Anesthetic Management of a Patient With Symptomatic May-Thurner Syndrome: A Case Report.

May-Thurner syndrome (MTS) is characterized by the compression of the left common iliac vein by the overlying right common iliac artery, which can lead to venous insufficiency, obstruction, and an increased risk of iliofemoral deep vein thrombosis (DVT) and pulmonary embolism (PE). We report the perioperative anesthetic management of a 38-year-old female with symptomatic MTS who underwent a total laparoscopic hysterectomy, bilateral salpingectomy, and lysis of adhesions for abnormal uterine bleeding. The patient had a history of persistent left lower extremity symptoms despite prior left commo

Effects of Intermittent Sequential Pneumatic Compression on Functional, Biochemical, and Perceptual Recovery Markers in Trained Cyclists: A Randomized Sham-Controlled Crossover Trial.

PURPOSE: Intermittent Sequential Pneumatic Compression (ISPC) has gained popularity as a recovery modality among athletes, yet its effectiveness in high-performance cycling settings remains unclear. This study aimed to evaluate the effectiveness of a 30-minute high-pressure ISPC intervention, in comparison with a sham condition, on the recovery of high-level road cyclists following a 30-minute all-out time trial on a stationary trainer. METHODS: Twenty-one male participants (age: 22.8 [4.8] y) were included in this crossover trial, with their recovery protocol sequence randomly assigned. Meas

Venous thromboembolism following foot and ankle surgery: A retrospective review with a prospective cohort study emphasizing the need for a chemoprophylaxis protocol.

BACKGROUND: Venous thromboembolism (VTE) is a potentially fatal complication following surgery. Incidence rates vary significantly depending on the procedure, ranging from 45-57% after pelvic surgery to 0.22-5.09% after foot surgery. Because of this variability, the use of prophylaxis in foot and ankle surgery has been debated. PURPOSE: This study aimed to evaluate the incidence of VTE in forefoot and rearfoot procedures and assess whether a patient-specific prophylaxis protocol reduced risk. STUDY DESIGN: Retrospective and prospective cohort study conducted over a four-year period. METHODS

Optimal prophylactic method of venous thromboembolism for gastrectomy in Korean patients: an interim analysis of prospective randomized trial.

BACKGROUND: Whereas routine prophylaxis for venous thromboembolism (VTE) is frequently utilized in the West, Asian physicians employ it much less often, based on its recorded rarity amongst their patients. This study was designed to examine the incidence of VTE and to determine the optimal method of thromboembolic prophylaxis following gastrectomy for cancer. METHODS: In this prospective, randomized trial, patients were assigned to either an intermittent pneumatic compression (IPC) only or an IPC plus enoxaparin. The primary end point of this study was to determine the VTE incidence rate with

Efficacy of prophylactic placement of inferior vena cava filter in patients undergoing spinal surgery.

STUDY DESIGN: Retrospective case series. OBJECTIVE: To evaluate the safety and efficacy of prophylactic inferior vena cava filter (IVCF) to prevent pulmonary embolism (PE) in high risk patients undergoing major complex spinal surgery. SUMMARY OF BACKGROUND DATA: PE has been reported to be the major cause of death after spinal reconstructive surgery. Mechanical prophylaxis alone is often not sufficient whereas anticoagulation therapy carries a significant risk of bleeding complications. Prophylactic IVCF placement is advocated in high-risk patients. METHODS: A total of 129 high-risk patients

The Effect of Intermittent Pneumatic Compression on Hemodynamics and Regional Cerebral Oxygen Saturation in Laparoscopic Bariatric Surgery with Mild Hypercapnia in the Steep Reverse Trendelenburg Position.

Obesity negatively affects hemodynamics and cerebral physiology. We investigated the effect of the utilization of an intermittent pneumatic compression (IPC) device on hemodynamics and cerebral physiology in patients undergoing laparoscopic bariatric surgery under general anesthesia with lung-protective ventilation. Sixty-four patients (body mass index > 30 kg/m2) were randomly assigned to groups that received an IPC device (IPC group, n = 32) and did not (control group, n = 32). The mean arterial pressure (MAP), heart rate (HR), need for vasopressors, cerebral oxygen saturation (rSO2), and ce

Breaking the Cycle of Recurrent Diabetic Foot Ulceration: A Novel and Sustainable Offloading Modality to Treat Diabetic Foot Ulceration and Prevent Recurrence.

