Multilocus variable-number tandem-repeat analysis-confirmed emergence of a macrolide resistance-associated mutation in Mycoplasma pneumoniae during macrolide therapy for interstitial pneumonia in an immunocompromised child.

A child with Job's syndrome was treated for pneumonia due to Mycoplasma pneumoniae. A mixed population of wild-type bacteria and an A2059G mutant was detected during josamycin treatment failure. The same multilocus variable-number tandem-repeat analysis (MLVA) type (MLVA type I) was isolated before and after treatment failure. The child recovered after ciprofloxacin treatment.

An update on the hyper-IgE syndromes.

The hyper-IgE syndromes (HIES; originally named Job's syndrome) are a collection of primary immunodeficiency syndromes resulting in elevated serum IgE levels and typified by recurrent staphylococcal skin abscesses, eczema and pulmonary infections. The disorder has autosomal dominant and recessive forms. Autosomal dominant HIES has been shown to be mainly due to STAT3 mutations and additionally results in connective tissue, skeletal, vascular and dental abnormalities. Autosomal recessive HIES has been shown to be mainly due to mutations in DOCK8; these patients are more prone to viral skin infe

STAT3 dominant negative Hyper-IgE syndrome: A patient report with actionable genomic findings.

Over the last two decades, diagnostic genetic testing for inborn errors of immunity has primarily relied on gene panel-based approaches organized by phenotype. In this report, we describe a patient with a clinical and molecular diagnosis of STAT3 dominant negative hyper-IgE syndrome referred to the National Institutes of Health for further evaluation including genome sequencing. Genome sequencing analysis included primary, secondary, and pharmacogenomic findings. This analysis confirmed the primary molecular diagnosis of STAT3 dominant negative hyper-IgE syndrome and found a pathogenic variant

Hyper-IgE syndromes in pediatrics: clinical spectrum, differential diagnosis, and management.

Hyper-IgE syndromes (HIES) are rare inborn errors of immunity (≈1 per million) caused by pathogenic variants in STAT3, DOCK8 or IL6ST. They present with very high serum immunoglobulin E (IgE), recurrent bacterial or fungal infections, eczema and characteristic organ involvement. The autosomal dominant STAT3-deficient form features early-onset eczema, “cold” abscesses, recurrent pneumonias with pneumatoceles and skeletal or dental anomalies. Autosomal recessive forms such as DOCK8 or PGM3 deficiency show a more severe phenotype with viral skin infections, allergy, asthma and increased malignanc

Hyper Immunoglobulin E Syndrome With Conjunctival Molluscum Contagiosum.

The authors report a rare case of conjunctival molluscum contagiosum in a 4-year-old male with hyperimmunoglobulin E syndrome and disseminated molluscum contagiosum infection. The patient presented with bilateral periorbital and right conjunctival lesions, along with lesions on the face, neck, ears, trunk, back, and genitalia. He was being prepared for a bone marrow transplant, and surgical excision of the lesions was performed to reduce the viral load. Although this case demonstrates the atypical presentation of molluscum contagiosum in immunocompromised individuals, the conjunctival lesions

Extracellular Vesicles based STAT3 delivery as innovative therapeutic approach to restore STAT3 signaling deficiency.

Extracellular Vesicles (EVs) have been proposed as a promising tool for drug delivery because of their natural ability to cross biological barriers, protect their cargo, and target specific cells. Moreover, EVs are not recognized by the immune system as foreign, reducing the risk of an immune response and enhancing biocompatibility. Herein, we proposed an alternative therapeutic strategy to restore STAT3 signaling exploiting STAT3 loaded EVs. This approach could be useful in the treatment of Autosomal Dominant Hyper-IgE Syndrome (AD-HIES), a rare primary immunodeficiency and multisystem disord

New Dominant-Negative IL6ST Variants Expand the Immunological and Clinical Spectrum of GP130-Dependent Hyper-IgE Syndrome.

Patients with autosomal dominant (AD) hyper-IgE syndrome (HIES) suffer from a constellation of manifestations including recurrent bacterial and fungal infections, severe atopy, and skeletal abnormalities. This condition is typically caused by monoallelic dominant-negative (DN) STAT3 variants. In 2020, we described 12 patients from eight kindreds with DN IL6ST variants resulting in a new form of AD HIES. These variants encoded truncated GP130 receptors, with intact extracellular and transmembrane domains, but lacking the intracellular recycling motif and the four STAT3-binding residues, resulti

Allele-Specific Disruption of a Common STAT3 Autosomal Dominant Allele Is Not Sufficient to Restore Downstream Signaling in Patient-Derived T Cells.

Dominant negative mutations in the STAT3 gene account for autosomal dominant hyper-IgE syndrome (AD-HIES). Patients typically present high IgE serum levels, recurrent infections, and soft tissue abnormalities. While current therapies focus on alleviating the symptoms, hematopoietic stem cell transplantation (HSCT) has recently been proposed as a strategy to treat the immunological defect and stabilize the disease, especially in cases with severe lung infections. However, because of the potentially severe side effects associated with allogeneic HSCT, this has been considered only for a few pati

Retained primary teeth in STAT3 hyper-IgE syndrome: early intervention in childhood is essential.

BACKGROUND: STAT3 hyper-IgE syndrome (STAT3-HIES) is a rare primary immunodeficiency that clinically overlaps with atopic dermatitis. In addition to eczema, elevated serum-IgE, and recurrent infections, STAT3-HIES patients suffer from characteristic facies, midline defects, and retained primary teeth. To optimize dental management we assessed the development of dentition and the long-term outcomes of dental treatment in 13 molecularly defined STAT3-HIES patients using questionnaires, radiographs, and dental investigations. RESULTS: Primary tooth eruption was unremarkable in all STAT3-HIES pat

Assessment of work-related risks factors for carpal tunnel syndrome.

BACKGROUND: The association of carpal tunnel syndrome with occupational risk factors is well established. However, in clinical practice these factors are only rarely considered and evaluated. Managing these risk factors could prevent the occurrence of future cases and alleviate treatment of the afflicted individuals. OBJECTIVES: To estimate the role of occupational risk factors in a large group of patients diagnosed by electrophysiological studies as suffering from CTS. METHODS: A group of 396 subjects (204 women, 165 men) who were tested in one laboratory by electrophysiological studies wer

The dental management of a patient with hyperimmunoglobulinemia E syndrome: a case report.

Hyperimmunoglobulinemia E recurrent infection syndrome (also known as Job's syndrome) is a rare multi-system primary immunological disorder in which non-immunological abnormalities of the dentition, bones and connective tissue are also seen. A previous study has reported the occurrence of dental abnormalities in three-quarters of individuals diagnosed as suffering from this condition. The present authors report the case of a boy whose prolonged retention of the primary dentition was associated with delayed eruption of permanent teeth. They emphasize the need for early intervention in order to

Burnout and its' associated factors among health workers: A hospital-based cross-sectional study from Pokhara, Nepal.

BACKGROUND: Health workers' burnout compromises patient safety and satisfaction, in addition to their own health. Over the last three decades or more, burnout syndrome has been getting more attention. High levels of burnout have been reported by health workers globally, and Nepal is no exception, including those mainly in hospital settings. Despite this, burnout issues have not been given enough attention in the literature. Hence, this study aims to assess the prevalence of burnout among health workers in hospital settings in Pokhara and to assess its' associated factors (personal, job-related
