Selective neurectomy of the zygomatic branches for oro-ocular synkinesis: A targeted approach to improve eye aperture.

BACKGROUND: Oro-ocular synkinesis is a common sequela of facial paralysis, characterized by involuntary eyelid closure during oral movement. Although botulinum toxin (BTX) is the first-line therapy, its transient effect necessitates repeated treatments. Selective neurectomy (SN) of the synkinetic ocular branches from the zygomatic-buccal nerves has emerged as an adjunct to BTX, though objective outcome data remain limited. METHODS: A retrospective cohort study was performed on patients with post-paralytic synkinesis treated with either BTX exclusively (BTX, n=13) or targeted SN of the synkine

Myoclonus in the critically ill: Diagnosis, management, and clinical impact.

Myoclonus is the second most common involuntary non-epileptic movement in intensive care units following tremor-like gestures. Although there are several types of myoclonus, they remain underappreciated, and their diagnostic and prognostic associations are largely ignored. This review discusses clinical, electrophysiological, neuroanatomical, and neuroimaging characteristics of different types of myoclonus in critically ill adults along with their prognostic impact and treatment options. Myoclonus is characterized by a sudden, brief, and sometimes repetitive muscle contraction of body parts, o

State intervention in pregnancy: Should the law respond thus to the problem of Foetal Alcohol Spectrum Disorder?

Maternal consumption of alcohol during pregnancy poses a serious threat to the life and health of unborn children. A submission to the Queensland Child Protection Commission of Inquiry proposed that the State's Child Protection Act be extended to allow intervention to protect unborn children, with a court empowered to order that the mother be taken into care pending birth, or otherwise impose conditions upon conduct. This article considers whether or not the law in Australia should respond to the problem of Foetal Alcohol Spectrum Disorder by allowing the involuntary treatment and detention of

Involuntary psychiatric hospitalization of children and adolescents in Northern Greece: Retrospective epidemiological study and related ethical issues.

The aim of the present study was to investigate epidemiological data on involuntary hospitalization of underage patients in psychiatric settings and illustrate the related ethical issues. The medical records of 131 involuntary psychiatric admissions of children and adolescents ordered by public prosecutor between 2005 and 2014 were examined carefully. The examined variables involved the place of origin, the place of residence of minors after discharge, the length of stay in hospitals, the discharge diagnosis, the rate at which the minors were introduced to police and other authorities before t

Palatal myoclonus--case report.

Palatal myoclonus is the name of a syndrome characterized by an involuntary rhythmic movement of the soft palate, occasionally involving the facial muscles, pharynx, larynx, and diaphragm. A review of the literature involving the etiology, pathology, symptoms, and treatment is presented. A case is reported with successful treatment with phenytoin sodium and phenobarbital. Also presented and discussed are the influences of acoustic and corneal stimulations on the myoclonic activity.

Clostridial toxins as therapeutic agents: benefits of nature's most toxic proteins.

Toxins are increasingly being used as valuable tools for analysis of cellular physiology, and some are used medicinally for treatment of human diseases. In particular, botulinum toxin, the most poisonous biological substance known, is used for treatment of a myriad of human neuromuscular disorders characterized by involuntary muscle contractions. Since approval of type-A botulinum toxin by the US Food and Drug Administration in December 1989 for three disorders (strabismus, blepharospasm, and hemifacial spasm), the number of indications being treated has increased greatly to include numerous f

[Effects of deep brain stimulation on urodynamic findings in patients with Parkinson's disease].

Electric stimulation therapy is one of the surgical treatments for Parkinson's disease whereby a chronic stimulating electrode is placed on the subthalmic nucleus (STN). Because medical treatments centered around L-dopa have limitations in severe Parkinson's disease, electric stimulation therapy is regarded as an appropriate treatment modality. Most Parkinson's disease patients experience lower urinary tract disorders such as urgency, daytime frequency or nocturia, due to detrusor overactivity. We conducted an International Prostate Symptom Score (IPSS) analysis and a pressure flow study (PFS)

Seizure As the Sentinel Event: An Atypical Presentation of Anti-N-Methyl-D-Aspartate (NMDA) Receptor Encephalitis in a Young Adult Male.

