Interventions for Health: Why and How Health Care Systems Provide Programs to Benefit Unhoused Patients.

Individuals experiencing homelessness have poorer health than housed individuals, while also utilizing more emergency department care and fewer preventive services. Several interventions - including permanent supportive housing, medical respite, and mobile medical clinics - are cost-effective means to improve health outcomes for homeless populations, yet few health systems have invested in such programs. This study aimed to determine the reasons some health systems initiated these interventions, and the early experience of those health systems that did. Quantitative analyses of health systems

Do health facility governing committees improve health system performance? An ecological study of Mainland Tanzania.

INTRODUCTION: Accountability is crucial for improved functionality of health systems and can be ensured through community participation in health governance. To engage the community in the governance of the local health system, health facility governing committees (HFGCs) have been implemented in several low-income and middle-income countries including Tanzania. However, the effect of HFGCs on health system performance is not well studied. The aim of this study was to investigate the relationship between the functionality of the HFGCs and health system performance in 180 districts of mainland

Overstimulated in the Emergency Room: A Parental Perspective on Improving Pediatric Hospital Care for Non-Speaking Children With Autism.

This patient perspective article presents practical recommendations, rooted in social psychological research, for pediatric emergency room and hospital care. The perspective is based upon the experience of a parent to a child with autism who is non-speaking and has required recurrent hospital care. The perspective highlights systemic challenges facing non-speaking autistic children and their families in hospital care contexts, including challenges in handoff communication and information transfer requiring repeated disclosure, as well as advocacy fatigue and dysregulating care environments. Mapping System Vulnerabilities in Anaesthesia Safety: A Role-Based Inquiry in Response to Fatal Fungal Meningitis Outbreaks in Mexico.

OBJECTIVES: Unsafe anaesthesia practices in Mexico have contributed to repeated outbreaks of fatal fungal meningitis, exposing broader health system vulnerabilities. This study aimed to analyse how structural weaknesses in perioperative care, spanning governance, workforce, and institutional practices, reflect deep gaps in health planning, regulation, clinical oversight and hospital management. METHODS: We applied a participatory, role-based systems mapping approach. A two-phase design method combined a survey to identify perceived system failures with structured, guided interprofessional, pa

The Promise of Community Health Workers for Addressing Structural Drivers of Inequities Among Hospitalized Patients.

Community health workers, trained health professionals from the community they serve, hold promise in their ability to combat structural racism affecting Black patients in the hospital. Structural racism leads to downstream, adverse effects on patient care, including lack of trustworthiness of health care institutions, impaired communication, and limited access to adequate pain control. To address these challenges, we launched a Health Advocate program integrating a community health worker into our hospital's medicine wards. The program sought to improve communication, patient experience, and

Exploring Police Officer Experiences During the Transfer of People in Crisis to Emergency Department Nurses and Staff.

Police officers are increasingly tasked with responding to people in crisis (PIC), often resulting in transfers to emergency departments (EDs) where they can encounter challenges like long wait times, safety concerns, and conflicting perspectives between the medical and legal systems. This qualitative study explores the experiences of police officers during the transfer of PIC to ED nurses and staff. Eleven police officers were recruited and interviewed, providing contextual information about ED transfers in the greater Montreal area. The findings revealed that police officers face varied chal

Care for the patient with AIDS in the ICU: outcomes and ethics.

HIV infection and AIDS are common diagnoses in many intensive care units (ICUs) in the United States. Although Pneumocystis carinii currently represents only one quarter of all diagnoses for which HIV-infected persons are admitted to the ICU, it is the disease with the most clinically applicable outcome data and, therefore, is a model for ethical decision-making regarding patients with HIV infection in the ICU. Despite advances in diagnosis and treatment of HIV-related P. carinii, recent studies show that only 20% to 25% of the patients with acute respiratory failure survive to hospital discha

From 5G smart hospitals to 6G-enabled learning hospitals: a governance-oriented framework for healthcare technology, policy, and responsible deployment.

OBJECTIVES: Discussion of 6G in healthcare often moves directly from anticipated network capabilities to proposed clinical applications, although application-level evidence, current connectivity alternatives, and 6G-specific evidence differ substantially in maturity. This narrative review develops an evidence-calibrated, governance-oriented framework for prospective 6G-enabled learning hospitals and examines when future 6G may add meaningful value beyond existing infrastructure. METHODS: A multidisciplinary narrative synthesis covered smart hospitals, learning health systems, 5G/6G communicat

Barriers and facilitators for treatment-seeking among women with genital fistula: a facility-based qualitative study in Bangladesh.

