Treatment of refractory status epilepticus with inhalational anesthetic agents isoflurane and desflurane.

BACKGROUND: Refractory status epilepticus (RSE) is defined as continued seizures after 2 or 3 antiepileptic drugs have failed. Several intravenous agents have been used for RSE; however, problems occur with their toxicity and/or effectiveness. OBJECTIVE: To report our experience with inhalational anesthesia (IA) in patients who were refractory to other antiepileptic drugs. DESIGN, SETTING, AND PARTICIPANTS: Retrospective review during a 4-year period of patients with RSE treated with isoflurane and/or desflurane. MAIN OUTCOME MEASURE: Efficacy of IA on therapy in terminating RSE. RESULTS:

Severe hypotension is not essential for isoflurane neuroprotection against forebrain ischemia in mice.

BACKGROUND: Volatile anesthetics provide protection in experimental models of global cerebral ischemia. To date, all models evaluated have included profound systemic arterial hypotension as a component of the ischemic insult. This study was designed to determine if isoflurane protection persists in a global insult devoid of hypotension. METHODS: C57BL/6J mice having a high incidence of posterior communicating artery atresia were anesthetized with isoflurane (1.2%) or fentanyl/N2O and subjected to bilateral carotid artery occlusion for 15 min or 20 min with normotension (80-110 mmHg mean arter

Effects of GABAergic agents on anesthesia induced by halothane, isoflurane, and thiamylal in mice.

The effects of gamma-aminobutyric acid (GABA) receptor modulators and GABA uptake inhibitors on volatile and intravenous anesthetic-induced anesthesia were examined in male ICR mice, as assessed by the loss of righting reflex (LORR). The GABA uptake inhibitors, NO-711 and SKF89976A, which are permeable to the blood-brain barrier (BBB), but not nipecotic acid or guvacine, which poorly permeate BBB, shortened the onset of LORR but did not affect the duration of LORR induced by 1.5% halothane and 2% isoflurane. NO-711 and SKF89976A shortened the onset of and prolonged the duration of LORR induced

Oxygen and glucose deprivation-induced neuronal apoptosis is attenuated by halothane and isoflurane.

UNLABELLED: Both in vitro and in vivo evidence supports the reduction of early ischemic, both global and focal, brain injury by volatile anesthetics. However, the protection afforded by volatile anesthetics in later neuronal death, i.e., apoptosis, caused by global ischemia has not been investigated. We induced oxygen and glucose deprivation in neuronal cortical cell cultures prepared from newborn rats on in vitro Days 10-14. This hypoxic (PO2 <50 mm Hg) condition was maintained continuously (30, 60, and 90 min). In a separate experiment, the neuronal cell cultures were exposed to isoflurane (

Isoflurane and pentobarbital reduce AMPA toxicity in vivo in the rat cerebral cortex.

BACKGROUND: Isoflurane and pentobarbital can reduce alpha-amino-d-hydroxy-5-methyl-4-isoxazole-propionate (AMPA) receptor-mediated toxicity in vitro. However, their effect on AMPA toxicity in vivo is not known. The present study was undertaken to evaluate the effects of isoflurane and pentobarbital on the in vivo neurotoxicity produced by AMPA. METHODS: Wistar-Kyoto rats were allocated to one of seven groups (n = 8 per group): isoflurane 1 minimum alveolar concentration, isoflurane electroencephalogram burst suppression (EEG-BS), low-dose pentobarbital, pentobarbital EEG-BS, NBQX, conscious,

Cardiopulmonary effects of three prolonged periods of isoflurane anesthesia in an adult elephant.

An adult 3,500-kg female African elephant was anesthetized 3 times for treatment of subcutaneous fistulas over the lateral aspect of each cubitus (anesthesia 1 and 2) and for repair of a fractured tusk (anesthesia 3). Lateral recumbency and anesthesia were achieved with etorphine (anesthesia 1 and 2) or etorphine and azaperone (anesthesia 3). The elephant's trachea was intubated and anesthesia was maintained by use of isoflurane and oxygen delivered via 2 standard large animal anesthesia machines joined in parallel. The range of total recumbency time was 2.4 to 3.3 hours. Breathing and heart r

Reversible focal ischemia in the rat: effects of halothane, isoflurane, and methohexital anesthesia.

Barbiturates and the volatile anesthetic isoflurane reduce CMR to similar values. If the mechanism of barbiturate protection against focal ischemic injury is due to a reduction in cellular energy requirements, then isoflurane should similarly reduce ischemic injury. To evaluate this, spontaneously hypertensive rats underwent 2 h of reversible middle cerebral artery occlusion (MCAO) while receiving deep methohexital, isoflurane, or halothane anesthesia. Ninety-six hours postischemia, neurologic deficits were present but without a difference between groups. Mean +/- SD infarct volume, as assesse

Effects of residual concentrations of isoflurane on the reversal of vecuronium-induced neuromuscular blockade.

Thirty-six anesthetized patients (ASA physical status 1 or 2) undergoing elective surgery were monitored (isometric adductor pollicis mechanical activity) to detect the effects of discontinuing isoflurane anesthesia upon the reversal of vecuronium-induced neuromuscular blockade. Neuromuscular blockade was produced by vecuronium 100 micrograms/kg and additional doses of 20 micrograms/kg until completion of surgery. The patients were randomly divided into three groups: in the control group (n = 12), only fentanyl/N2O was given; in the "isostable" group (n = 12), isoflurane at an end-tidal concen

Circulatory effects of isoflurane in patients with ischemic heart disease: a comparison with halothane.