This case report details an atypical presentation of anti-N-methyl-D-aspartate (NMDA) receptor encephalitis in a 21-year-old male, characterized by a "seizure-first" phenotype. The patient's initial generalized tonic-clonic seizure led to a misdiagnosis of epilepsy, and subsequent psychiatric symptoms were incorrectly attributed to the side effects of antiepileptic medication or a new, undiagnosed psychiatric disorder. This resulted in an involuntary psychiatric admission and a delay in diagnosis and, thus, definitive treatment. Once the Graus criteria for probable anti-NMDA receptor encephali

Riluzole Administration to Rats with Levodopa-Induced Dyskinesia Leads to Loss of DNA Methylation in Neuronal Genes.

Dyskinesias are characterized by abnormal repetitive involuntary movements due to dysfunctional neuronal activity. Although levodopa-induced dyskinesia, characterized by tic-like abnormal involuntary movements, has no clinical treatment for Parkinson's disease patients, animal studies indicate that Riluzole, which interferes with glutamatergic neurotransmission, can improve the phenotype. The rat model of Levodopa-Induced Dyskinesia is a unilateral lesion with 6-hydroxydopamine in the medial forebrain bundle, followed by the repeated administration of levodopa. The molecular pathomechanism of

Violence in first-episode psychosis: a systematic review and meta-analysis.

BACKGROUND: Violence towards others is a recognised complication of first-episode psychosis. AIMS: To estimate the rate of violence and the associations with violence in first-episode psychosis. METHOD: A systematic review and meta-analysis of 9 studies. RESULTS: Pooled estimates of the proportion of patients with first-episode psychosis committing any violence, serious violence and severe violence were 34.5%, 16.6% and 0.6%, respectively. Violence of any severity was associated with involuntary treatment (OR=3.84), a forensic history (OR=3.28), hostile affect (OR=3.52), symptoms of mania (

Limb-shaking transient ischemic attack with facial muscles involuntary twitch successfully treated with internal carotid artery stenting.

INTRODUCTION: Limb-shaking transient ischemic attack (LS-TIA) is a clinical disease with severe carotid stenosis, which is characterized by unilateral rhythmic dance or tremor like involuntary movements of arms and/or legs, but facial muscles are usually unaffected. METHODS: Today, we report a 42-year-old man with transient ischemic attack who suffered from right limb shaking and right facial muscle twitching due to the obvious stenosis of left internal carotid artery (ICA). Written informed consent was obtained from participants according to the Declaration of Helsinki, and a local ethic com

Psychogenic non-epileptic seizures treated with guided transcranial direct current stimulation: A case report.

INTRODUCTION: Psychogenic non-epileptic seizures (PNES) are paroxysms of either altered subjective or objective manifestations that may mimic epileptic seizures (ES), without abnormal neuronal epileptiform activity. In this report, we present the case of a 39-year-old woman with PNES and functional movement disorders, who was successfully treated with neuro-guided transcranial direct current stimulation (tDCS). METHODS: We used a PET-guided tDCS approach, as a hypometabolism of the frontal region was revealed by FDG TEP scan. TDCs was performed 5 days/week, 2 times/day, during 3 weeks. All cl

[Limb myokymia with involuntary finger movement caused by peripheral neuropathy due to systemic lupus erythematosus (SLE)--a case report].

A 24-year-old woman with SLE since the age of 12 developed sensory-motor peripheral neuropathy in 1990. Her sensory neuropathy was symmetrical in all limbs and distally dominant. Electrophysiological study showed marked reduction of motor and sensory conduction velocities. Sural nerve biopsy revealed vasculitis and axonopathy. Symptoms of neuropathy and nerve conduction velocities were improved by high dose steroid treatment. After one year she had a fever of unknown origin, and then presented limb myokymia with involuntary finger movement which was fine, irregular, and continuous. Needle EMG

The dawn of a new ice age: social egg freezing.

