Mid-term outcomes after distally locked-to-standard primary stem exchange in 29 hip-prosthesis patients.

BACKGROUND: Cementless locked femoral stems are used for revision surgery in patients with bone loss to induce spontaneous bone reconstruction, allowing subsequent replacement by a standard primary stem. The small number of patients and short follow-ups available to date preclude a valid assessment of this strategy. HYPOTHESIS: After distally locked stem revision, replacement by a standard primary stem does not induce complications, and the quality of the bone reconstruction allows strong fixation of a regular primary stem. MATERIALS AND METHODS: We retrospectively evaluated 29 patients in w

Total hip arthroplasty using S-ROM prosthesis in elder patients with type C and B bone.

OBJECTIVE: The purpose of this study was to evaluate the clinical and radiological results using S-ROM prosthesis in patients with type C and B femoral bone requiring primary total hip arthroplasty (THA) for multiple reasons. METHODS: Sixteen hips were followed up for a mean of 38 months (range, 26-48), with a mean age at surgery of 72 years (range, 65-75). RESULTS: The average Harris hip score improved from 52.2 points to 88.5 points. All femoral stems showed stable fixation. Neither osteolysis around the femoral stem nor subsidence of the femoral stem were found at the final follow-up. CO

Forgotten intrauterine contraceptive device - A threat to total hip prosthesis: A case report with review of the literature.

Primary total hip replacement has become a routine procedure these days. With improvement in surgical techniques and implant designs, the survival rate of prosthesis has increased significantly but unfortunately, prosthetic infections though uncommon continue to be a threatening complication. We present a detailed review of the literature along with a case report of infected total hip prosthesis in a 36-year-old female who had been operated 6 years back. The causative organism was found to be Actinomyces israelii which was related to an infected intrauterine device used for contraception that

Rapid progression of a severe femoral bone loss in a stable revision hip prosthesis: causes and management.

We report a case of a severe bone loss of the proximal femur with a rapid progression, in a 72-year-old male patient with a stable total revision hip prosthesis. The patient had a persistent mechanical thigh pain. The blood laboratory values were normal. Infection disease and osteolytic bone tumor were excluded. A surgical procedure was performed. The native bone of the proximal femur was resorbed and replaced by a dense fibrous tissue with some sclerotic bone fragments. A large amount of a brownish fluid and a red-brown and friable tissue were found in the pseudojoint cavity. The proximal fem

Biomechanical evaluation of different offset versions of a cementless hip prosthesis by 3-dimensional measurement of micromotions.

BACKGROUND: Cementless hip prostheses with different offsets are frequently used to restore the rotation center of the hip. However, a rising offset is theoretically associated with a potential risk for increased interface stresses and early loosening. METHODS: To assess this potential risk for cementless stems, the primary stability of the CLS Spotorno stem was examined with respect to three different femoral neck versions (125°, 135° and 145°) measuring 3-dimensional micromotions. For this purpose 18 stems were implanted in composite femurs and tested dynamically using physiological loading

New insight into the mechanism of hip prosthesis loosening: effect of titanium debris size on osteoblast function.

The incidence of rheumatoid arthritis and osteoarthritis is on the rise due to our expanding elderly population. Total joint arthroplasty is the most successful, prevalent treatment modality for these and other degenerative hip conditions. Despite the wide array of prosthetic devices commercially available, hip prostheses share a common problem with a gradual and then accelerating loss of bone tissue and bone-implant interface integrity, followed by implant instability and loosening. Implant failure is largely the result of inevitable wear of the device and generation of wear debris. To provid

Metallosis due to impingement between the socket and the femoral head in a total hip prosthesis. A case report.

Metallosis is usually defined as aseptic fibrosis, local necrosis, or loosening of a device secondary to metallic corrosion and release of wear debris. It is an occasional but characteristic clinical finding in patients who have a metal-on-metal design of total hip replacement, or when metal surfaces contact after a failure or erosion of the polyethylene component. We report the case of a patient who had severe metallosis caused by wear of the femoral head with the titanium socket, after the complete erosion and destruction of the polyethylene cup, with failure of the accetabular component. Frozen autograft-prosthesis composite reconstruction with total hip arthroplasty for proximal femoral osteosarcoma: a case report.

