Comprehensive Gene Panel Testing for Hearing Loss in Children: Understanding Factors Influencing Diagnostic Yield.

OBJECTIVE: To evaluate factors influencing the diagnostic yield of comprehensive gene panel testing (CGPT) for hearing loss (HL) in children and to understand the characteristics of undiagnosed probands. STUDY DESIGN: This was a retrospective cohort study of 474 probands with childhood-onset HL who underwent CGPT between 2016 and 2020 at a single center. Main outcomes and measures included the association between clinical variables and diagnostic yield and the genetic and clinical characteristics of undiagnosed probands. RESULTS: The overall diagnostic yield was 44% (209/474) with causative

Neonatal hearing screening - does failure in TEOAE screening matter when the AABR test is passed?

PURPOSE: Newborns who fail the transient evoked otoacoustic emissions (TEOAE) but pass the automatic auditory brainstem response (AABR) in universal newborn hearing screening (UNHS), frequently have no further diagnostic test or follow-up. The present study aimed to investigate whether hearing loss might be missed by ignoring neonatal TEOAE failure in the presence of normal AABR. METHODS: A retrospective analysis was conducted in newborns presenting between 2017 and 2021 to a tertiary referral centre due to failure in the initial UNHS. The main focus was on infants who failed TEOAE tests, but

A 3D Printed Human Ear Model for Standardized Testing of Hearing Protection Devices to Blast Exposure.

HYPOTHESIS: A 3D printed human temporal bone (TB) that is anatomically accurate would cost-effectively reproduce the responses observed in blast testing of human cadaveric TBs with and without passive hearing protection devices (HPDs). BACKGROUND: HPDs have become critical personal protection equipment against auditory damage for service members. Acoustic test fixtures and human TBs have been used to test and develop HPDs; however, the lack of a cost-effective, standardized model impedes the improvement of HPDs. METHODS: In this study, the 3D printed TB model was printed with flexible and ri

Adult Normative Data for the Adaptation of the Hearing in Noise Test in European French (HINT-5 Min).

Decreased speech-in-noise (SpIN) understanding is an early marker not only of presbycusis but also of auditory processing disorder. Previous research has shown a strong relationship between hearing disorders and cognitive limitations. It is therefore crucial to allow SpIN testing in subjects who cannot sustain prolonged diagnostic procedures. The objectives of this study were to develop a rapid and reproducible version of the Hearing in Noise Test (HINT-5 min), and to determine its adult normative values in free-field and monaural or binaural headphone conditions. Following an adaptive signal-

Contralateral occlusion test: The effect of external ear canal occlusion on predicting conductive hearing loss.

INTRODUCTION AND OBJECTIVES: The contralateral occlusion test (COT) has the potential to allow the quantitative evaluation of unilateral conductive hearing loss. The purpose of this study was to determine the accuracy of the test in predicting the degree of hearing loss. MATERIALS AND METHODS: Fifty-three subjects with unilateral conductive hearing loss were recruited from an otolaryngology department of a tertiary hospital. The COT was performed using 128, 256, 512, 1024 and 2048Hz tuning forks with the non-affected ear canal totally occluded to determine lateralization. Pure-tone audiometry

Using Speech Recall in Hearing Aid Fitting and Outcome Evaluation Under Ecological Test Conditions.

In adaptive Speech Reception Threshold (SRT) tests used in the audiological clinic, speech is presented at signal to noise ratios (SNRs) that are lower than those generally encountered in real-life communication situations. At higher, ecologically valid SNRs, however, SRTs are insensitive to changes in hearing aid signal processing that may be of benefit to listeners who are hard of hearing. Previous studies conducted in Swedish using the Sentence-final Word Identification and Recall test (SWIR) have indicated that at such SNRs, the ability to recall spoken words may be a more informative meas

The Performance-Perceptual Test (PPT) and its relationship to aided reported handicap and hearing aid satisfaction.

OBJECTIVE: Results of objective clinical tests (e.g., measures of speech understanding in noise) often conflict with subjective reports of hearing aid benefit and satisfaction. The Performance-Perceptual Test (PPT) is an outcome measure in which objective and subjective evaluations are made by using the same test materials, testing format, and unit of measurement (signal-to-noise ratio, S/N), permitting a direct comparison between measured and perceived ability to hear. Two variables are measured: a Performance Speech Reception Threshold in Noise (SRTN) for 50% correct performance and a Percep

Automated Question-and-Answer-Based Speech-In-Noise Hearing Test Using a Large Language Model.

