Sexually transmitted diseases in people living with HIV after implementation of cost coverage of HIV pre-exposure prophylaxis by statutory health insurance in Germany.

OBJECTIVES: After pre-exposure prophylaxis (PrEP) was introduced, rates of sexually transmitted infections (STIs) increased among PrEP users. However, data on STI trends in people living with HIV since then are limited. Since September 2019, PrEP has been covered by statutory German health insurance (SHI) in vulnerable groups. This study aimed to determine whether this coverage of PrEP costs affected STI rates in people living with HIV (specifically, men who have sex with men). METHODS: All patients of the HIVCENTER Frankfurt diagnosed with at least one STI within the observation period were

Health insurance coverage is associated with access to substance use treatment among individuals with injection drug use: Evidence from a 12-year prospective study.

OBJECTIVE: Understand how insurance impacts access to services among people who have injected drugs. METHODS: 1748 adults who have injected drugs were assessed at twice-annual study visits between 2006 and 2017 (18,869 visits). Use of specialty substance use treatment, receipt of buprenorphine, and having a regular source of medical care were assessed for association with concurrent insurance coverage. Random intercept logistic regression was used to adjust for potential confounders. RESULTS: When participants acquired insurance, they were more likely to report specialty substance use treatm

Insurer Coverage of Invasive Coronary Angiography and Percutaneous Coronary Intervention for Stable Coronary Artery Disease in the United States Compared With Guidelines and Landmark Trials.

BACKGROUND: Invasive coronary angiography (ICA) and percutaneous coronary intervention (PCI) are common procedures for the diagnosis and treatment of coronary artery disease (CAD). These procedures are typically performed within the parameters of insurance coverage, but little is known about how insurance policies align with guidelines and landmark randomized clinical trials. METHODS: We developed 6 use cases (3 each for ICA and PCI) of clinical scenarios for stable CAD commonly encountered in clinical practice and compared policies of the largest US public and private payers (based on total

Associations Between Health Insurance Coverage with HIV Detection and Prevention Behaviors Among Individuals with Undiagnosed HIV or at Increased Risk for HIV Infection in the USA.

BACKGROUND: Improving HIV detection and prevention remains a critical public health initiative that requires policy-based solutions. This study sought to compare HIV detection/prevention behaviors before and after healthcare reform in Massachusetts, USA, among heterosexually active persons - the group with the highest reported number of undiagnosed HIV cases. The current study sought to (1) characterize differences in insurance coverage and HIV detection/prevention behaviors between cycles 1 (2006) to 5 (2019); (2) evaluate socio-demographic disparities in insurance coverage accounting for cyc

Health insurance coverage for medical nutrition therapy in older women with cancer: a comparative health policy perspective on long-term care, frailty, and survivorship equity.

Older women with cancer face overlapping risks of malnutrition, sarcopenia, frailty, treatment toxicity, functional decline, and dependence on long-term care. Medical nutrition therapy (MNT) can translate nutrition screening into individualized dietary counseling, symptom-responsive support, oral nutritional supplements, and enteral or parenteral nutrition when clinically indicated. Yet access is strongly conditioned by health insurance design. Eligibility restrictions, cost sharing, limited dietitian networks, fragmented authorization, and discontinuity between hospital, home, and long-term c

A cross-sectional study of latent tuberculosis infection, insurance coverage, and usual sources of health care among non-US-born persons in the United States.

More than 70% of tuberculosis (TB) cases diagnosed in the United States (US) occur in non-US-born persons, and this population has experienced less than half the recent incidence rate declines of US-born persons (1.5% vs 4.2%, respectively). The great majority of TB cases in non-US-born persons are attributable to reactivation of latent tuberculosis infection (LTBI). Strategies to expand LTBI-focused TB prevention may depend on LTBI positive non-US-born persons' access to, and ability to pay for, health care.To examine patterns of health insurance coverage and usual sources of health care amon

Did the Expansion of Insurance Coverage for Oral Health Reduce Self-reported Oral Health Inequalities in Korea? Results of Repeated Cross-Sectional Analysis, 2007-2015.

BACKGROUND: In 2009, the South Korean government expanded universal health insurance to include oral health services. In the present study, we sought to examine whether improved access resulted in a reduction in income-based self-reported oral health inequalities. METHODS: We analyzed repeated cross-sectional data from the Korea National Health and Nutrition Examination Survey (KNHANES) waves IV through VI (2007-2015). We analyzed self-reported oral health status among 68,431 subjects. Changes in oral health inequalities across four income levels (low, middle-low, middle-high, and high) were

Total healthcare costs and cost savings after bariatric surgery and glucagon-like peptide-1 receptor agonist treatment for obesity and diabetes vary with insurance coverage.

