Endoscopic Denker's Approach for the Treatment of Extensive Sinonasal Tumors: Our Experience.

OBJECTIVES: Surgical management of large sinonasal tumors, spreading to the pterygopalatine, and infratemporal fossae have always been a challenge for the ENT surgeon and the multidisciplinary team. METHODS: Endoscopic Denker's approach allows complete exposure of the anterior, inferior, and lateral recesses of the maxillary sinus, without necessitating a gingivobuccal or a transseptal incision. Moreover, endoscopic Denker's approach facilitates direct, straight entry to the pterygopalatine and infratemporal fossae. RESULTS: Twenty-two patients with extensive sinonasal pathology underwent re

Recent advances in neuro-otology.

Four advances in the field of neuro-otology are described: (1) auditory brain-stem response audiometry, (2) electroneuronography, (3) the cochlear implant, and (4) the infratemporal fossa approach to the skull base. Auditory brain-stem response audiometry is an accurate method for the early detection of hearing impairment and early diagnosis of acoustic neurinomas. Electroneuronography accurately assesses the degree of injury to the facial nerve, thereby improving management of facial nerve lesions. The cochlear implant restores the sensation of sound to totally deaf patients. The infratempora

Unusual delayed presentation of a nail gun injury through the skull base.

Published reports of nail gun injuries to the face are uncommon. We describe an unusual delayed presentation with injury through the infratemporal fossa and a literature review. A 55-year-old patient presented 2 weeks after an unrecognized injury with complaints of a headache. Imaging revealed a nail traversing the infratemporal fossa with intracranial extension. The nail was removed through a preauricular approach without sequelae. Nail gun missiles to the face are uncommon, dramatic, but often nonfatal because of their relative low velocity. Patients are usually diagnosed at the time of inju

Maxillary-to-petrous internal carotid artery bypass: an anatomical feasibility study.

The possibility for maxillary artery (MA) to petrous internal carotid artery (ICA) bypass was investigated. Five adult cadavers were dissected bilaterally. After zygomatic arch osteotomy, the coronoid process was sectioned at its base. An extensive infratemporal craniotomy was performed at the level of foramina ovale, rotundum and spinosum. The petrous portion of the ICA was exposed by drilling away the floor of the middle fossa, posterior to the foramen ovale and medial to the foramen spinosum. The MA was identified medial to the infratemporal crest and was followed in the pterygopalatine fos

Microsurgical Techniques for Exposing the Internal Maxillary Artery in Cerebral Revascularization Surgery: A Comparative Cadaver Study.

BACKGROUND: The internal maxillary artery (IMAX) is currently considered one of the main donor vessels in extracranial-to-intracranial bypass surgeries. Four main techniques for harvesting the IMAX have been described: via the anterior medial infratemporal fossa (AMITF), the middle infratemporal fossa (MITF), the anterior lateral middle fossa (ALMF), and the lateral middle fossa (LMF). The advantages and limitations of these techniques have not yet been systematically evaluated and compared. METHODS: Twenty-five cadaver specimens were used to evaluate the harvesting technique. The length and

Musculus pterygoideus proprius: A meta-analysis.

The pterygoideus proprius muscle can be found incidentally in the infratemporal fossa, where it is spatially associated with the muscles of mastication, the maxillary artery, and the trigeminal nerve and its branches. Anatomists have described the muscle in various ways over the past 160 years, chiefly as a musculotendinous structure that originates from the infratemporal crest of the sphenoid bone and inserts into the lateral pterygoid plate and the lateral pterygoid muscle. It is present in non-human primates, albeit rarely, with similar anatomical findings. Embryologically, the pterygoideus

Combined transmandibular-zygomatic approach and infratemporal craniotomy for intracranial skull base tumors.

Large intracranial skull base tumors remain surgically treacherous lesions. The preauricular transzygomatic infratemporal approach to this area has rapidly gained popularity. This article presents modifications to this approach which improved visualization and access to the infratemporal fossa. The procedure increases visualization, minimizes brain retraction, and results in improved resection and outcome. Reconstruction is rapid and there are minimal functional and cosmetic deficits.

Unilateral upper and lower subtotal maxillectomy approaches to the cranial base: microsurgical anatomy.

OBJECTIVE: The relationship of the maxilla, with its thin walls, to the nasal and oral cavities, the orbit, and the infratemporal and pterygopalatine fossae makes it a suitable route for accessing lesions involving both the central and lateral cranial base. In this study, we compared the surgical anatomy and exposure obtained by two unilateral transmaxillary approaches, one directed through an upper subtotal maxillectomy, and the other through a lower subtotal maxillectomy. METHODS: Cadaveric specimens examined, with 3 to 40x magnification, provided the material for this study. RESULTS: Both

Endoscopic Endonasal Resection of Trigeminal Schwannomas.