BACKGROUND: Women living with genital fistula often endure prolonged suffering and face multiple barriers to accessing treatment. Bangladesh's government has enhanced referral mechanisms, enabling case detection in communities and facilitating surgical interventions at medical college hospitals through nationwide initiatives. However, research on barriers and facilitators for fistula treatment in Bangladesh remains limited. Detailed insights into treatment-seeking paths with time sequences are scarce. This study aimed to explore facilitators and barriers to completing fistula treatment with th

BACKGROUND: In March 2020 the World Health Organization declared the coronavirus disease 2019 (COVID-19) a pandemic. Management of this pandemic had significant implications for clinical departments across the world. Healthcare systems were urgently required to reorganise and redesign patient care as well as repurpose staff. OBJECTIVES: We will share the lived experience of our response as speech therapy and audiology (STA) clinicians to the COVID-19 pandemic. METHOD: This study adopted an autoethnographic approach within Bronfenbrenner's bioecological model to describe STA clinicians' respo

Pressure Sores and Systemic Inflammatory Response Syndrome: UC Davis Quality Improvement Initiative.

BACKGROUND: The National Pressure Ulcer Advisory Panel estimates pressure sore care to approach $11 billion annually. It is not uncommon for these patients to present to the emergency department (ED) with a chief concern of a pressure sore, while concurrently carrying an undiagnosed infectious process that is the culprit for the acute presentation, rather than the chronic pressure injury. We aim to identify patients who met systemic inflammatory response syndrome (SIRS) criteria at ED presentation who were referred to plastic and reconstructive surgery for pressure sore debridement prior to a

EHR-Based Quality Interventions Associated With Differential GDMT Prescribing in Community Versus Academic Hospitals.

BACKGROUND: Institutional barriers are associated with suboptimal guideline-directed medical therapy (GDMT) prescribing at heart failure discharge. PROJECT RATIONALE: Practice variations between academic medical centers (defined as "teaching" hospitals with residents) and community hospitals are poorly understood. Evaluating how diverse settings respond to quality improvement strategies is vital. PROJECT SUMMARY: A multicenter study across 10 electronic health record-unified hospitals evaluated 3,535 heart failure encounters. Phase 1 implemented educational work groups and financial incentiv

The San Francisco Health Systems Collaborative: Public Health and Health Care Delivery Systems' Response to the Covid-19 Pandemic.

The Covid-19 pandemic challenged health care delivery systems worldwide. Many acute care hospitals in communities that experienced surges in cases and hospitalizations had to make decisions such as rationing scarce resources. Hospitals serving low-income communities, communities of color, and those in other historically marginalized or vulnerable groups reported the greatest operational impacts of surges. However, cross-institutional collaborations within jurisdictions offer unique opportunities to prevent or mitigate health disparities in resource utilization and access to care. In January 20

Expansion of community-based perinatal care in California.

OBJECTIVE: In California, hospitals with Community Neonatal Intensive Care Units (NICUs) increased from 17 in 1990 to 52 in 1997. The purpose of this study was to investigate the effects of their growth on level-specific distribution of births, acuity, and neonatal mortality. STUDY DESIGN: A total of 4,563,900 infants born from 1990 to 1997 were analyzed by levels of care. We examined shifts in birth location and acuity. Neonatal mortality for singleton very-low-birth-weight (VLBW) infants without congenital abnormalities was used to assess differences in level-specific survival. RESULTS: Li

Health Care Leaders' Perspectives on the Maryland All-Payer Model.

IMPORTANCE: Since 2014, all hospitals in Maryland have operated under an all-payer global budget system. Hospital global budgets have gained renewed attention as a strategy for constraining cost growth, improving patient outcomes, and preserving health care access in rural and underserved communities. Lessons from the implementation of the Maryland All-Payer Model (MDAPM) may have implications for policy makers, payers, and hospitals in other settings seeking to adopt global budgets or other value-based payment models. OBJECTIVE: To examine perspectives on the implementation of the MDAPM amon

Wearable Multispectral Sensor for Newborn Jaundice Monitoring.

Newborn immaturity transcends their bodies, immune systems, and communication and perception capabilities, making them vulnerable to the environment. Neonatal jaundice is a common condition, with higher levels of unconjugated bilirubin concentration having neurotoxic effects. Newborns are routinely monitored visually or non-invasively with transcutaneous bilirubinometry (TcB) due to their biological immaturity to conjugate bilirubin. Higher levels of bilirubin are a sign that there is either an unusual rate of red blood cells breaking down or that the liver is not able to eliminate bilirubin t

Use of WhatsApp messaging technology to strengthen obstetric referrals in the Greater Accra Region, Ghana: Findings from a feasibility study.

In Ghana, the high-risk obstetric referral system is inadequate. Delay is common and patients often arrive to receiving hospitals in compromised states. An effective referral system should include an adequately resourced referral hospital, communication across sectors, accountability, transport, monitoring capability and policy support, which are currently lacking. A pilot program was undertaken to facilitate communication between hospital staffs. Additionally, data was collected to better understand and characterize obstetric referrals in Accra. Thirteen institutions were selected based on re

Process improvement for follow-up radiology report recommendations of lung nodules.

UNLABELLED: In the modern healthcare system, there are still wide gaps of communication of imaging results to physician and patient stakeholders and tracking of whether follow-up has occurred. Patients are also unaware of the significance of findings in radiology reports. With the increase in use of cross-sectional imaging such as CT, patients are not only being diagnosed with primary urgent findings but also with incidental findings such as lung nodules; however, they are not being told of their imaging findings nor what actions to take to mitigate their risks. In addition, patients at high r
