Transcranial direct current stimulation for the treatment of focal hand dystonia.

The treatment of writer's cramp, a task-specific focal hand dystonia, needs new approaches. A deficiency of inhibition in the motor cortex might cause writer's cramp. Transcranial direct current stimulation modulates cortical excitability and may provide a therapeutic alternative. In this randomized, double-blind, sham-controlled study, we investigated the efficacy of cathodal stimulation of the contralateral motor cortex in 3 sessions in 1 week. Assessment over a 2-week period included clinical scales, subjective ratings, kinematic handwriting analysis, and neurophysiological evaluation. Twel

[Hemianopsia and agnosia].

Hemianopsia and agnosia, the coexistence of a disorder of perception and a disorder of recognition, is exemplified by two patients chosen for their clinical similarity. These two patients each had a stroke, one of the left and the other of the right posterior cerebral artery, resulting right and left hemianopsia respectively. Primary visual functions such as visual acuity and contrast sensitivity were intact. The differences were only apparent as regards the severity of the agnostic disorders. The patient with the lesion on the left failed to recognize stylized drawings, was alexic, could not

Multitype hand writing as a digital marker for Parkinson's disease.

BACKGROUND: Handwriting impairment is a distinctive symptom of movement disorders (MDs), such as Parkinson's disease (PD) and Parkinsonian syndrome (PDS). OBJECTIVES: Custom-designed digital handwriting (DHW) tools were developed to strengthen the diagnosis of MDs. METHODS: Participants with MDs from the Neurology and PET departments at the First Affiliated Hospital of Guangzhou Medical University were recruited to perform digital handwriting (DHW) tasks, which included drawing a line and a cube, and writing a sentence in Chinese (Ch), English (E), and Korean (K). AV133 PET-CT/MRI was used t

Modes of Alphabet Letter Production during Middle Childhood and Adolescence: Interrelationships with Each Other and Other Writing Skills.

Although handwriting is typically taught during early childhood and keyboarding may not be taught explicitly, both may be relevant to writing development in the later grades. Thus, Study 1 investigated automatic production of the ordered alphabet from memory for manuscript (unjoined), cursive (joined), and keyboard letter modes (alphabet 15 sec) and their relationships with each other and spelling and composing in typically developing writers in grades 4 to 7 (N = 113). Study 2 compared students with dysgraphia (impaired handwriting, n=27), dyslexia (impaired word spelling, n=40), or oral and

Effective treatment of manganese-induced occupational Parkinsonism with p-aminosalicylic acid: a case of 17-year follow-up study.

OBJECTIVE: Chronic manganese (Mn) intoxication induces syndromes resembling Parkinson disease. The clinical intervention has largely been unsuccessful. We report a 17-year follow-up study of effective treatment of occupational Mn parkinsonism with sodium para-aminosalicylic acid (PAS). METHODS: The patient, female and aged 50 at the time of treatment, was exposed to airborne Mn for 21 years (1963-1984). The patient had palpitations, hand tremor, lower limb myalgia, hypermyotonia, and a distinct festinating gait. She received 6 g PAS per day through an intravenous drip infusion for 4 days and

A rare patient with Parkinson's disease presenting with isolated progressive micrographia.

Although handwriting impairment is a frequent sign of Parkinson's disease (PD), its significance in the evaluation processes of these patients may be overlooked among physicians. Therefore, we would like to report an illustrative patient who presented with isolated micrographia initially; but received the diagnosis of PD in the follow-up.

Apraxic agraphia following thalamic damage: Three new cases.

Apraxic agraphia (AA) is a so-called peripheral writing disorder following disruption of the skilled movement plans of writing while the central processes that subserve spelling are intact. It has been observed in a variety of etiologically heterogeneous neurological disorders typically associated with lesions located in the language dominant parietal and frontal region. The condition is characterized by a hesitant, incomplete, imprecise or even illegible graphomotor output. Letter formation cannot be attributed to sensorimotor, extrapyramidal or cerebellar dysfunction affecting the writing li

Corticobasal degeneration as a cognitive disorder.

The presence of cognitive impairment in corticobasal degeneration (CBD) is now widely recognised. Our review of the literature reveals that, although the pattern and severity of neuropsychological impairments can be highly variable across patients, several general trends can be identified. The most characteristic impairments are limb apraxia (usually ideomotor), constructional and visuospatial difficulties, acalculia, frontal dysfunction, and nonfluent aphasia. The limb apraxia is associated with deficits in drawing, copying, and handwriting, but there is emerging evidence that the problems wi

What type of tremor did the medieval 'Tremulous Hand of Worcester' have?

The thirteenth-century medieval scribe, the ‘Tremulous Hand of Worcester’ is known for the tremor visible in his script. Thorpe & Alty combine historical analysis with the first neurological study of the scribe’s handwriting. After considering various differential diagnoses, they conclude that the balance of evidence favours essential tremor.

