Preoperative localization of clinically occult breast lesions: experience at a referral hospital.

The authors performed 601 preoperative needle localization procedures for occult breast abnormalities in 1987. Whereas most previous series have been based on mammograms obtained and interpreted at the reporting institutions, this study population includes a large number of patients (79%) examined and referred from other facilities. One hundred nine cancers were diagnosed in the 601 cases (18%). Biopsies performed for masses yielded a 23% (64 of 278) frequency of cancer and those for calcification, 14% (45 of 323). The benign to malignant ratio (4.5:1) in this mixed group of patients is higher

Analyzing dengue fever with constrained medical resources: A dynamical systems approach.

Dengue fever, a major mosquito-borne viral disease, poses a significant public health threat, particularly in high-incidence countries like Brazil, where rising cases strain limited medical resources. We analyze the impact of constrained medical resources, specifically hospital bed capacity, on dengue transmission dynamics. A novel compartmental model is developed where the availability of hospital beds (B) is a key parameter governing treatment access. Employing classical linearization theory, we conduct a comprehensive stability analysis of the system equilibrium points. Systematic bifurcati

Prevalence and cost savings of therapeutic interchange among U.S. hospitals.

The prevalence and cost savings of therapeutic interchange (TI) among teaching, nonteaching, and investor-owned hospitals in the United States was studied. A survey was sent to all directors of pharmacy at hospitals listed in the 1999 American Hospital Association directory as having more than 100 beds; 463 (29.8%) hospitals responded. The survey elicited data about hospital demographics, the policies and personnel involved in TI, and the estimated cost savings incurred by the use of TI. Eighty-eight percent of teaching, 89% of nonteaching, and 100% of investor-owned hospitals reported having

A socio-medical study of infant mortality among disadvantaged blacks.

This report profiles the woman at highest risk of a low birthweight delivery and infant death, in a generally disadvantaged, Black population in a major northeastern city which maintains one of the highest infant mortality rates in the nation. It discusses the existence of a very high risk subpopulation which contributes disproportionately and repeatedly to the infant and perinatal death statistics. If adequately identified, these women can be subject to intervention programs. Results show that within an inner-city Black population, where educational levels are relatively low and much of the r

Trends and Disparities in Postpartum Sterilization after Cesarean Section, 2000 through 2008.

PURPOSE: Tubal sterilization patterns are influenced by factors including patient race, ethnicity, level of education, method of payment, and hospital size and affiliation. However, less is known about how these factors influence tubal sterilizations performed as secondary procedures after cesarean sections (C-sections). Thus, this study examines variations in the prevalence of postpartum tubal sterilizations after C-sections from 2000 to 2008. METHODS: We used data from the National Hospital Discharge Survey to estimate odds ratios for patient-level (race, marital status, age) and system-lev

A comparison of infection control program resources, activities, and antibiotic resistant organism rates in Canadian acute care hospitals in 1999 and 2005: pre- and post-severe acute respiratory syndrome.

BACKGROUND: The Resources for Infection Control in Hospitals (RICH) project assessed infection control programs and rates of antibiotic-resistant organisms (AROs) in Canadian acute care hospitals in 1999. In the meantime, the severe acute respiratory syndrome (SARS) outbreak and the concern over pandemic influenza have stimulated considerable government and health care institutional efforts to improve infection control systems in Canada. METHODS: In 2006, a version of the RICH survey similar to the original RICH instrument was mailed to infection control programs in all Canadian acute care ho

Primary total elbow arthroplasty for distal humeral fractures in elderly patients: a nationwide analysis.

BACKGROUND: Displaced intra-articular distal humeral fractures are a challenging injury in elderly patients. High rates of complications have led to the increasing use of total elbow arthroplasty (TEA) for primary treatment. This study presents US nationwide trends in primary TEA for distal humeral fractures in elderly patients (65 years and older) from 2002 to 2012. We hypothesized that there was an increase in the rate of TEA utilization. METHODS: Data were obtained from the Nationwide Inpatient Sample for the years 2002 to 2012. All inpatients 65 years and older with distal humeral fractur

Two-way referral bias: evidence from a clinical audit of lymphoma in a teaching hospital.

The objective of this study was to evaluate the effect of referral bias in a clinical audit of lymphoma in a university hospital. We compared demographic and clinical characteristics as well as survival for Jerusalem residents (local) and referred (distant) patients diagnosed from 1987 to 1992 and treated in our institution. Referred patients were younger (p < 0.0001), and less likely to be immigrants (p < 0.0001), than local patients. Aggressive non-Hodgkin's lymphomas (NHL) were more common in the referred population (p = 0.015). Survival for Hodgkin's disease was consistently better for loc

Clinical use of androgenic steroids at a major medical center: who prescribes androgens and how well do they do it?

To assess prescribing practices for androgens at Wilford Hall USAF Medical Center, the authors analyzed prescriptions for all patients receiving therapy during a 12-month period (n = 201) and reviewed the available outpatient records not maintained elsewhere (n = 105). The most commonly prescribed androgens were testosterone enanthate (144/201; 56.7%), and danazol (52/201; 25.9%). Review of the available outpatient medical records of 73 male patients using testosterone enanthate showed a mean age of 59.5 years, mean frequency of injection of 20 days, mean duration of therapy of 3.5 years, and

The cesarean birth rate: influence of hospital teaching status.

