increasingly recognized as an active and adaptable component of sensorimotor function, contributing to motor control, pain modulation, and recovery after neurological injury. Within this framework, the Hoffmann reflex (H-reflex) has evolved from a classical electrophysiological phenomenon into a useful probe of spinal circuit function. Rather than reflecting motoneuron excitability alone, H-reflex amplitude and modulation arise from the interaction of Ia afferent transmission, presynaptic inhibition, homosynaptic depression, and interneuronal networks that regulate sen

Speed-related spinal excitation from ankle dorsiflexors to knee extensors during human walking.

Automatic adjustments of muscle activity throughout the body are required for the maintenance of balance during human walking. One mechanism that is likely to contribute to this control is the heteronymous spinal excitation between human ankle dorsiflexors and knee extensors (CPQ-reflex). Here, we investigated the CPQ-reflex at different walking speeds (1-6 km/h) and stride frequencies (0.6-1.3 Hz) in healthy human subjects to provide further evidence of its modulation, and its role in ensuring postural stability during walking. The CPQ-reflex was small or absent at walking speeds below 2-3 km

Electrophysiology in Fisher syndrome.

Fisher syndrome (FS), a variant of Guillain-Barré syndrome (GBS), is characterized by the clinical triad of ophthalmoplegia, ataxia, and areflexia. The lesion sites for these unique clinical features include the oculomotor nerves and group 1a neurons in the dorsal root ganglion, and the presence of FS is determined by the expression of ganglioside GQ1b in the human nervous system. Neurophysiological findings suggest that ataxia and areflexia are due to an impaired proprioceptive afferent system. Typically, the soleus H-reflex is absent and a body-sway analysis using posturography shows a 1-Hz

Postexercise facilitation of reflexes is not common in Lambert-Eaton myasthenic syndrome.

Postexercise facilitation (PEF) with clinical reflexes, H-reflex, and T-reflexes at the ankle and knee was systematically studied in 16 patients with Lambert-Eaton myasthenic syndrome (LEMS). PEF was observed in ankle and knee deep tendon reflexes in five patients, in H-reflex in three patients, and in T-reflexes in six patients. When all reflex tests were combined, 7 (43.7%) of 16 patients showed PEF by at least one test. The authors conclude that the PEF of reflexes, the most helpful diagnostic clinical marker for LEMS, is not common.

Changes of reflex size in upper limbs using wrist splint in hemiplegic patients.

We evaluated the effect of prolonged wrist extension on H reflex in the flexor carpi radialis (FCR) muscle and tendon jerk (T) reflex in the biceps brachii (BB) muscle of 17 chronic hemiplegic patients. H reflex of the FCR and T reflex of the BB were assessed every 5 minutes within 20 minutes during prolonged wrist extension and post-20 minutes after the extension. As a result, H reflex in the FCR was reduced by passive wrist stretch in 82% of the spastic limbs. The effect was larger in the higher spastic group. In 45% of the spastic limbs, T reflex in the BB also was reduced by passive wrist

Acute tolerance to the hypnotic effect of morphine in rats.

To demonstrate the development of acute tolerance to the hypnotic effects of morphine, loss and recovery of the righting reflex with a constant-rate morphine infusion was studied in rats. In one group of animals, brain and serum concentrations of morphine were detected (radioimmunoassay) at the time of loss of the righting reflex, and in another group, at the time of the reflex recovery. The morphine infusion at a constant rate of 14 mg.kg-1.h-1 caused a loss of the righting reflex in all animals that was achieved by 2.5 h. However, this level of response could not be maintained, and at 5 h it

Peroneal motoneuron excitability increases immediately following application of a semirigid ankle brace.

STUDY DESIGN: Within-session, within-subject comparison of 2 conditions. OBJECTIVES: To determine the influence of application of a semirigid ankle brace on the excitability of the peroneus longus muscle motoneuron pool as measured by the H-reflex. BACKGROUND: The literature suggests that cutaneous mechanoreceptors can contribute to proprioception, especially during conditions in which skin experiences displacement. Further, skin displacement and stimulation of cutaneous mechanoreptors have been shown to increase motoneuron excitability. METHODS AND MEASURES: H-reflexes and M-waves of the p

Botulinum neurotoxin-A in a patient with post-stroke spasticity: a neurophysiological study.

INTRODUCTION: Post-stroke spasticity (PSS) is a disorder of the sensory-motor control, leading to upper motor neuron lesions manifesting either as intermittent or sustained involuntary activation of muscles. Botulinum neurotoxin-A (BoNT-A) is mostly utilized in a variety of therapeutic indications, and it is effective and safe in the management of focal PSS in the rehabilitation scenario. The study aimed to evaluate the effect of BoNT-A administration on H-reflex of upper and lower limbs following PSS. In addition, the investigation of the association among the degree of spasticity (assessed b

Carbon dioxide alters the Hoffmann reflex independent of hydrogen ions.

The purpose of this study was to examine the effect of changes in capillary blood pH on the resting soleus Hoffmann (H) reflex in the intact human. H-max size, H-wave at 20% of H-max, M-max and H-reflex latency were recorded in 10 subjects (apparently healthy, ages 19-36) before and after exposure to 3 hours of NaHCO(3), CaCO(3), NH(4)Cl (all at 0.3 g/kg) or 10 minutes 7% Carbon dioxide (CO(2)) administration. NaHCO(3) increased capillary blood pH, CaCO(3) did not change capillary blood pH, and NH(4)Cl and 7% CO(2) decreased capillary blood pH. H-max and H-wave at 20% of M-max size were signif

Central effect of bladder filling and voiding.