[The importance of writing: screening and rehabilitation tools].

Dysgraphia, a neurodevelopmental disorder, can be diagnosed and, in addition to the resources available to therapists, rehabilitated using an interactive tablet that analyzes aspects invisible to the naked eye, such as pressure, inclination and speed of the tracing.. This data, which is highly informative on handwriting quality, enables more detailed analysis and personalized rehabilitation for each patient.

Copy Skills and Writing Abilities in Children With and Without Specific Learning Disabilities.

Copying a text quickly and accurately is important both in school and in daily life. However, this skill has never been systematically studied, either in children with typical development (TD) or in children with specific learning disabilities (SLD). The aim of this research was to study the features of a copy task and its relationship with other writing tasks. For this purpose, 674 children with TD and 65 children with SLD from Grades 6 through 8 in Italy were tested with a copy task and other writing assessment tasks, measuring three aspects of writing: handwriting speed, spelling, and expre

Practice schedule and testing per se affect children's transfer abilities in a grapho-motor task.

Children's ability to transfer the gains of a motor experience, such as learning to write a letter, to novel conditions, such as cursive writing of the same letter, are affected by the way in which the learning experience is parsed. Parsing may have limitations because a short session may hamper the engagement of procedural memory consolidation processes. Here, we compared the effects of two practice schedules with the total amount of practice identical training provided in a single-session practice versus multi-session practice, wherein each session on its own was insufficient for generating

Throat clicking as the initial symptom of Parkinson's disease.

The presenting manifestations of Parkinson's disease (PD) are variable, but a majority of patients note tremor as the initial symptom. Others complain of slowing of movements, loss of dexterity, fatigue, or changes in handwriting as initial symptoms. We describe a patient who developed an unusual clicking sound emanating from his throat as the initial manifestation of PD.

[Dysgraphia].

Dysgraphia (or handwriting difficulties) is common. It is often associated with neurodevelopmental disorders. In France, it is confirmed by the BHK test. Associated anxiety must be taken into account, as many young people can fall into vicious circles of avoidance. Subtypes of dysgraphia are found according to underlying neurodevelopmental difficulties (motor, visuo-spatial or reading difficulties).

Presentation matters: Surface text features and text quality in written narratives of Dutch high school students with and without dyslexia.

Presentation features such as spelling, punctuation and handwriting can influence the evaluation of general text quality. High school students with dyslexia might therefore be at a disadvantage, as at least their spelling performance is typically poor(er). Furthermore, these students might show less sophisticated linguistic features of texts, such as word length and sentence complexity, that might also be related to text quality. We compared narratives written by Dutch high school students (mean age 13.7 years) with (n = 28) and without (n = 29) dyslexia. Students with dyslexia's texts contain

Clinical Reasoning: a woman with rapidly progressive apraxia.

A 56-year-old woman presented with changes in balance, handwriting, and thinking. Approximately 1 year before her first visit, the patient developed difficulty walking, which caused multiple falls without serious injury. She also developed bilateral upper-extremity tremors that worsened with movement. At the time of her visit, she could barely sign her name.

Gait and graphology changes after ventriculoperitoneal shunting for idiopathic normal pressure hydrocephalus: an exploratory pilot study.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological syndrome characterized by gait impairment, cognitive decline, and urinary dysfunction. While gait improvement after ventriculoperitoneal shunting (VPS) is well established, postoperative changes in handwriting-related graphomotor function remain poorly investigated. This exploratory pilot study aimed to evaluate whether a multidimensional motor-oriented assessment, including clinical gait scores, instrumental gait analysis, and handwriting-related graphomotor features, may capture postoperative

Underlying functions associated with keyboarding performance of elementary-school students.

BACKGROUND: Keyboarding (Typing) is a major writing mode in educational settings in addition to, or as an alternative to, handwriting. Therefore, it is important that occupational therapists become experts on this activity, to support students' performance. Yet, the knowledge of keyboarding performance of elementary-school students, and the underlying functions it entails, is limited. AIM: To compare keyboarding performance (speed and accuracy) of 4th-grade students in copying and dictation keyboarding tasks, and to examine the role of underlying functions in predicting keyboarding performanc

Computerized Writing and Reading Instruction for Students in Grades 4 to 9 With Specific Learning Disabilities Affecting Written Language.

Computer scientists and educational researchers evaluated effectiveness of computerized instruction tailored to evidence-based impairments in specific learning disabilities (SLDs) in students in grades 4 to 9 with persisting SLDs despite prior extra help. Following comprehensive, evidence-based differential diagnosis for dysgraphia (impaired handwriting), dyslexia (impaired word reading and spelling), and oral and written language learning disability (OWL LD), students completed 18 sessions of computerized instruction over about 3 months. The 11 students taught letter formation with sequential