BACKGROUND: Glucagon-like peptide-1 receptor agonist (GLP-1-RA) options continue to evolve. Given different levels of reimbursement across insurance payors, we compared healthcare costs across payors for patients treated with metabolic and bariatric surgery (MBS) or GLP-1-RAs. SETTING: Academic Health Center. METHODS: Using the STATinMED RWD Insights all-payor claims database, we performed a retrospective 1:1 propensity-matched cohort analysis of patients with class II or III obesity and type 2 diabetes (T2D) treated with sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), or GLP-1-RAs

Risk-reducing mastectomy and salpingo-oophorectomy in women with hereditary breast and ovarian cancer: a single-institute experience following coverage by Japanese national medical insurance.

BACKGROUND: Risk-reducing mastectomy (RRM) and risk-reducing salpingo-oophorectomy (RRSO) are preventive options for women with hereditary breast and ovarian cancer (HBOC). The Japanese national medical insurance began covering RRM and RRSO for patients with HBOC in April 2020. METHODS: We retrospectively analyzed 59 individuals with pathogenic germline variants (PGVs) from 55 families diagnosed with HBOC between 2010 and 2024 to assess the prevalence of RRM and RRSO after April 2020. RESULTS: The median age at diagnosis was 50 years. Ten individuals (16.9%) were diagnosed before April 2020,

Demographic, socio-economic, behavioral and clinical factors predicting virologic failure with generic fixed-dose combination antiretroviral therapy before universal health insurance coverage in northern Thailand.

We conducted a 2-year prospective cohort study to investigate multiple aspects of factors predicting the outcome of fixed-dose combination antiretroviral (ARV) therapy with lamivudine, stavudine, and nevirapine (GPOvir) at a government referral hospital in northern Thailand. At 6 and 24 months after the initiation of GPOvir, viral load (VL) was measured to determine virologic failure (>400 RNA copies/ml) and demographic, socio-economic, behavioral and clinical data were collected. From 10 April 2002 to 31 January 2004, 409 patients participated in this study: 64/364 (17.0%) at 6 months and 55/

Vertebral Fracture Assessment by dual-energy X-ray absorptiometry: insurance coverage issues in the United States. A White Paper of the International Society for Clinical Densitometry.

Clinical trial data and fracture risk prediction models unequivocally demonstrate the utility of identifying prevalent vertebral fractures to predict future fractures of all types. Knowledge of prevalent vertebral fractures can alter patient management decisions and result in initiation of therapy to reduce fracture risk in some patients who would not otherwise be treated. Cost-benefit analysis demonstrates that identifying and treating patients with vertebral fractures, even those with a densitometric classification of osteopenia, is cost effective. Vertebral fractures can be readily identifi

The impact of digital interventions on health insurance coverage for reproductive, maternal, newborn and child health services utilization in Kakamega, Kenya: a cluster randomized controlled trial.

The National Hospital Insurance Fund (NHIF) of Kenya was upgraded to improve access to healthcare for impoverished households, expand universal health coverage, and boost the uptake of essential reproductive, maternal, newborn and child health (RMNCH) services. However, premiums may be unaffordable for the poorest households. The Innovative Partnership for Universal Sustainable Healthcare (i-PUSH) programme targets low-income women and their households to improve their access to and utilization of quality healthcare, including RMNCH services, by providing subsidized, mobile phone-based NHIF co

Population-Level Trends in Gastric Cancer Mortality and Incidence After Expansion of Japanese National Health Insurance Coverage for Helicobacter pylori Eradication: A Descriptive Time-Trend Study.

BACKGROUND: In February 2013, Japanese national health insurance coverage for Helicobacter pylori eradication was expanded to patients with H. pylori-positive, endoscopically diagnosed chronic gastritis. This policy broadened access to eradication treatment, but its relationship with subsequent population-level gastric cancer trends requires cautious interpretation. AIM: To describe national trends in gastric cancer mortality and incidence before and after the 2013 insurance expansion, with attention to age-specific patterns, endoscopic screening, treatment uptake, and alternative explanation

Lessons learned from the movement behind the first over-the-counter daily birth control pill in the United States and recommended strategies for implementing full insurance coverage.

July 13, 2023, marked a historic day for contraception access when the Food and Drug Administration (FDA) approved the first-ever daily over-the-counter (OTC) oral contraceptive pill (OCP) in the United States. This approval did not happen overnight. It took two decades of research, advocacy, and partnerships led by the Free the Pill coalition (formerly the Oral Contraceptives Over the Counter Working Group). The Free the Pill coalition's vision for OTC OCPs goes beyond the FDA approval; it also includes efforts to make other types of OCPs available without a prescription in the United States
