Catheterization and cardiovascular interventions : official journal of the Society for Cardiac Angiography & Interventions2016eng
Bicuspid aortic stenosis (BAS) has been excluded in clinical trials on transcatheter aortic valve replacement (TAVR) due to the presumed uneven expansion of the aortic prosthesis, leading to significant paravalvular regurgitation (PVR). There is no transcatheter heart valve (THV) commercially approved for treating BAS. The Lotus Valve System mitigates PVR by possessing an adaptive seal and being fully re-positionable. The latter is also important in preventing embolization, as the location of prosthesis fixation in BAS could be variable due to the presence of less expandable raphe. We report o

Patients with rheumatic aortic stenosis and prior mechanical mitral valve replacement present unique challenges for transcatheter aortic valve replacement, particularly when the aortomitral distance is short. This case series examines 4 such patients, focusing on procedural complexities, imaging strategies, and outcomes. Preprocedural multidetector computed tomography and intraprocedural transesophageal echocardiography were critical in assessing anatomical constraints and optimizing valve deployment. Key considerations included preventing mitral prosthesis interference, mitigating left ventri

The choice of prosthesis type in patients with valvular heart disease should always be individualised. The treating heart team must weigh the concerns surrounding durability of bioprosthetic valves compared with mechanical valves and the need for lifelong anticoagulation required with mechanical valves. In general, guidelines recommend that patients under the age of 60 would benefit from a mechanical valve, and those over 70 would benefit from a bioprosthetic valve. We would argue, in this context, that the most appropriate choice for this patient would be undertaking a mitral valve replacemen

The authors report the case of a 49-year-old female with a history of rheumatic valvular heart disease who underwent valve surgery in 1997, with implantation of St. Jude prosthetic valves in aortic and mitral position. She was asymptomatic until the end of July 2001, when she was admitted to Garcia de Orta Hospital emergency unit because of heart failure, fever and suspicion of endocarditis. Cardiologic evaluation was requested and the transthoracic (TTE) and transesophageal (TEE) echocardiograms revealed vegetations on the prosthetic mitral valve. Blood cultures were negative. She started emp

BACKGROUND: Carcinoid tumor with consecutive endocardial fibroelastosis of the right heart, known as carcinoid heart valve disease (CHVD) or Hedinger's syndrome, is accompanied by combined right-sided valvular dysfunction with regurgitation and stenosis of the affected valves. Cardiac surgery with replacement of the tricuspid and/or pulmonary valve is an established therapeutic option for patients with Hedinger's syndrome. Little is known about the long term outcome and the choice of prosthesis for the pulmonal position is still a matter of debate.
METHODS: The authors report three cases of p

We report the case of a 77-year-old man with heart failure (NYHA class IV), angina (CCS class III) and multiple comorbidities. The evaluation showed severe aortic sten osis and left main coronary disease. Due to the very high perioperative risk, conventional surgery was denied. It was therefore decided to perform a combined percutaneous intervention. First, left main PCI was successfully performed. A few days later, a percutaneous aortic valve prosthesis was implanted via a femoral approach using the CoreValve ReValving system. Immediately after prosthesis implantation, the control angiogram s

In this article, we report the case of a 58-year-old man who was admitted to our institution with acute dyspnea and poor hemodynamic condition. He had undergone double mitral-aortic valve replacement five years earlier for rheumatic heart disease. Transthoracic echocardiography (TTE), cinefluoroscopy, and transesophageal echocardiography (TEE) revealed a significant reduction in the opening of the mechanical mitral valve leaflets. An urgent redo surgery was performed, during which the pannus was removed, ensuring the restoration of normal leaflet motion. This clinical case presents two rare ci

Over the last 50 years medicine and technology have progressed to the point where it has become commonplace to safely replace damaged or diseased heart valves with mechanical and biological prostheses. Despite the advancements in technology current valve substitutes continue to have significant limitations with regards to thrombogenicity, durability, and inability to grow or remodel. In an attempt to overcome the limitations of currently available valve prosthesis, heart valve tissue engineering has emerged as a promising technique to produce biological valve substitutes. Currently, the field

Percutaneous placement of transcatheter heart valves for treatment of degenerated surgical valves in the aortic and mitral position is an emerging therapy for selected high-risk patients. Here we describe in detail the first case in the literature of a patient (female, 72 years old, log EuroSCORE 22.9%) with a degenerated biological mitral prosthesis which was successfully treated by transapical implantation of a Lotus valve. The case described demonstrates the very controlled feasibility of valve-in-valve treatment for a degenerated mitral bioprosthesis with a mechanically expanding Lotus val

A 64-year-old man was referred for aortic valve re-replacement due to moderate-to-severe stenosis that developed 10 years after complete aortic root replacement using a stentless valve composite graft. He also had coronary heart disease and a mitral valve defect with predominant insufficiency. The patient underwent re-do surgery consisting of coronary artery bypass grafting, mitral valve replacement, and replacement of the valve prosthesis within the aortic conduit, which I believe is the first report of such a procedure.

