Seizures in the adult intensive care unit.

Seizures are a common occurrence in the intensive care unit (ICU). The presentation of seizures is usually as focal or generalized motor convulsions, but other seizure types may occur. Etiologies of the seizures are typically secondary either to primary neurologic pathology or a consequence of critical illness and clinical management. Particularly important as precipitants of seizures are hypoxia/ischemia, drug toxicity, and metabolic abnormalities. It is important to properly diagnose the seizure type and its cause to ensure appropriate therapy. Most seizures occur singly, and recurrence is u

Tracheotomy and the intensive care unit patient.

Transportation of the intensive care unit (ICU) patient to the operating room for tracheotomy has been implicated as an unnecessary source of complications and has been cited as a relative indication for percutaneous tracheotomy. However, there is very little evidence in the literature to support this claim. We evaluated 100 consecutive patients who were transported from the ICU to the operating room for tracheotomy. There were no complications related to patient transportation. A total of five complications occurred, all unrelated to patient transportation. Two patients receiving pressure con

Bedside tracheostomy in the intensive care unit.

OBJECTIVE: To prove that tracheostomy performed at the bedside in the intensive care unit is a safe, cost-effective procedure. DESIGN: Retrospective review of all adult patients undergoing elective bedside tracheostomy in the intensive care unit between January 1983 and December 1988. Two hundred four patients were identified. SETTING: A private 1200-bed tertiary care center with a 120-bed critical care facility. MAIN OUTCOME MEASURES: Major and minor perioperative complications, cost savings, and comparison of risk between bedside tracheostomy and that performed in the operating room. RES

The Role of Laparoscopy in the Intensive Care Unit.

Evaluation of the abdomen in critically ill intensive care unit patients remains problematic. Sedation, neuromuscular blockade, and head injury frequently make physical examination unreliable. As intra-abdominal processes frequently contribute to the development of multiple organ failure, it is imperative to establish a diagnosis and initiate treatment as soon as possible while the organ failures remain reversible. A diagnostic algorithm is presented as well as a summary of recent literature that places diagnostic laparoscopy in context with other diagnostic studies for evaluation of the abdom

Neurologic intensive care unit monitoring.

Monitoring modalities unique to the neurologic intensive care unit include intracranial pressure monitors and neuroelectrophysiologic monitors. Each modality fullfills criteria for accuracy, responsivity during clinical change, and stability over time for trend analysis. Intracranial pressure monitoring may be accomplished by any of three approaches--ventricular catheter, subarachnoid bolt, or epidural pressure transducer. Intracranial pressure control has proved beneficial in at least three different illnesses--acute closed head injury, acute noncommunicating hydrocephalus, and Reye's syndrom

Infections as a problem in the intensive care unit.

Problems of infections in the intensive care unit (ICU) center on patients admitted because of sepsis, patients with compromised host defenses and patients subject to invasive procedures. The mortality rate in septic shock is high, 47% in our own study. Escherichia coli was found in half the cases. Multiple organ failure is often the common denominator in patients with sepsis. Hypovolaemia and low serum proteins make the patients more susceptible to infections. Typical ICU procedure, i.e. central venous catheters, arterial lines, Swan-Ganz catheters, endotracheal tubes and urinary catheters vi

Use of etomidate in an intensive care unit.

A resumé of two studies and retrospective survey of 52 patients in an intensive care unit receiving infusions of etomidate has demonstrated that the technique can be used effectively and safely to provide sedation in patients requiring artificial ventilation and those breathing spontaneously. In both studies all measured respiratory and cardiovascular indices remained within normal limits. There was a low incidence of muscle movements in all groups and there was no nausea, vomiting, or allergic reactions. Additionally, the technique was readily accepted by the patients.

Osteomyelitis in a neonatal intensive care unit.

Neonatal osteomyelitis presents with few clinical signs despite multiple sites of involvement. Four cases of osteomyelitis due to Staphylococcus aureus or Candida albicans were encountered in a neonatal intensive care unit. Three were unsuspected clinically and were detected as incidental radiologic findings. The fourth presented with soft-tissue abscesses. Long bone metaphyses were most frequently affected. Other sites included iliac bones, clavicles, and spine. On follow-up the bones healed, but one patient was left with hip deformity secondary to destruction of the cartilaginous femoral hea

[The pregnant patient in the Intensive Care Unit].

Pregnant patients require intensive medical treatment relatively rarely: in industrialized countries they account for only about 2% of ICU-patients. Compared to pregnancy, the need for intensive care occurs more often in the immediate postpartum setting, primarily due to peripartum complications such as severe haemorrhage, hypertensive crises, or sepsis. The treatment of such patients requires a multiprofessional and interdisciplinary approach involving obstetricians, neonatologists, intensive care physicians, nursing staff, and other specialists, as well as, if necessary, psychologists. [Liver diseases in the intensive care unit].

The frequency of liver diseases in the intensive care unit has increased significantly in recent years and is now observed in up to 20% of critically ill patients. The occurrence of liver disease is associated with significantly increased morbidity and mortality. Two groups of liver diseases in the intensive care unit can be distinguished. First, the group of "primary hepatic dysfunctions", which includes primary acute liver failure as well as acute-on-chronic liver failure in patients with pre-existing liver cirrhosis. The second group of "secondary or acquired liver diseases" includes choles

Plasma transfusion in the intensive care unit.

BACKGROUND: Current guidelines discourage prophylactic plasma use in non-bleeding patients. This study assesses global plasma transfusion practices in the intensive care unit (ICU) and their alignment with current guidelines. STUDY DESIGN AND METHODS: This was a sub-study of an international, prospective, observational cohort. Primary outcomes were in-ICU occurrence rate of plasma transfusion, proportion of plasma events of total blood products events, and number of plasma units per event. Secondary outcomes included transfusion indications, INR/PT, and proportion of events for non-bleeding i

Reigniting Intensive Care Unit Liberation.

BACKGROUND: The Society of Critical Care Medicine has established guidelines to manage pain, sedation, delirium, immobility, family participation, and sleep disruption in the intensive care unit, a set of interventions known as the intensive care unit liberation (ABCDEF) bundle. Adherence to these guidelines has shown positive results. LOCAL PROBLEM: In the intensive care units of a level I trauma academic teaching hospital in central Texas, the rate of bedside nursing staff adherence to the ABCDEF bundle was only 67.1% in January 2022. The aim of this quality improvement project was to impro