The surgical management of trigeminal schwannomas (TNs) entails the use of a variety of cranial base approaches for their effective surgical management. Although an extended middle fossa or posterior petrosal approach may be more appropriate for disease with primarily posterior fossa involvement, the expanded endoscopic approaches are suited for tumors with predominately middle fossa and/or extracranial involvement along the V2 and V3 divisions and limited posterior fossa extension. The endoscopic endonasal resection of TNs within the middle fossa, pterygopalatine fossa, and infratemporal foss

Treatments in sinonasal inverted papilloma malignancy invading the pterygopalatine fossa: A case report.

Sinonasal inverted papilloma (SNIP) is less malignant and usually occur in the maxillary sinus. However, cases invading the pterygopalatine fossa are extremely rare. In this article, we describe a rare case of a man in his early 60s who presented with left nasal congestion, headache, epistaxis, and facial numbness. The patient underwent a comprehensive treatment primarily based on surgery. Nasal endoscopy showed that the mass had invaded the left infratemporal fossa laterally and was poorly separated from the internal maxillary artery, which increased the complexity and difficulty of the surge

Temporal Approach to Removal of a Large Orbital Foreign Body.

Accidents with firearms can result in extensive orbital trauma. Moreover, gun parts can come loose and impale the maxillofacial region. These injuries can cause the loss of visual acuity and impair eye movements. Multidisciplinary treatment is required for injuries associated with this type of trauma. Computed tomography with three-dimensional reconstruction is useful for determining the precise location and size of the object lodged in the facial skeleton, thereby facilitating the planning of the correct surgical approach. The temporal approach is a fast, simple technique with few complicatio

Image-guided endonasal endoscopic excision of Meckel's cave trigeminal schwannoma from cavernous and petrous carotid artery.

Trigeminal schwannoma is the second most common schwannoma after vestibular schwannoma. Symptoms vary depending on the anatomical structures affected by the tumor, including facial pain, paresthesia, dizziness and ataxia. The primary goals are controlling the symptoms and the maintenance of cranial nerves' integrity perioperatively. We report a 39-year-old lady who was complaining of mild right-sided headache, vision and hearing loss, right facial weakness and dysphagia. CT and MRI showed a large dumbbell-shaped tumor originating from the trigeminal fossa abutting the petrous and cavernous car

Extensive nasopharyngeal angiofibromas: the maxillary swing approach.

The objective of this study was to evaluate the efficacy and outcome using the maxillary swing approach for the management of extensive nasopharyngeal angiofibromas. A retrospective analysis in a tertiary care center revealed five cases with extensive nasal angiofibromas operated using the maxillary swing approach between 2010 and 2012. All patients had tumor extension to the lateral-most portions of the infratemporal fossa with complete occupation and destruction of the lateral wall of the sphenoid sinus causing abutment to the cavernous sinus and complete involvement of the pterygopalatine f

Severe destruction of the temporomandibular joint with complete resorption of the condyle associated with synovitis, acne, pustulosis, hyperostosis, and osteitis syndrome.

The synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome consists of a combination of inflammatory bone disorders and dermatologic pathology. Bone lesions as a form of diffuse sclerosing osteomyelitis in the mandible occur in the posterior body and ramus. Bone lesions rarely spread to the temporomandibular joint (TMJ) where ankylosis may result. Herein we present an unusual case of SAPHO syndrome with TMJ involvement in which severe destruction of the TMJ occurred. We observed an extension of the invasive soft tissue lesion into the infratemporal fossa from the TMJ with com

High-pressure grease injury of the face.

We describe a rare case of high-pressure grease injury of the face in a 48-year-old man with severe facial edema, soft palate swelling, and bleeding from the nose and mouth. Preoperative computed tomography (CT) revealed multiple low-density areas in nasal cavity, subcutaneous tissue of the cheek, soft palate, right maxillary sinus, and right infratemporal fossa. The patient complained of a dyspnea feeling, and immediate tracheostomy was performed. The grease was removed by the Caldwell-Luc incision approach but was partially left in the infratemporal fossa. After the operation, the patient st

Infratemporal and intraorbital metastasis of the glioblastoma multiforme. A case report.

Primary brain tumors rarely metastasize outside of the central nervous system. A case of 37-year-old woman with a temporal lobe glial tumor which shows infratemporal and orbital invasion is presented. She had undergone a cranial operation because of temporal lobe epilepsy 8 years ago. Defects on the dura and bone flap were thought to be the mechanisms of the infratemporal fossa invasion. The case illustrates extra-axial metastasis of glioblastoma multiforme, an exceptional finding.

Huge pleomorphic adenoma of the upper retromolar area.

The case of a 41-year-old female is described wherein a minor salivary gland pleomorphic adenoma originating in the right retromolar area had extended into the infratemporal fossa and enlarged to 5 x 6 x 12 cm. The patient first became conscious of the mass as a slight swelling of the buccal mucosa when she was about 30 years of age, and the swelling since then had gradually gained in prominence. Preoperative computed tomography scan demonstrated a well-circumscribed mass occupying the right infratemporal fossa and medially displacing the pterygoid laminas. The mass was easily removed without
