Painless giant angioleiomyoma in the subfascia of the lower leg.

Angioleiomyoma is rare, benign vascular smooth muscle tumor originating from the tunica media of the veins and arteries. It usually presents as a slow-growing, small, and painful mass arising from the cutaneous or subcutaneous tissue. We report an unusual case of angioleiomyoma that was located in the subfascia of the lower leg and had grown to massive size (≤5 cm in diameter) during a 3-year period without pain. A 57-year-old female presented with a 3-year history of a slowly growing pain-free mass on the anterior portion of her right lower leg, just above the ankle joint. Magnetic resonance

The risk injury to the posterior interosseous nerve in standard approaches to the proximal radius: a cadaver study.

PURPOSE: The aim of this study was to provide guidance on the safe zones for the exposure of the proximal radius by measuring the distance from the PIN to various anatomical landmarks in the proximal forearm in pronation and supination. METHODS: Twenty cadaveric arms were used for this study. On the anterior aspect of the forearm, the distance between insertion of the biceps tendon and the arcade of Frohse as well as the shortest distance between the PIN and the ulnar aspect of the radial neck were measured. On the posterior aspect of the forearm, the shortest distance between the PIN and the

Lunate-triquetral and midcarpal joint instability.

Instability of the ulnar side of carpus centers around the triquetrum, which is suspended by the ulnar triquetral ligaments and supported proximally by the TFCC. The triquetrum guides the lunate by an interosseous membrane and stout palmar ligaments that provide a relatively rigid connection between the two bones. Disruption of the LT ligament is frequently associated with pathology in the ulnar carpal area and may progress to triquetral instability, VISI, and finally, degenerative arthritic changes on the ulnar side of the carpus. The diagnosis of LT injuries is made by stress radiographs, ar

Posterior tibial tendon transfer: a review of the literature and analysis of 74 procedures.

At Rancho Los Amigos Hospital on the Children's Orthopedic Service, 43 patients underwent anterior transfer of the posterior tibial tendon through the interosseous membrane between 1969 and 1979. This included 74 procedures for the following diagnoses: Duchenne muscular dystrophy, club-foot, cerebrospastic disease (cerebral palsy and head injury), Charcot-Marie-Tooth disease, scapuloperoneal dystrophy, and peroneal palsy. The purpose of this paper is to present the 54 of these transfers that have a minimum 2 year follow-up were performed with a uniform surgical technique, and had uniform posto

Osteogenesis Imperfecta: Current and Prospective Therapies.

Osteogenesis Imperfecta (OI) is a group of connective tissue disorders with a broad range of phenotypes characterized primarily by bone fragility. The prevalence of OI ranges from about 1:15,000 to 1:20,000 births. Five types of the disease are commonly distinguished, ranging from a mild (type I) to a lethal one (type II). Types III and IV are severe forms allowing survival after the neonatal period, while type V is characterized by a mild to moderate phenotype with calcification of interosseous membranes. In most cases, there is a reduction in the production of normal type I collagen (col I)

Extensor indicis proprius muscle and its variants together with the extensor digitorum brevis manus muscle: a common classification. Clinical significance in hand and reconstructive surgery.

PURPOSE: The extensor indicis proprius (EIP) is a muscle of the forearm that originates from the posterior surface of the ulna and the adjacent interosseous membrane and attaches to the index finger. Many anatomical variations of this muscle have been reported in the literature. The extensor digitorum brevis manus (EDBM) is a variant muscle found on the back of the wrist and hand. These muscle variants should be considered in the context of clinical syndromes and reconstructive hand surgery. The aim of the present study was to describe in detail the normal and variant anatomy of these muscles

Vascular anatomy and harvesting of the free fibula flap in pigs.