The circulatory effects of isoflurane (I) were compared with those of halothane (H) in two groups of patients premedicated with morphine and scopolamine and scheduled for coronary artery bypass surgery. Both groups were similar with respect to age, weight, sex distribution, body surface area, left ventricular function, and preoperative dose of propranolol. While the patients were awake and breathing 100% oxygen, cardiac output and related hemodynamics were measured. The patients were then anesthetized by the same anesthesiologist with either isoflurane or halothane plus 50% N2O in O2. Ventilat

Isoflurane aggravates pre-existing proteotoxicity in adult nematodes by suppressing mitochondrial fitness.

Post operative delirium (POD) is an acute complication, characterized by fluctuating attention and confusion, which may develop and persist into cognitive decline. POD is most commonly observed in elderly patients, particularly those with preexisting cognitive impairments. Isoflurane, a widely used volatile anesthetic, is associated with POD. However. how exposure to isoflurane affects the integrity of the proteome is largely obscure. Utilizing the nematode C. elegans, we found that isoflurane leads to a long-lasting decline in protein homeostasis (proteostasis) of adult animals that express n

Ramelteon Ameliorates Isoflurane-Induced Hyperactivity and Social-Recognition Impairment in Aged Mice.

BACKGROUND: Ramelteon, a melatonin receptor agonist, has been reported to alleviate postoperative delirium (POD), although its efficacy remains controversial. The mechanisms of ramelteon's effects are unclear and few animal studies have addressed POD-related behavioral and molecular changes. We investigated the specific postoperative behavioral and molecular changes that result from ramelteon pretreatment. METHODS: Ramelteon (0.03 or 0.3 mg/kg, p.o.) was given to mice once a day for 7 consecutive days before abdominal laparotomy under 2 h of isoflurane anesthesia. Postoperative locomotor acti

Frequency-Dependent Block of Excitatory Neurotransmission by Isoflurane via Dual Presynaptic Mechanisms.

Volatile anesthetics are widely used for surgery, but neuronal mechanisms of anesthesia remain unidentified. At the calyx of Held in brainstem slices from rats of either sex, isoflurane at clinical doses attenuated EPSCs by decreasing the release probability and the number of readily releasable vesicles. In presynaptic recordings of Ca2+ currents and exocytic capacitance changes, isoflurane attenuated exocytosis by inhibiting Ca2+ currents evoked by a short presynaptic depolarization, whereas it inhibited exocytosis evoked by a prolonged depolarization via directly blocking exocytic machinery

Medetomidine-ketamine-isoflurane anaesthesia in pygmy hippopotami (Choeropsis liberiensis)--a case series.

HISTORY: Medical knowledge of pygmy hippopotami is limited. Anaesthesia has been considered a challenge because of the anatomy, semi-aquatic life style and aggressive behaviour. Polycystic kidney disease (PKD) has been described and can contribute to active kidney disease potentially affecting anaesthesia. PHYSICAL EXAMINATION AND MANAGEMENT: Fourteen pygmy hippopotami were anaesthetized for general health assessment and reproductive procedures. Animals (estimated bodyweight 250 kg) were darted intramuscularly with 0.08 mg kg(-1) medetomidine and 1.2 mg kg(-1) ketamine. After endotracheal intu

Brainstem regions affecting minimum alveolar concentration and movement pattern during isoflurane anesthesia.

BACKGROUND: Spinal transection or selective delivery of volatile anesthetics to the spinal cord reduces minimum alveolar concentration (MAC), whereas precollicular decerebration does not. The authors sought to determine which brainstem regions influence anesthetic requirements and movement responses with isoflurane. METHODS: Movement (biceps femoris electromyogram) and MAC were measured in adult rats before and after decerebration at the precollicular, mid-collicular, pontine or medullary level, or decerebellation. Additional experiments assessed the effects of lidocaine inactivation of the m

Time to extubation in infants undergoing pyloromyotomy -- isoflurane inhalation vs remifentanil infusion.

BACKGROUND: Infantile hypertrophic pyloric stenosis (IHPS) associated with metabolic alkalosis, could induce late anesthesia recovery, especially when opioids are used. The aim of this study was to compare the time of extubation and the quality of perioperative analgesia in infants scheduled for pyloromyotomy, receiving either isoflurane inhalation or remifentanil infusion. METHODS: Thirty full-term infants scheduled for pyloromyotomy were prospectively studied. A standardized anesthetic induction was performed. For maintenance of anesthesia, infants were randomly allocated to receive either

Isoflurane sensitizes the cardiac sarcolemmal adenosine triphosphate-sensitive potassium channel to pinacidil.

BACKGROUND: Cardioprotective effects of isoflurane are partially mediated by the sarcolemmal adenosine triphosphate-sensitive potassium (sarcK ATP ) channel. The authors tested the hypothesis that isoflurane sensitizes sarcK ATP channels to a potassium channel opener, pinacidil, adenosine- and phospholipid-mediated pathways. METHODS: Activation by pinacidil of the K ATP current (I KATP ) was monitored in guinea pig ventricular myocytes at 0.5 and 5 mm intracellular ATP in the whole cell configuration of the patch clamp technique. The sensitization effect was evaluated by pretreating each myoc
