Heterotopic ossification after hip surgery in cerebral palsy.

Heterotopic ossification is a post-surgery complication occasionally observed in patients with infantile cerebral palsy. In the majority of the cases such lesions are found at the hip after surgery on the skeleton and the soft tissues. At our Institute in the last five years, this complication has been observed four times, in 39 patients, who underwent mainly soft tissue releases because of flexion and adduction contracture of the hip. In these cases, as in a further patient with myelomeningocele, the periarticular heterotopic ossification appeared in the hip after tenotomy of the ilio-psoas a

The relationship of lower extremity strength and gait parameters in patients with post-polio syndrome.

Relationships between lower extremity strength and stride characteristics were studied in 24 patients with post-polio syndrome. Maximum isometric torques were measured in the ankle plantar flexors, hip and knee extensors, and hip abductors. Gait velocity, stride length, and cadence were recorded during free and fast walking. Step-wise regression analysis was performed to determine which muscle groups best predicted ambulatory function. Plantar flexion torque was the best predictor of velocity (r = .55 free walking and r = .76 fast) and cadence (r = .46 free and r = .58 fast). The combination o

Avascular necrosis of the femoral head as sequela of fracture of the greater trochanter. A case report and review of the literature.

After a fall from a tree, a 12-year-old boy sustained a fracture of the greater trochanter and was subsequently treated by open reduction. Twenty months after surgery, the hip developed to a flexion contracture, limb-length discrepancy, a collapsed, irregular-shaped femoral head with sclerotic areas, and a metaphyseal cyst. A growing child with avascular necrosis of the femoral head after fracture of the greater trochanter seems not to have been reported in the English language literature.

Primary recognition and treatment of congenital dislocation of the hip.

The pathology associated with congenital dislocation of the hip has been reviewed. The pathophysiology as it affects the development of the hip under treatment makes a strong case for the avoidance of the "frog leg" position with fixed flexion and abduction of the hips in the plaster cast. It is apparent that tightness of the iliopsoas muscle and the underlying capsule makes that flexed abduction position necessary to hold the hip in position. The "frog leg" position is seriously implicated in the development of aseptic necrosis, not only of the dislocated hip but of the normal hip as well. Th

Surgical treatment of the hip in cerebral palsy.

Hips are affected generally in CP but in favourable cases we can prevent dislocation by adductor tenotomy performed in time (530 cases). Posterior transposition of the origins of the adductor muscles is performed if the patient walks with inwood rotated inferior extremities (105 cases). In the treatment of real subluxation and valgus deformity the open tenotomy of adductor muscles and femur osteotomy with derotation and varisation were used (180 cases). We accomplish open reduction in frank dislocation (35 cases). In such cases it is often necessary to shorten the femur. Also normal acetabulum

Dislocations following arthroplasties of the hip. Incidence, prevention, and treatment.

Three methods of avoiding postoperative prosthetic dislocation following total or bipolar hip arthroplasty are described; each approach is recommended for a specific population. The anterior surgical approach was utilized for bipolar hip endoprosthetic arthroplasty in 62 minimal or bed-to-chair ambulators with established flexion contractures about the hip. Two of these patients subsequently experienced a prosthetic dislocation, one of which was secondary to infection. An additional 98 patients who underwent arthroplasty using the posterior approach wore a knee immobilizer postoperatively, wit

Outcomes of gastrocnemius-soleus complex lengthening for isolated equinus contracture in children with cerebral palsy.

BACKGROUND: The goal of the gastrocnemius-soleus complex (GSC) lengthenings in children with cerebral palsy (CP) is to achieve a plantigrade foot and normalize kinematics during gait. The study purpose was to evaluate the results of GSC lengthening for isolated equinus contracture in individuals with CP. It was hypothesized that GSC lengthenings would normalize passive ankle range of motion, kinematic, kinetic, and temporal spatial parameters. METHODS: Gait data from 15 able-bodied participants from the laboratory normal database and passive range of motion, kinematic, kinetic, and temporal s

Expanding the phenotypic spectrum for CDK8-related disease: A case report.

