BACKGROUND: The clinical course of children with advanced heart block secondary to Lyme disease has not been well characterized.
OBJECTIVE: To review the presentation, management, and time to resolution of heart block due to Lyme disease in previously healthy children.
METHODS: An IRB approved single-center retrospective study was conducted of all patients < 21 years old with confirmed Lyme disease and advanced second or third degree heart block between 2007 and 2017.
RESULTS: Twelve patients (100% male) with a mean age of 15.9 years (range 13.2-18.1) were identified. Six patients (50%) had

Long-term prostaglandin use is commonly associated with side effects such as cortical proliferation of the bones, hypertrophic pyloric stenosis, and soft tissue swelling of the extremities. We report a neonate with critical coarctation of the aorta, who developed second and third degree atrioventricular blocks associated with prolonged prostaglandin E1 (PGE1) infusion. Interestingly, these conduction blocks only occurred at low PGE1 dose. The rhythm disturbances resolved promptly with the discontinuation of PGE1 following surgical repair.

Autoimmune congenital heart block (ACHB) is an immune-mediated cardiac disease included among the manifestations collectively referred to as neonatal lupus. The placental transference of maternal Ro/La autoantibodies may damage the conduction tissues during fetal development leading to blocking of signal conduction at the atrioventricular (AV) node in an otherwise structurally normal heart. Irreversible complete AV block is the main cardiac manifestation of ACHB, but some babies may develop endocardial fibroelastosis, valvular insufficiency, and/or frank cardiomyopathies with significantly red

Autoimmune congenital heart block (CHB) is an immune-mediated acquired disease that is associated with the placental transference of maternal antibodies specific for Ro and La autoantigens. The disease develops in a fetal heart without anatomical abnormalities that could otherwise explain the block, and which is usually diagnosed in utero, but also at birth or within the neonatal period. Autoantibody-mediated damage of fetal conduction tissues causes inflammation and fibrosis and leads to blockage of signal conduction at the atrioventricular (AV) node. Irreversible complete AV block is the pri

BACKGROUND: Rotational atherectomy (RA) performed during percutaneous coronary intervention is commonly associated with distal embolization of calcific particles resulting in conduction abnormalities. As a result, temporary pacing is often used during these procedures. It is unknown whether temporary pacing should be routinely used.
METHODS: We performed an analysis of 134 consecutive patients who underwent RA at our institution between January 2011 and September 2013 for the presence of intraprocedural heart block or pacemaker activation.
RESULTS: A temporary pacemaker was inserted in 50% o

We report a case of syncope in a young patient who presented with high-degree, variable atrioventricular heart block. Despite having no other classic manifestations of Lyme disease, she was treated with intravenous ceftriaxone for Lyme carditis on high clinical suspicion due to geographic location. The heart block resolved within 24 h of treatment. Although rare, we demonstrate the importance of considering Lyme carditis in patients who present with new-onset heart block and a history of living in an endemic area. Initiation of empiric antibiotic therapy can lead to rapid resolution of this co

A 36-year-old man with a short neck, low hairline, and mild kyphoscoliosis, presented with history of syncope. Chest radiography revealed a diaphragmatic hernia. Computed tomography demonstrated fusion of C2-C6 vertebral bodies, Electrocardiography indicated complete heart block. Ultrasonography showed a right pelvic kidney. He was diagnosed with Klippel-Feil syndrome and underwent permanent pacemaker implantation and corrective surgery for the congenital diaphragmatic hernia.

A variety of diseases, other than the common Lev-Lenègre disease, are associated with cardiac conduction system abnormalities. These include acute processes, such as acute rheumatic fever, and other disorders, such as sarcoidosis, connective tissue disorders, neoplasms, and bacterial endocarditis with cardiac abscess formation. The purpose of the study is to raise awareness of these rare conditions. We present 10 adult patients (4 males and 6 females) with a mean age of 47 years (range: 19-69), with various rare diseases associated with heart block, who needed temporary or permanent pacemaker

BACKGROUND: Congenital complete heart block (CCHB) is an uncommon although important and potentially serious condition contributing to neonatal morbidity and mortality.
AIMS: To study the characteristics and outcomes of infants born with CCHB at a single tertiary centre.
METHODS: A retrospective review of all infants with CCHB over the last 20 years was carried out to determine the outcomes, and the indications and timing of pacemaker insertion.
RESULTS: Fifteen live born infants (10 male, 5 female) with CCHB were identified. Their mean (and SD) gestation and birth weight were 37 (3.3) week

Describe a case of a female patient having anti-Ro/SSA antibodies without any other risk factor or collagen disease. In her first pregnancy a congenital heart block and hydrops in the fetus were diagnosed, and these caused stillbirth. In a second pregnancy an in utero treatment resulted in the succesful delivery of a normal child.

