The usefulness of echocardiography in the surgical management of infants with congenital heart disease.

Between January 1988 and August 1991, at The Children's Heart Center, Atlanta, 83 infants with congenital heart defects were diagnosed by echocardiography and underwent surgery without cardiac catheterization. The diagnostic categories included 46 infants with left heart obstructive lesions, 19 infants with cyanotic heart lesions, and 18 infants with miscellaneous lesions. Forty-five infants (55%) underwent surgery under cardiopulmonary bypass. There were three errors in diagnosis, yielding a diagnostic accuracy of 95%. Many infants with congenital heart disease can be accurately and completel

Predicting outcomes in patients with low ejection fraction undergoing coronary artery bypass graft.

INTRODUCTION: Reduced left ventricular ejection fraction (LVEF) is a well-known predictor of adverse events after cardiac surgery. We aimed to assess the outcomes in patients with low LVEF undergoing coronary artery bypass graft. METHODS: In this retrospective cohort, we included all patients with left ventricular ejection fraction ≤ 40 who underwent coronary artery bypass grafting between March 2007 and March 2016 (with a median follow-up of nine years) at Tehran Heart Center. Demographics and clinical characteristics were extracted from the data registry. Akaike information criterion (AIC)

Charcot-Marie-Tooth syndrome and surgical management for left main coronary artery disease.

A 36-year-old male had coronary heart disease with a 12-year history of muscular weakness in his bilateral lower extremities and gait disturbance characterized by classical features of gastrocnemius and peroneal muscular atrophy. This is a clinical report of the first case of Charcot-Marie-Tooth disease (type 2) associated with the left main coronary artery disease requiring bypass surgery with cardiopulmonary bypass. The surgical procedure and postoperative course were uneventful. The patient was followed for 18 months postoperatively.

Does adenosine pharmacologically precondition human myocardium during coronary bypass surgery?

AIM: Adenosine (Ado) triggers ischemic preconditioning. We investigated whether Ado provides additional myocardial protection in patients during intermittent aortic cross-clamping (IAC) bypass surgery. METHODS: The placebo group was made of 15 male of 66+/-8 years while the Ado group was made of 19 male of 65+/-10 years. The patients of the Ado group had a 3-vessel heart disease and were treated with elective surgery. With the aortic cross-clamping, Ado or vehicle were infused over 10 min at systemic pressure together with sufficient blood via the aortic root. Blood samples before anaesthesia

Combined antegrade/retrograde cardioplegia for myocardial protection: a clinical trial.

The role of retrograde coronary sinus perfusion in the preservation of ischemic myocardium is controversial. We evaluated the use of combined antegrade and retrograde cardioplegia in 59 patients undergoing coronary artery bypass surgery. Nineteen patients were administered antegrade cardioplegia, whereas 40 patients were administered antegrade plus retrograde cardioplegia. Hemodynamic data were obtained before the onset of cardiopulmonary bypass and at 1, 2, 4, 8, 16, and 24 hours after cessation of cardiopulmonary bypass. Myocardial function was assessed by measuring systemic blood pressure,

Heart Transplantation of a Preterm Infant With HLHS.

A 33-week gestation, 1.75-kg female infant with mitral stenosis/aortic atresia variant of hypoplastic left heart syndrome and severe ventriculo-coronary connections underwent surgical septectomy and bilateral pulmonary artery banding at five weeks of age (2.10 kg). After separation from bypass, she developed hemodynamic instability requiring venoarterial extracorporeal membrane oxygenation support. She was listed for heart transplantation and transplanted after three days of support with an oversized heart (4.7:1 donor-recipient weight ratio).

Transapical aortic valve implantation in patients with severely depressed left ventricular function.

OBJECTIVES: Transapical aortic valve implantation significantly reduces operative risk in elderly patients with aortic valve stenosis and comorbidities. However, it is unknown whether this procedure is feasible in patients with advanced heart failure. METHODS: Between April 2008 and July 2010, 258 patients underwent transapical aortic valve implantation. Twenty-one patients had advanced heart failure with decompensation and a left ventricular ejection fraction of 10% to 25%. The mean age of these patients was 74 ± 11 years (range, 36-88 years). The mean left ventricular ejection fraction was

Surgical Management of a Giant Left Atrial Appendage Aneurysm in a Symptomatic Neonate.

A 4-day-old, 3.7-kg neonate presented to our institution with a systolic heart murmur and signs of respiratory distress and was found to have a giant left atrial appendage aneurysm on transthoracic echocardiogram. Given the compression of the left ventricle and signs of respiratory failure necessitating mechanical ventilation, the decision was made to proceed with surgical resection under cardiac arrest with cardiopulmonary bypass.

