Cochlear hearing loss in tympanoplasty.

In tympanoplasty, trauma to the cochlea is usually caused by the transmission of vibration induced during the removal of cholesteatoma, granulation tissue, and tympanosclerosis surrounding the ossicular chain. In 100 sequential cases 20 showed evidence of cochlear trauma. Hearing was recovered in 10 of these cases during the immediate postoperative period. In nine the hearing was partially recovered but remained below the preoperative bone-conduction level, and in one there was a total cochlear hearing loss. When the stapes footplate was fractured or dislocated and a perilymphatic fistula deve

Meningitis and sensorineural hearing loss.

A 12 month retrospective study was conducted on 54 children discharged from the Children's Hospital, Birmingham, Alabama, with a diagnosis of generalized meningitis, a major cause of post-natal sensorineural hearing loss (SNHL). Of these high risk patients, 38 or 70% had Haemophilus influenza meningitis and fully 40% of those children tested audiometrically were determined to have SNHL. Because there would appear to be an increase in SNHL in the post meningeal population, all children with a diagnosis of Haemophilus influenza, pneumococcal, or meningococcal meningitis should have an audiologic

[Pathology of labyrinthine hearing loss].

The most common causes auf sensorineural deafness lie in the cochlea. Other causes are less numerous, such that our presentation will be limited to cochleal disturbances. In a general introduction we shall attempt to analyse the structural reactions, metabolism and circulation of the cochlea and the behaviour of the inner ear lymph to a noxa. The monotony of the reactions will be described as well as the reversibility of some of the changes which are observed. All these reactions play a part in the normal functioning of the inner ear. The special part of the presentation will discuss the diffe

Hearing loss in skull fractures.

One hundred three cases of skull fractures in or around the temporal bone were reviewed for hearing loss. Of these, 100 patients had skull series, 66 had computed tomographic head scans, and 44 had polytomographic studies of the temporal bone. Hearing loss in head trauma can be grouped into four categories: conductive hearing loss, peripheral sensorineural hearing loss, central sensorineural hearing loss, and combinations of these hearing losses. The cause of conductive hearing loss and peripheral sensorineural hearing loss was usually identified by the type of temporal bone fracture. However,

Hearing loss in multiple sclerosis.

Hearing loss in an uncommon symptom in multiple sclerosis (MS). In nine patients with MS, seven with unilateral hearing loss and two with bilateral impairment, accompanying symptoms and signs included facial numbness, hemifacial paresis or spasms, ipsilateral limb ataxia, nystagmus, vertigo, tinnitus, and spastic-ataxic gait. Central auditory dysfunction was suggested by audiometric findings and/or by brainstem auditory evoked potentials in all nine patients. Clinical improvement in two was accompanied by return toward normal in the results of audiometric or electrophysiologic studies. Hearing

Sarcoidosis and fluctuating hearing loss.

Neurological manifestations of sarcoidosis occur in 5% of affected patients. Of the cranial nerves, the eighth nerve is the fourth most frequently involved. The central nervous system lesion is presumed to be a granulomatous meningitis that directly infiltrates the cranial occur in concert with other neurological findings, but may be the only presenting complaint. Sarcoidosis is not generally recognized as a cause of fluctuating sensorineural hearing loss. The fluctuating quality of the hearing loss has been noted by other investigators, but no patient has been previously reported with hearing

Hearing loss in Pfeiffer's syndrome.

A 14-year-old boy with conduction hearing loss, acrocephaly, minimum syndactyly, broad thumbs and big toes is classified as being affected with a form of acrocephalosyndactyly, termed Pfeiffer's syndrome (McK+10160) [12]. It is the first case of this syndrome presented in the otorhinolaryngological literature. During exploratory tympanotomy, the ossicular chain was found to be fixed. The incus seemed to be fixed in the epitympanum and there was ankylosis of the stapes. After antrotomy during a first stage, stapedectomy and teflon interposition during a second stage the air-bone gap disappeared

Cystic fibrosis and hearing loss.

The mucosal epithelium of the middle ear and Eustachian tube is in direct continuity with the upper respiratory tract. Since the otolaryngological aspects of cystic fibrosis (CF) are dominated by involvement of the paranasal sinuses, it might be assumed that children with CF would be expected to have a higher than usual incidence of middle ear disease. Eighty patients who were afflicted with CF had audiological evaluations, which consisted of hearing threshold levels (250 to 8,000 Hz) and speech and impedance audiometry. We found no greater incidence of a conductive or sensorineural hearing lo

Paget's disease and hearing loss.

Hearing loss, tinnitus, and vertigo have long been associated with Paget's disease of the bone. We reviewed the records of 463 patients with Paget's disease who were seen in the otolaryngology department. Hearing loss was common. It was our observation that mixed hearing losses were part of the disease process. Sensorineural hearing loss was the most frequent loss, but it usually was not part of the disease process. Tinnitus, vertigo, or both were seen in about 20% of these patients.

Sudden Hearing Loss Research Clinic.

There is still a considerable lack of knowledge about the etiology and treatment of sudden hearing loss, despite renewed interest among investigators. There is still no treatment regimen that has been demonstrated to be effective. Other clinics should be established to elucidate the factors surrounding the etiology and to as with sudden hearing loss have another diagnosable disorder, but careful follow-up is necessary. In our series this figure was approximately one-third. Luetic labyrinthitis, perilymphatic fistula, fluctuant hearing loss or endolymphatic hydrops, and acoustic neuroma are amo

Detection of syphilitic hearing loss.

Syphilis can cause sensorineural hearing loss that is potentially treatable. To determine the incidence of positive serological findings in patients with sensorineural hearing loss, the rapid plasma reagin card test (RPRCT) and the FTA-ABS test were performed in a study group composed of patients with sensorineural hearing loss of obscure origin. Of the 306 patients with sensorineural hearing loss, 20 (6.5%) demonstrated a positive FTA-ABS as compared with two of 100 (2%) of the control group. Considerable variation was found in the rate of the false-negative and the biological false-positive

Hearing loss. The physician's responsibility.

Hearing loss is a very important and common problem in the United States, affecting 13 to 14 million people. It is primarily a medical problem, requiring diagnosis and, when indicated, treatment by a physician, preferably in collaboration with an audiologist. History taking is the most important part of the diagnostic process, followed by a physical examination with special attention to the head, neck, and ears. The primary object of the workup is to exclude all possible extrinsic causes of hearing loss. Intrinsic or genetic deafness is a diagnosis of exclusion. After the hearing loss has been

Definition of fluctuant hearing loss.

In summary, fluctuant hearing loss is defined as a disorder of the inner ear characterized by fullness, roaring tinnitus, and fluctuations in hearing. It is believed to be caused by an inadequate absorption of endolymph from the endolymphatic sac, with or without one or more metabolic disorders, that interferes with the delicate balance between the production and absorption of endolymph and thus produces cochlear hydrops. This triad of fullness, roaring tinnitus, and fluctuant hearing loss resulting from cochlear hydrops is much more common than the quadrad of true turning vertigo, fullness, r