In normal subjects and in patients with spasticity and/or urological complaints changes in somatic muscle reflexes associated with filling of the urinary bladder were observed. Two different patterns of reflex changes occurred. One pattern was associated with easy initiation of voiding: it consisted of increasing H and T reflexes and decreasing flexor reflexes with concomitant decreasing sphincter muscle activity. The other pattern was associated with difficulty in initiation of voiding: it consisted of increasing flexor relfexes including sphincter muscle activity and decreasing H and T refle

Hoffmann reflex is increased after 14 days of daily repeated Achilles tendon vibration for the soleus but not for the gastrocnemii muscles.

In a previous study, Achilles tendon vibrations were enough to improve the triceps surae (TS) activation capacities and also to slightly increase TS Hoffmann reflex (H-reflex) obtained by summing up soleus (Sol) and gastrocnemii (GM and GL) EMGs. The purpose of the present study was to analyze separately Sol and GM or GL reflexes to account for different effects of the vibrations on the reflex excitability of the slow soleus and of the gastrocnemii muscles. A control group (n = 13) and a vibration group (n = 16) were tested in pre-test and post-test conditions. The Achilles tendon vibration pr

Hoffmann reflex in idiopathic scoliosis.

This study was carried out in order to determine the dependence of selected Hoffmann reflex parameters on type, progression and morphology of idiopathic scoliosis (IS). Data collected from 129 girls with IS (59 progressive and 70 non-progressive cases) aged 7-16 years and 24 healthy subjects were analysed. H-reflex index (IH) and H/M amplitude index (IH/M) were calculated. Progressive left lumbar scoliosis expressed a significant decrease of IH values and a distinct tendency to IH/M depletion compared to non-progressives and controls. Progressive right thoracic scoliosis expressed marked tende

Neurophysiological changes following traumatic spinal lesions in man.

Neurophysiological observations were made on normal subjects and on 57 patients who had had injuries to the spinal cord. The amplitude of the muscle compound action potential (M response) recorded from triceps surae in response to supramaximal stimulation of the tibial nerve was reduced in the patients indicating that there are changes in motor units below the level of a spinal lesion in man. In the patients who were clinically spastic it was found that: (1) The proportion of the triceps surae motoneuron pool reflexly activated either by tapping the Achilles tendon or by stimulating the tibial

H wave and spinal root potentials in neuromonitoring of S1 root function during evacuation of herniated disc: preliminary results.

AIM: To determine the changes in the tibial H reflex and spinal nerve root potentials (SRPs) of the S1 root during posterior discectomy and the effects of surgical manipulation. METHODS: Tibial H reflex responses (M and H waves) were intermittently recorded from the soleus muscle by surface electrodes during different stages of surgery in 5 patients with S1 radiculopathy. All patients had Achilles reflex preserved bilateraly and no paresis on manual strength testing preoperatively. SRPs were additionally obtained by direct epidural recordings from the surgically exposed S1 root in 2 of them.

Self-modulation of rectus femoris reflex excitability in humans.

Hyperreflexia is common after neurological injury such as stroke, yet clinical interventions have had mixed success. Our previous research has shown that hyperreflexia of the rectus femoris (RF) during pre-swing is closely associated with reduced swing phase knee flexion in those with post-stroke Stiff-Knee gait (SKG). Thus, reduction of RF hyperreflexia may improve walking function in those with post-stroke SKG. A non-pharmacological procedure for reducing hyperreflexia has emerged based on operant conditioning of H-reflex, an electrical analog of the spinal stretch reflex. It is currently un

Reflexes in prior polio and their relation to weakness and anterior horn cell loss.

The aim of this study was to evaluate the reflex pattern in patients with prior polio and to relate these findings to the degree of anterior horn cell (AHC) involvement and loss of muscle force. Twenty-five prior polio subjects were investigated with electromyography (EMG), force testing and reflex studies, which included the patellar and Achilles reflex, H-reflex, T-response and interlimb reflex (ILR). The clinical reflexes, H/M-ratio and T-response amplitude at rest were positively correlated with force and negatively correlated with the degree of AHC loss. The H/M-ratio was decreased compar

Modification of reflexes in normal and abnormal movements.

The trajectories observed for the limb during human locomotion are determined by a mixture of influences, some arising from neural circuits entirely within the central nervous system and others arising from a variety of sensory receptors. Muscle reflexes are highly modulated during locomotion in an adaptive manner within each phase of the step cycle. Furthermore, the modulation can be modified quickly for different tasks such as standing, walking and running, probably by changes in presynaptic inhibition. This modulation is often lost or severely reduced in patients with spasticity after spina

Age-Related Changes in Presynaptic Inhibition During Gait Initiation.

Age-related changes in presynaptic inhibition (PSI) have not been observed during gait initiation, which requires anticipatory postural adjustment (APA). As APA is centrally modulated and is impaired in older compared to young adults, here we aimed to study the presynaptic control and co-contraction levels in the ankle muscles during gait initiation in older compared to young adults. Fifteen older (age range 65-80 years) and 15 young adults (age range 19-30 years) performed a gait initiation task on a force platform under 3 conditions: (i) without electrical stimulation; (ii) test Hoffman refl