PURPOSE: Pigs have long been used as an animal model for fibula flaps. However, the vascular anatomy of the pig fibula has not been described comprehensively. This study was designed to explore the vascular anatomy of the pig fibula and explore the feasibility of using pigs as animal models for surgical training. METHODS: Twenty fresh cadaveric hindlimbs from hybrid pigs (Shanghai White pig) were used in this study. The arteries were infused with black lard. The origin, course, and branches of the vessels nourishing the fibula were observed in 15 samples. The primary parameters included varia

The triangular fibrocartilage complex (TFCC): updated anatomy, variants, and MRI diagnostic pitfalls.

The triangular fibrocartilage complex (TFCC) is a key stabilizer of the ulnar wrist, integrating both the distal radioulnar and ulnocarpal joints. Recent advances in arthroscopic anatomy have led to a three-dimensional, tripartite conceptualization of the TFCC, describing it as comprising three interconnected components: the articular disc, the distal radioulnar ligaments providing primary stability, and a continuous peripheral capsular structure, or "peripheral wall," which encompasses the palmar and dorsal ulnocarpal ligaments, the meniscus homologue, and the extensor carpi ulnaris tendon sh

Delayed anterolateral radial head dislocation secondary to radial shaft fracture malunion: A case report.

RATIONALE: Traumatic radial head dislocation (RHD) can occur due to hyperpronation injury with sequential disruption of the annular ligament, quadrate ligament, and the interosseous membrane. Although studies have shown that traumatic RHD is generally associated with Monteggia fracture-dislocation, traumatic RHD occurring with ipsilateral radial shaft fractures has rarely been reported. Delayed RHD secondary to the malunion of isolated radial shaft fractures is extremely rare. PATIENT CONCERNS: We report the case of a 12-year-old right-handed boy with progressive pain and limited range of mot

New tendon transfer for correction of drop-foot in common peroneal nerve palsy.

UNLABELLED: Common peroneal nerve palsy has been reported to be the most frequent lower extremity palsy characterized by a supinated equinovarus foot deformity and foot drop. Dynamic tendon transposition represents the gold standard for surgical restoration of dorsiflexion of a permanently paralyzed foot. Between 1998 and 2005, we operated on a selected series of 16 patients with traumatic complete common peroneal nerve palsy. In all cases, we performed a double tendon transfer through the interosseous membrane. The posterior tibialis tendon was transferred to the tibialis anterior rerouted th

Ulna Rotation Osteotomy in Complete Dislocation of the Distal Radioulnar Joint.

Background Complete dislocation of the distal radioulnar joint (DRUJ) is a rare injury. Dislocation can present as uni- or multidirectional, acute or chronic, reducible or irreducible. It can occur due to isolated loss of ligamentous restraints or more frequent in Galeazzi - or Essex-Lopresti-type fractures. Case Description A 28-year-old man sustained a distal radius fracture and presented after conservative treatment with blocked supination. Examination revealed a malunion of the distal radius in combination with an irreducible dorsal dislocation of the DRUJ. Corrective osteotomy of the dist

A framework for parametric modeling of ankle ligaments to determine the in situ response under gross foot motion.

Ligament sprains account for a majority of injuries to the foot and ankle complex, but ligament properties have not been understood well due to the difficulties in replicating the complex geometry, in situ stress state, and non-uniformity of the strain. For a full investigation of the injury mechanism, it is essential to build up a foot and ankle model validated at the level of bony kinematics and ligament properties. This study developed a framework to parameterize the ligament response for determining the in situ stress state and heterogeneous force-elongation characteristics using a finite

The undiagnosed Essex-Lopresti injury.

The Essex-Lopresti injury is rare. It consists of fracture of the head of the radius, rupture of the interosseous membrane and disruption of the distal radioulnar joint. The injury is often missed because attention is directed towards the fracture of the head of the radius. We present a series of 12 patients with a mean age of 44.9 years (26 to 54), 11 of whom were treated surgically at a mean of 4.6 months (1 to 16) after injury and the other after 18 years. They were followed up for a mean of 29.2 months (2 to 69). Ten patients had additional injuries to the forearm or wrist, which made diag

Rheumatological symptoms due to retinoids.

