Atypical cleft hand with complex syndactyly: An unusual form of hand oligodactyly.

Hand oligodactyly refers to a developmental defect which results in the presence of less than 5 digits on a hand. It presents as a component of 4 main categories of congenital upper limb malformations. Herein, we present a 50-year-old man with an unusual form of hand oligodactyly which is characterized by atypical cleft hand accompanied by complex syndactyly between the thumb and the index finger. Accurate characterization of hand oligodacticity can sometimes be challenging due to unusual phenotypic appearances accompanying the abnormality. Radiological evaluation is of great importance for co

Shift work and sleep disorder comorbidity tend to go hand in hand.

Taking into consideration that shift work has a wide-ranging impact on circadian and sleep functioning, it seems likely that shift work increases the risk of a general sleep disturbance, spread out over a multitude of comorbid sleep disorders. The aim of the present study is to analyze and present the sleep disorder data of 250 shift workers and 971 permanent day workers, taken from a nationally representative sample. Additional data concerning duration, timing, and quality of sleep, daytime functioning and social/family variables were added to the analyses. The results showed that the shift w

Lack of emergency hand surgery: discrepancy between elective and emergency hand care.

Wrist, hand, and finger trauma are the most common injuries presenting to emergency departments. Shortage of emergency hand care is an emerging problem, as on-call hand coverage declines. This study evaluates the availability of elective and emergency hand surgery services in Tennessee, with the use of telephone surveys administered to emergency department and operating facility management. One hundred eleven Tennessee hospitals completed the surveys (93% response rate). In all, 77% of hospitals offer elective hand surgery, 58% offer basic emergency hand services, 18% offer occasional hand spe

The hand that has forgotten its cunning--lessons from musicians' hand dystonia.

Focal task-specific dystonia of the musicians' hand (FTSDmh) is an occupational movement disorder that affects instrumental musicians and often derails careers. There has been speculation on the role of intense practice or the specific technical demands of various instruments as triggers for the development of FTSDmh. In this study, we review the clinical features of all published cases (899 patients) and 61 previously unpublished cases of FTSDmh. Our primary goals were to search for patterns in the clinical phenotype, and to discern if specific instrumental technical demands might be related

Hand preference, extent of laterality, and functional hand use in Rett syndrome.

Residual hand use in functional tasks, extent of laterality, and right or left preference were studied in 145 2- to 24-year-old, postregression Australian subjects with Rett syndrome via parent questionnaire. Hand use was markedly restricted, more for complex than simple and for external (touching food and objects) than internal tasks (scratching, rubbing eyes), suggesting a deficit in cerebral control of external, goal-oriented hand use, which is perhaps genetically determined because there is significantly greater restriction of external tasks in subjects with demonstrated MECP2 mutations. Practical approaches to obtaining hand radiographs and special techniques in hand radiology.

Radiography of the hands and joints involves a certain amount of decision making by the radiographer. Many disorders of the wrists are evaluated with radiography, and the positions and radiographic technique factors should be adjusted to reveal as much of the desired anatomy as possible. Immobilizing the patient can be of significant value in obtaining good quality radiographs. It is essential that the processed radiograph contains all pertinent anatomy and that all structures have been demonstrated anatomically as well as radiographically. A number of radiographic positions and procedures are

Evaluation of commercially available hand wipes for hand hygiene in water-safe care: removal of bacterial spores from hands.

BACKGROUND: Hand hygiene is essential for preventing healthcare-associated infections (HCAIs), but traditional handwashing relies on a water source. Hand wipes may offer a practical water-free alternative that supports water-safe care and reduces reliance on hospital-based hand hygiene protocols. AIM: To determine whether using hand wipes could reduce bacterial hand contamination to a level comparable to that achieved using hand soap. METHODS: The removal of Escherichia coli K12 and Geobacillus stearothermophilus (GS) spores (used as a surrogate for Clostridioides difficile) from the hands o

Hand Hygiene Knowledge and Hand Dirtiness Assessment to Inform Alcohol-Based Hand Rub Appropriateness in Community Settings in Uganda and Kenya.