Congenital heart block (CHB) is a conduction abnormality that affects hearts of foetuses and/or newborn to mothers with autoantibodies reactive with the intracellular soluble ribonucleoproteins 48-kD La, 52-kD Ro and 60-kD Ro. CHB carries substantial mortality and morbidity, with more than 60% of affected children requiring lifelong pacemakers. Several hypotheses have been proposed to explain the pathogenesis of CHB. These can be grouped under three main hypotheses: Apoptosis, Serotoninergic and Ca channel hypothesis. Here, we discuss these hypotheses and provide recent scientific thinking tha

Lyme disease is a recognised cause of atrioventricular heart block. In the majority of cases conduction disturbances are reversed with antibiotic treatment. The diagnosis of Lyme disease is not always obvious and its recognition is important if unnecessary implantation of a permanent pacemaker is to be avoided. A 34-year-old man presented to our hospital with complete heart block due to Lyme carditis. This resolved completely within a few days after treatment with appropriate antibiotics.

BACKGROUND: Listeria monocytogenes is a food-borne pathogen that primarily affects pregnant women. Cardiac involvement is an uncommon complication of infection. We present a case of a gravida with Listeria bacteremia at 36 weeks of gestation.
CASE: Two of a patient's blood cultures grew L monocytogenes after she experienced chills, headache, myalgia, and diarrhea. The patient was treated with antibiotics for 48 hours, and then labor was induced, resulting in a normal delivery with a healthy neonate. On day 5 postpartum, the patient developed progressive heart block, resulting in a third-degre

OBJECTIVES: A method for the induction of complete heart block (CHB) by ethanol injection and its success rate in a pig model of acute right ventricular failure is reported. Additionally, a review of the literature for the induction of CHB in laboratory animals is detailed. The literature review was undertaken to both compare our rate of success with other methods and provide insight into our technique and refine its implementation.
BACKGROUND: Animal models of CHB have facilitated the understanding of therapeutics for various cardiac pathologies in humans. In our laboratory, CHB in pigs is u

Complete heart block associated with Wegener's granulomatosis (WG) is rare, and has not previously been reported with 'limited' WG. The case of a 36-year-old man who presented with complete heart block due to 'limited' WG [positive cytoplasmic antineutrophil cytoplasmic antibodies (c-ANCA) on indirect immunofluorescence, positive serum antibodies to proteinase-3, and inflammatory sinus disease seen on computerized tomography (CT) without renal or pulmonary involvement] is presented. In addition, a gallium-scan fused with a myocardial perfusion scan and cardiac magnetic resonance imaging (MRI)

PURPOSE: Treatment with H2 receptor antagonists may cause the heart to be more susceptible to atrioventricular conduction delay when exposed to an overwhelming insult by histamine released during an anaphylactic reaction. We present the case of a woman, pretreated with ranitidine, who developed 3:1 heart block secondary to latex anaphylaxis. We propose that H2 antagonist premedication alone in patients susceptible to anaphylaxis increases their risk of heart block.
CLINICAL FEATURES: A 38 yr old obese woman with cervical cancer presented for a radical hysterectomy. Systems review yielded a hi

Doxapram, a respiratory stimulant, is used to treat idiopathic apnea of prematurity. The side effects reported are minimal. We present three cases of second-degree atrioventricular block caused by QT interval prolongation associated with doxapram administration. All three infants returned to normal sinus rhythm after doxapram administration was stopped.

BACKGROUND: Congenital complete heart block is an uncommon fetal arrhythmia characterized by complete dissociation of the atrial and ventricular contractions.
CASES: Two patients were diagnosed with isolated congenital complete heart block in the second trimester of pregnancy. Successful vaginal delivery was achieved by both.
CONCLUSION: A monitoring protocol algorithm to be used by the clinician in managing term patients with isolated complete congenital heart block is presented. Vaginal delivery can be safely accomplished in patients with isolated congenital complete heart block, with the