Left ventricular pseudoaneurysm after coronary artery bypass and valve replacement for post-infarction mitral regurgitation.

Most left ventricular inferior wall aneurysms are classified as false, and they have a narrow neck and exhibit rapid flow. Inferior wall pseudoaneurysms that develop soon after primary percutaneous intervention and coronary artery bypass grafting for acute myocardial infarction are rare. We report the case of a 64-year-old man who had a wide-necked left ventricular inferior wall pseudoaneurysm that developed soon after surgery for an acute myocardial infarction and post-infarction mitral regurgitation that occurred as a mechanical complication. The surgery consisted of coronary artery bypass g

Hybrid palliation in hypoplastic left heart syndrome.

PURPOSE OF REVIEW: Despite progressive improvement in surgical results, hypoplastic left heart syndrome remains one of the congenital heart abnormalities with the greatest morbidity and mortality. Hybrid approaches to management, combining surgical and interventional catheterization procedures, have been introduced to minimize exposure to cardiopulmonary bypass, and improve outcomes for these high-risk infants. RECENT FINDINGS: First-stage palliation of hypoplastic left heart syndrome has been performed as a hybrid procedure combining surgical pulmonary artery banding with catheterization ste

A 57-year-old man was diagnosed with dilated cardiomyopathy and coronary stenosis. His electrocardiogram showed a complete left bundle branch block and a prolonged QRS interval. As appropriate medical therapy improved his symptoms, he did not visit out-patient clinic after discharge. A year later, he presented with exertional chest oppression and was readmitted with severe heart failure. Although medical therapy was provided, his condition did not improve. Left ventricular systolic dysfunction and stenosis of the left anterior descending artery were aggravated. We performed coronary artery byp

Orthotopic heart transplantation with preoperative unsuspected left superior vena cava and absence of right superior vena cava.

We report the case history of a 47-year-old man who underwent orthotopic heart transplantation for ischemic cardiomyopathy. At the time of cardiectomy, the patient was found to have a persistent left superior vena cava draining into the coronary sinus and complete absence of his right superior vena cava. The donor heart had been removed without knowledge of this venous anomaly; consequently, the donor's superior vena cava and innominate vein were not harvested. The persistent left superior vena cava was cannulated for cardiopulmonary bypass. The recipient's heart was excised along the atrial v

Late surgical results for ischemic mitral regurgitation. Role of wall motion score and severity of regurgitation.

The indication for concomitant valve operations for ischemic mitral regurgitation is examined in 120 consecutive patients with regurgitation who had coronary bypass. Ischemic mitral regurgitation was mild in 56%, moderate in 18%, and severe in 27%. Compared with patients without mitral regurgitation who underwent coronary bypass, significantly more patients with regurgitation had cardiomegaly (31% versus 5%), left heart failure (42% versus 6%), and abnormal wall motion scores (71% versus 42%). Eighty-three patients (69%) with ischemic mitral regurgitation had coronary bypass alone and 37 (31%)

[Ischemic cardiomyopathy: possibilities of surgical treatment].

The high morbidity and mortality in patients with serious heart failure is a therapeutic challenge for current medicine. The leading cause of left ventricular dysfunction remains ischemic heart disease. Coronary artery bypass surgery is a treatment of choice in moderate-to-severe ischemic cardiomyopathy. The scarcity of completed prospective randomized clinical trials and high surgical risks create significant uncertainty concerning the optimal current treatment strategy. The role of imaging techniques of assessment for myocardial viability based on current guidelines may be very important in

[Atresia of the left coronary ostium. Repair in a 2-month-old infant].

A case of atresia of the left coronary ostium revealed by neonatal heart failure is reported. The initial diagnosis was anomalous origin of the left coronary artery from the pulmonary artery. At surgery performed in this 6-week old infant the diagnosis was amended and the malformation was repaired. Soon after the operation the child rapidly developed hypertrophic "myocardiopathy" of the left ventricle. Seven and a half months later, he is asymptomatic and the echocardiographic parameters of left ventricular systolic function are gradually returning to normality. Atresia of the left coronary os

Phenoxybenzamine in the treatment of hypoplastic left heart syndrome: a core review.

Perioperative management of neonates after the Norwood procedure is extremely complex. Limited reserve of the neonatal single ventricle and the parallel arrangement of the pulmonary and systemic circuits result in a tenuous balance between pulmonary and systemic blood flows. Precise manipulations of both pulmonary and systemic vascular resistance are necessary to prevent excessive pulmonary blood flow at the expense of systemic oxygen delivery. An emerging treatment strategy aimed at improving early mortality is the intraoperative administration of phenoxybenzamine, a profound systemic vasodil