INTRODUCTION AND IMPORTANCE: Reconstruction after proximal femoral resection requires restoration of both a large osseous defect and stable hip joint function. Frozen autograft-prosthesis composite (FAPC) reconstruction using liquid nitrogen-treated autograft represents a viable biological option for this indication. We present a case of proximal femoral osteosarcoma managed with this technique. CASE PRESENTATION: A 25-year-old man presented with a 3-month history of atraumatic right hip pain. Imaging and biopsy confirmed Enneking stage IIB high-grade osteosarcoma of the proximal femur. Wide

Conversion total hip arthroplasty with a proximally modular, distal fixation reconstruction prosthesis following cephalomedullar nail failure.

BACKGROUND: The goals of intertrochanteric hip fracture (IHF) treatment are stable fixation, early mobilisation and function restoration. If the attempt to reduce, stabilise and fracture healing utilising a femoral cephalomedullar nail (CMN) fails, options for subsequent attempts are limited. PURPOSE: Evaluate the clinical and radiographic outcomes of conversion total hip arthroplasty (THA) using a modular stem following a CMN failure. MATERIALS AND METHODS: We retrospectively reviewed a consecutive series of patients with an IHF between 2012 and 2014 to identify CMN patients that went on to

Carbon/PEEK composite materials as an alternative for stainless steel/titanium hip prosthesis: a finite element study.

Total hip replacement (THR) has been ranked within the most typical surgical processes in the world. The durability of the prosthesis and loosening of prosthesis are the main concerns that mostly reported after THR surgeries. In THR, the femoral prosthesis can be fixed by either cement or cementless methods in the patient's bones. In both procedures, the stability of the prosthesis in the hosted bone has a key asset in its long-term durability and performance. This study aimed to execute a comparative finite element simulation to assess the load transfer between the prosthesis, which is made o

Serum interleukin 8 levels correlate with synovial fluid levels in patients with aseptic loosening of hip prosthesis.

Levels of inflammatory cytokines (tumor necrosis factor alpha, interleukin [IL] 6, and IL-8) in serum from patients with osteolysis on radiographs after hip arthroplasty [osteolysis(+), n = 28], patients without osteolysis after hip arthroplasty [osteolysis(-), n = 24], and nonoperated healthy subjects [controls, n = 20] were determined. In addition, cytokine levels in synovial fluid from patients undergoing revision total hip arthroplasty (n = 14) for loosening were measured and compared with each other and with the area of osteolysis on radiographs. Serum IL-6 and IL-8 levels were significan

From bench to bedside: a novel suture-augmented prosthesis for greater trochanteric fixation in osteoporotic hip arthroplasty.

BACKGROUND: Stable fixation of the greater trochanter during hip arthroplasty for unstable osteoporotic intertrochanteric fractures remains a significant challenge. Conventional techniques depend on securing the bone-implant interface, which is particularly compromised in osteoporotic bone. Here, we propose a novel conceptual approach that establishes a direct mechanical bridge from the abductor tendon to the prosthesis, thereby reducing reliance on the fragile bone-implant interface. METHODS: In this two-stage translational study, we progressed from biomechanical validation to clinical appli

Long-term prosthesis survival of total hip and total knee arthroplasty in people with inherited bleeding disorders.

BACKGROUND: Joint arthroplasty provides pain relief and functional improvement, but higher peri- and postoperative risks in people with bleeding disorders historically lead to lower prosthesis survival rates (PSRs) than in the general population. Long-term outcomes after advances in hemophilia care remain unclear. In people with bleeding disorders, PSR is 84.0% at 15 years after total knee arthroplasty (TKA) and 91.9% at 5 years after total hip arthroplasty (THA), compared with 92.8% and 93.7% in the general population. OBJECTIVES: To determine PSRs of TKA and THA up to 25 years after surgery