During the coronavirus disease 2019 pandemic, alcohol-based hand rub (ABHR) availability and use increased as a prevention measure. ABHR is a convenient, effective way to kill microbes on hands but is hampered by the presence of organic material, whereas handwashing with water and soap (HWWS) can physically remove microbes and dirt. Although ABHR is preferred in most health care settings, the suitability of ABHR use in community settings has not been measured. We compared characteristics between community members and health care personnel (HCP) to inform considerations for promoting ABHR in co

Reconstruction of a Monodactylous Hand with Microsurgical Free Foot-to-Hand Transfer in Split-Hand/Split-Foot Malformation with Tibial Aplasia.

Split-hand/split-foot malformation with long bone deficiency (SHLFD syndrome) is a rare congenital disorder, which may be sporadic or autosomal dominant with incomplete penetrance. When complete tibial aplasia is seen, the mainstay of treatment is amputation and lower limb prosthesis. This rare constellation of congenital differences presents an opportunity for microsurgical free tissue transfer using the principle of "spare parts" to improve the functionality of the hand. We present a rare case of split-hand/split-foot malformation with a monodactylous right hand and complete tibial aplasia,

Optimizing Hand Hygiene Compliance.

Although hand hygiene is considered a fundamental defense against infection spread, maintaining high, consistent compliance remains an ongoing challenge. Even after the COVID-19 pandemic placed hand hygiene in the spotlight, initially high compliance levels dropped, facilitating the spread of multidrug-resistant pathogen transmission in some settings. This article details the various methodologies for monitoring hand hygiene with diverse solutions for feedback. Ultimately, infection preventionists and hospital epidemiologists must consider what will be most effective for their facility when de

Gunshot Injury to Hand.

The anatomic complexity of the hand and the proximity of its structures make gunshot injuries especially prone to severe, multi-tissue damage. These injuries often involve multiple structures, including bones, tendons, nerves, and skin, simultaneously, requiring individualized and frequently staged treatment. Various reconstructive strategies are available for each affected structure. In this article, we provide an overview of treatment choices and a representative case that illustrates the clinical reasoning behind selecting among these alternatives in the context of a complex hand gunshot in

Epinephrine and hand surgery.

PURPOSE: The growing emphasis on value-based care and the adoption of the "Wide Awake Local Anesthesia with No Tourniquet" (WALANT) surgical technique have led to increased use of epinephrine in hand surgery. Although this practice is considered safe, some surgeons still worry about elective epinephrine use in the finger. The aim of this systematic review was to determine the rate of ischemic complications associated with epinephrine use in hand surgery to better inform surgeons and patients. METHODS: A systematic search of the Scopus database was conducted for studies published between 2005

Minor Hand Surgery Procedures.

While hand surgery has developed as its own unique subspecialty since the mid-twentieth century, other providers, especially those practicing in emergency or more rural settings, may be called upon to perform minor surgical procedures of the hand. With understanding of the relevant anatomy and basic surgical principles, minor hand procedures can be done safely and effectively. This article will discuss common hand injuries and pathologies and their respective management.

Common Pediatric Hand Anomalies.

Upper extremity congenital anomalies in the newborn are second only to congenital heart anomalies. Some of the more commonly encountered upper extremity anomalies are trigger thumb, thumb hypoplasia, polydactyly, syndactyly, and amniotic band syndrome. While some conditions occur in isolation, others are known to commonly occur in association with syndromes. Familiarity with these conditions is important not only to provide adequate evaluation and workup of these patients but also to deliver appropriate surgical intervention and prepare parents with appropriate expectations. In this article, w

[Pathogenesis of hand eczema].

BACKGROUND: The pathogenesis of hand eczema is multifactorial. Exogenous factors such as skin irritation and contact sensitization contribute to this as well as an endogenous atopic predisposition. OBJECTIVE: This article provides a review of the pathogenetic factors relevant to the development of hand eczema. MATERIAL AND METHODS: An evaluation of the current literature on the pathogenesis of hand eczema was carried out. RESULTS: Impairment of the epidermal barrier function plays an essential role for the development of hand eczema. The molecular biological basis and the possible significa

[Prevention of hand eczema].

Apart from endogenous factors, hand eczema is often caused by irritant or allergic contact dermatitis related to occupational or non-occupational exposure to skin hazards. This enables several complementary approaches on the level of primary, secondary and tertiary prevention. A high priority is set on identification and subsequent reduction or elimination of causative exposures. Important preventive approaches include legal regulations, technical and organizational measures as well as correct use of adequate personal protective equipment (e.g. protective gloves). Interventions based on health
