BACKGROUND: Secondary prophylaxis following treatment of CMV infection in solid organ transplant recipients (SOTRs) may be considered in high-risk SOTRs, although utilization criteria are not well described. The addition of CMV-specific T-cell immunity assay testing may help guide selection of SOTRs for secondary prophylaxis.
METHODS: All SOTRs at a single center with CMV T-cell assays obtained during secondary prophylaxis between January 2020 and August 2023 were reviewed. CMV antiviral secondary prophylaxis discontinuation rates and subsequent need for antiviral therapy re-initiation were r

Toxoplasma gondii infection in cats is mostly subclinical. Systemic disease usually occurs in immunocompromised individuals. This report describes an unusual systemic toxoplasmosis, presenting as parasitic arthritis in an adult 14-year-old neutered male indoor domestic shorthair cat, with no identifiable cause of immunosuppression, and referred for lethargy, anorexia, and fever. Laboratory abnormalities included non-regenerative anemia, thrombocytopenia, increased serum amyloid A concentration, and polyclonal hypergammaglobulinemia. With persistent discomfort on palpation of the pelvic region

Bacterial infection poses a significant threat to clinical treatment due to the emergence of drug resistance and the high risk of recurrence. Here, we report the de novo design of a spiky pollen-based vanadium artificial enzyme particle (VAE-Pollen) that integrates potent reactive oxygen species (ROS)-catalytic activity with immune priming to prevent both primary and secondary bacterial infections. Experimental and theoretical analyses confirm that bacteria are efficiently captured by the micro-structured surface of VAE-Pollen, and the introduction of oxygen vacancies modulates the electronic

Pregnancy is an immunological paradox where despite a competent maternal immune system, regulatory mechanisms at the fetoplacental interface and maternal secondary lymphoid tissues (SLTs) circumvent rejection of semi-allogeneic concepti. Small extracellular vesicles (sEVs) are a vehicle for intercellular communication; nevertheless, the role of fetoplacental sEVs in transport of antigens to maternal SLTs has not been conclusively demonstrated. Using mice in which the conceptus generates fluoroprobe-tagged sEVs shed by the plasma membrane or released from the endocytic compartment, we show that

Antibody deficiency may be primary, because of an underlying inborn error of immunity, or secondary, due to another disease process or medication, leading to decreased antibody production or increased antibody loss. Secondary antibody deficiency is much more common than primary. It can, however, be difficult to distinguish primary from secondary, and both should be considered when assessing patients with hypogammaglobulinaemia. The case presented highlights the importance of confirming the underlying pathophysiology in patients presenting with antibody deficiency, the contribution of genetic a

BACKGROUND: The COVID-19 (SARS-CoV-2) epidemic has posed a major challenge to global public health, especially in children. Some children may experience secondary infection with Mycoplasma pneumoniae after SARS-CoV-2 infection, which has attracted widespread attention. Studies have shown that eosinophils play an important role in respiratory tract infections and are involved in regulating immune responses and inflammatory processes. However, there is a lack of systematic research on the specific manifestations and mechanisms of eosinophils in secondary infection with Mycoplasma pneumoniae afte

Measles is an important re-emergent infectious disease globally. Vaccine immunity at the population level is key to the prevention of outbreaks, as unvaccinated or immunosuppressed adults are particularly vulnerable to severe infection. With the increasing use of immunomodulatory treatments for autoimmune and malignant conditions, the long-term effects of CD20-expressing cell-depleting therapies on the vaccine-induced humoral immunity to measles remain unclear. Haemophagocytic lymphohistiocytosis (HLH) is a hyperinflammatory syndrome with high mortality commonly triggered by infection and is p

The present study reports the case of a patient with diffuse large B-cell lymphoma (DLBCL) and monoclonal gammopathy (MG) secondary to immune thrombocytopenia purpura (ITP). The clinical diagnoses and investigations of this case are reported. To the best of our knowledge, this is the first study to report DLBCL and MG secondary to ITP. The patient presented with a rare constellation of diseases, which made the diagnosis and treatment difficult for the physicians. The patient was followed up for 10 years using the morphological examination of bone marrow cells after chemotherapy, and currently

Case presentation: A 66-year-old gentleman with metastatic non-small-cell lung cancer developed a wide-based gait following treatment on a clinical trial with cytotoxic chemotherapy and an anti-PD-L1 drug. He had no other significant past medical history of note. Brain imaging, blood tests and lumbar puncture did not reveal a structural, biochemical, paraneoplastic or infective cause. The main differential diagnoses were immune-mediated toxicity or a paraneoplastic syndrome. He was started on prednisolone on the suspicion that his symptoms represented an immune-mediated toxicity. His condition

A 36-year-old male presented with a secondary, but anti-neutrophil cytoplasmic antibody (ANCA) (proteinase-3) positive, vasculitis with renal insufficiency due to a pauci-immune necrotizing glomerulonephritis. An infective process was initially excluded by blood cultures and an echocardiogram prior to immunosuppression. The patient's condition failed to improve and re-evaluation confirmed infective endocarditis requiring valve replacement. Subsequent tissue cultures identified Bartonella henselae. Antibiotic treatment led to full resolution of physical, biochemical and immunological markers. T

Animal models of viral respiratory disease often use weight loss as a marker of disease severity; however, this may relate to dehydration and malnutrition that would be corrected clinically. We tested whether parenteral fluid therapy improved weight loss from influenza infection. BALB/c and C57BL/6 mice were infected with A/X31 (H3N2) influenza and randomized to intraperitoneal fluid therapy. Blood glucose was also measured post-viral infection on day 3 and 6 in BALB/c mice and on day 6 in C57BL/6 mice. Parenteral fluids did not alter weight loss or the immunological response to infection, and

Renewed interest in smallpox and the need for safer vaccines have highlighted our lack of understanding of the requirements for protective immunity. Since smallpox has been eradicated, surrogate animal models of closely related orthopoxviruses, such as ectromelia virus, have been used to establish critical roles for CD8 T cells in the control of primary infection. To study the requirements for protection against secondary infection, we have used a prime-challenge regime, in which avirulent ectromelia virus was used to prime mice that were then challenged with virulent ectromelia virus. Dengue hemorrhagic fever (DHF), the most severe form of illness following infection with a dengue virus, is characterized by plasma leakage, thrombocytopenia, and hepatic inflammation. The interrelationships among virus burden, immune activation, and development of DHF were examined in 54 children with secondary dengue-3 virus infections participating in a prospective, hospital-based study. DHF was associated with higher mean plasma viremia early in illness and earlier peak plasma interferon-gamma levels. Maximum plasma viremia levels correlated with the degree of plasma leakage and thrombocyt