The long-term follow-up of patients with bioprosthetic valves manufactured at the Instituto Nacional de Cardiología Ignacio Chávez is presented. From February 1983 to May 1990, 1068 patients were operated and 1252 valves were replaced. Eighty two percent had rheumatic heart disease with one or more injured valves. In 17% it was a replacement of a failing valve. Seventy three percent of the patients had a low or medium-low socioeconomical status. There were 176 perioperative deaths in the first month after the operation and 150 patients were lost to follow-up. The rest (740) are the subject of

BACKGROUND AND AIMS OF THE STUDY: Prompted by anecdotal evidence and observations by surgeons, an investigation was undertaken into the potential differences in implanted aortic valve prosthesis sizes, during aortic valve replacement (AVR) procedures, between northern and southern European countries.
METHODS: A multi-institutional, non-randomized, retrospective analysis was conducted among 2,932 patients who underwent AVR surgery at seven tertiary cardiac surgery centers throughout Europe. Demographic and perioperative variables including valve size and type, body surface area (BSA) and early

Valve ring abscess complication of infective endocarditis increases the expected morbidity and mortality rates of patients, but is seldom recognized by available noninvasive techniques. In our study, two-dimensional echocardiography successfully detected valve ring abscesses in eight patients with infective endocarditis affecting aortic valve prosthesis. Echocardiography showed the perivalvular abscess as an echo-free space in all patients. Prosthetic vegetations were seen in the only patient who had a biological prosthesis and excessive prosthetic rocking was observed in cases with severe aor

We report a case of a 57-year-old patient with a history of a Starr-Edwards mitral valve prosthesis and DeBakey-Surgitool aortic valve prosthesis implanted 30 years ago who presented with symptoms consistent with class IV heart failure. The patient had been on no anticoagulation for approximately 30 years secondary to recurrent epistaxis occurring two years after starting warfarin therapy postoperatively. Throughout the patient's lifetime he experienced no thromboembolic complications from the lack of anticoagulation, despite developing concomitant atrial fibrillation approximately ten years p

BACKGROUND: The St. Jude Medical prosthesis has become the most commonly used artificial heart valve, yet few data are available in the pediatric population. This study addresses results of the use of this prosthetic valve in pediatric patients over a 13-year time frame.
METHODS: From January 1982 through June 1994, 73 patients (49 male) underwent 75 valve replacements using the St. Jude Medical prosthesis at Minneapolis Children's Medical Center. Follow-up was complete in 71 patients, with a mean follow-up of 39 +/- 32 months (mean +/- standard deviation; range, 4 to 142 months).

BACKGROUND: Transcatheter aortic valve implantation is a well-established treatment option in older patients with symptomatic severe aortic stenosis. Balloon-expandable valve migration is a rare but potentially life-threatening complication.
CASE SUMMARY: We report a case of an 87-year-old man with severe aortic stenosis who underwent transfemoral Edwards Sapien 3 valve implantation complicated by delivery-balloon rupture. During deployment, we observed a pressure drop, resulting in incomplete valve expansion. Acute prosthesis migration into the left ventricle occurred. The prosthesis was cro

BACKGROUND: Infective endocarditis (IE) is an infection involving either native or prosthetic heart valves, the endocardial surface of the heart or any implanted intracardiac devices. IE is a rare condition affecting 3-15 patients per 100,000 population. In-hospital mortality rates in patients with IE remain high at around 20% despite treatment advances. There is no consensus recommendation favoring either bioprosthetic valve or mechanical valve implantation in the setting of IE; patient age, co-morbidities and preferences should be considered selecting the replacement prosthesis.

BACKGROUND: The UFO procedure is an established surgical technique to treat extensive endocarditis of the aortic or mitral valve with involvement of the intervalvular fibrous body (IVFB). This technique can be used for radical resection of the whole infected tissue. Independently of the size of the infected region it is unavoidable to replace both valves. In this high-risk re-do case we performed a modified so-called hemi-UFO procedure with preservation of the mitral valve. We present a 71-year-old male patient initially diagnosed with severe stenosis of the aortic valve. An aortic valve repla

We describe the pathologic findings at autopsy in 96 persons who had undergone prosthetic heart valve implantation. The most common indication for implantation was rheumatic heart disease (58%), followed by infective endocarditis (22%). Cardiac failure led to death in two thirds of the cases, most often from infective endocarditis (18%) and congestive heart failure (13%). Failure of the prosthetic valve caused death in eight cases. There were 13 deaths intraoperatively or in the immediate postoperative period. Patients with mechanical prostheses survived longer than did those with bioprosthese

BACKGROUND AND AIM OF THE STUDY: Mechanical heart valve implantation requires long-term anticoagulation because of thromboembolic complications. Recent studies have indicated that the relatively high wall shear stresses and negative pressure transients developed during the valve closing phase may be dominant factors inducing thrombus initiation. The study aim was a two-dimensional (2D) functional simulation of flow past bileaflet heart valve prosthesis during the closing phase, incorporating the fluid-structure interaction analysis to induce motion of the leaflets.
METHODS: The fluid-structur
