Management of portal hypertension based on portal hemodynamics.

Portal hypertension is most commonly caused by chronic liver disease. As liver damage progresses, portal pressure gradually elevates and hemodynamics of the portal system gradually change. In normal liver, venous returns from visceral organs join the portal trunk and flow into the liver (hepatopetal blood flow). As portal pressure increases due to liver damage, congestion of some veins of the visceral organ occurs (blood flow to and from). Finally, the direction of some veins (the left gastric vein in particular) of the visceral organ change (hepatofugal blood flow) and develop as collateral v

Clarifications on Continuous Renal Replacement Therapy and Hemodynamics.

OBJECTIVE: Continuous renal replacement therapy (CRRT) is a continuous process of bedside blood purification which is widely used in the treatment of acute kidney injury (AKI) and for fluid management. However, since AKI and fluid overload are often found to be associated with hemodynamic abnormalities, determining the relationship between CRRT and hemodynamics remains a challenge in the treatment of critically ill patients. The aim of this review was to summarize key points in the relationship between CRRT and hemodynamics and to understand and monitor renal hemodynamics in critically ill pat

Echocardiographic Evaluation of Hemodynamics in Neonates and Children.

Hemodynamic instability and inadequate cardiac performance are common in critically ill children. The clinical assessment of hemodynamic status is reliant upon physical examination supported by the clinical signs such as heart rate, blood pressure, capillary refill time, and measurement of the urine output and serum lactate. Unfortunately, all of these parameters are surrogate markers of cardiovascular well-being and they provide limited direct information regarding the adequacy of blood flow and tissue perfusion. A bedside point-of-care echocardiography can provide real-time hemodynamic infor

Cerebrovascular hemodynamics in fetuses with congenital heart disease.

BACKGROUND AND OBJECTIVE: It is hypothesized that diminished cerebral vascular resistance or the "brain sparing effect" is associated with fetuses with complex congenital heart defects (CHD) and may affect their neurodevelopmental outcome. An alternative explanation is that it is related to the location, cardiac output, pressure, and resistance in left heart obstructive CHDs. We sought to determine the effects of various left and right heart obstructive defects on the cerebral and placental hemodynamics and to evaluate the utility of these variables for the assessment and prognosis of CHDs. Hemodynamics of patient-specific aorta-pulmonary shunt configurations.

Optimal hemodynamics in aorta-pulmonary shunt reconstruction is essential for improved post-operative recovery of the newborn congenital heart disease patient. However, prior to in vivo execution, the prediction of post-operative hemodynamics is extremely challenging due to the interplay of multiple confounding physiological factors. It is hypothesized that the post-operative performance of the surgical shunt can be predicted through computational blood flow simulations that consider patient size, shunt configuration, cardiac output and the complex three-dimensional disease anatomy. Utilizing

Fetal cardiovascular hemodynamics in twin-twin transfusion syndrome.

Twin-twin transfusion syndrome (TTTS) complicates 10-15% of monochorionic-diamniotic (MCDA) pregnancies. It originates from unbalanced transfer of fluid and vasoactive mediators from one twin to its co-twin via placental anastomoses. This results in hypovolemia in the donor and hypervolemia and vasoconstriction in the recipient twin. Consequently, the recipient demonstrates cardiovascular alterations including atrioventricular valve regurgitation, diastolic dysfunction, and pulmonary stenosis/atresia that do not necessarily correlate with Quintero-stages. Central Hemodynamics and Arterial Stiffness in Systemic Sclerosis.

Although microvascular disease is a hallmark of systemic sclerosis (SSc), a higher prevalence of macrovascular disease and a poorer related prognosis have been reported in SSc than in the general population. The simultaneous assessment of prognostically relevant functional properties of larger and smaller arteries, and their effects on central hemodynamics, has never been performed in SSc using the state-of-the-art techniques. Thirty-four women with SSc (aged 61±15 years, disease duration 17±12 years, and blood pressure 123/70±18/11 mm Hg) and 34 healthy women individually matched by age and m

Ventilatory efficiency and resting hemodynamics in hypertrophic cardiomyopathy.

PURPOSE: In patients with systolic heart failure, the ability of cardiopulmonary exercise testing (CPX) variables to reflect pathophysiology is well established. The relationship between CPX and pathophysiology has, however, not been thoroughly investigated in patients with nonobstructive hypertrophic cardiomyopathy (NHCM). The objective of this study was to assess the ability of CPX variables to reflect resting hemodynamics in patients with nonobstructive hypertrophic cardiomyopathy NHCM. METHODS: We performed CPX and right heart catheterization on 83 subjects with NHCM (51 male/32 female, m

Pulmonary hemodynamics in children living at high altitudes.