Knowledge of how cesarean birth rates vary by hospital characteristics may aid in understanding and perhaps modifying some of the structural and process components of newborn delivery services to decrease the necessity of birth by cesarean procedure. To examine the influence of select hospital characteristics, data on hospital newborn deliveries in Illinois for 1986 among women 10-50 years of age inclusive (N = 130,249) were obtained from computerized hospital discharge abstract files. Characteristics of the hospitals were obtained from the annual American Hospital Association survey. Adjustin

A clinical audit of neurosurgical bed usage.

AIM: To investigate inefficiencies during patients' stay in neurosurgical beds. METHODS: A prospective audit of neurosurgical inpatients was conducted over approximately 2 months. We recorded the causes and duration of inappropriate delays, defined as actions requiring more that 24 h from initiation until completion. RESULTS: One-hundred-and-thirty-eight patients were studied (61 elective and 77 emergency admissions). The audited patients were in hospital for a total of 1665 days. Of these, 482 days (29%) were due to delays. 92.5% of these wasted days involved emergency admissions. The main

The case for using flexible healthcare capacity constraints to optimize pandemic control strategies.

During the COVID-19 pandemic, a key challenge was designing control strategies that balance the benefits and costs of public health measures while preventing healthcare systems from becoming overwhelmed. This was often addressed by epidemiological modellers by implementing a binding (hard) constraint based on the maximum number of hospital beds that could be occupied at any given time. However, experience from the pandemic demonstrated that healthcare capacity depends not only on bed availability, but also on staffing and the availability of other resources. We argue that defining healthcare c

The Impact of Hospital Accessibility on Interregional Patient Mobility in Italy.

Patient mobility represents a proxy measure to assess the quality and availability of hospital services, especially in decentralized health systems. Different studies have been focused on the interregional mobility in Italy to capture factors influencing this phenomenon. Among them, hospital capacity is generally captured through the number of beds per population. However, this indicator does not consider the distance to hospitals and the accessibility of extra-regional beds, in particular for patients living at the regional borders. The aim of this paper is to analyse the effect of extra-regi

[Does the nursing shortage in neonatal intensive care units (NICU) threaten the warranty of healthcare for newborns? : The "Mary and Joseph" project].

BACKGROUND: Germany has been experiencing a dramatic shortage of nursing staff for years that particularly affects neonatal intensive care units (NICUs). It is assumed that this situation leads to reductions in bed capacities, resulting in negative effects on the healthcare of newborns. These were investigated through a retrospective observational study using the example of three NICUs at the University Hospital of Munich (LMU). METHODS: For the four-year observation period from August 2017 to May 2021, time series data from the "Quality Assurance Guideline for Premature and Mature Infants" (

Healthcare-associated viral gastroenteritis among children in a large pediatric hospital, United Kingdom.

Viruses are the major pathogens of community-acquired (CA) acute gastroenteritis (AGE) in children, but their role in healthcare-associated (HA) AGE is poorly understood. Children with AGE hospitalized at Alder Hey Children's Hospital, Liverpool, UK, were enrolled over a 2-year period. AGE was classified as HA if diarrhea developed > or =48 hours after admission. Rotavirus, norovirus, adenovirus 40/41, astrovirus, and sapovirus were detected by PCR. A total of 225 children with HA-AGE and 351 with CA-AGE were enrolled in the study. HA viral gastroenteritis constituted one fifth of the diarrhea

Incidence and impact of infection in a nursing home care unit.

In this study we examined the frequency of infection and its consequences in a Veterans Administration medical center nursing home care unit during its first 9 months of operation. A total of 231 patients were enrolled and were followed up for an average stay of 115 days. Sixty-nine infections occurred in 50 patients and yielded a period prevalence rate of 22% and an infection incidence rate of 2.6 infections per 1000 days of patient care. Symptomatic urinary tract infections, pneumonia, and skin and soft tissue infections accounted for 41%, 32%, and 17% of the infections, respectively. Staphy

[How good is our social network?--Experiences with referral and admission of paraplegic patients in Germany].

The care of patients with spinal cord injury is regarded as well organized in the FRG. Nevertheless one can state that by lack of bed-capacity in our SCIC the social service is insufficient. Recent cases often cannot be admitted immediately. Patients getting their impairment by diseases cannot be treated all, because there is few capacity left. Life-long care--demanded since the time of Sir L. Guttmann--today is an utopia. Increasing capacity for treatment of patients in hospital and after discharge is necessary.

Utilization of Alternate Care Sites During the COVID-19 Surge and Mass Care: California, 2020-2021.

The coronavirus disease (COVID-19) pandemic caused critical hospital bed and staffing shortages in parts of California for most of 2020 and 2021. Alternate Care Sites (ACS) were established in several regions to alleviate the hospital patient surge and to maximize staffed bed capacity. Over 1900 patients were successfully provided medical care (with physician, nursing, respiratory therapy, oxygen, and pharmacy services) in relatively austere settings. This paper examines the challenges faced at these ACS facilities and how adaptations were incorporated according to the changing dynamics of the