INTRODUCTION: Delayed healing and recurrence of diabetic foot ulcerations (DFUs) is often related to excessive pressure. Offloading, a mainstay of treatment, can be achieved through a variety of methods. Although pressure mitigation should be continued after wound resolution to prevent recurrence, many offloading modalities are discontinued at that point, with providers instead relying on patient self-directed use of appropriate inserts and shoe gear. Use of a novel offloading modality continued upon wound healing may help break the cycle of recurrent DFUs. CASE REPORT: A patient presented wi

Mechanical thromboembolic prophylaxis with risk stratification in total knee arthroplasty.

The purpose of this study was to determine the rate of thromboembolic and bleeding complications when using mechanical prophylaxis with preoperative risk stratification following total knee arthroplasty (TKA). Between 1994 and 2007, 4037 TKAs were performed on 3144 patients at our institution. Mechanical VTE prophylaxis was used for standard risk patients, which included AV impulse foot pumps, thigh high stockings, and early mobilization. Chemoprophylaxis was only given to patients who were at increased thromboembolic risk. The incidence of DVT identified by ultrasound following TKA was 2.1%.

Evaluation of a venous-return assist device to treat severe post-thrombotic syndrome (VENOPTS). A randomized controlled trial.

Severe post-thrombotic syndrome (PTS) is responsible for considerable disability, reduced quality of life and increased health care costs. Current therapies are limited and often ineffective. We performed a two-centre, randomized, cross-over controlled trial to evaluate Venowave, a novel lower-limb venous-return assist device, for the treatment of severe PTS. Eligible subjects were allocated to receive, in randomized order, Venowave for eight weeks and a control device for eight weeks. The eight-week treatment periods were separated by a four-week period when no device was used (i.e. wash-out

Incidence of venous thromboembolism after robotic-assisted hysterectomy in obese patients with endometrial cancer: do we need extended prophylaxis?

Robotic surgery for gynecologic malignancy is associated with a lower rate of venous thromboembolism (VTE) than laparotomy. Obese patients represent a particularly high-risk group for VTE, but prior studies tend to focus on patients with a much lower BMI and without extended VTE prophylaxis. Our objective was to examine the role of extended thromboprophylaxis in obese patients who underwent robotic-assisted surgery for endometrial cancer. We conducted a retrospective cohort study of obese patients (BMI ≥ 35 kg/m2) who underwent robotic surgery for newly diagnosed endometrial cancer. The primar

[Self-active physical thrombosis prophylaxis in the patients' bed with the Phlebostep: acceptance and measurement of venous blood flow in immobilized patients].

BACKGROUND: Despite the broad use of low molecular weight heparin, deep vein thrombosis is still a relevant risk for immobilized patients in orthopedic surgery. Patients can reduce this risk by active training exercises with a muscle pump. The aim of this study was to test the acceptance and effect of a self-developed training device to accelerate venous return as well as a technical optimization. The device was installed for in-patients in orthopedic and traumatology departments. PATIENTS AND METHODS: A simple pillow-like device was developed, which can be positioned against the foot end of

Preoperative patient preparation in enhanced recovery pathways.

Enhanced recovery pathways are a novel approach focused on enhancing the care of surgical patients. "Prehabilitation" is the term applied to any intervention administered before surgery to reduce surgery-related morbidity, decrease the length of hospital stay, expedite the return of organ function, and facilitate the patient's return to normal life. A PubMed search was performed with the following key words: enhanced recovery, preoperative preparation, cessation of smoking and euvolemia. The results from this Pubmed search revealed that female patients may have higher levels of anxiety than ma

Superior Clinical, Quality of Life, Functional, and Health Economic Outcomes with Pneumatic Compression Therapy for Lymphedema.

BACKGROUND: Pneumatic compression therapy is one of several options for the management of lymphedema. The lack of clarity around clinical outcomes, quality of life, cost of care, and its proper application, as a function of lymphedema complexity, limit its use in clinical practice. This is compounded by difficulties associated with insurance approval and uncertainty about the role of this modality in the treatment algorithm. The purpose of this study is to elucidate the healthcare economics and value of pneumatic compression therapy for lymphedema. METHODS: All patients who underwent treatmen