Given the age-related decline in ovarian reserve and oocyte quality, it is unsurprising the global trend of deferring motherhood has resulted in increased levels of involuntary childlessness. The development of oocyte vitrification, with pregnancy and livebirth rates now comparable to using fresh oocytes, has provided an opportunity to cryopreserve oocytes electively for future use, empowering women with the capacity to delay their childbearing years. While it enhances reproductive autonomy, age-related obstetric complications, economic implications and the risk of unsuccessful future treatmen

[Physiology and evaluation of overactive bladder].

Detrusor overactivity is a urodynamic observation characterized by involuntary detrusor contractions during the filling phase which may be spontaneous or provoked. There are certain patterns of detrusor overactivity. Phasic detrusor overactivity is defined by a characteristic wave form and may or may not lead to urinary incontinence. Terminal detrusor overactivity is defined as a single involuntary detrusor contraction occurring at cystometric capacity, which cannot be suppressed. Neurogenic detrusor overactivity is qualified as neurogenic when it is a relevant neurological condition (old term

Lesioning the thalamus for dyskinesia.

Recent advances on understanding the pallidothalamic relation lead us to perform Vim-Vo thalamotomy (combined thalamic lesion in ventralis intermedius nucleus and ventralis oralis nucleus) for cases with dyskinesia. In our recent series of thalamotomies, there are 12 cases of dyskinesia caused by various etiologies. Therefore the clinical manifestation of the involuntary movement was different in each case, including, more or less, some elements of irregular involuntary hyperkinetic movement. Stereotactic operation was performed using Leksell's apparatus aided by Surgiplan and MRI. The Vim nuc

Sleep-related periodic leg movements associated with spinal cord lesions.

We describe three patients who developed progressive paraparesis and sleep-related periodic leg movements (SRPLM) associated with thoracic spinal cord lesions; one patient had a schwannoma and two had intramedullary lesions. The patients showed periodic repetitive involuntary movements involving one or both lower limbs. The involuntary movements consisted of a single rapid dorsiflexion of the great toe or ankle, two to four repetitive dorsiflexions of the toes and ankle, and a mixture of repetitive jerks and prolonged spasms causing flexion of the hip and knee and dorsiflexion of the ankle and

[Hemichorea/hemiballism a rare complication of hyperglycemia].

We present a case of hemichorea/hemiballism, a rare complication of hyperglycemia. Diagnosis is made clinically by signs of unilateral involuntary movements of the extremities combined with typical neuroradiological findings in the basal ganglia. Guidelines for treatment of the condition are lacking but in many cases correction for hyperglycemia is sufficient for full symptom relief. In other cases, symptomatic treatment with haloperidol and tetrabenazine can be used.

How does European mental health legislation promote autonomy for those in involuntary care? A comparative descriptive analysis of 27 countries.

PURPOSE: A growing emphasis on autonomy, manifest through the influence of recovery orientations, user involvement, and the broader shift towards patient-centered healthcare, has significantly influenced mental health care. The Convention on the Rights of Persons with Disabilities (CRPD) is a pivotal development in this regard, challenging longstanding practices in mental health care and particularly involuntary care. This study examined how European mental health legislation seeks to promote the autonomy of those subjected to involuntary care, by analyzing laws from 27 jurisdictions. METHODS

Hemorrhage Accompanying Psychogenic Nonepileptic Seizure During Cesarean Section: A Case Report.

INTRODUCTION: Psychogenic nonepileptic seizures (PNES) during the perioperative period remain an underrecognized and under-investigated phenomenon. This type of disorder is characterized by sudden and short-lived convulsions, but they are which triggered by psychological factors. In cases of lack of experience, PNES is prone to be identified as epilepsy. Correct identification and proper treatment can more effectively protect patient safety. PATIENT CONCERNS: A pregnant woman, with a history of two miscarriages, developed unexplained convulsions after gynecologic surgery two years ago. The cu