Isotretinoin and etretinate are synthetic derivatives of vitamin A widely used in the treatment of dermatological diseases, mainly those affecting keratinization. They have numerous side-effects, among which the rheumatic symptoms are not the most common or the most severe. The main skeletal adverse reaction of retinoids is hyperostosis. It mainly occurs with protracted treatments and high dosages, and its incidence may exceed 80% after a few years of administration. Hyperostosis is axial, located in the cervical and thoracic spine, and may be responsible for limitation of movement; in the app

Effects of axial forearm instability on force transmission across the elbow.

BACKGROUND: The interosseous membrane (IOM) and distal radioulnar joint (DRUJ) provide axial stability to the forearm. Our hypothesis was that injury to these structures alters force transmission through the elbow. METHODS: A custom-designed apparatus that applies axial loads from the wrist to the elbow was used to test 10 cadaveric upper limbs under the following simulated conditions (1) intact, (2) DRUJ injury, (3) IOM injury, or (4) IOM + DRUJ injury. IOM injury was simulated by osteotomies of the IOM attachment to the radius, and DRUJ injury was simulated by distal ulnar oblique osteotomy

A nonclassical IFITM5 mutation located in the coding region causes severe osteogenesis imperfecta with prenatal onset.

Osteogenesis imperfecta (OI) is a hereditary connective tissue disorder characterized by a wide range of skeletal symptoms. Most patients have dominantly inherited or de novo mutations in COL1A1 or COL1A2. Up to 5% of patients have OI type V, characterized by hyperplastic callus formation after fractures, calcification of the interosseous membrane of the forearm, and a mesh-like lamellation pattern observed in bone histology. Recently, a heterozygous mutation in the 5'-untranslated region (UTR) of IFITM5 (c.-14C > T) was identified as the underlying cause of OI type V, and only this specific m

The utilization of a suture bridge construct for tibiofibular instability during transtibial amputation without distal bridge synostosis creation.

Symptomatic distal tibiofibular instability is a known complication of trauma-related transtibial amputations. Overt proximal tibiofibular dislocations, which are easily recognized on routine radiographs, may occur concurrently with the traumatic injury or amputation. More commonly, however, the proximal tibiofibular joint remains structurally intact in the presence of distal instability due to the loss of the distal syndesmotic structures and damage to the interosseous membrane, resulting in fibular angulation and distal tibiofibular diastasis. Some authors have espoused treating this instabi

How does a reduction in the radial bow, in addition to a rotational deformity of the radius, affect the range of motion of the forearm: an anatomical cadaver study.

BACKGROUND: This study aimed to investigate the anatomical effect of radial rotational deformity and reduced radial bow on the forearm range of motion (ROM) to improve outcomes in severe forearm diaphyseal fractures and malunion. METHODS: Twenty upper limbs of 10 fresh-frozen cadavers were amputated at the humerus and distally at the carpometacarpal joint and examined. Soft tissues, except the interosseous membrane, articular capsule, and pronator quadratus muscle, were removed. The deformity was modeled by cutting at the maximal bow of the radial diaphysis and fixing it with an external fixa

The IFITM5 mutation in osteogenesis imperfecta type V is associated with an ERK/SOX9-dependent osteoprogenitor differentiation defect.

Osteogenesis imperfecta (OI) type V is the second most common form of OI, distinguished by hyperplastic callus formation and calcification of the interosseous membranes, in addition to the bone fragility. It is caused by a recurrent, dominant pathogenic variant (c.-14C>T) in interferon-induced transmembrane protein 5 (IFITM5). Here, we generated a conditional Rosa26-knockin mouse model to study the mechanistic consequences of the recurrent mutation. Expression of the mutant Ifitm5 in osteo-chondroprogenitor or chondrogenic cells resulted in low bone mass and growth retardation. Mutant limbs sh