A conversational speech-in-noise test administered without human supervision is desirable to reduce the need to attend the clinic. This study aimed to create and evaluate a novel question-and-answer-based speech-in-noise test called 'CHAT', where stimuli were synthesised using text-to-speech (TTS), responses captured using automatic speech recognition (ASR), and responses evaluated using a large language model (LLM). Three tests were compared: the newly developed CHAT, the Adaptive Sentence List (ASL) and the Four Alternative Auditory Feature Test (FAAF). CHAT used synthetic speech while ASL a

Toward a Real-World Technical Test Battery for Remote Microphone Systems Used with Hearing Prostheses.

Remote microphones (RMs) enable clearer reception of speech than would be normally achievable when relying on the acoustic sound field at the listener's ear (Hawkins, J Sp Hear Disord 49, 409-418, 1984). They are used in a wide range of environments, with one example being for children in educational settings. The international standards defining the assessment methods of the technical performance of RMs rely on free-field (anechoic) delivery, a rarely met acoustic scenario. Although some work has been offered on more real-world testing (Husstedt et al., Int J Audiol 61, 34-45. 2022), the area

"The Dichotic Digit Test" as an Index Indicator for Hearing Problem in Dementia: Systematic Review and Meta-Analysis.

BACKGROUND: Patients with dementia commonly have problems processing speech in the presence of competing background speech or noise. This difficulty can be present from the very early stages of dementia, and may be a preclinical feature of Alzheimer's disease. PURPOSE: This study investigates whether people with dementia perform worse on the dichotic digit test (DDT), an experimental probe of speech processing in the presence of competing speech, and whether test performance may predict dementia onset. RESEARCH DESIGN: Systematic review and meta-analysis. DATA COLLECTION AND ANALYSIS: A lit

Subclinical conductive hearing loss significantly reduces otoacoustic emission amplitude: Implications for test performance.

OBJECTIVES: Distortion product otoacoustic emissions (DPOAEs) are a time-efficient, non-invasive means of assessing the integrity of active inner ear mechanics. Unfortunately, the presence of even relatively minor conductive hearing loss (CHL) has been suggested to reduce the clinical utility of DPOAEs significantly. The primary aims of this study were to systematically evaluate the impact of CHL on DPOAE amplitude and to determine if ear-specific primary tone level manipulations can be used to mitigate CHL impact and recover DPOAE measurability. METHODS: For 30 young adults (57 ears) with no

Large-scale academic achievement testing of deaf and hard-of-hearing students: past, present, and future.

The first large-scale, nationwide academic achievement testing program using Stanford Achievement Test (Stanford) for deaf and hard-of-hearing children in the United States started in 1969. Over the past three decades, the Stanford has served as a benchmark in the field of deaf education for assessing student academic achievement. However, the validity and reliability of using the Stanford for this special student population still require extensive scrutiny. Recent shifts in educational policy environment, which require that schools enable all children to achieve proficiency through accountabi

Hearing loss in patients with enlarged vestibular aqueduct: air-bone gap and audiological Bing test.

The Bing test is based on the principle that occlusion of the external auditory meatus improves the perception of bone-conducted sounds unless there is a conductive hearing impairment. An air-bone gap has been reported in patients with large vestibular aqueduct (LVA) syndrome without apparent middle ear dysfunction. We therefore performed the Bing test on nine patients with this syndrome to evaluate whether it is associated with an air-bone gap or middle ear dysfunction. Bone conduction thresholds did not change significantly during the Bing test in any patient. Because an air-bone gap is obse

[200 years testing hearing disorders with speech, 50 years german speech audiometry -- a review].

UNLABELLED: FROM THE BEGINNIGS TO CLASSICAL SPEECH TESTS: The need for classification of different degrees of hearing disorders first arose when it seemed possible to treat deafness. Grapengiesser in Berlin 1801 had applied galvanic current to the ears of deaf children and reported some success. Pfingsten in Kiel in 1804 using this method was the first to use speech in diagnosing different degrees of deafness. He divided speech sounds into three classes: vowels, voiced consonants and voiceless consonants. Itard in Paris in 1821 gave a classification of five classes according to which sounds co