There are numerous publications on altitude-related diseases in adults. In addition, an International Consensus Statement published in 2001 deals with altitude-related illnesses occurring in lowland children who travel to high altitudes. However, despite the millions of children living permanently at high altitudes around the world, there are few publications on altitude-related diseases and pulmonary hemodynamics in this pediatric population. In this paper, we review the published literature on this subject. First, the pulmonary hemodynamics of healthy children (newborns, infants, children, a

Hyperventilation before resistance exercise: cerebral hemodynamics and orthostasis.

UNLABELLED: Hyperventilation performed by athletes during preparation for resistance exercise might contribute to reports of postexercise orthostatic instability. PURPOSE: To test the hypothesis that post-resistance exercise orthostatic instability is associated with exaggerated reductions of cerebral blood-flow velocity after hyperventilation. METHODS: We recorded the ECG, end-tidal CO2, beat-by-beat finger arterial pressure, and cerebral blood-flow velocity in 10 healthy subjects. Subjects performed 10 repetitions of recumbent leg press using resistance equivalent to 80% of their six-repet

Stenosis hemodynamics: from physical principles to clinical indices.

Clinical evaluation of patients with aortic stenosis entails hemodynamic determinations and interpretations that depend on complex blood flow patterns. Although pressure gradient and Doppler velocity are intrinsically adjusted for a wide range of species and body size, they are highly flow-dependent indices that can vary among patients with physically identical stenosis areas and within individuals between determinations. Other indices, such as the Gorlin area, are adjusted for flow. All stenosis indices derived from hemodynamic measurements, however, must exhibit some degree of flow dependenc

Thoracic sympathectomy: effects on hemodynamics and baroreflex control.

UNLABELLED: Endoscopic thoracic sympathectomy at T2-T4 is an effective and safe treatment for primary axillary and palmar hyperhidrosis and facial blushing refractory to conventional treatment. T2 and T4 ganglia however are in the direct pathway of sympathetic innervation of the heart and part of the vasomotor nerves. In this study we investigate possible changes in steady-state hemodynamics as well as in beat-to-beat cardiovascular control after thoracoscopic sympathectomy of T2-T4. In 12 patients we measured continuously heart rate (HR) and blood pressure (BP) (non-invasively with Finapres)

[Splanchnic hemodynamics in acute cholecystitis and mechanical jaundice].

The authors analyze results of the investigation of hemodynamics in the abdominal cavity vessels by the method of duplex scanning in 30 patients with acute cholecystitis, in 30 patients with mechanical jaundice and cholelithiasis and in 30 healthy subjects. It was found that these patients had less blood flow along the upper mesenterial artery and the portal vein. As a compensation the blood flow increased along the hepatic artery.

[The correction of prostatic hemodynamics in chronic prostatitis].

Prostatic rheography findings evidence for marked disturbances of venous outflow in patients with chronic prostatitis. To correct the hemodynamics, rectal magnitotherapy was used in two regimens: with constant frequency of the magnetic field (5000 Hz) and fluctuating magnetic field (1-20000 Hz). The comparison of the results gave priority to the latter regimen because it provided reduction of the treatment duration and normalization of rheographic parameters in 37 (90.2%) patients. The authors recommend rectal magnitotherapy for combined treatment of chronic prostatitis.

Influence of anti-hypertensive drugs on glomerular hemodynamics.

Alterations in glomerular hemodynamics may play an important role in the progression of renal dysfunction. Accordingly, treating not only systemic hypertension, but also glomerular hypertension is important for conservation of renal function in patients with renal disease. However, glomerular capillary pressure does not necessarily change in parallel with systemic blood pressure due to unique mechanisms that control the resistance of glomerular afferent and efferent arterioles. While myogenic response and tubuloglomerular feedback play an important role in controlling afferent arteriolar resis

Strangulation injuries in children. Part 2. Cerebrovascular hemodynamics.

The cerebrovascular hemodynamics were recorded in two children with comparable hypoxic-ischemic injuries after strangulation. Monitoring was initiated within 13 hours of injury and continued for at least 38 hours. The profile included continuous measurements of cortical regional cerebral blood flow (rCBF) with a subdural thermal diffusion probe, intracranial pressure, mean arterial pressure, and expired CO2 tension. Data sets were obtained every 15 minutes or every 5 minutes during epochs of hyperventilation and inotropic support. Arterial CO2 and oxygen content and pH and, in the second patie
