Racial Disparities in Hospitalization Among Patients Who Receive a Diagnosis of Acute Coronary Syndrome in the Emergency Department.

Background Timely hospitalization of patients who are diagnosed with an acute coronary syndrome (ACS) at the emergency department (ED) is a crucial step to lower the risk of ACS mortality. We examined whether there are racial and ethnic differences in the risk of being discharged home among patients who received a diagnostic code of ACS at the ED and whether having health insurance plays a role. Methods and Results We examined 51 022 910 discharge records of ED visits in Florida, New York, and Utah in the years 2008, 2011, 2014, and 2016/2017 using state-specific data from the Healthcare Cost

Hospitalisations for emergency-sensitive conditions in Germany during the COVID-19 pandemic: insights from the German-wide Helios hospital network.

BACKGROUND: While there are numerous reports that describe emergency care during the early COVID-19 pandemic, there is scarcity of data for later stages. This study analyses hospitalisation rates for 37 emergency-sensitive conditions in the largest German-wide hospital network during different pandemic phases. METHODS: Using claims data of 80 hospitals, consecutive cases between 1 January and 17 November 2020 were analysed and compared with a corresponding period in 2019. Incidence rate ratios (IRRs) comparing the two periods were calculated using Poisson regression to model the number of hos

Health Insurance Status and Unplanned Surgery for Access-Sensitive Surgical Conditions.

IMPORTANCE: Access-sensitive surgical conditions, such as abdominal aortic aneurysm, ventral hernia, and colon cancer, are ideally treated with elective surgery, but when left untreated have a natural history requiring an unplanned operation. Patients' health insurance status may be a barrier to receiving timely elective care, which may be associated with higher rates of unplanned surgery and worse outcomes. OBJECTIVE: To evaluate the association between patients' insurance status and rates of unplanned surgery for these 3 access-sensitive surgical conditions and postoperative outcomes. DESI

Cost comparison between medicare and private insurance for robotic transhiatal esophagectomy.

Esophageal cancer is a significant health concern, with the robotic platform being increasingly adopted for transhiatal esophagectomy (THE). While literature exists regarding the cost of robotic THE and its benefits, there is limited data analyzing cost and concurrent hospital reimbursement based on payor or provider. This study aimed to compare hospital reimbursement after robotic THE for patients with Medicare versus private insurance. With IRB approval, a prospective study of 85 patients from 2012 to 2022 who underwent robotic THE was conducted. Private insurance was defined as coverage exc

Access to post-discharge inpatient care after lower limb trauma.

BACKGROUND: Most hospitals in the United States are required to provide emergency care to all patients, regardless of insurance status. However, uninsured patients might be unable to access non-acute services, such as post-discharge inpatient care (PDIC). This could result in prolonged acute hospitalization. We tested the hypothesis that insurance status would be independently associated with both PDIC and length of stay (LOS). METHODS: An observational study was undertaken using the California State Inpatient Database (2007-2011), which captures 98% of patients admitted to hospital in Califo

[Reconstruction or amputation in complex fractures of the tibia. A comparative study with special reference to insurance medicine aspects].

As an introduction to the subject, a historical retrospective review of the pension-related outcome of fractures of the tibia and fibula is presented. Analysis of the past 50 years shows a continuous reduction in the frequency of the award of pensions in such cases, from 36% to 2.2%. A retrospective Swiss study is then presented, in which 18 patients with below-the-knee amputations are compared with 21 patients with limb-salvage of the lower leg. Medical insurance parameters such as length of hospitalisation, number of operations, duration of rehabilitation and the level of rehabilitation achi

Health Insurance and Interhospital Transfer for Critically Ill Patients With Respiratory Failure.

IMPORTANCE: Critically ill patients with acute respiratory failure may benefit from transfer to higher-volume centers with specialized care. However, health insurance is often considered prior to interhospital transfer and may represent a factor other than severity of illness that influences transfer processes and outcomes for patients with respiratory failure. OBJECTIVE: To examine the association between patient health insurance, interhospital transfer, and mortality for critically ill patients with acute respiratory failure. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort stu

Association of Early Primary Care Follow-Up With Acute Care Use After ICU Discharge.

OBJECTIVE: To determine if early primary care follow-up after pediatric intensive care unit (PICU) hospitalization is associated with reduced acute care use. METHODS: Using claims data from publicly and privately insured children, we identified a cohort of PICU hospitalizations from January 2016 to December 2019. We matched children with and without early primary care follow-up (within 7 days of hospital discharge) 1 to 1 on demographic and clinical characteristics. We compared the number of days of acute care use (emergency department or rehospitalization) during days 8 to 90 postdischarge u