BACKGROUND: Cyclin-dependent kinase 8 (CDK8) is part of a regulatory kinase module that regulates the activity of the Mediator complex. The Mediator, a large conformationally flexible protein complex, goes on to regulate RNA polymerase II activity, consequently affecting transcriptional regulation. Thus, inactivating mutations of the genes within the kinase module cause aberrant transcriptional regulation and disease, namely, CDK8-related intellectual developmental disorder with hypotonia and behavioral abnormalities (IDDHBA). CASE PRESENTATION: We describe, for the first time, a likely patho

Unique use of botulinum toxin to decrease adductor tone and allow surgical excision of vulvar carcinoma.

Here, we present the case of an 86-year-old woman with vulvar carcinoma requiring surgical resection and with Parkinson's disease with severe spasticity and contractures of the lower extremities. Because of the patient's severe contractures and spasticity (her knees could only be separated by 2 cm with sustained abducting force), surgical positioning and access to the vulva were impossible. The patient was admitted, intending to undergo surgery after injection with botulinum toxin (BTX) to hip adductors and intensive physical therapy. After confirmed healed hip arthroplasty, the patient underw

Orthopedic surgical management of hip and knee involvement in patients with juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis is the most common arthritic disease of childhood and a leading cause of childhood disability, affecting an estimated 300,000 US children and adolescents aged < or =16 years. Approximately 10% to 30% of patients experience functional deficits resulting from both the articular and systemic manifestations of their disease, including leg length inequality and deformity, that are often more crippling than joint destruction. Surgical intervention to treat bone and soft-tissue deformity, leg length inequality, and joint destruction is indicated when medical therapy has

Effect of pelvic obliquity on the orientation of the acetabular component in total hip arthroplasty.

In infrapelvic obliquity, coronal pelvic malrotation entails a change in the spatial location of the bony acetabulum. In the present study, 77 patients presented with infrapelvic obliquity with types 1 and 2 hip pathologies in which the pelvis is lower and higher, respectively, on the short-leg side. The 2 types were classified into 3 subtypes (A, B, and C) according to the severity of the pelvic obliquity (0°-3°, 3°-6°, and >6°). Angles of inclination of pelvic obliquity postoperatively, anteversions, and inclinations (abduction angle) for acetabular components were measured after total hip a

Total hip arthroplasty after hip arthrodesis: functional outcomes, and mid-term survivorship integrating the fusion technique complexity score.

BACKGROUND: Conversion of hip arthrodesis to total hip arthroplasty (THA) is technically demanding because of altered anatomy, soft-tissue contractures, and long-standing biomechanical adaptations. Preoperative complexity varies widely, and contemporary data on functional recovery and complications remain limited. This study evaluates functional and radiographic outcomes after conversion THA and introduces a Fusion-to-THA Surgical Complexity Score (FTCS) to characterise preoperative difficulty and explore its association with perioperative outcomes. PATIENTS AND METHODS: We retrospectively re

The midlumbar myelomeningocele hip: mechanism of dislocation and treatment.

We define the "bowstring" force of the iliopsoas, the force applied to the femoral head as the tendon angles across the hip joint, which we believe is an important cause of dislocation of the hip in patients with a midlumbar myelomeningocele. An operative procedure consisting of iliopsoas recession and suture of its tendon to the anterolateral hip joint capsule has been developed and used in 10 dislocated and 9 subluxated hips. The procedure was performed on patients with an average age of 6.6 months; the average follow-up was 45 months. Arthrography demonstrated the subluxation pattern of Lev

The Price of Poor Cementing: A Case Report on Chronic Hip Dislocation and Revision.

INTRODUCTION: Hemiarthroplasty is commonly performed for displaced femoral neck fractures in elderly patients. While generally effective, poor cementing techniques can lead to serious complications, including implant migration and chronic dislocation. Cement interposition in the acetabulum is a rare but preventable cause of persistent instability. This case emphasizes the importance of meticulous cement handling and intraoperative vigilance to avoid avoidable revision surgeries. CASE REPORT: A 76-year-old woman presented 8 months after left hemiarthroplasty with persistent hip pain and immobi

Juvenile fibromatosis and other diseases of connective tissue.

There are a number of features which are common to such variable conditions as juvenile fibromatosis on the one hand and torticollis and congenital dislocation of the hip, and possibly scoliosis, on the other. Various pathological syndromes are presented, all of them characterized by disturbance and contracture of connective tissue. Surgery is carried out in severe cases when it is a practicable proposition. The administration of an enzyme inhibitor at cellular level has introduced a new dimension in the management of localized fibromatosis.
