Relationship between urinary excretion of fibronectin degradation products and proteinuria in diabetic patients, and their suppression after continuous subcutaneous heparin infusion.

To explore the possibility that the excretion of urinary fibronectin degradation products (U-FnDP) can be an indicator of the progression of diabetic nephropathy, U-FnDP and urinary protein(U-P) were determined in 64 diabetic patients and 11 healthy volunteers. Moreover, to determine whether continuous subcutaneous heparin infusion (CSHI) reduces elevated U-FnDP and U-P in diabetic patients with persistent proteinuria, heparin sodium was administered as a bolus subcutaneous injection of 5000 IU, followed by subcutaneous infusion of 250 IU/kg per 24 h heparin sodium for 7 days. U-FnDP excretion

Transition from Continuous Subcutaneous Insulin Infusion to IDeg-Based Regimens in Hospitalized T2DM Patients: A Single-Center Retrospective Analysis.

BACKGROUND: Chinese patients with type 2 diabetes mellitus (T2DM) often initiate insulin therapy in the hospital, requiring rapid glycemic control and simplified post-discharge plans. Few clinical studies have focused on the transition protocols for continuous subcutaneous insulin infusion (CSII). OBJECTIVE: To evaluate the efficacy, safety, and patient satisfaction of transitioning from CSII to three treatment regimens: insulin degludec (IDeg), insulin degludec/insulin aspart (IDegAsp), or insulin degludec and liraglutide (IDegLira). METHODS: Clinical information and continuous glucose moni

Impact of Early Subcutaneous Basal Insulin With Intravenous Insulin Infusion for Diabetic Ketoacidosis and Glycaemic Outcomes: A Systematic Review and Meta-Analysis.

BACKGROUND: Diabetic ketoacidosis (DKA) is a life-threatening metabolic emergency requiring prompt insulin therapy. Although intravenous (IV) insulin infusion remains the standard treatment, early initiation of subcutaneous (SC) basal insulin during IV therapy may improve outcomes. We evaluated the efficacy and safety of early basal insulin administration in adults with DKA. METHODS: A systematic review and meta-analysis following PRISMA 2020 guidelines searched PubMed, Embase and Cochrane Library from inception to February 2026 for randomised controlled trials (RCTs) comparing early SC basal

Transitioning from intravenous to subcutaneous ocrelizumab in a high-volume infusion center: A time-and-motion-informed capacity and service-flow analysis.

BACKGROUND: Subcutaneous (SC) ocrelizumab offers a streamlined alternative to intravenous (IV) infusion in the management of multiple sclerosis (MS). Building on time-and-motion (T&M) methodologies validated in oncology, we conducted a single-center operational study to quantify procedure-level time savings, estimate the resulting care-time dividend, and assess changes in patient flow following SC implementation. METHODS: We employed a cross-sectional observational design in a five-chair day hospital treating ∼600 patients with MS treated with infusive therapies. T&M data were collected for 1

Recent Developments in Convenience of Administration of the Anti-CD38 Antibody Isatuximab: Subcutaneous Delivery and Fast Intravenous Infusion in Patients With Multiple Myeloma.

Isatuximab-based combinations are among the accepted standard-of-care regimens for early-line treatment of patients with relapsed/refractory multiple myeloma (RRMM), based on the results of the Phase 3 ICARIA-MM and IKEMA trials. Further study findings have shown benefit with Isa-based combinations in patients with newly diagnosed MM, as reported from the randomized GMMG-HD7 and CONCEPT trials. Isa is currently approved in various countries for intravenous (IV) administration in patients with RRMM. A more convenient route of administration, such as subcutaneous (SC) injection, and faster IV in

Continuous subcutaneous infusion of glucagon-like peptide 1 lowers plasma glucose and reduces appetite in type 2 diabetic patients.

OBJECTIVE: The gut hormone glucagon-like peptide 1 (GLP-1) has insulinotropic and anorectic effects during intravenous infusion and has been proposed as a new treatment for type 2 diabetes and obesity. The effect of a single subcutaneous injection is brief because of rapid degradation. We therefore sought to evaluate the effect of infusion of GLP-1 for 48 h in patients with type 2 diabetes. RESEARCH DESIGN AND METHODS: We infused GLP-1 (2.4 pmol.kg-1.min-1) or saline subcutaneously for 48 h in randomized order in six patients with type 2 diabetes to evaluate the effect on appetite during fixe

Occult ectopic ACTH secretion due to recurrent lung carcinoid: long-term control of hypercortisolism by continuous subcutaneous infusion of octreotide.

We report on a woman with clinical Cushing's syndrome confirmed by biochemical data. The Cushing's syndrome was shown to be ACTH dependent and inferior petrosal sinus sampling pointed to an ectopic source. After resection of a lung carcinoid a well documented remission of Cushing's syndrome was obtained. At recurrence of Cushing's syndrome 18 months later the ACTH source could not be located despite extensive diagnostic procedures. Clinical and biochemical control of hypercortisolism was achieved by continuous subcutaneous infusion of octreotide. During a brief interruption of treatment recurr

Subcutaneous bone formation in the naevoid basal-cell carcinoma syndrome: normal urinary cyclic AMP response to parathyroid hormone infusion.

A patient with the naevoid basal-cell carcinoma syndrome had subcutaneous bone formation in the areas other than those occupied by the carcinomata. The patient did not show a significant phosphaturic response after parathyroid hormone infusion, but this extract produced a normal excretion of cyclic AMP. This proves that the subcutaneous bone does not signify an affinity to pseudo-hypoparathyroidism.

Continuous subcutaneous insulin infusion versus multiple daily injections in children and young people at diagnosis of type 1 diabetes: the SCIPI RCT.

BACKGROUND: The risk of developing long-term complications of type 1 diabetes (T1D) is related to glycaemic control and is reduced by the use of intensive insulin treatment regimens: multiple daily injections (MDI) (≥ 4) and continuous subcutaneous insulin infusion (CSII). Despite a lack of evidence that the more expensive treatment with CSII is superior to MDI, both treatments are used widely within the NHS. OBJECTIVES: (1) To compare glycaemic control during treatment with CSII and MDI and (2) to determine safety and cost-effectiveness of the treatment, and quality of life (QoL) of the pati

Chronic subcutaneous infusion therapy with apomorphine in advanced Parkinson's disease compared to conventional therapy: a real life study of non motor effect.

BACKGROUND: Apomorphine infusion therapy remains under-used and there are no comparative studies of motor and non-motor effects of apomorphine infusion. METHODS: In this paper we report preliminary results from an ongoing clinical observational "real life" surveillance-based study focused on effects of this therapy on non-motor symptoms and health-related quality of life in a group of patients on apomorphine. RESULTS: Apomorphine infusion led to highly significant improvements in UPDRS 3 (p = 0.0003), UPDRS 4 (p = 0.0003), PDQ-8 (Parkinson's disease questionnaire, p = 0.001) and NMSS total (
